1.Safety and efficacy of laparoscopy-assisted radical gastrectomy for advanced gastric cancer

Objective To evaluate the safety and efficacy of laparoscopy-assisted gastrectomy (LAG) for advanced gastric cancer.Methods The clinical data were retrospectively analyzed in 108 patients undergoing radical LAG for advanced gastric cancer from March 2008 to November 2011.Results 98 out of 108 patients were followed-up,the follow-up rate was 90.7%,with medium follow-up period of 30 months (range 6-70 months).26 patients died and 3 patients developed recurrence during the follow-up period.The overall recurrence rate was 26.9% (29/108),in which the recurrence rate was 0 for Ⅰ B cases,18.8% (6/32) for ⅡA,30% (6/20) for ⅡB,30.8% (8/26) for ⅢA,and37.5% (9/24) for ⅢB,respectively.The overall 1,2,3-year survival rates were 95.4%,88.9%,73.8%,and 1,2,3-year progression free survival rates were 93.5%,83.3%,73.8%,respectively.Conclusions Compared with open gastrectomy LAG is a safe and feasible surgical procedure with satisfactory 3-year survival in patients of advanced gastric carcinoma.
2.Analysis of the dispassed reason of the reporting staff of senior professional and technical positions qualifications evaluation of Anhui provincial health series

Objective To explore the dispassed reason of Anhui provincial health senior titles evaluation.Methods The senior professional titles of the year 2009-2011 assessment results which were announced by the office of personnel titles of Anhui Provincial Health Department and the recorded results of the jury assessment were analyzed.Results In Anhui Province from 2009 to 2011 for three years,a total of 4 523 people participated in the senior title review by staff 3 039 people,by the proportion of 67.2%.2009,2010,2011 pass rate was 69.14%,66.85%,65.89%,respectively.1 484 people did not pass,and the rate was 32.8%.The main reason was the lack of reporting by professional and technical proficiency,non-standard format of application materials,assessment reporting material falsification.Conclusion Increasing the professional title appraisal policy propaganda,assessment focus which should be focused,the RAE quantitative score standards of health senior professional title,the establishment of a senior title of the series of health reporting assessment information system can improve this conditions.

6.A Study of Mental Health Status and Its Determinants among the Rural Middle School Students

Objective: To investigate the rural middle school students' mental health and explore its determinants. Methods: 989 rural middle school students in Jiangxi province were investigated with SCL-90, Social Support Rating Scale and the Chinese versions of EMBU. Results: The major problem of mental health were interpersonal sensitivity, obsessive-compulsive, depression, hostility and paranoid ideation. There was remarkable difference between boys and girls, senior and junior high school students. Multiple regression analysis indicated that the major determinants of the rural middle school students' mental health were subjective social support, usage of social support, father's and mother's rejection and denying, father's over-protection, mother's punishment and rigidity, different sex, grades and only children or non-only children. Conclusion: The students in rural middle school, especially in senior high school, had severe mental health problems, social support and parental rearing patterns have a significant influence on mental health.

[Objective] To observe the effect of Puji Xiaodu Decoction (PXD) on immune function in mice. [Methods] Thirty NIH mice were randomized into three groups. Group A was treated with normal saline, group B with 2.9 g/kg PXD and group C with 11.6 g/kg PXD for 7 days. After treatment, interleukin 2 (IL-2) production and natural killer cell (NK) activity in splenocytes and lymphocyte proliferation in the spleen were detected. AIM: To observe the changes of plasma content of Glu-plasminogen (plasmin, Pln) in patients during blood coagulation and/or fibrinolytic system activation. METHODS: Using specific McAb to antigenic determinant in NH 2 terminal (1-65 aa) of Pln and specific McAb to antigenic determinant in heavy chain of plasminogen, the sandwiched ELISA method was established to detect Glu-plasminogen and total plasminogen in human plasma collected from 220 normal controls and 40 patients after heart surgery. RESULTS: The average total plasminogen was (231.8?62.1) mg/L and average Glu-plasminogen was (231.9?45.8) mg/L in 220 normal controls, the ratio of Glu-plasminogen to total plasminogen (G/P) was 0.91. Objective To evaluate the nutritional status of extremely low birth weight (ELBW) infants and the effects of nutritional support strategy alterations on their growth during hospitalization.Method From 2005 to 2014,clinical data of ELBW infants admitted to the neonatal intensive care unit (NICU) in our hospital were retrospectively analyzed.The clinical data included their general status,enteral and parental nutritional support strategy and complications during hospitalization The patients were assigned into pre5 group and late5 group.Those who survived and discharged from 2005 to 2009 were the pre5 group,and those who survived and discharged fromn 2010 to 2014 were the late5 group.The independent t test and chi square test were used for statistical analysis.Result A total of 58 ELBW infants were enrolled in the study,including 18 patients in the pre5 group and 40 in the late5 group.No statistically significant differences existed between the two groups on gestational age,birth weight,Z score (weight for length and gender),birth length,head circumference and main complications during hospitalization (P >0.05).Pre5 group had higher incidence of small for gestational age (SGA) than late5 group (16/18 vs.25/40,P =0.037),while the EUGR ratio at discharge (14/18 vs.21/40,P =0.061) was similar.When compared with pre5 group,late5 group had larger amount of initial enteral feeding volume [4.4 ml/(kg · d) vs.2.4 ml/(kg · d),P =0.014] and feeding volume at the end of the first week [(19.8 ± 16.0) ml/(kg · d) vs.(12.2 ±9.5) ml/(kg · d),P =0.036].Similarly,the starting dose of amino acids in parenteral nutrition [2.0g/(kg· d) vs.1.0 g/(kg· d),P<0.001],maximum dose of amino acids [4.0g/(kg.d) vs.3.5 g/(kg · d),P < 0.001],total calories at the end of the first week [(82.6 ± 12.6) kcal/(kg · d) vs.(71.1±15.2) kcal/(kg· d),P=0.004] and the second week [(103.7 ±19.8) kcal/(kg· d) vs.(92.3 ± 17.9) kcal/(kg · d),P =0.041],the weight gain velocity from birth to discharge [(18.7 ± 2.9) g/(kg.d) vs.(16.9±2.8) g/(kg· d),P=0.031] and change of Z scores (AZ) [-0.6 (-1.0,-0.4) vs.-1.2 (-1.6,-0.8),P =0.004] showed significantl differences between the two groups,with better outcomes in late5 group.However,the duration of parenteral nutrition,the total amount of amino acids,the time reaching total enteral feeding,the length of hospital stay were similar between the two groups.Ten cases(61.1%)of infants in pre5 group were breastfed,and four of them used human milk fortifier (HMF) (fortified rate was 22%).32 cases (80%) in late5 group were breastfed and 23 cases used HMF (fortified rate was 57.5%).The time to initiate HMF in the late5 group was at (30.2 ± 13.2) days,and human milk amount was (89.9 ± 34.5) ml/kg,fortified duration was (32.8 ± 15.7) days.Conclusion The enteral feeding strategy were more vigorous in the last 5 years than before,the initial feeding volume,the increasing rate,the initial dosage of amino acid,and maximum dosage of amino acid had been increased.Human milk and HMF of preterm infants were preferred.The vigorous nutritional support strategy were effective for the weight gain of ELBW infants without obvious side effects during hospitalization.
