2.Effect of general anaesthesia with combination of acupuncture and enflurane applied in radical operation of laryngocarcinoma.

OBJECTIVETo observe the effect of general anaesthesia with combination of acupuncture [conducted with Han's acupoint nerve stimulator (HANS) applied] and enflurane in radical operation of laryngocarcinoma (LC).
METHODSSixty patients with LC of grade I - II , classified according to the standard of American Society of Anesthesiologists (ASA), were assigned by randomizing number table to the control group and the tested group, 30 patients in each group. The control group received anaesthesia with enflurane alone for inducing and maintaining; the tested group was anaesthetized with enflurane like that given to the control group but also received additionally needling stimulation conducted by HANS. The dosage of enflurane used, the minimum effective concentration of enflurane in alveolar air (MACEnf) and the changes of heart rate (HR) as well as blood pressure (BP) in patients at different time points in the operational process were observed.
RESULTSAs compared with those in the control group, in the tested group, both the MACEnf and the dosage of enflurane were reduced, with the difference between the two groups significant (P<0. 01). The changes of HR and BP among different time points in the tested group were slight, and as compared with those in the control group at the corresponding time points, the difference was significant ( P <0. 05 or P<0.01).
CONCLUSIONGeneral anaesthesia with combination of enflurane and needling conducted by HANS applied in radical operation of LC has definite effect with less complication. Needling could be cooperated with narcotics, and so it could be taken as an auxiliary measure of anaesthesia for radical operation of LC.

Objective To analyze the relationships between the drinking water fluoride and bone mineral density (BMD), and serum osteocalcin (BGP) and to explore the BMD and serum BGP as significant early screening biomarkers for fluorosis especially for early bone damage in endemic fluorosis areas. Methods Wamiao (severe endemic fluorosis area, as fluoride exposed group) and Xinhuai (non endemic fluorosis area, as control group) Village were selected in 2006. One hundred and fouty-six objects were chosen from 2 villages (103 in Wamiao, 43 in Xinhuai). The sex, age, body height, body weight, drinking water fluoride in each object's household well, BMD, and serum BGP were investigated, and the dose-response relationships were analyzed between the drinking water fluoride and BMD, and serum BGP. CurveExpert 1.3 Software was used to fit the dose-response relationships between the rate of abnormal BMD, the rate of abnormal serum BGP, and the drinking water fluoride. Results The levels of drinking water fluoride in males' and females' families in fluoride exposed group were [(2.38±0.68), (2.62±0.91 )mg/L] significant higher than that in control group [(0.35±0.08), (0.36±0.07)mg/L], the difference being statistically significant(t values were 14.27 and 11.08,and P<0.01, respectively). BMD in males in fluoride exposed group [(0.78±0.07)g/cm2] was significant lower than that in control group[(0.83±0.08)g/cm2], the difference being statistically significant (t=2.37,P<0.05). Serum BGP in males and females in fluoride exposed group [(4.17±0.67), (4.11±0.57) μg/L] were significant higher than that in control group [(1.48±0.40), (1.44±0.39)μg/L], the difference being statistically significant (t values were 17.64 and 19.40, and P<0.01, respectively]. BMD in the group with drinking water fluoride≥2.92 mg/L[(0.66±0.15 )g/cm2] was significant lower than that in the group with drinking water fluoride<0.42 mg/L [(0.76±0.12)g/cm2], the difference being statistically significant (P<0.01). The levels of serum BGP in the groups with the drinking water 0.42-,2.05-, ≥.92 mg/L[(3.83±1.07), (4.22±0.72), (3.99±0.63) μg/L] were significant higher than that in the group with the drinking water<0.42 mg/L [(1.44±0.37) μg/L], the difference being statistically significant (P<0.01). The equation for the dose-response relationship between the drinking water fluoride and the rate of abnormal BMD was y=(0.284-0.058x)-1.260, r=0.999 94; and y=100.05/(1+78.62e-4.5x), r=0.999 99 for the drinking water fluoride and the rate of abnormal serum BGP. Conclusions There were significant dose-response relationships between drinking water fluoride and BMD and serum BGP. It indicated that BMD and BGP might be considered as early screening biomarkers for endemic fluorosis, especially for the bone damage.

Objective To observe the clinical curative effect and security of the deproteinised calf serum enteric capsules on nerve functions defect caused by cerebral isehemia damage.Methods A randomized,double-blind,multicentric trial design was conducted.Treatment group:based on routine treatment,the patients were given deproteinised calf serum enteric capsules for 90 days.Control group: based on routine treatment,the patients were given placebo for 90 days.The NIHSS,BI,mRS measuring scale were used for evaluation on the 7th,14th and 28th day before and after treatment.BI and mRS measuring scale were used for evaluation on the 90th day.The change of numerical value was inspected before and after treatment.Results Before treatment,there was no significant difference in the NIHSS,BI and mRS measuring scale evaluation between the groups of treatment and control.After treatment,there were significant differences in the NIHSS,BI and mRS measuring scale evaluation between the groups of treatment and control on the 14th and 28th day (P
6.Hypotensive action of luohuo capsule and its effect on plasma adrenal medullin and tissue factor pathway inhibitor.

OBJECTIVETo study the therapeutic effect of Luohuo Capsule (LHC) clinically.
METHODSClinical trial on 90 patients were carried out adopting randomized, stratified, single-blinded and positive drug controlled method, by divided patients into the treated group (n = 60) and the control group (n = 30). The treated group was treated with LHC, which was mainly consisted of Leech, Radish seed, Water-plantain Tuber, Chuanxiong, etc. The control group was treated with Beijing Hypotensive. No. 0. The therapeutic course was 4 weeks. The diagnosis and efficacy evaluation were in accord with corresponding national standards, using the indexes including safety, clinical symptoms, blood pressure, hemorrheologic parameters, blood lipid, adrenal medullin (AdM) and plasma tissue factor pathway inhibitor (TFPI).
RESULTSThe total effective rate of LHC in lowering blood pressure in the treated group and the control group was 85.00% and 86.67% respectively, comparison of them showed no significant difference (P = 0.915). Most of the improvement of clinical symptoms in the treated group were better than those in the control group (P < 0.05 or P < 0.01). LHC could also improve the hemorrheologic status (P < 0.01), alleviate the blood lipids disorder (P < 0.05), and adjust AdM and TFPI (P < 0.05).
CONCLUSIONLHC is safe in treating hypertension grade I and II with no adverse reaction.

7.Study on the quantitative evaluation on the degree of TCM basic syndromes often encountered in patients with primary liver cancer.

OBJECTIVETo establish a quantitative model for evaluating the degree of the TCM basic syndromes often encountered in patients with primary liver cancer (PLC).
METHODSMedical literatures concerning the clinical investigation and TCM syndrome of PLC were collected and analyzed adopting expert-composed symposium method, and the 100 millimeter scaling was applied in combining with scoring on degree of symptoms to establish a quantitative criterion for symptoms and signs degree classification in patients with PLC. Two models, i.e. the additive model and the additive-multiplicative model, were established by using comprehensive analytic hierarchy process (AHP) as the mathematical tool to estimate the weight of the criterion for evaluating basic syndromes in various layers by specialists. Then the two models were verified in clinical practice and the outcomes were compared with that fuzzy evaluated by specialists.
RESULTSVerification on 459 times/case of PLC showed that the coincidence rate between the outcomes derived from specialists with that from the additive model was 84.53 %, and with that from the additive-multificative model was 62.75 %, the difference between the two showed statistical significance (P<0.01).
CONCLUSIONSIt could be decided that the additive model is the principle model suitable for quantitative evaluation on the degree of TCM basic syndromes in patients with PLC.

8.Clinical value of serum level of ryanodine receptor antibodies in diagnosis of myasthenia gravis

Objective To investigate the clinical value of detecting the serum level of ryanodine receptor antibodies (RyR-Ab) in the diagnosis and evaluation of myasthenia gravis (MG). Methods Enzyme-linked immunosorbent assay (ELISA) and Western blotting were used to detect the serum level of RyR-Ab in 81 patients with MG, 45 non-MG patients with other neurological disorders and 50 healthy control subjects. Results The results of ELISA showed a serum RyR-Ab positivity rate of 23.4% in the in MG patients, significantly higher than the rates in non-MG patients and the healthy subjects (P<0.05). The serum RyR-Ab positivity rate was 77.2% in MG patients with thymoma (MGT), 14.2% in those with thymie atrophy (MGA), and 6.6% in those with thymie hyperplasia (MGH). A positive correlation was found between the titer of serum RyR-Ab and the severity of muscle weakness (r=0.547, P<0.05). Compared with CT scanning, serum RyR-Ab level had low sensitivity (77.2%) but high specificity (91%, P<0.05) for diagnosis of MGT. According to the results of Western blotting, the serum RyR-Ab positivity rate was 81.8% in MGT patients, significantly higher than that in thymomβ-free MG patients and NMG patients (P<0.05). Conclusion RyR-Ab positivity is mainly found in MGT patients, and detection of serum RyR-Ab may help evaluate the severity of MG and assist in CT-based diagnosis of MGT.

To screen differentially expressed genes involved in osteogenic differentiation of human bone marrow stromal cells (BMSCs) at defined stages, subtractive cDNA library was established by means of suppression subtractive hybridization. The BMSCs cultured for 12 and 21 d were used as driver and tester, respectively. A subtract library was successfully constructed and five positive clones were selected from the library. Sequencing analysis and homology comparison showed that the five clones differentially expressed in BMSCs cultured for 21 d were at least 90% homologous with the known genes in human GenBank. It was interestingly found that the osteogenic BMSCs cultured for 21 d differentially expressed decorin and Bax inhibitor 1. RT-PCR was performed to confirm the differentially expressed genes. The results showed that the expression of Bax inhibitor 1 was significantly higher in the cells of 21-day than that of 12-day, while the expression of decorin was only detected in the cells of 21-day.

10.Percutaneous coronary intervention combined cardiac resynchronization therapy for refractory heart failure secondary to ischemic cardiomyopathy.

OBJECTIVETo evaluate the efficacy and safety of percutaneous coronary intervention (PCI) combined cardiac resynchronization therapy (CRT) for refractory heart failure secondary to ischemic cardiomyopathy (ICM).
METHODSPCI and CRT were performed in 7 ICM patients confirmed by angiography with NYHA class IV, QRS duration >/= 130 ms in 6 of them, III degrees AVB in 1 patient, fast ventricular heart rate Af in 1 patient, ventricular fibrillation history in 2 patient. All of them had their LVEDD >/= 55 mm, and LVEF = 0.40 detected by UCG. PCI was performed first in 5 patients, and their follow-up angiography showed no restenosis 6 months after PCI, then CRT was given. CRT was performed first in 2 patients and 2 weeks later PCI was combined.
RESULTSThe procedures of PCI and CRT were performed successfully in all patients. Five patients received right atrial and biventricular pacing, one patient with Af received biventricular pacing and atrial-ventricular node radiofrequency ablation at the same procedure, and the another one patient received CRTD. One out of seven patients died of re-AMI 4 months after the combination therapy, and the other 6 patients had been alive 5 - 41 (23.2 +/- 13.8) months during the follow-up period. The heart function of the 7 patients had further improved after PCI and CRT combined therapy compared to that of PCI or CRT only. Their NYHA class decreased from IV to II, 6-minute walking distance increased steadily, and mitral regurgitation reduced and QRS duration shortened significantly. The LVEDD decreased and LVEF increased significantly in 2 patients without ventricular aneurysm, and slight improvement or no change were in the other 5 patients.
CONCLUSIONFor patients with refractory heart failure secondary to ICM, the combination of PCI and CRT could obviously improve their heart function, quality of life and prognosis, which also very safe in perforation.
