2.Influnece of early postoperative feeding in formation of gastrointestinal anastomotic fistula and healing time in rabbits

Objective To observe the influence of early postoperative feeding in the healing of intestinal anastomosis in rabbits,and to clarify preliminarily the relationships between early postoperative feeding after gastrointestinal surgery and gastrointestinal anastomotic fistula formation and healing time in rabbits.Methods 48 rabbits were randomly divided into experimental group and control group, then they were treated with gastrointestinal anastomosis.The rabbits in experimental group were fed with liquid diet 24 h after operation,and the rabbits in control group were fed nothing after operation and supplied by total parenteral nutrition.Two rabbits of each group were selected for exploratory laparotomy on the 3rd,5th,7th,10th and 15th day after operation,and the healing rate of anastomosis,the anastomotic bursting pressure,the anastomotic breaking strength,and the hychoxyproline level of anastomosis were observed.Results The healing rate of anastomosis in control group was 91.6%(22/24), and the healing rate of anastomosis in experimental group was 95.8%(23/24),there was no significant difference between two groups(P>0.05).The anastomotic bursting pressures of the rabbits in two groups were decreased remarkably at the 72nd hour after operation,which was the lowest point,and they were increased remarkably on the 5th day after operation,but the anastomic bursting pressure in experimental group was a little lower than that in control group,and it reached the peak on the 7th day after operation in control group. On the 10th day after operation,the anastomic bursting pressure in control group was a little lower than that on the 7th day after operation,but the anastomic bursting pressure in experimental group reached the peak.There were no significant differences of anastomic bursting pressure at different time points between two groups(P>0.05).The anastomotic breaking strength had no significant difference between two groups at the 72nd hour after operation,both of them reached the lowest points,however the anastomtic breaking strengths in two groups were increased remarkably on the 10th day after operation,and reached the peaks.but there were no significant differences of anastomic breaking strength at different time points between two groups (P>0.05 ). The hychoxyproline level of anastomosis:in experimental group was a little lower than that in control group at the 72tnd hour after operation,and both of them reached the peaks on the 7th day after operation;but there were no significant differences of hychoxyproline levels of anastomosis at different time points between two groups(P>0.05).Conclusion Early postoperative feeding can not cause the increase of anasmotic healing time and the incidence rate of gastrointestinal anastomotic fistula.

Objective To evaluate the safety and accuracy of CT-guided cutting needle biopsy for the hepatic lesions near diaphragmatic dome.Methods A total of 25 cases with hepatic lesions near the diaphragmatic dome were undertaken CT-guided cutting needle biopsy using 16 gauge or 18 gauge core biopsy needles.Results Histological examination showed malignancy in 17 cases and benign in 8 with 2 false negative results(8%),and there were no false positive results.The specificities of malignant and benign lesions were 100% and 75%,respectively.Overall accuracy was 92%.Pneumothorax,needle tract hemorrhage,and subcapsular hepatic hemorrhage occurred in 2(8%),1(4%)and 1(4%),respectively.Conclusion CT-guided cutting needle biopsy for the hepatic lesions near diaphragmatic dome is a reliable and relatively safe diagnostic method.(J Intervent Radiol,2007,16:838-840)

6.Nuss procedure for the correction of pectus excavatum by using double braces in adolescence

Objective To investigate the indication, feasibility and technique of minimally invasive nuss procedure with thoracoscope by using double braces in the treatment of wide-scope pectus excavatum repairing in adolescence. Methods 31 patients including 24 boys and 7 girls, suffered from pectus excavtum were corrected by nuss procedure under thoracoscope.The average age was (15.32 ± 3.89)years (ranged, 14 years and 18 years). All cases were moderate to severe degree according to Hallar index with depression scope of 4 ribs or more. A couple of braces of two-point or multipoint fixation for thoracoscopic-assisted nuss procedure were used. Results The procedure was successfully completed under thoracoscopy in all patients. Double braces were utilized in 27 cases, double are bars were required in 4 cases, and multipoint fixations were in 6 patients. The postoperative hospital stay was 5-11 days [average, ( 7.48 ± 1.95 ) days]. The duration of following up was one month to three years. The perioperative complications included intraoperative hemorrhage in 2 patients, bar invertion with displacement in 1, and interrupted pain for two months in 1. All patients recovered after expectant treatment. Conclusion Nuss procedure with double braces for the correction of a large area of asymmetric pectus excavatum under thoracoscopy is safe and effective technique for adolescence.
7.Additional diagnostic value of low dose CT in ventilation/perfusion hybrid SPECT for pulmonary embolism

Objective To evaluate the assistant diagnostic value of low dose CT in patients with pulmonary embolism (PE) based on ventilation/perfusion (V/Q) SPECT imaging.Methods One hundred and two patients with clinical suspected PE had been enrolled for this retrospective study.The final diagnosis of PE was made according to the 2008 guidelines of European Society of Cardiology (ESC).All patients underwent V/Q SPECT/CT (Hawkeye 4,GE).The imaging findings from low dose CT lung window were used for differential diagnoses of abnormal regions in SPECT imaging.The diagnostic efficiency of V/Q SPECT alone was compared with that of V/Q SPECT combined with low dose CT scan.Crosstabsx2 test was performed using SPSS 13.0 software.Results Twenty-nine patients (28.43%,29/102) were finally diagnosed as PE.V/Q SPECT alone had a sensitivity of 93.10% (27/29),a specificity of 90.41% (66/73),and an accuracy of 91.18% (93/102).With additional diagnostic information from low dose CT,the diagnostic specificity increased to 95.89% (70/73,X2 =1.72,P > 0.05 ),and the accuracy increased to 95.10% (97/102,x2 =1.23,P > 0.05) though the sensitivity remained the same.Conclusion Imaginginformation from low dose CT in hybrid SPECT/CT may enhance V/Q diagnostic accuracy for PE.

Objective To study the expression of Mage A1、A2、A3 in breast cancer tissues and four breast cancer cell lines Methods The expression of Mage A1、A2、A3 in breast cancer tissues and four breast cancer cell lines, MCF 7、Sk Br 3、MDA MB 435s and TM40D was detected by reverse transcription polymerase chain reaction (RT PCR) Results The positive expression rate of Mage A in breast cancer tissues was 13/33 (39%), of Mage A1 was 4/33 (12%), of Mage A2 was 8/33 (24%), and of Mage A3 was 7/33 (21%), respectively Both Mage A1 and Mage A3 were positive in breast cancer cell line MCF 7 and Sk Br 3 MDA MB 435s expressed Mage A2 and Mage A3 Mage A3 was positive in TM40D Conclusion Mage genes were often expressed in breast cancer, but expression of Mages varies in the breast cancer cell lines Mage genes encoding proteins are eligible for Mage peptide based active immunotherapy
9.Effect of House Indoor Air Pollution on Children Respiratory System in Lanzhou City

Objective To study the adverse effect of house indoor air pollution on children respiratory system health in Lanzhou city.Methods A retrospective questionnaire investigation on 1 700 children respiratory health was carried out in three kindergartens,three elementary schools involving lower grade pupils in three districts in Lanzhou city.At the same time,the information about the home environment of the children and family background was collected.Applying logistic regression model, the influencing factors were analyzed.Results The over-standard rates of indoor air quality were from 12.4% to 43.8% in Lanzhou.In the non-heating period,the over-standard rates of formaldehyde,benzene,SO_2,NO_2,CO and PM10 in indoor air were 22.6%,14.5%,13.3%,12.4%,0% and 23.3% respectively.In the heating period,the over-standard rates of the pollutants were 27.3%,16.4%,34.6%,28.5%,32.3% and 43.8% respectively.Indoor air pollution was statistically associated with children respiratory symptoms and diseases,the related risk factors were someone smoking in home,house decorated,opening windows, ventilation,heating in winter,sharing bedroom and maternal education.The outdoor air pollution was one of the influencing factors. Conclusion Children respiratory diseases and symptoms are related to many air pollution factors,such as outdoor and indoor air pollution,personal characteristic and family factors,so to prevent and control indoor air pollution,a comprehensive way should be taken in Lanzhou city.
10.Related Factors of Hyperhomocysteinemia among Non-hypertensive People Aged between 40-70 Years in Tianjin

Objective To investigate the concentration of homocysteine (Hcy) and related factors of hyperhomocyste-inemia (hHcy,Hcy≥15μmol/L) among non-hypertensive people aged between 40-70 in Tianjin. Methods Non-hyperten-sive community residents aged 40-70 years were enrolled randomly from May 2011 to December 2012 in six districts in Tian-jin. Plasma Hcy was assessed by enzyme cycling method. Factors related to hHcy were analyzed in multivariate logistic re-gression models. Results Our study included 874 participants (mean age is 57 ± 6 years, 25.5%of all are males) and the con-centration of Hcy was 12.0 μmol/L. The OR (odds ratio)(95%CI; P)for hHcy were 1.048(1.015,1.083; P=0.004), 4.191 (2.359,7.448;P<0.001), 1.280(0.896,1.829;P=0.175), 0.460(0.259,0.816;P=0.008)respectively for age, male, smoking, exercise, and the odd ratio for hHcy were 0.290(0.179, 0.469;P<0.001), 0.168(0.092,0.309;P<0.001)for consumption of vegetable and fruits 250-500 g/d and>500g/d, compared with<250 g/d. Conclusion Male and age were adverse factors for hHcy, consumption of vegetable and fruits, and exercise were positive factors.
