1.Free forearm flap and free iliac bone reconstruct the composite tissue defects of the floor of mouth and the lower part of face in the same period

ObjectiveTo probe into the clinical efficiency of reconstruction to person who had composite tissue defect of the floor of mouth and the lower part of face with the free forearm flap and free iliac bone in the same term.MethodsBefore the surgery, the surgeon should carefully evaluate the scope and shape of defects in 11 cases who had the composite tissue defects of the floor of mouth and the lower part of face, to design the individual forearm flap, so that it matches with the defect region. During surgery, the first resumption of defects using titanium plates forming the basic shape of mandible and the occlusal relationship,then used to re-sawing to take modeling of the iliac bone,transplanted free iliac bone and fixed after the inside of the titanium plate, thereby restoring the continuity of mandible missing.ResultsFree forearm flap and free iliac bone all survived in 11 cases, the success rate of 100%. The patients were followed up for 6 to 12 months, although the shape of restoration areas were different levels of fat, but eating, swallowing and other oral function had been well improved, 7/11 could enter normal diet, 4/11 to enter liquid diet, At the same time, the patients in social activities can communicate in a language daily, sensory function of skin flap and facial appearance has been satisfactory recovery.ConclusionConformal free forearm flap and free iliac bone is an ideal way to reconstruct the composite tissue defects of the floor of mouth and the lower part of face in the same period, not only safe, practical prognostic effect, and also can significantly improve the patients quality of life, which is worthy of reference and use.

Objective To understand the status of knowledge,attitude and behavior on malaria of residents in endemic areas of Yunnan Province.so as to provide evidences for implementation of Global Fund MaIaIia Control Program.Methods The subjects were selected by the method of multi-level sampling in 47 countries and were investigated by questionnaire.Results A toatl of 40 940 households in 883 natural villages were investigated.39 234 questionnaires were effective with an effective rate of 95.83%.A total of 60.32%interviewees knew malaria symptom and signs,22.41%of them knew the route of transmission,17.65% knew the preventive measures.57.61% had the willing to use insecticide-treated bed nets(ITNs),and 8.68%of the households owned ITNs.Conclusion Health edueion and promotion regarding critical malria interventions should be strengthened in the following implementation.

Objective To investigate the evaluation value of magnetic resonance spectroscopy (MRS) combined with S-100B protein in the severity and prognosis in patients with acute subdural hematoma ( ASDH).Methods Eighty cases of ASDH patients and 20 cases of healthy check-up were selected.MRS was used to test NAA/Cr, Cho/Cr, NAA/Cho, and Glx /Cr in thalamus and corpus callosum.The blood S-100B protein was detected in 72 h after injury.The relationships of those MRS detection indices with glasgow coma scale ( GCS) and glasgow prognostic score ( GOS) for 2 months after injury were analyzed .Results MRS detection in-dex and the S100B protein in ASDH were compared between each group relative to normal control group , all difference had statistical significance ( P <0.05).As aggravating the severity of traumatic brain injury , Cho/Cr, Glx/Cr ratio, and S-100B protein concentra-tion were elevated , and NAA/Cho and NAA/Cr were reduced .All differences were statistically significant among poor recovery , good recovery, and normal control groups ( P <0.05).For patients with traumatic brain injury, there were worse prognosis, the higher ra-tios of Cho/Cr and Glx/Cr, higher concentration of S-100B protein, and lower ratios of NAA/Cho and of NAA/Cr.GCS score and GOS scores were negatively correlated with Cho/Cr and Glx/Cr ratios of corpus callosum , and were positively correlated with NAA/Cho, NAA/Cr ratios of corpus callosum .S-100B protein was positively correlated with Cho/Cr and Glx/Cr, and was negatively correlated with NAA/Cho and NAA/Cr.MRS combined S-100B can improve the prognosis of patients with up to the accuracy of 81%.Conclu-sions MRS detection in the early stage after injury of ASDH patients has important value in assessment of the severity of the injury and its prognosis , the accuracy of assessment of prognosis is improved with a combination of MRS detection and blood S -100 B protein meas-urement.
8.A quantitative study on ultrasonographic texture analysis of injured porcine striped muscle induced by thermal and chemical factors

Objective To quantitatively analyze and compare the texture features of thermal and chemical lesions on the porcine striated muscle, in vitro extracted from high-frequency ultrasonograms using computer-assisted image analysis technique, and to investigate the application values. Methods The thermal lesion and chemical lesion were induced in vitro in porcine striated muscle by microwave ablation and anhydrous acetic acid injection, respectively. The two dimension (2D) ultrasonographic ifndings were qualitatively compared between the groups of thermal and chemical lesion models, in which eight textural features in geometric mathematics extracted from 2D ultrasonograms were quantitatively analyzed by a technique of computer-assisted image analysis named multiscale decomposition method of echo intensity of interface relfections. Results As expected, microwave ablation and anhydrous acetic acid caused signiifcant changes of several texture features extracted from ultrasonograms. There were significant differences between the normal group and microwave ablation group in grayscale mean (Mean), irregularity (IRGL) and periodicity of distribution (POD) as follows (Mean: 1.9143±0.2914 vs 1.2334±0.3357, t=-5.306, P=0.000; IRGL: 0.5577±0.0334 vs 0.5092±0.0459, t=-2.957, P=0.007; POD: 0.000 27±0.000 005 vs 0.000 29±0.000 008, t=4.782, P=0.000). There were signiifcant differences between the normal group and anhydrous acetic acid injection group in number of blobs (NOB), size of blobs (SOB) and periodicity of distribution (POD) as follows (NOB: 51.0324±13.6998 vs 31.6042±4.8315, t=4.633, P=0.000; SOB:16.4843±3.9349 vs 25.6230±2.3555, t=6.903, P=0.000;POD:0.000 26±0.000 015 vs 0.000 29±0.000 008, t=-4.459, P=0.000). For each group of injured regions, there were significant differences between the microwave ablation group and anhydrous acetic acid injection group in Mean, IRGL, NOB and SOB as follows (Mean: 1.2664±0.2688 vs 1.9143±0.2914, t=-5.661, P=0.000; IRGL: 0.5220±0.0422 vs 0.5577±0.0334, t=-2.295, P=0.032;NOB:51.0324±13.6998 vs 34.5856±2.6362, t=4.048, P=0.000;SOB:16.4843±3.9349 vs 25.3176±2.3501, t=-6.676, P=0.000). Conclusion Technique of computer-assisted image analysis named multiscale decomposition method of echo intensity of interface relfections, based on multiscale blob features extraction, was useful to differentiate ultrasonic texture features between the groups injured in our study, which established quantitative muscle ultrasound as a practical and reliable tool for the muscle injury diagnosis to distinguish the structural changes induced by different physiochemical factors.
9.Protective effect of nicotinamide in a mouse Parkinson's disease model.

OBJECTIVETo examine the protective effect of nicotinamide on 1-methyl-4-phenyl-1, 2, 3, 6-tetrahydropyridine (MPTP)-induced Parkinson's disease (PD) in mouse model and its mechanisms.
METHODSParkinson's disease was induced by injection of MPTP in adult male C57BL/6 mice, nicotinamide (500 mg/kg,i.p.) was given prior to subacute (30 mg/kg/d × 5 d,i.p.) MPTP administration. Locomotor activities, striatal dopamine levels, lactate dehydrogenase (LDH) and NO synthase (NOS) activities of whole brains and striatum were analyzed at d5 after last MPTP injections.
RESULTSPretreatment with nicotinamide significantly improved the locomotor activity in the open-field test (P<0.01), but not in the swimming test and grip & climbing test. Nicotinamide administration resulted in sparing striatal dopamine levels from MPTP-induced dopamine depletion. There was no significant difference in LDH and NOS activities in the whole brains among the groups; but the activities in the striatum were drastically elevated after MPTP treatment. Nicotinamide pretreatment markedly inhibited MPTP-induced LDH and NOS activities (P<0.01) and showed no significant difference compared to controls (P>0.05).
CONCLUSIONNicotinamide protects dopaminergic neurons against MPTP-induced neurodegeneration,which suggests that the neuroprotective effects be associated with the inhibition of cell injuries and NOS activities.

10.Relationship of the A1166C polymorphism of ATI R gene with TCM syndrome and efficacy of Chinese hypotensor in patients with essential hypertension.

OBJECTIVETo investigate the polymorphic distribution of the A1166C, the angiotensin II type I receptor (AT1R) gene, in the Chinese people with essential hypertension (EH), and to study the relationship of A1166C with the TCM syndrome types and the efficacy of hypotensor.
METHODSBlood pressure (BP), body weight index (BWI), fasting blood glucose (FBG), cholesterol (Ch), concentration of triglyceride, levels of serum plasma Ang II , endothelin (ET) and calcitonin-gene related peptide (CGRP) in 206 patients with EH and 86 health subjects of Chinese nationality were determined. DNA of peripheral white blood cells were extracted by salting out method and the polymorphism of AT1R gene A1166C was assessed by PCR and limited enzymatic segmentation. The 206 EH patients were differentiated into different types according to TCM syndrome differentiation, and the gene distribution appeared in each TCM syndromes was compared. Qingxin Capsule (QC) or captopril was used respectively to treat 34 and 32 patients with hypertension of Yin-deficiency and excessive Yang type to observe the relationship between the hypotensive effect of treatment and the distribution of gene types.
RESULTS(1) No significant difference was found in BWI, FBG, blood lipid or serum related hormones, only the difference in systolic and diastolic pressure was shown between the hypertension group and the control group. (2) The rate of AT1R AC plus CC gene type in EH patients was 0.126, which was significantly higher than that in the normal control group (0.047, P < 0.01). The frequency of C allele distribution was obviously higher than that in the normal control group (0.068 vs 0.023, P<0.05). No significant difference was found in AT1R gene types or frequency of allele distribution between different sexes (male and female). (3) Analysis on relationship between gene types and clinical phenotype showed that no significant difference was found in systolic pressure, diastolic pressure, blood glucose, blood lipid, plasma Ang II, ET, or CGRP between patients with AA gene type and with AA plus CC gene type by t test (P > 0.05). (4) Analysis on relationship between gene type and TCM syndrome type showed that the distribution of AA gene type and AA plus CC type was insignificantly different in patients with various TCM syndrome types. (5) Analysis on relationship between gene type and hypotensive effect of treatments showed that both QC and captopril were effective to patients with hypertension of different gene types. The hypotensive effect of either was similar in spite of what gene types the patients with.
CONCLUSIONA1166C gene type is related to EH in the Chinese, C allele might a susceptible gene to hypertension, while TCM syndrome type of EH patients might not be related to whether C allele was borne. The hypotensive effect of QC or captopril was not related to whether C1166 allele was borne. No significant difference of blood pressure, blood glucose, blood lipid, plasma Ang II, ET, or CGRP in patients with different gene types, which suggested the effect of gene types on hypertension might not be by path of glucose or lipid metabolism, ET, CGRP or plasma Ang II system.
