1.Clinical comparative study on conventional surgical and subfascial endoscopic perforator surgery in great venous varicosity

Objective To compare the curative effect of high ligation+exfoliation and subfascial endoscopic perforator surgery(SEPS)for superficial varicose veins in calf+invagination spot-striping surgery in great venous varicosity.Methods Study group(42 patients)accepted SEPS+invagination spot-striping surgery and control group (42 patients)accepted traditional surgeries.Operation duration,bleeding volume in operation,the time of beginning movement away from bed after operation,hospitalization duration,the degree of pain,the scar,the recrudescence af- ter operation and the instance of the ulcer heals of two groups were compared.Results Operation duration,bleeding volume in operation,the time of begin movement away from bed after operation and hospitalization durations of study group were significantly lower than those of control group(P0.05).All of the patients in study group recovered without severe syndromes such as venous thrombosis,skin necrosis,lower limb functional disorder etc.They had no recrudesce after 4~16 months and were satisfied with the curative effect.Con- elusions The clinical curative effect of SEPS+invagination spot-striping surgery in great venous varicosity is superi- or to that of traditional operation and it has the advantages such as minor wound,few scars,light pains,short hospi- talization duration,without recrudescence,the ulcer heals quickly and so on.
2.Effect of Rosmarinic Acid on Antioxidative Systems in Mesangial Proliferative Glomerulonephritis in Rats

Objective To investigate the change of oxidation system and antioxidation system in mesangial proliferative glomerulonephritis(MsPGN) induced by anti-Thy1.1 antibody,and further to study the intervention of rosmarinic acid(RAD).Methods Anti-THy1.1 serum was produced,and then intravenously injected into rats for establishing an experimental model of MsPGN.The experiment was designed for control with or without RAD,glomerulonephritis with or without RAD,respectively.The activity of superoxide dismutase(SOD) and the content of malondialdehyde(MDA) in tissue homogenate were detected by spectrophotomerty.Results The activity of SOD significantly decreased,while the content of MDA increased in MsPGN.RAD could inhibit oxidation in the mesangial cells.Conclusion Lipid peroxidation participates in MsPGN and RAD can control the changes of the mesangial cells and show the activity of antioxidation.
3.Clinical study on treatment of acute eczema by Shuangfujin.

OBJECTIVETo evaluate the effects and safety of Shuangfujin (SFJ) on acute eczema.
METHODSOne hundred and twenty patients with acute eczema were randomly assigned to 4 groups equally, the saline group, the boric acid group, the Pifukang lotion group and the SFJ group, treated with respective medicine.
RESULTSAfter 4 days of treatment, the symptom score was remarkably lower in the SFJ group than in the other three groups (P<0.01), score in the saline group was higher than that in the boric acid group and the Pifukang lotion group, and insignificant difference was shown between the latter two groups. The total effective rate and cured rate were 46.4% and 14.3% in the SFJ group, which were matched to those in the Pifukang lotion group and significantly higher than those in the other two groups (P <0.01), and the saline group showed the lowest efficacy. The effect of stopping itch in the SFJ and the boric acid group were matched, which was higher than that in the Pifukang lotion group, and the lowest was in the saline group. No skin irritant reaction and other adverse reactions were found.
CONCLUSIONSFJ is effective, safe and convenient in treating acute eczema.

4.Construction of platform on the three-dimensional finite element model of the dentulous mandibular body of a normal person.

OBJECTIVETo develop a method to construct a three-dimensional finite element model of the dentulous mandibular body of a normal person.
METHODSA series of pictures with the interval of 0.1 mm were taken by CT scanning. After extracting the coordinates of key points of some pictures by the procedure, we used a C program to process the useful data, and constructed a platform of the three-dimensional finite element model of the dentulous mandibular body with the Ansys software for finite element analysis.
RESULTSThe experimental results showed that the platform of the three-dimensional finite element model of the dentulous mandibular body was more accurate and applicable. The exact three-dimensional shape of model was well constructed, and each part of this model, such as one single tooth, can be deleted, which can be used to emulate various tooth-loss clinical cases.
CONCLUSIONThe three-dimensional finite element model is constructed with life-like shapes of dental cusps. Each part of this model can be easily removed. In conclusion, this experiment provides a good platform of biomechanical analysis on various tooth-loss clinical cases.

5.Effect of heavy metals exposure on neurobehavioral function in welders.

OBJECTIVETo explore the relationship between heavy metals exposure and neurobehavioral function impairment in welders.
METHODSThe metals exposure in 82 welders and 51 operators were investigated with blood Pb, Cd and Mn via AAS, and the nervous impairment was evaluated with neurobehavioral core test battery (NCTB).
RESULTSPb [(115.49 +/- 79.22) microg/L] and Cd [(3.67 +/- 3.19) microg/L] in welders were significantly higher than operators [(69.32 +/- 50.79) and (0.83 +/- 0.76) microg/L respectively] (P < 0.05). Welders had worse standard scores of NCTB 13 items such as depression-dejection than non-welders (P < 0.05). Significant difference of confusion-bewilderment and forward digit span in welders only existed in different groups of Pb and Mn, respectively. A dose-effect relationship was found between forward digit span and serum Mn level in welders. General linear regression analysis indicated that Pb exposure, Mn exposure and alcohol consume had negative relation with the loss of nervous system function.
CONCLUSIONThe nervous impairment in welders is attributed to occupational exposure to Pb and Mn, concomitantly.

6.Testicular mixed nonseminomatous germ cell cancer: a case report and review of the literature.

OBJECTIVETesticular mixed nonseminomatous germ cell cancer (TMNGCC) is rarely reported. This study aimed to explore the clinical symptoms, pathological characteristics and treatment methods of TMNGCC.
METHODSWe analyzed the clinical data of 1 case of TMNGCC, observed its pathological characteristics under the light microscope by histology, cytochemistry, immunohistochemistry and immune marking, and investigated the clinical features of such tumors by reviewing the relevant literature.
RESULTSThe patient presented with a chief complaint of painless testicular swelling for 3 years. Histopathological examinations revealed a tumor of papillary, fissural or adenoid structure, with large polygonal or columnar cells with one or more irregular vesicular nuclei, the nuclear membrane clear, the cytoplasm eosinophilic or basophilic, and the interstitium infiltrated by a few lymphocytes. Here are the immunohistochemical results: CD117 -, CK8-18 + +, CD30 + +, CK + + +, vimentin -, PLAP +/-, P53 +, AFP + and EMA + +. The tumor was pathologically diagnosed as teratogenic embryonic testicular cancer, and treated by radical surgery, followed by adjuvant chemotherapy according to the treatment of TMNGCC. One-year follow-up found the patient to be alive.
CONCLUSIONTMNGCC is a rare malignant tumor, mostly with unobvious clinical symptoms. Its diagnosis primarily depends on physical examination, ultrasonography, CT, and measurement of serum tumor markers; its confirmation necessitates pathological examination, and its first-choice treatment is surgical resection.

7.Analysis of the pelvic stability after type I resection of iliac tumor.

OBJECTIVETo analyze the pelvic stability after type I resection of iliac tumor.
METHODSSix adult cadaveric specimens were tested. The iliac subtotal resection models were established according to Ennecking's type I resection. Markers were affixed to the key region of the pelves. Axial loading from the proximal lumbar was applied by MTS load cell in the gradient of 0-500 N in the double feet standing state. Images in front view were obtained using CCD camera. Based on Image J software, displacements of the first sacral vertebrae (S1) of the resected pelves and the intact pelves were calculated using digital marker tracing method with center-of-mass algorithm.
RESULTSSerious instabilities were found in the resected pelves. S1 rotational movements around the normal side femoral head of the resected pelvis were found. The average vertical displacement of S1 of the resected pelvis was (7 +/- 3) mm under vertical load of 500 newtons, which were 8.3 times compared to the intact pelvis. The average angle of S1 rotation around the normal side femoral head of the resected pelvis was (4.0 +/- 1.8) degrees, which were 12.5 times compared to the intact pelvis.
CONCLUSIONSBiomechanical model of type I resection of iliac tumor are established. Essential pelvic reconstruction must be introduced because of the serious instability of the bone defection after tumor resection.

8.Clinical investigation on treatment of children leukemia with non-T-cell depleted haploidentical bone marrow transplantation.

To investigate the efficacy and feasibility of parent non-T cell depleted haploidentical bone marrow transplants (haploidentical BMT) for children with leukemia, the efficacy of haploidentical BMT was evaluated in 8 leukemia children (1.9-9 years) received hematopoietic stem cell transplantation, donors were their parents with HLA-mismatched for two or three loci. Five children were pre-conditioned with a myeloablative regimen consisting of high-dose cytarabine (Ara-C), cyclophosphamide (CY) and total body irradiation. Busulfan (BU), Ara-C and CY were used for preconditioning regimen in other three children. The donors were given G-CSF prior to marrow harvest and the non-T-cell depleted grafts were used. A combination of CsA, MTX, ATG, mycophenolate mofetil (MMF) and CD25 monoclonal antibody were used for GVHD prophylaxis. The results showed that rapid engraftment was observed in all cases after transplantation by cytogenetic evidence. The mean time of neutrophil count exceeded 0.5 x 10(9)/L and the mean time of platelet count exceeded 20 x 10(9)/L were 16 and 17 days after transplantation respectively. Incidence of lethal aGVHD was lower, II-III acute aGVHD was found only in one out of eight patients. Chronic GVHD was observed in five patients, 4 from which showed local cGVHD, one developed extensive cGVHD. During the follow-up of 33 months (range 7-56 months), two patients died from relapsed leukemia, including one relapsed as donor-origin leukemia. Disease-free survival was achieved in the remaining six patients. No death occurred during the follow-up of 6 months. It is concluded that above-mentioned preconditioning regimen and GVHD prophylaxis procedure in non-T-cell depleted bone marrow transplantation from HLA-mismatched parents are effective approaches and safe strategy for the treatment of children leukemia.

9.A finite element analysis of the pelvic reconstruction using fibular transplantation fixed with four different rod-screw systems after type I resection.

BACKGROUNDThe pelvis often needs to be reconstructed after bone tumor resection. A major challenge here for the orthopedic surgeons is to choose a method that gives the best performance which depends upon its biomechanical properties. In this study, a 3-dimensional finite element analysis (FEA) was used to analyze the biomechanical properties of reconstructed pelvis using fibula transplant fixed by four commonly used rod-screw systems.
METHODSA total pelvic finite-element model including the lumbar-sacral spine and proximal femur was constructed based on the geometry of CT image from a healthy volunteer. Three-dimensional finite element models of different implants including fibula, rod and screw were simulated using ways of solid modeling. Then various reconstructed finite element models were assembled with different finite element implant model and type I resected pelvic finite element model. The load of 500 N was imposed vertically onto the superior surface of L3 vertebral body, and the pelvis was fixed in bilateral leg standing positions. FEA was performed to account for the stress distribution on the bones and implants. The pelvis displacement of the different rod-screw fixation methods and the maximum equivalent stress (max EQV) on all nodes and element were figured out to evaluate the advantages and disadvantages of different reconstructive methods.
RESULTSStress concentration in the fibula transplant was extremely high in the reconstructed pelvis, but could be substantially decreased by internal fixation, which partially transferred the stress from the fibula to the rod-screw systems. High stress concentration was also found in the implants, especially in the connection sites between screw and rod. Among the four methods of fixation, a double rod system with L5-S1 pedicle and ilium screws (L5-S1 HR) produced the best performance: least stress concentrations and least total displacement.
CONCLUSIONAccording to the stability and stress concentration, the method of L5-S1 HR fixation combined with fibula transplantation is better than other fixation methods in pelvic reconstruction after type I resection.

10.Applications of magnetic resonance imaging in diagnosis of the complications after breast augmentation with polyacrylamide hydrogel injection.

OBJECTIVETo explore the clinical value of magnetic resonance (MR) imaging in the diagnosis of the complications after breast augmentation with polyacrylamide hydrogel injection.
METHODSMRI was performed in 16 patients who had breast augmentation with polyacrylamide hydrogel injection. The results of magnetic resonance imaging were compared with the clinical symptoms.
RESULTSMRI examinations of the 16 patients showed that (1) 4 breasts in 2 patients were comparatively natural; (2) a large lacteal cyst was detected in one patients in lactation; (3) breast asymmetry occurred in 3 patients (6 sides); (4) in 10 patients (20 sides), the injected gel was in irregular form. The results of MRI were in accord with the clinical symptoms.
CONCLUSIONMRI is of great diagnostic value for the complications after breast augmentation with polyacrylamide hydrogel injection.
