ObjectiveTo explore the diagnostic methods and surgery pattern at the first time of spontaneous perforation of congenital choledochal cyst.MethodsEleven cases(4 male,7 female) with spontaneous perforation of congenital choledochal cyst were 6 months to 5 years old,and their average course of disease were 4 days.Gustily abdominal distension, abdominal pain, crying and fever were present in all cases.Jaundice(7 cases) and emesis(5 cases) appeared. All cases were detected with physical sign of peritonitis by physical examination.Choledochal cysts were confirmed by CT or B ultrasound in 8 cases. All cases accepted abdominal paracentesis and biliary ascites was drawn.Three different operative procedures were performed:choledochocyst excision & Roux-Y choledocho-jejunostomy(2 cases),choledochotomy with T-tube drainage(3 cases),and cholecystostomy(6 cases).Nine children receiving external drainage operation accepted a second operation to rebuild biliary tract(choledochocyst excision & Roux-Y choledochoje-junostomy) after 3 to 6 months.ResultsAll cases had got satisfactory therapeutic efficacy without any grave complication such as fistula of anastomotic stoma, infection of biliary tract or obstruction of biliary tract.During operation, perforations were located in the juncture of choledochus and cystic duct in 5 children and were not found in the other 6 children. In the second operation, the cases receiving cholecystostomy had less peritoneal adhesion, anatomic structure changes,haemorrhage[(30-50) mL vs(100-200) mL] and operation time[(2.5-3.0) h vs(3.5-5.0) h] than those receiving choledochotomy with T-tube drainage, and did not appear inadequate drainage for cystic duct obstruction.ConclusionsFor children with more organ inflammatory edema and adherence and in a bad overall condition, the first-time operation of cholecystostomy is more reasonable.

BACKGROUND: Ascariasis is one of the most common human parasitic infections worldwide. In some rare cases, ascariasis may cause serious consequences even sudden death. This study was undertaken to review the life-threatening complications of ascariasis in trauma patients reported in the literature. DATA SOURCES: Relevant articles about ascariasis and trauma were searched from Pubmed, Google scholar, Scirus, and Wanfang databases. RESULTS: Twenty-four patients with ascariasis were collected from 21 articles searched. Most of these patients were from tropical and subtropical countries. Of the 24 patients, 12 were children. Their major complications occurred in the airway passage and digestive tract. There were 3 fatal cases in these patients. Twelve of the 24 patients described in 10 articles were reported in the last 10 years. CONCLUSIONS: Early diagnosis and prompt intervention are essential to minimize the high morbidity and mortality of these serious complications in trauma patients. Physicians should be aware of the possibility of Ascaris infection in a trauma patient from endemic area of ascariasis. History of Ascaris infection and routine examination of feces forAscaris eggs may be helpful to make a correct diagnosis.

Phenazine-1-carboxylic acid(PCA)is one of the major antifungal compounds produced by plant growth-promoting rhizosphere(PGPR)pseudomonads and has been shown to contribute to the biological control of soil-borne plant pathogens.Model on secondary metabolite phenazine-1-carboxylic acid(PCA)fermentation nutrition conditions of gacA inactivate mutant Pseudomonas sp.M18G was constructed by Plackett-Burman design(PB)and Response Surface Method(RSM).In PB design four key components selected from 12 different factors have shown to play an important role for promoting PCA production.Center Composite Design(CCD)was adopted to establish a fermentation model for the four nutrition components using RSM.The optimal concentration of the four components based on analysis of regression equation were determined that soybean meal 33.4 g/L,glucose 12.7 g/L,soy peptone10.9 g/L,ethanol13.8 g/L,and the highest PCA production could reached 1.89 g/L after 60h fermentation and the yield increased to 6 fold over that before optimization. The contour graphs depicted interactions of the two nutrition components showed that soybean meal and ethanol played an even more crucial role for the highest production of PCA in fermentation. It establishes a method for high PCA production and lays a foundation for the further PCA commercial development.
