1.The effect of embolization of gastric coronary and short gastric veins on the control of hemorrhage from (gastric) varices

Objective To study the effect of embolization of gastric coronary and short veins on the control of hemorrhage from gastric fundal varices. Methods Thirty-two patients with liver cirrhosis and hemorrhage from esophagus and fundal varices, who still had variceal bleeding after endoscopic banding or sclerosing of esophageal varices were included in the study. Gastric coronary and short veins were (embolized) by percutaneous, transhepatic embolization with absolute alcohol, steel ring, or gelfoam. The follow-up of 3 to 11 months was carried out after embolization. Results The follow-up endoscopy showed that the varices of gastric fundus disappeared completely in 21 of 29 patients (72.4%), and (eleminated) partially in 8 patients (27.6%). Only one patient (3.1%) had re-bleeding due to portal (hypertensive) gastropathy in the follow-up period. No obvious complication was found in these patients. Conclusions The varices of gastric fundus could be obliterated by percutaneous, transhepatic embolization of gastric coronary and short veins.

[Objective] To explore the therapeutic mechanism of modified Zhuling Decoction (MZD) in treating primary mesangial proliferative glomerulonephritis (PMPG). 3.Combined anterior and posterior approaches to treat severely unstable lower lumbar burst fractures

Objective To evaluate the efficacy of combined anterior and posterior approaches in treatment of severely unstable lower lumbar burst fractures.Methods A retrospective case series study was made on clinical data of 14 patients with lower lumbar burst fractures collected from August 2009 to August 2014.There were 12 males and 2 females,with a mean age of 39 years.Seven fractures occurred at L3,five at L4,and two at L5.Nine patients were associated with injury to the posterior ligament complex (PLC).According to the American Spinal Injury Association (ASIA) classification,the spinal injuries were rated as grade B in two patients,grade C in four,grade D in five and grade E in three.All patients underwent posterior pedicle screw fixation combined with anterior spinal canal decompression and titanium mesh or iliac bone grafting.Lumbar lordosis angle,vertebral height,spinal canal decompression,ASIA grade and complications were evaluated after operation.Results Two patients experienced cerebrospinal fluid leakage postoperatively,which were healed after 2 weeks' local pressure treatment.Three patients experienced recurrent lumbar pain postoperatively,which were relieved after the removal of internal fixation 18 months after operation.All patients were followed up for 12-36 months (mean,18 months).Compared to the detection before operation,final follow-up showed improved lumbar lordosis [(30.2 ± 7.3) ° vs.(41.3 ± 6.5) °],decreased loss of the anterior vertebral height [(62.3 ± 21.5) % vs.(11.8 ± 7.8) %] and reduced canal compromise [(65.7 ± 30.5) % vs.(21.9 ± 12.7)%] (all P < 0.05).ASIA grade was significantly improved at the final follow-up,including grade C in one patient,grade D in three and grade E in ten (P < 0.05).Follow-up showed no apparent graft loosening,pseudarthrosis,implant breakage and severe kyphosis.Conclusion Combined anterior and posterior approaches to treat severely unstable lower lumbar burst fractures can effectively reconstruct the height and stability of the vertebral body,restore the spinal canal volume,and attain satisfactory clinical outcome.
4.Treatment of fresh subtrochanteric fracture combined with old femoral neck fracture with hemiarthoplasty through anterolateral approach.

OBJECTIVETo explore the clinical effects of one-stage hemiarthroplasty in treating fresh subtrochanteric fracture combined with old femoral neck fracture.
METHODSFrom October 2003 to October 2013, 7 patients with fresh subtrochanteric fracture and old femoral neck fracture were treated with hemiarthroplasty in one stage. There were 5 males and 2 females, aged from 69 to 80 years old with an average of 74.5 years. According to the Garden classification, 2 cases were type IV and 5 cases were type V. According to the A0 classification of subtrochanteric fracture, all cases were type A1 (long oblique femoral subtrochanteric fractures). Biological coating long handle femoral prosthesis was used in the patients. The hip joint functions were assessed according to Charnley scoring system after all hemiarthroplasty.
RESULTSSeven patients were followed up from 12 to 24 months with an average of 18 months. There were 1 case with urinary system infection and 1 case with deep venous thrombosis. Of all, 5 cases recovered the walk ability at 6 months after operation and 2 cases could walk with the help of the crutch at 1 year after operation. According to Charnley scoring system to assess the hip joint function, the total Charnley scores reached 15.0±2.5, and 4 cases got an excellent results, 2 good, 1 fair.
CONCLUSIONArthroplasty through anterolateral approach can obtain satisfactory clinical effects in treating fresh subtrochanteric fracture combined with old femoral neck fracture, it has advantages of good joint stability, little pain, less complication, quickly recover, early activities on fields.

7.Preliminary observation on the interference effect of Helicobacter hepaticus infection on the immune response in BALB/c mice

Objective To determine the interference effect of H. hepaticus infection on the functional characteris-tics of dendritic cell ( DC) surface molecules and immune response in mice. Methods Male BALB/c mice were inocula-ted with H. hepaticus (ATCC 51450). Murine bone marrow-derived dendritic cells (DC) were isolated and co-cultured which were stimulated by GM-CSF and IL-4 at the fifth month after the last inoculation. Then the DCs were subjected to FACS analysis for surface markers (CD11c, CD40, CD80 and MHCII) detection. On this basis, virus suspension of New-castle disease virus( NDV) ZJ1 strain was inoculated into the mice. Serum was collected for detection of the NDV antibody titer in serum weekly to explore the difference of antibody titer between the two groups. Results The expression rates of CD40 and MHCII on the mouse DCs in experimental group were higher than that in the control group. The NDV antibody ti-ter of experimental group was slightly lower than that in the control group in the first week. During the 2nd to 5th weeks, the titer was higher than that in the control group, with a very significant difference. In the 6th week, the titer of both the two groups tended to fall. Conclusions H. hepaticus infection can promote bone marrow DC maturation in mice, stimulate the expression rates of MHC II and CD40, and enhance the NDV antibody levels.
8.The study of near and long term changes of bone cement and cancellous bone after percutaneous vertebroplasty

Objective To observe the research of the biologically performance,interface micro-structure and nano-indentation situation when polymethyl methacrylate bone cement-PMMA injected into rabbit in the near future and far future individually.Methods New Zealand white rabbits were used ovariectomy plus dcxamethasone intramuscular injection to establish osteoporosis animal model.All animal models were divided into 3 groups with 35 in each osteoporosis group,testing group,and normal group.Execute the lumbar centrum of New Zealand rabbit as osteoporosis bone defect model,injected bone cement,with the help of material dynamo experiment machine to do the axial compression tests,three point bending test,and torsion test in order to observe the biological dynamic changes.Optical microscope,fluorescence microscope and scanning electron microscope were used to observe the changes of micro bone cement and the changes of bone looseness.Nano-indentation testing was applied by the triboindenter to study the critical load.Results PMMA can rapidly establish the strong support with stable function in the near future.Biomechanical experiments showed that biomechanical properties of bone cement group was significantly higher than those in the other two groups.Biomechanical properties of bone cement group may decline with the time,but which were still better than those of OP in the control group.Histo-morphological observation result shows that under osteoporosis state the bone grows slower,bone's rebuilding time also extended.And in the later period,main bone's continuous osteoporosis had some impact on the interface.Nano-indentation testing showed that the young modulus and stiffness of the interface among bone,material and interface were significantly differences.Bone cement had showen the best nano indentation hardness,then was interface and bone tissue.Conclusion The polymethyl methacrylate bone cement llⅢ has a good effect in filling,on account of it has great plasticity and liquidity and can penetrate into the minuteness aperture between trabecula,then after solidifying,it will combine firmly with the host bone.PMMA can rapidly establish the strong support with stable function after operation.The performance is decreasing in the far future but still with satisfaction.
