1.Observation of effect of comprehensive rehabilitation on elder hypertension

ObjectiveTo observe the effect of comprehensive rehabilitation on elder hypertension.Methods136 old patients with hypertension were divided randomly into the simple recuperating group (control group) and comprehensive rehabilitation group (observing group) with 68 cases in each group. Patients in simple recuperating group were treated with routine medicine and meal therapy. Cases in comprehensive rehabilitation group were added with health education, recuperating with natural factors, kinesiatrics, psychology and view treatment, etc. The blood pressure, symptoms and dosage of medicine of patients of two groups were compared.ResultsAfter two months treatment, blood pressures of all patients of two groups were dwon and symptoms improved, but that of comprehensive rehabilitation group were more obvious (P<0.01 and P<0.05). While the number of persons ceasing or reducing dosage of medicine in comprehensive rehabilitation group was greater than that of in simple recuperating group (P<0.01).ConclusionThe comprehensive rehabilitation is effect on elder hypertension.

4.Clinical and biomedical features, on adult patients infected Dengue virus at Hue Central Hospital

99 patients over 15 year old were treated at Hue central Hospital from July 2002 to May 2003. 45.5% was in male and 54.5% in female. The most prevalence of age group was from 15 to 24 years old (72.7%). The symptom of abdominal pain in Dengue haemorrhagic fever was 61.8% and 46.2% in Dengue fever, alvine flux in Dengue haemorrhagic fever was 58.8% and 47.7% in Dengue fever. Liver intumescence in Dengue haemorrhagic fever was 29.4% and 7.7% in Dengue fever, pain in epigastric region in Dengue haemorrhagic fever was 32.4% and 9.2% in Dengue fever. The signs of headache, retroorbital pain, muscles and joints, supracondylar intumescence and along to sternocleidomastoid muscle had equal prevalence in Dengue haemorrhagic fever and Dengue fever. Based on Dengue haemorrhagic fever degree of WHO, most of cases were in degree II (82.4%), degree I (11.8%) and degree III (5.8%). All 99 cases, who were diagnosed with Dengue haemorrhagic fever (100%) by basic systems, only 34.3% of cases with Dengue haemorrhagic fever were re-diagnosed by hospitals, the rest 65.7% cases were Dengue fever

5.Study on overcoming the conditions of anorexia, flatulence ans dyspepsy on acute viral hepatitis patients

At Hue Central Hospital, Transmitted disease department, 41 viral hepatitis with anorexia, flatulence and dyspepsia were divided into 2 groups: the control group using fortex, polyvitamin and the intervened group using pantyrase in addition. The study showed that acute hepatitis occurred more in male than in female patients, the most age at 16-45 years old (61%). The group using pantyrase got improvement on the symptoms of flatulence, dyspepsia, promoted the digestion and the appetition. The anorexia interrupted after 7.18 1.91 days in pantyrase group and 16.84 4.91 days in the control group

6.Tick fever with complication of encephalitis meningtis

2 patients with tick fever had had an encephalo-meningitis complication, a rare condition with clinical symptoms similar with other type of encephalo-meningitis. In the diagnosis of encephalo-meningitis fever caused by Ricketsia, it must pay attention to the signs of conjunctive congestion, erythrema, especially the ulcerous lesions with high diagnostic value. The change in cerebro spinal fluid is not usually specific as meningitis caused by pyogenes bacteria. These are an increase of protein level, light decrease of glucose level and a minime increased of cell count. The early detection and specific treatment reduce the severity of the condition and cerebral symptoms can be reduced slower than the fever

7.Study on predictive factors for chronic HbsAg among acute hepatitis B patients

51 patients with acute hepatitis B were studied at Infectious and Tropical Diseases Department, Hue Central Hospital with criteria of clinical features, transaminase 5 times higher than normal upper limit and positive HbsAg at hospitalized time. 62% of them admitted hospital from 1 to 14 days after developed disease, reminder admitted hospital later than 15thday. Symptoms: 45,1% patients no fever; 88,2% oligouria and dark urine; 58,8% without swell of liver; 9,8% bleeding; 9,8% faint. Test results: there were 11% of patients with prothrombin <60%, 82,4% with bilirubin <340µg/l. 62,7% with De Ritis<1, 35,3% with HbeAg(+) and 33,3% with HbsAg (+) remained for 6 months. HbsAg positive remained for 6 months is associated with the group of patients admitted hospital later than 15thday (X2=5.1, P<0,05), the group with no fever (X2=10.1, P<0,05), the group with oligouria and dark urine (X2=13.6, P<0,05) and the group with jaundice (X2=5.1, P<0,05). The associations also existed between bilirubin level <340µg/l and the remained HbsAg (+) for 6 months and HbsAg (+) at the patients’ admission and the remained HbsAg (+) for 6 months (X2=5.5, P<0,05 and X2=31.3, P<0,05, respectively). The latter is strongly positive association.

8.Research progress in autophagy on dry age-related macular degeneration

Age-related macular degeneration (AMD) is a common cause of vision loss and living quality impairment in elderly people.Dry AMD is considered to be a neurodegenerative disease, and there has been no preventive method or effective therapy for it so far.Recent studies reveal that accumulation of lipofuscin may lead to dysfunction and even death of retinal pigment epithelium (RPE) cells.Autophagy is a major lysosome-dependent degradation pathway in eukaryotes involved in the disposal of damaged cytoplasmic proteins and organelles.Autophagy is revealed to be involved in the pathological processes of several neurodegenerative diseases such as Alzheimer's disease and dry AMD.Therefore,studies focus on autophagy may provide a new target for the prevention and treatment of dry AMD.This paper reviewed the research progress of autophagy in the pathogenesis of AMD in recent years.The roles of autophagy,lysosomal damage,oxidative stress and immune inflammatory reaction were described.
9.The clinical effect observation of the non invasive positive pressure mechanical ventilation for the treatment of chronic obstructive pulmonary disease with Ⅱrespiratory failure

Objective To explore the noninvasive positive pressure ventilation combined with pulmonary function and blood gas analysis of chronic obstructive pulmonary disease(COPD)with type Ⅱ respiratory failure. Methods 120 COPD patients with type II respiratory failure were selected,they were randomly divided into the con-trol group and observation group by using the random number table method,with 60 cases each group.The control group was given conventional low flow oxygen therapy,the observation group was application of noninvasive positive pressure ventilation treatment.The change of blood gas levels,the forced expiratory volume in one second (FEV1 ), forced expiratory volume in one second to forced vital capacity ratio (FEV1 /FVC)before treatment and after one week treatment,and the intubation rate,the average hospitalization time were observed.Results After treatment,FEV1 , FEV1 /FVC (%),PaO2 ,pH of the two groups were higher than those before treatment,while PaCO2 was lower than that before treatment,the differences were statistically significant (t =8.33,12.56,14.71,10.82,7.88,10.14, 13.20,15.63,11.48,8.37,all P <0.05).FEV1 and FEV1 /FVC (%)of the observation group after treatment were (2.84 ±0.42)L and (69.93 ±0.72),which were higher than those of the control group after the treatment (2.69 ± 0.39)L and (67.11 ±0.78),the difference was statistically significant (t =8.95,10.76,all P <0.05).PaO2 and pH of the observation group after treatment were (77.90 ±1.12)mmHg and (7.41 ±0.10),which were higher than those of the control group after treatment (70.88 ±1.14)mmHg and (7.29 ±0.08),the difference was statistically significant (t =11.58,7.94,all P <0.05).PaCO2 of the observation group was (50.14 ±1.23)mmHg,which was lower than that of before treatment (54.26 ±1.12)mmHg,the difference was statistically significant (t =9.81,P <0.05).The intubation rate of the observation group was 1.67% (1 /60),which was lower than that of the control group 13.33% (8 /60),the difference was statistically significant (χ2 =7.90,P <0.05).The average hospital stay time of the observation group was (11.69 ±1.32)d,which was lower than that of the control group(15.10 ±1.28)d, the difference was statistically significant (t =9.84,P <0.05).Conclusion Noninvasive positive pressure ventila-tion could significantly improve lung function of COPD patients with type II respiratory failure,increase PaO2 ,improve the respiratory status of the patients,reduce intubation rate and shorten the hospitalization time.
10.CT findings of testicular torsion

Objective:To study CT findings of testicular torsion and improve the recognition of the disease.Methods:The clini-cal and CT findings in 13 cases with testicular torsion proved by operation were analyzed retrospectively.All patients underwent low dose precontrast and postcontrast helical CT.Results:All patients of the disease had feature of urgent onset and short course of disease(two hours~3 days).Two patients of these cases had patient history of injury.CT findings:precontrast CT found the volume of sick testes increased and their densities increased slightly and nonuniform.Postcontrast CT found their features of nonuniform enhancement and gyrate enhancement.Conclusion:CT manifestations of testicular torsion have some specific features,which can help to confirm the diagnosis by referring the patient’s history.
