1.Recent progresses in neurosurgery

Objective To investigate the present status and developments of Chinese military neurosurgery, make an objective analysis about the achievements and deficiencies, and propose a developing trend for the field of Chinese military neurosurgery. Methods The scientific literatures of neurosurgery published domestically and abroad in recent 5 years were partially reviewed. The advances and achievements in basic researches and clinical works of neurosurgery acquired during the period of the 11th Five Year Plan were analyzed. Results During the period of the 11th Five Year Plan, Chinese military neurosurgery was still in the lead position in traditional areas such as traumatic brain injury. In the meantime, great achievements were obtained in the researches of brain and spinal cord tumors, endovascular treatment and functional neurosurgery, and 47 items of Army Science and Technology Advancement Prizes or Military Medical Achievement Awards were acquired. A great progress was made in personnel training. However, unbalanced development in neurosurgery field still existed within military departments, between military and civil departments and between different regions, so an enhanced technical interchange and cooperation is needed. Conclusions On the coming period, the research work of military neurosurgery should focuse on basic and clinical research of gene therapy to keep in step with neurosurgical trends in the world. On the other hand, attention should be paid to technological innovation with military features such as professional and general treatment of traumatic brain injury and researches on the development of protective equipments.
2.Inhibitory effect of extracts from asparagus filicinus rhizome on proliferation of human osteosarcoma cells

Objective To investigate the inhibitory effect of extracts from asparagus filicinus rhizome on prolifieration of human osteosarcoma Saos-2 cells and its molecular mechanism .Methods MTT assay was used to detect the cytotoxic activity and growth inhibition of three different extracts from asparagus filicinus rhizome against Saos-2 cells ;plate colony formation assay was per-formed to detect active fraction of asparagus filicinus rhizome on the anchorage dependent growth of Saos-2 cells ;the cell cycle alter-ation was determined by propidium iodide staining and flow cytometry analysis ;the alteration of protein expression level of COX-2 was determined by using Western blotting .Results Ethyl acetate fraction of asparagus filicinus rhizome (AF-A) exerted the potent cytotoxicity on Saos-2 cells(IC50 =26 .7 μg/mL);AF-A induced the inhibitory effect on the anchorage dependent growth of Saos-2 cells in a dose dependent manner(P<0 .05);Saos-2 cells treated by AF-A at the concentration of 30 .0 and 100 .0 μg/mL for 48 h induced the increase of percentages of S phage from (31 .8 ± 4 .8)% in the control group to (43 .7 ± 2 .5)% and(51 .9 ± 1 .9)% ,the difference showing statistical significance (P< 0 .05) .Western blotting showed that AF-A at different concentrations decreased COX-2 protein expression .Conclusion AF-A posseses the inhibitory effect on the proliferation and growth of human osteosarcoma cells in vitro ,and its mechanism might be associated with the induction of S phage arrest and the inhibition of COX-2 protein ex-pression level .
3.Development and Application of Medical Equipment Management Network System

Objective To consummate modern hospital equipment management.Methods Medical equipment management network system was developed based on the platform of 'No.1 Military Medical Project'.The software could locate the site,register and count the maintenance and usage of the equipment.Results The accurate cost-accounting report and benefit analysis report for equipment were created.Conclusion The scientific and standard management for equipment impels the in-depth application of hospital informationization.[Chinese Medical Equipment Journal,2008,29(2):58-59]
4.Follow-up of total hip replacement using domestic prosthesis in 33 hips

BACKGROUND: Total hip replacement has been widely used in the treatment of severer hip diseases in China.However,most of prostheses are designed according to Caucasian osteometric measurements,whether those prostheses are suitable for Orientals is still in dispute.It is emerging to design prostheses for Orientals,there are few reports addressing therapeutic effect after replacement.OBJECTIVE: To follow up the long-term results of total hip replacement using domestic designed dual-assembly total hip prosthesis.METHODS: Totally 30 patients(33 hips)who underwent total hip replacement at the Department of Orthopaedic Surgery,China-Japan Union/Affiliated Hospital of Jilin University,from June 1987 to December 1996,were followed up.The designed dual-assembly total hip prosthesis was produced by Central Iron & Steel Research Institute,which was similar to bipolar prosthesis with a polyethylene acetabular cup.First generation cementing techniques were applied.The average age at the time of the operation was 56.5 years(30-81 years)and the average follow-up period was 8.5 years(5.0-14.4 years).Standard anteroposterior radiographs of the hip and clinical examinations were used to analyze therapeutic efficacy.RESULTS AND CONCLUSION: The total excellent and good rate was 68.6%.The imaging examination showed that there were16 hips(48.5%)had aseptic loosening and 11 hips(33%)had heterotrophic bone formation.Additionally,two cases suffered from infection and one case suffered from dislocation.The utilization of prosthesis achieved certain curative effects.However,the design and cement technology have problems due to limitation of conditions,which should be solved in further experiments.
5.Effects of methylprednisolone on the secreted function of Schwann cells

BACKGROUND: Secretion of various neurotrophic factors by Schwann cells plays important roles in neural regeneration. However, the secretion capability is affected by many factors. To seek a feasible method for promoting nerve growth factor secretion by Schwann cells is a key of regeneraion following neurologic defect.OBJECTIVE: To explore the effects of methylprednisolone(solu-medrol) on the secreted function of Schwann cells of cultured rats.METHODS: Schwann cells were isolated and cultured by enzyme digestion method. Cell growth was observed under an inverted phase contrast microscope. Following passage, purity of some Schwann cells was identified using S-100 protein immunity. Other Schwann cells were regulated using cell counting plate into 1×10~9/L, and incubated in a 6-well culture plate (15 wells) for further incubation. Following 4 days of culture, different concentrations of solu-medrol (10~(-3), 10~(-4), 10~(-6), 10~(-8) mol/L) were administrated to the cell, while blank control group (1 well) was given no drug. 24, 48 and 72 hours after administration, reverse trancription-polymerase chain reaction (RT-PCR) was used in the detection of the levels of nerve growth factor mRNA.RESULTS AND CONCLUSION: Number of primarily cultured cells was significantly increased at day 7, and 80% cells were confluent. Subcultured cells were spindle-shaped, with 2 thin long processes, showing positive fluorescence staining. Fibroblasts were round or flat, showing negative reaction of fluorescence staining. Reserve transcription-polymerase chain reaction demonstrated that nerve growth factor number at 72 hours affected by 10~(-8) mol/L radiosone was increased compared with the blank control group and other concentrations and other time points (P < 0.05). Number of nerve growth factor was reduced following treatment of 10~(-3) mol/L radiosone compared with the blank control group and other concentrations (P < 0.05). These results suggested that high concentration of solu-medrol prohibits secreted function of Schwann's cells, but long time and low dosage solu-medrol promotes secreted function of Schwann's cells.

8.Expression of Survivin in colorectal cancer tissue and its relationship with clinicopathological features

Objective To explore the expression of Survivin in colorectal cancer tissue and its relationship with clinicopathological features.Methods In situ hybridization was used to examine the expression of Survivin mRNA,and immunohistochemical method was used to detect the expression of Survivin protein in 60 cases of colorectal cancer and 30 cases of colorectal normal mucos tissue.The relationship of the expression of Survivin protein with clinicopathological features of colorectal cancer was analyzed.Results The positive expression rate of Survivin protein in colorectal normal mucos tissue (10.00%,3/30) was lower than that in colorectal cancer tissue (73.33%,44/60),and there was significant difference (P < 0.05).The positive expression rate of Survivin mRNA in colorectal normal mucos tissue (6.67%,2/30) was lower than that in colorectal cancer tissue (63.33%,38/60),and there was significant difference (P < 0.05).The expression of Survivin mRNA was positively correlated with depth of invasion,lymph node metastasis,liver metastasis,peritoneal micrometastasis and TNM stage in patients with colorectal cancer (P < 0.01),but had no correlation with gender,age,tumor size,location,degree of differentiation and histological type (P > 0.05).Conclusions The specific high expression of Survivin protein has a high correlation with the occurrence,development,infiltration of colorectal cancer.Survivin may be used as a marker for prognosis of patients with colorectal cancer.

Objective To evaluate the relevance of PET CO 2 ,TcPCO 2 and PaCO 2 in pediatric patients with congenital heart disease.Methods Eighty pediatric patients with congenital heart disease were scheduled for elective cardiac operation with cardiopulmonary bypass,aged 1 month to 5 years, 42 male and 38 female,were divided into 4 groups:group N1,cyanotic type congenital heart disease with severe pulmonary hypertension;group N2,cyanotic type congenital heart disease without pul-monary hypertension;group N3,non-cyanotic congenital heart disease with severe pulmonary hyper-tension;group N4,non-cyanotic congenital heart disease with mild pulmonary hypertension or normal of pulmonary artery pressure,20 cases in each group.The same anesthesia methods were selected in each group.PET CO 2 ,TcPCO 2 and PaCO 2 before operation were recorded,and the relevance among them was analyzed.Results In groups N1,N2 and N3,the differences of PET CO 2 and PaCO 2 were significant,,while the difference was relatively small in group N4;the difference of TcPCO 2 and PaCO 2 were relatively small between the four groups.In Groups N1,N2 and N3,the values of TcP-CO 2-PaCO 2 value were significantly less than that of PET CO 2-PaCO 2 ;in group N4,TcPCO 2-PaCO 2 value was obviously higher than that of PET CO 2-PaCO 2 values (P < 0.01 ).Correlation analysis of PET CO 2 and PaCO 2 showed that R2 in the groups N1,N2,N3,N4 were 0.028,0.1 97,0.390, 0.71 1(P >0.05,0.02

RESULTS: Compared with control group, apoptosis of bEnd. 3 cells in 25 mg/L group was significantly inhibited ( P <0.05), but apoptosis in the 100 mg/L group was significantly increased (P < 0.05). In apoptosis inhibited group, the Fas immunocytochemical staining was weaker, the positive cells were significantly decreased ( P < 0.05) and caspase - 3 expression was decreased compared with control group; however, the Bcl - 2 staining was stronger and the positive cells were significantly increased ( P < 0.05). On the other hand, in apoptosis increased group ( 100 mg/L group), the changes were just opposite. CONCLUSIONS: The effect of crenulatin on apoptosis of mouse cerebral microvascular endothelial cells possesses a dual - direction change, inhibitive effect in 25 mg/L and stimulative effect in 100 mg/L group, respectively. The mechanism is related to the alterations of Fas/Bcl - 2 expression and caspase - 3 activity.
