1.Replacement and support therapy in the patients with liver failure

The patients with liver failure have a high risk of death.Liver transplant is known as the most curative therapy for those patients,but the transplant operations are often restricted because of the donor deficiency and high medical cost.Now artificial liver is still the best choice to treat the liver failure.In the article,the research progress on the replacement and the support therapy in the patients with liver failure is reviewed.
2.The development of surgery for macular hole with retinal detachment in high myopic eyes

Treatment of macular hole associated retinal detachment in high myopia has progressively evolved over the years,including the scleral buckling,simple intravitreal gas injection,pars plana vitrectomy (PPV),PPV combined with internal limiting membrane (ILM) peeling or transplantation and so on.Simple vitreous gas injection is less traumatic and good for the patients with small holes and localized retinal detachment.PPV combined with ILM peeling can achieve better treatment effects for small holes with wide retinal detachment.But for large holes with wide retinal detachment,PPV combined with ILM transplantation is necessary to improve the macular holes closure rate.If the ILM has been peeled before,the lens capsule could be a nice substitute for ILM.The scleral surgery can solve the problem of posterior scleral staphyloma and effectively control the elongation of the axial length.However,there is still no one surgery could deal with all the problems of high myopia,we should consider all the circumstances like the size of the macular hole and the range of the retinal detachment to choose the best individualized therapy.
3.Application of point-of-care testing in blood purification and extracorporeal membrane oxygenation therapy

Point-of-care testing(POCT)is a kind of testing technology that uses portable testing instruments to test and quickly report the detection results at the bedside of testing object.Critically ill children need bedside examination, diagnosis and treatment in time according to the changes of their condition because of their critical condition and rapid progress.Therefore, the application of POCT in pediatric intensive care medicine has developed rapidly.Blood purification technology and extracorporeal membrane oxygenation(ECMO)are important treatment methods for critically ill children, while POCT is a necessary technology and an important link in the management of extracorporeal circulation.In this review, the application of POCT in blood purification and ECMO therapy is summarized.

5.THE UPPER AIRWAY CT SCAN OF OBSTRUCTIVE SLEEP APNEA PATIENTS

Compared with CT scan results of OSAS patients and normal adults to explore the value of CT diagnosis of upper airway stricture in patients with OSAS, 35 patients with OSAS and 35 normal adults were included in the study. 22, 19, 12, 8 cases had obstruction at palate, uvula, lingua and epiglottis region respectively, the awaked CT scan results of 61 obstructive sites were compared with those of normal adults. Obvious difference was found between patients and normal adults in area, left right dimension and coronal dimension. difference was also found in thickness of lateral pharyngeal tissue of palate, uvula and lingua region and retropharyngeal tissue of palate and uvula. The normal extent was determined by unilateral 95% confidence interval in normal adult group. And 43 sites had abnormal CT scan results, the positive ratio was 72 13%. The results sugget that there are obvious differences between the patients with OSAS and normal adults, the CT scan measure can show the upper airway obstructive site of patients with OSAS.
6.Effects of He-Ne irradiation on matrix metalloproteinases- 9 expression during experimental tooth movement in rabbits

Objective To explore the feasibility of He-Ne laser irradiation in promoting orthodontic tooth movement and periodontal remodeling through observing the effect of low power laser on the expression and distribution of matrix metalloproteinases- 9(MMP- 9) during orthodontic tooth movement in rabbit. Methods Thirty rabbits were divided into 1d,3d,5d,7d,14d and 21d groups, each group were given a coil spring connected to the maxillary upper first molar to the upper incisor and fixed bilaterally by a ligature wire and set force to 80g. He-Ne laser irradiation was performed on the right side of the orthodontic tooth, and the left side served as control. Radiation time ranged from 1 day to 5 days, once a day, 15min each time. All of the tissue sections were proceeded with MMP- 9 immunohistochemical staining. MMP- 9 average gray scale in periodontal tissue of the orthodontic teeth was analyzed by a Computer Image Analyzing System. Results The expression level of MMP- 9 increased earlier and lasted for a longer time in the compression and tension area on the side of irradiation by He-Ne laser as compared to those on the control side. There was significant difference between the irradiation side and the control side in terms of the MMP- 9 expression gray scale in the tension zones at 5 and 7 days post-operation, and in the pressure zones at 3,5,7 day post-operation. Conclusion During the movement of orthodontic teeth, local irradiation with He-Ne laser could increase the expression of MMP-9. It was suggested that the irradiation with low power laser accelerate blood vessel and bone remodeling.
7.Endoplasmic reticulum stress within hypoxia microenvironment and its related dieases

Endoplasmic reticulum stress,an critical component of cell stress,is a protective response for eukaryotic cells. The change of partial pressure of oxygen (for instance hypoxia) is a primary condition for cell stress,significantly impacting the biological phenotype and behavior of cells (including survival,migration and invasion et al). Through endoplasmic reticulum stress,intracellular concentration of unfolded protein is reduced,and agglutination inhibited. Recently,endoplasmic reticulum stress has been demonstrated to play an important role in pathogenesis of some diseases such as cancer,diabetes and inflammation,which are characterized by the micro-environment hypoxia in local tissue. Therefore,investigations on the mechanism of endoplasmic reticulum stress caused by hypoxia may promote the novel therapeutic strategy for tumors,cardiovascular diseases and diabetes.

9.Pathogenesis and therapy in sepsis-induced cardiomyopathy

Sepsis-induced cardiomyopathy(SICM)is a reversible cardiac insufficiency in the early stage of sepsis, and mainly manifests as left ventricular dilation, decreased ejection fraction, and recovery within 7~10 days.Although it is reversible, the incidence and mortality in sepsis are high.The specific mechanism is still unclear.Inflammatory reaction, mitochondrial dysfunction, apoptosis and other pathophysiological processes play an important role.Its process is complex and involves the interaction between organism and pathogen.The management of SICM is still based on the etiologic treatment of septic shock guided by hemodynamic monitoring and tissue perfusion, with cardio-protective therapy and specific measures.This review summarizes the literatures on the mechanisms and treatments of SICM.
