1.Enhancement of systemic and CNS delivery of meptazinol hydrochloride by intranasal administration to rats

Aim To investigate the extent of systemic absorption and uptake of meptazinol (MEP) hydrochloride in cerebrospinal fluid (CSF) after intranasal administration on rats and compare with oral administration. Methods CSF samples were collected by a serial sampling method. The concentration of MEP in the biological samples was measured by HPLC with fluorescence detector. Results Rapid and significant levels of MEP in plasma and CSF can be achieved after nasal administration whereas the oral administration resulted in considerably lower drug concentrations. AUC in plasma and CSF from the nasal route are 7.375 and 15.6 folds compared with those of the oral route, respectively. Conclusion Intranasal MEP is able to show quick absorption and improve the bioavailability, which could be a promising alternative to oral administration.
2.Study on correlation between erythrocyte indices and genotype in cases with alpha-thalassemia

Objective To investigate the correlation between the erythrocyte indices and the genotypes of alpha thalassemia.Methods337 carriers with various genotypes of alpha-thalassaemia ( iron deficiency,alpha-thalassemia double heterozygote and homozygote,α-compounding β-thalassemia and abnormal hemoglobinopathy were excluded) were classified into three groups based on different genotypes of alpha-thalassaemia including silent thalassemia group (ST,83 cases),α-thalassemia trait group (TT,210cases) and intermediate thalassemia group( IT,44 cases),and 154 healthy adults were randomly choosed as normal control The erythrocyte indices involving in RBC,Hb,MCV,MCH,MCHC and RDW-CV were retrospectively analyzed and the difference of which was compared by analysis of variance and SNK test among aboved-mentioned groups.ResultsThere were statistical significance among groups about erythrocyte indices except Hb F.The order of the level of MCV and MCH was NC [( 86.6 ± 5.2) fl,( 29.5 ± 2.1 ) pg] >ST[(80.1 ±3.3) fl,(26.7±1.3) pg] >TT[(68.5 ±3.4) fl,(22.0 ±1.2) pg] >IT[(66.6±7.1)fl,(20.0 ±2.2) pg,F =580.67,761.19,P <0.05].And the size of RDW-CV was IT(22.3 ±3.4)% >TT (14.9±1.2) % >ST(13.8±1.6)% >NC(13.2±1.4)%(F=347.25,P<0.05).In ST group,the value of MCHC of -α3.7/αα subgroup( 335.6 ± 8.0) g/L was higher than that of -α4.2/αα subgroup( 330 ±7.2) g/L and αTα/αα subgroup (328.4 ±9.5) g/L(F=6.07,P <0.05).Meanwhile,in IT group,the value of MCV of αTα/--SEA subgroup( 70.1 ± 7.2 ) fl was higher than that of -α3.7/--SEA subgroup ( 63.4 ±5.9) fl and -α4.2/--SEA subgroup ( 64.1 ± 4.0 ) fl ( F =5.55,P < 0.05 ).However,the value of MCHC of αTα/--SEA subgroup( 289.7 ± 21.2 ) g/L was lower than that of other two subgroups [( 306.3 ± 8.4 ),(306.1 ± 8.7) g/L,F =8.72,P <0.05].Except Hb A2 and Hb F,there was positive correlation between the number of deleted α-globin gene and that of RBC and RDW-CV ( r =0.318 and 0.580,P <0.01 ).Nevertheless,there was negative correlation between the number of deleted α-globin gene and that of the other erythrocyte indices (r =-0.483,-0.827,-0.744 and -0.684,P all <0.01 ).ConclusionsThere is close correlation between the degree of anemia and the number of deleted α-globin gene characterized by Hb reduction and RBC increasing.In addition,the anemia degree of non-deletional Hb H disease is severer than that of deletional Hb H,which of Hb H disease with -α4.2/--SEA is severer than that with -α3.7/--SEA.
3.Comparison between probe melting curve analysis based on real-time fluorescent PCR and reverse dot blot assay for gene and prenatal diagnosis of β-thalassemia

ObjectiveTo evaluate the reliability of the probe melting curve analysis (PMCA) based on real-time fluorescent PCR assay for the genetic diagnosis and prenatal diagnosis of β-thalassemia.MethodsA total of 135 cases of peripheral blood samples were collected from Zhuhai Municipal Maternity and Child Healthcare Hospital between 2008 and 2010.All the samples were performed preliminary diagnosis according to the hematological data.Of these,119 cases were diagnosed as β-thalassemia trait,4 cases were diagnosed as severe thalassemia and 12 cases were normal.In addition,44 cases of amniotic fluid and 8 cases of cord blood with high-risk for severe β-thalassemia were also collected.The diagnostic reliability of the PMCA assay and reverse dot blot assay was evaluated on 187 above-mentioned cases by direct DNA sequencing analysis in a double-blind study.ResultsThe genotypes of 185 cases were detected accurately based on the PMCA assay except for two cases:one heterozygote with Ini( ATG > AGG) was omitted and another heterozygote couldn't be distinguished between CD43 ( G > T) and CD37 ( G > A ).For the RDB assay,only one heterozygote with CD71-72 ( + T) was not detected accurately in the above-mentioned cases.Compared with the DNA sequencing analysis,the sensitivity,specificity,negative predictive value,positive predictive value and diagnostic efficiency of the PMCA assay were 98.75%,100.00%,93.10%,100.00% and 98.93%,respectively.The corresponding value of the RDB assay were 99.38%,100.00%,96.42%,100.00% and 99.47%,respectively.There were no significant between-group differences in the diagnostic efficiency of the two assays ( P > 0.05 ).The results of prenatal diagnosis were in complete concordance with the follow up results,after the birth or induced labour of the fetuses.ConclusionsThe PMCA assay can be used as an alternative and verified method of RDB assay for the genetic diagnosis and prenatal diagnosis of β-thalassemia.
4.Efficacy of Saxagliptin on glucose and lipids metabolism and adipokines in high-glucose and fat dietinduced insulin resistance rats

Objective To investigate the efficacy of Saxagliptin on the glucose and lipids metabolism and adipokines levels in diet-induced insulin resistance rat,and to explore the regulatory mechanism of saxagliptin in insulin resistance.Methods 36 4-week-old male Wistar rats were randomly divided into two groups:control group (n=12) and high-glucose and fat diet group(HF,n =24).The HF rats were randomly divided into two subgroups:HF group and saxagliptin group,and continued the treatment for 8 weeks.Levels of blood glucose,insulin,triglyceride (TG),total cholesterol (TC),TNF-α and adiponectin were determined.Results After four months,serum levels of fasting glucose,insulin,TG,TC were significantly increased in HF group as compared with control group [(7.07±1.61) mmol/L vs.(5.10±0.44) mmol/L,(23.70±7.37) mU/L vs.(19.35 ± 6.38) mU/L],[(3.21± 1.44) mmol/L vs.(0.83 ± 0.39) mmol/L,(3.04 ± 1.62) mmol/L vs.(1.14±0.24) mmol/L,all P<0.05],and were all decreased after the treatment of saxagliptin.The glucose infusion rate was lower in HF group than in control group and was higher in saxagliptin group than in HF group [(18.80±1.57) mg · kg-1 · min-1 vs.(24.31±2.65) mg · kg 1 · min 1,(21.45 ±1.80) mg· kg-1 · min-1 vs.(18.80±1.57) mg· kg 1 · min-1,P<0.01 or 0.05].Compared with the control group,serum THF-α was higher and adiponectin level was lower in HF group [(1.38 ±0.18) μg/L vs.(2.33±0.21)μg/L,(2.65±0.29) mg/L vs.(1.38±0.20)mg/L,both P<0.01].Saxagliptin can significantly increase the serum level of adiponectin and decrease the level of TNF-α in HF group (P<0.01 or 0.05).Conclusions Saxagliptin treatment can improve high-fat dietinduced insulin resistance,decrease blood lipids levels and regulate the levels of adipokines in HF rats.
5.Clinical study on effect of sequential colon dialysis in patients with chronic renal failure in early and medium stage

Objective: To observe the effect of sequential colon dialysis in patients with chronic renal failure(CRF) in early and medium stage.Methods: One hundred and forty-two CRF patients in early and medium stage were treated with sequential colon dialysis and traditional Chinese medicine according to(traditional) Chinese medical syndrome differentiation and typing,and clinical syndromes were observed.(Sixty-three) patients with CRF were selected as controls treated with low protein diet plus essential amino acids.The serum levels of blood urea nitrogen(BUN),creatinine(Cr),uric acid(UA),the creatinine(clearance) rate(CCr),electrolyte and medium molecular substance(MMS) were measured before and after treatment.Results: The clinical symptoms of the 142 patients were improved,the serum levels of BUN,Cr,UA and MMS were decreased significantly(BUN:(15.3?4.5)mmol/L vs.(8.6?3.3)mmol/L;Cr:((443.6?73.5)?mol/L) vs.(283.5?38.7) ?mol/L;UA:(512.6?86.5)?mol/L vs.(243.1?33.6)?mol/L;MMS: 0.44?0.06 vs.0.32?0.04),and CCr increased markedly((9.87?3.31)ml/min vs.(15.60?(4.63)ml/min)).The difference between treatment group and control group was significant.The changes in electrolyte levels were not significant before and after treatment in both groups.Conclusion: Sequential colon dialysis has obviously curative effects on CRF patients in early and medium stage,and could ameliorate the(develope) of CRF in early and medium stage.
6.Effects of smoking on serum paraoxonase 1 activity and carotid atherosclerosis

Objective To explore the effects of smoking on serum paraoxonase1 (PON1) activity and carotid atherosclerosis.Methods 478 subjects from residents of health screening for cardiovascular disease were enrolled from June 2012 to July 2014 in Huangpu district,shanghai.Smoking,drinking,exercise and cardiovascular disease risk factor data were recorded and gathered.All subjects accepted carotid artery ultrasound examination and were measured serum PON1 activity.The lowest quartile of serum PON1 activity level was taken as low PON1 activity level.Results (1) Serum PON1 activity in smokers was lower than that in non-smokers ((206.5±25.6) kU/L vs (230.9±38.1)kU/L,P<0.01),incidence of carotid artery atherosclerosis in smoking group was higher than non-smoking group(75.7% vs 56.1%,P<0.01).(2) Multivariate logistic regression analyses indicated smoking,lack of exercise,creatinine,LDL-C were the independence factors of PON1 activity.(3) Multivariate logistic regression analyses indicated smoking,serum PON1 activity,age,gender,systolic pressure,were the independence factors of carotid atherosclerosis.Conclusion Smoking reduces serum activity.Smoking and lower serum PON1 activity level are independent risk factors of carotid atherosclerosis.
7.Relationship among serum homocysteine level, paraoxonase 1 activity and carotid atherosclerosis

Objective:To study relationship among serum homocysteine (Hcy) level ,paraoxonase 1 (PON1) activity and carotid atherosclerosis .Methods:A total of 179 residents from a community of Shanghai ,who participated in cardiovascular risk factor screening from 2012 to 2014 ,were selected .They received carotid ultrasonic examination and measurements of serum Hcy ,PON1 and other biomarkers .According to serum Hcy level ,subjects were divided into elevated Hcy group (n=85) and normal Hcy group (n=94) .Results:Spearman correlation analysis indicated that serum Hcy level was significant inversely correlated with PON1 activity (r= -0.738 ,P=0.001) .Compared with normal Hcy group ,there were signifi‐cant rise in age [(60.66 ± 7.18) years vs .(64.57 ± 7.29) years] ,male proportion (27.66% vs .63.53% ) ,serum creati‐nine [(69.62 ± 12.76)μmol/L vs .(88.47 ± 20.86)μmol/L] ,uric acid [(267.85 ± 63.02)μmol/L vs .(307.51 ± 76.07)μmol/L] ,triglyceride [(1.33 ± 0.79) mmol/L vs .(1.76 ± 1.70) mmol/L]and systolic blood pressure [(134.93 ± 15.82) mmHg vs .(142.72 ± 17.86) mmHg] ,and significant reductions in levels of high density lipoprotein cholesterol [HDL‐C , (1.17 ± 0.26) mmol/L vs .(1.06 ± 0.27) mmol/L]and PON1 [(288.58 ± 73.80) kU/L vs .(187.81 ± 16.31) kU/L]in el‐evated Hcy group , P<0. 05 or <0. 01. Incidence rate of carotid atherosclerosis in elevated Hcy group was significantly higher than that of normal Hcy group (64. 7% vs .44. 7% ) , P=0. 001 .Multi‐variate gradual Logistic regression analysis indicated that serum creatinine and Hcy levels were independent risk factors for serum PON 1 activity(OR=1.055 ,1.139 , P<0.01 ,<0.05);Hcy isn′t an independent risk factor (OR=1.020 ,P=0.497) for carotid atherosclerosis .Conclusion:Serum Hcy level is significant inversely correlated with serum PON 1 activity ,and both of them are related to occurrence of carotid atherosclerosis .

9.Survey of endemic situations in schistosomiasis transmission controlled or interrupted regions in Anhui Province

Objective To understand the variation rules of schistosomiasis endemic situation before and after schistosomia-sis transmission controlled or interrupted,so as to provide the evidence for the consolidation of control achievements. Meth-ods In Anhui Province,3 counties reaching the criteria of schistosomiasis transmission controlled or interrupted were selected and their historical endemic data were collected and analyzed statistically from 10 years before the schistosomiasis transmission controlled to 2008. Results In Tianchang City,the Oncomelania hupensis snail area was 3.54 hm2 in a part of the lake marsh-land in the year reaching the criteria of transmission controlled(1998),and no snails were found in the year of transmission in-terrupted(2008). No stool-test-positive resident was detected except there were 58 acute schistosomiasis cases in 1993,and the sero-test-positive rate of local residents remained at low levels(0-1.55%). In Taihu County,the snail area was 0.84 hm2 account-ing for 0.06%of its historical accumulative snail areas in the year of transmission controlled(1971),and no snails were found three years before the transmission interrupted (1983). However,the schistosomiasis endemic rebounded 12 years later (1995),there was an outbreak of acute schistosome infections(73 cases),and the snail area increased again to 133.7 hm2 ac-counting for 2.91%of historical accumulative snail areas. After that,the snail area kept on rising and the infection rates of resi-dents and bovine remained higher than 1%. In Guangde County,the snail area was 32.4 hm2 accounting for 1.90%of its histori-cal accumulative snail areas in the year of transmission controlled(1995). After that,the snail area increased progressively, and the schistosomiasis endemic rebounded and there were acute schistosome infections 5 years later. The Spearman tests showed that the sero-test-positive rate(Tianchang City)and the stool-test-positive rates of residents and bovine(Taihu County)had positive correlations with the snail areas(r=0.582,0.401,0.596,all P<0.05). Conclusion The snail status is a key for the consolidation of schistosomiasis transmission controlled and interrupted. Therefore,a valid surveillance system of snail situa-tion should be established as quickly as possible.

Anxiety disorder is one of the most common mental disorders and seriously impairs the physical and mental health of patients. Due to the efficacy of acupuncture for tranquilization, acupuncture displays its unique advantage on the treatment of anxiety disorder, but the relevant biological mechanism has not been elaborated. The modern medicine study has proved that the heart and brain have their own independent natriuretic peptide (NP) system. The dysfunction of ANP and its receptor are closely related to the occurrence of anxiety disorder. The ANP acts on anti-anxiety. Hence, focusing on the three aspects, named the anti-anxiety effect of acupuncture based on its tranquilizing effect, the anti-anxiety effect of ANP and the positive regulation of acupuncture on NP, the mechanism on ANP and its receptor was explored in anti-anxiety with acupuncture based on tranquilizing effect, and the idea was put forward on that the anti-anxiety effect of acupuncture was possibly based on its action of tranquilization through regulating the ANP and its receptor. As a result, it is expected to provide the theoretic support for the mechanism study on anti-anxiety with acupuncture based on its tranquilizing effect.
