1.Clinical Observation of Compound Glycyrrhizin Combination with Acyclovir in the Treatment of Children with Infectious Mononucleosis

OBJECTIVE:To observe the clinical efficacy and safety of compound glycyrrhizin combined with acyclovir in the treatment of children with infectious mononucleosis. METHODS:90 children with infectious mononucleosis were randomly divid-ed into control group and research group. Control group was given Acyclovir injection 10 mg/(kg·d)adding into 150 ml 5% Glu-cose solution by intravenous infusion,twice a day. Research group was additionally given Compound glycyrrhizin injection 2-4 ml/kg,adding into 150 ml 5% Glucose solution by intravenous infusion,twice a day. The treatment course for both group was 2 weeks. Clinical efficacy,hospitalization time,improvement time of clinical symptoms,T lymphocyte subsets(CD3+,CD4+,CD8+ and CD4+/CD8+)before and after treatment and incidence of adverse reactions in 2 groups were observed. RESULTS:The total effective rate in research group was significantly higher than control group,hospitalization time and improvement time of clinical symptoms were significantly better than control group,the differences were statistically significant(P<0.05). After treatment,CD3+ and CD8+were significantly lower than before,and research group was lower than control group,CD4+and CD4+/CD8+were significantly high-er than before,and research group was higher than before,the differences were statistically significant(P<0.05). There was no sig-nificant difference in the incidence of adverse reactions between 2 groups(P>0.05). CONCLUSIONS:Compound glycyrrhizin combined with acyclovir has better efficacy than only acyclovir in the treatment of children with infectious mononucleosis,with similar safety.
2.Safe dosage of intrathecal injection of sufentanil for preemptive analgesia in lower limb orthopaedic surgery

ObjectiveTo selected the safe dosage of intrathecal injection of sufentanil for preemptive analgesia in lower limb orthopaedic surgery.MethodsSelected 96 patients who ASA classification Ⅰ - Ⅱ grades undergoing elective lower limb orthopaedic surgery with subarachnoid epidural block down,they were divided into six groups by random number table and double blind method,each group of 16 cases.Group Ⅰ,Ⅱ,Ⅲ,Ⅳ,Ⅴ,Ⅵ received 0.05,0.10,0.15,0.20,0.25,0 μ g/kg of intrathecal sufentanil and 2.5 ml of 0.5% levobupivacaine.Observed and recorded the duration of block level of pain to T10,visual analog score (VAS)postoperative at 2,4,6,8,12,24,48 h,and side effects.ResultsThe duration of block level of pain to T10 of group Ⅰ,Ⅱ,Ⅲ,Ⅳ,Ⅴ,Ⅵ was (245.6 ± 34.2),(259.3 ± 36.7),(268.9 ± 37.8),(286.6 ± 38.8),(287.7 ± 39.1 ),(228.6 ± 31.4) min.There was no significant difference between group Ⅳ and group Ⅴ (P > 0.05 ),the differences of other groups were statistically significant (P < 0.05 ).Compared with group Ⅴ,VAS of group Ⅲ and group Ⅳ at 6,8,12,24,48 h after operation was not statistically significant (P > 0.05),and they were lower than the other three groups(P < 0.05 ).There were no respiratory depression occurred after operation in group Ⅰ,Ⅱ,Ⅲ,Ⅳ,Ⅵ,4 cases occurred in group Ⅴ.ConclusionThe best of effective dose of intrathecal injection of sufentanil for preemptive analgesia in lower limb orthopaedic surgery is 0.20 μ g/kg.
3.Efficacy of esophago-pharyngeal tube for airway management in patients undergoing operation on language areas

Three ASA Ⅱ male patients aged 22-46 yr weighing 50-75 kg underwent operation on language areas from June to September 2009 in our hospital. Anesthesia was induced with propofol 2-2.5 mg/kg, fentanyl 4-5 μg/kg and atracurimm 0.6 mg/kg. The esophago-pharyngeal tube was inserted into esophagus and oropharynx under direct vision. The esophageal balloon was inflated with 10-15 ml of air and the pharyngeal balloon with 40-60 ml of air. After the correct position of the tube was confirmed, the patients were mechanically ventilated.Anesthesia was maintained with propofol 50-70 μg · kg- 1 · min- 1 , remifentanil 0.15-0.25 μg· kg- 1 · min- 1 and atracurium 0.3-0.4 μg·kg-1·h-1. The pharyngeal balloon was deflated during wake-up test. The patients were asked to count continuously and answer question as instructed to locate the language areas. After the language areas had been located, the pharyngeal balloon was inflated again. The tidal volume was set at 500-510 ml and measured 500-512 ml. The airway pressure measured 16-18 cm H2O. The hemodynamics was stable and blood gas indexes were within the normal range. The language in response to questions was clear in one patient, not very clear in the second patient and vague in the third patient. Hypoxemia hypercapnea and intracranial hypertension were not observed during operation and wake-up test. After the patients became conscious, the balloons were deflated and the vital signs were stable. The patients were transferred to ICU after removal of esophago-pharyngeal tube. No postoperative complications occurred.
4.Minimal and optimal concentration of ropivacaine for epidural anesthesia in mastectomy

Objective To investigate the minimal and optimal concentration of ropivacain for epidural anesthesia in patients with breast cancer.Methods 103 patients with breast cancer ASA Ⅰ-Ⅱ undergoing elective radical mastectomy were conducted with epidural anesthesia.T2-3 were selected as a puncture site.A catheter was inserted into the epidural space in a cephalic direction for 3.5-4.0 cm.The concentration of ropivacain was produced by the test of up-down sequential allocation technique.20-25 ml of 0.20 % ropivacaine was given after a test of 0.20 % 5 ml ropivacaine.If the anesthesia was effective (VAS≤3),the next concentration was down 0.01%,while the VAS was more than 5,the next concentration was up 0.01%.The anesthesia effect and segments of block were measured with acupuncture.Blood pressure (NIBP),heart rates(HR),blood oxygen saturation (SPO2),blood gas and VAS were monitored during operation.Nasal catheter oxygen inhalation was used in routine method.Results The median effective concentration (EC50) of ropivacaine was 0.10 % determined by the formula of dixon and massey.The 95 % confidence intervals was 0.1022 %-0.1065 %.The least and the best analgesic concentration of ropivacaine for the upper thoracic epidural block was 0.14 % and 0.17 %-0.18 % respectively.Conclusion The minimal and the optimal analgesic concentration of ropivacaine for the upper thoracic epidural block are 0.14 % and 0.17 %-0.18 %respectively,and which provides a safe and rational use of ropivacaine for the clinic.
5.Application of covered stent in treatment of carotid blowout after head and neck tumors resection

OBJECTIVE To assess the treatment reliability of covered stent for carotid artery blowout after head and neck tumors resection. METHODS Five cases with postoperative rupture of carotid artery invaded by the head and neck tumor were reviewed. They presented with life threatening massive neck or oral bleedings. All of them were treated with self expanding covered stents through intervention therapy approach. RESULTS The covered stent were successfully deployed in the target arteries in all cases,the instant isolation effect was quite satisfactory. After treatment, angiography showed successful occlusion of the pseudoaneurysm, patency of carotid artery lumen, and significant improvement of clinical symptoms without neurologic dysfunction. Following up 2 to 36 months, 3 patients were alive with no disease, two patients died of recurrence. CONCLUSION For the treatment of carotid blowout, endovascular occlusion with covered stent is a minimally-invasive, safe and reliable methods.
6.Value of afterdischarges monitoring for intraoperative electrical stimulation for brain mapping

Objective To explore the value of afterdischarges monitoring for intraoperative electrical stimulation for brain mapping. Methods 34 patients received cerebral cortex electrical stimulation for brain mapping during operation of brain function area , afterdischarges were monitored simultaneously to determine the upper limit of stimulus intensity. Results 34 cases underwent electrical stimulation successfully , and received surgery without neurologic decline except 2 cases of hypokinesia. Conclusion After discharges monitoring improve the accuracy, reduce the risk of intraoperative cerebral cortex electrical stimulation.
7.Reliability and validity of the Chinese version of International Consultation on Incontinence Modular Questionnaire for Male Lower Urinary Tract Symptoms

Objective To translate the International Consultation on Incontinence Modular Questionnaire for Male Lower Urinary Tract Symptoms (ICIQ-MLUTS) and ICIQ-MLUTS long form (ICIQ-MLUTS LF) into Chinese and assess their metric properties and applicability.Methods After obtaining permission from the International Consultation on Incontinence Questionnaire (ICIQ),original ICIQ-MLUTS and ICIQ-MLUTS LF were translated into Chinese according to ICIQ validation protocol and cultural adaptation guideline.During November 2017 to August 2018,Chinese version of ICIQ-MLUTS and ICIQ-MLUTS LF were tested by administering them in 6 and 12 hospitals in China respectively.To validate the translated questionnaires,the following tests were undertaken.The content validity was determined by indepth interviews with participants and experts.The correlation coefficients of ICIQ-MLUTS and ICIQ-MLUTS LF with International Prostate Symptom Score (IPSS) were assessed to validate criterion validity.Cronbach's alpha test was used to explore internal consistency.And the test-retest reliability was evaluated by calculation of intraclass correlation coefficient.Results In total,Chinese ICIQ-MLUTS and ICIQ-MLUTS LF were administrated to 135 and 230 male patients with lower urinary tract symptoms respectively.Both questionnaires had good content validity and good criterion validity with IPSS (Pearson correlation 0.846 and 0.833 for ICIQ-MLUTS and ICIQ-MLUTS LF respectively,both P < 0.001).The Cronbach's alpha coefficient was 0.797 for ICIQ-MLUTS,and 0.853 for ICIQ-MLUTS LF.Intraclass correlation coefficient was 0.986 and 0.985 respectively (both P < 0.001),showing good test-retest reliability.Conclusions The Chinese version of ICIQ-MLUTS and ICIQ-MLUTS LF had good validity and reliability,which can be used to assess Chinese male patients with lower urinary tract symptoms.
8.Correlation between job burnout and social support among rural preschool teachers

Objective:
To analyze the relationship between job burnout and social support of rural preschool teachers.
Methods:
Multistage stratified cluster random sampling was used to select 809 rural preschool teachers from 3 prefectures cities in Sichuan province. Teachers’ Job Burnout Scale (MBI-ESN) and Social Support Assessment form (SSRS) was implemented. Pearson correlation analysis and canonical correlation analysis were used to explore the correlation between preschool teachers’ job burnout and social support.
Results:
Person correlation analysis found negative associations between all dimension of job burnout with each dimension of social support, as well as total score (P<0.01) except the emotional exhaustion dimension with objective support dimension of social support. In the canonical correlation analysis, a pair of typical variables was identified, with correlation coefficient of 0.421, contributing 81.4% (P<0.01); the first typical variable reflecting job burnout was mainly decided by X3 (low achievement feelings). The first typical variable W1 reflecting social support was mainly determined by Y2 (subjective support) and Y3 (utilization of support).
Conclusion
Job burnout of rural preschool teachers correlates with social support. The results point to the need to develop different forms of social support to deal with job burnout among teachers working in preschools.
9.Identification of the wild germplasm of Prunus mume based on AFLP markers.

Mei (Prunus mume Sieb.et Zucc.) is traditionally not only a famous special ornamental plant but also a fruit tree origined in China. In order to conserve and ultilize scientifically the germplasm resources of wild mei, we identified and analysed the germplasm of mei flower in 65 samples collected from the habitat, using AFLP makers in combination with morphological anaylsis. This study amplified clearly 1 728 polymorphic bands, using the 8- pair-primer of Mse I -EcoR I screened totally from 64 -primer combination. According to the Nei' 72 distance coefficient clustering, all of the formas and varieties used in this study, including Prunus mume var. mume, P. mume, P. mume var. goethartiana, P. mume var. pallescens, P. mume var. microcarpa, P. mume var. cenrnus-sempervirens (newly recoeding variety), P. mume var. cernua, P. mume var. pallidus, P. mume var. taomei, were identified at the point of Nei' 72=0.26. Due to the genetic difference obviously among the formas and varieties, we suggested that wild germplasmtypes of all formas and varieties in P. mume should be conserved in their habitats in the furture.

10.The distribution and structure of class Ⅰ integron in the multidrug-reisistant Pseudomonas aeruginosa

Objective To investigate the antimicrobial susceptibility of Pseudomonas aeruginosa (P. aeruginosa) isolated from Zhenjiang area to 13 routinely used antibiotics and identify the structure and dissemination of class Ⅰ integron. Methods K-B test was used to determine the resistant rate of 71 strains of P. aeruginosa. DNA template was extracted by boiling method, PCR method was utilized to detect class Ⅰintegron, and subsequently gene cassettes were analyzed by sequencing. Results The resistant rates to 13 routinely used antibiotics were quite different from 18. 3 to 77.5% among 71 strains of P. aeruginosa. The prevalence of class Ⅰ integron was 38%. These integrons include 5 gene cassettes ( aadB, aac (6) - Ⅱ , PSE-Ⅰ , dfrA17 and aadAS), in which dfrA17 and aadA5 gene cassette were frequently found. Comparing with the negative strains of integron, the positive strains of integron has obviously higher resistance to ten the antibiotics including piporacillin, piperacillin-tazobactam, ceftriaxone, cefepime, ceftazidime, gentamicin,amikacin, tobmmycin, levofloxacin, and ciprofloxacin. Conclusions The resistant rates of P. aeruginosa to 13 drugs were different, and the resistant rates of integron positive strains were obviously higher than integron negative strains, which indicates that integron may play an important role in multidrug reisistance of P. aeruginoosa.
