1.Non-cultural methods for the laboratory diagnosis of invasive fungal diseases

Non-cultural laboratory diagnostic methods include fungal antigen detection, anti-fungal antibody detection and molecular methods. Fungal antigen detection aim at ( 1, 3 )-β-D-glucan, galactomannan, cryptococcal capsular polysaccharide antigen and candida mannan antigen.Different antigen is used to diagnose different fungal infection.Antifungal antibody include antibody against Histoplasma capsulatum, Coccidioides immitis/Paracoccidiodes and Blastomyces dermatitidis.Molecular methods include non-amplification nucleic acid detection, nucleic acid amplification test, direct sequencing and matrix-assisted laser desorption/ionization time of flight mass spectrometry ( MALDI-TOF MS ) technology.Even though molecular methods are limited because of no standardization, the future of its usage is promising.
2.THE TOLERANCE OF HUMAN SPINE TO THE IMPACT RESULTED FROM OPENING OF PARACHUTE

Various vertebrae taken from 34 cadavers were tested separately on a Schenck tester with static and dynamic loads to evaluate their strengths. For an impact lasting for a relatively long period, the tolerance of each vertebra is under 21 G. Through an electronic model of human spine based on its dynamic characteristics under the impact of an opening parachute, an allowable magnitude of impact was estimated as 25 G for 50 ms. This data might be of use for our aviation industry.
3.Comparison and Observation about the Immediate Effect of Postoperative Distension of the Abdomen with Electroacupunctuving Zusanli and Shangjuxu Points

[Objective]To contrast the influence of using the electro-needling machine on the Zusanli(ST36) or Shangjuxu(ST37) ,or on the two points both to treat the postoperative distension of the abdomen.And simultaneously to obverse whether using the two points together has the coordination effect or antagonistic effect.[Method] Adopting the stochastic grouping contrastive technique,dividing the 100 patients into the Zusanli group,the Shangjuxu group,the cooperative group and the blank group. To contrastively observe each situation,including the abdominal rumbles sound,the abdominal pain,the transformation of the incidental symptoms and the first time of exhaust,before and after the acupuncture treatment in each group.[Result]The treatment group has the therapeutic effect,with the blank group comparison,having the significant difference(P0.05)with each other.There's no obvious coordination effect or antagonistic effect during the treatment in the cooperative group. [Conclusion] Using the electro-acupunture on Zusanli,Shangjuxu can play an important part in improving the recuperation of the gastrointestinal function after surgical treatment. It also can obviously relieve uncomfortable postoperative symptoms.
4.Interpretation and application of antimicrobial susceptibility testing

Antimicrobial susceptibility testing is an important tool for guiding rational use of antimicrobial agents in clinical settings.Proper use of antimicrobial susceptibility testing results requires clinicians and clinical microbiologists to have the knowledge of antimicrobial susceptibility testing and breakpoints,and have the ability to use pharmacokinetic and pharmacodynamic (PK/PD) principles to optimize antimicrobial therapy regimen.(Chin J Lab Med,2013,36:289-291)
5.In vitro activity of ciprofloxacin or Amikacin combined with ?-lactams against Multidrug-Resistant Pseudomonas aeruginosa Strains

Objective The aim of this study was to evaluate the in vitro activity of Ciprofloxacin(CIP)or Amikacin(AK)combined with four clinically-used antipseudomonas ?-lactams against multidrug-resistant Pseudomonas aeruginosa strains.Methods Determining the MICs of six antibiotics to 35 clinically isolated Pseudomonas aeruginosa strains which have different resistance patterns by 2-fold broth dilution method;determining the in vitro activity of Ciprofloxacin or Amikacin combined with Ceftazidime(CAZ),Imipenem(IMP),Meropenem(MEM)and Piperacillin/Tazobactam(TZP)by broth checkerboard method.Results The combinations of Ciprofloxacin or Amikacin with four ?-lactams have mainly synergistic or additive effect,the synergistic rate of AK-group(AK+CAZ:71.43%;AK+IMP:42.86%;AK+MEM:54.29%;AK+TZP:85.71%)was higher than that of CIP-group(CIP+CAZ:20%;CIP+IMP:25.71%;CIP+MEM:14.29%;CIP+TZP:31.43%).The combination of CIP+TZP has the highest synergistic rate in CIP-group;the combination of AK+TZP has the highest synergistic rate in AK-group.None of the combinations had an antagonistic effect.Conclusion the combination of ciprofloxacin or amikacin with ?-lactams have good in vitro activity to multidrug-resistant Pseudomonas aeruginosa strains.
6.Influence of psychological nursing on patients with pregnancy-induced hypertension combined with obstructive sleep apnea hypopnea syndrome

Objective To analyze the clinical efficacy of psychological nursing on patients with pregnancy-induced hypertension (PIH) combined with obstructive sleep apnea hypopnea syndrome (OSAHS).Methods 96 patients with pregnancy-induced hypertension (PIH) combined with OSAHS from January 2009 to July 2012 were admitted.They were randomly divided into the experimental group and the control group with 48 cases in each group.All patients were given routine drug therapy,based on this,the control group received routine care,and the experimental group was given routine care and psychological nursing.The anxiety and depression score and the treatment effect were compared between the two groups.Results After eight-week treatment,in the experimental group,the scores of SAS,SDS declined obviously and the levels of blood pressure were significantly decreased compared with those in the control group.The difference was statistically significant.Conclusions Psychological nursing for patients with pregnancy-induced hypertension(PIH) combined with OSAHS played a positive role in nursing of these patients,it can remit their anxiety and depression emotion and also can significantly decrease the patients'blood pressure.
7.The impact of score and weight of regular grade on the total mark in medical microbiology

Objective To explore the influence of the scores and weights of the regular grade on the evaluation of students' learning effect.Method To compare the impact of the regular grade before and after adjustment on the total mark,analyze the problems exposed during scoring and search for the solutions according to Medical Microbiology results of four grades including Grade 2010 to 2013.SPSS 13.0 software was used for statistical analysis and Pearson method was used for correlation analysis and theory achievement scores.Results The regular grade of four grades scored highly,with the average (95.00 ± 3.80),(96.00 ± 4.55),(95.00 ± 2.84) and (95.00 ±-2.82) respectively.What was more,it had randomness.The correlation coefficient between regular grade and total mark were 0.069,0.149,0.984 and 0.285 respectively.The regular grade of Grade 2010 was the same as Grade 2013 and the theoretical score of Grade 2013 was 4 points lower than Grade 2010(71 vs.75),however the total mark of Grade 2013 was 1 point higher than Grade 2010 (80 vs.79),which showed the more the regular grade weights,the greater it impacted on the total mark.Conclusion The appropriate score and weight of the regular grade is important to evaluate the students' learning effect objectively.
8.Evaluation of the capabilities of disc diffusion and Vitek2-compact GN13 methods for testing antimi-crobial susceptibility and screening of ESBLs Enterobacteriaceae clinical isolates

Objective To evaluate the capabilities of disc diffusion and Vitek2-compact GN13 methods for testing antimicrobial susceptibility of screening ESBLs ( extended-spectrumβ-lactamase) in En-terobacteriaceae clinical isolates.Methods A total of 93 Enterobacteriaceae strains were isolated from pa-tients with intra-abdominal infections in 21 hospitals during 2011 to 2012.The in vitro minimum inhibition concentration ( MIC ) values of ampicillin-sulbactam, piperacillin-tazobactam, ertapenem, ceftazidime, ceftriaxone, cefepime, imipenem, amikacin, ciprofloxacin and levofloxacin were determined by disc diffu-sion, Vitek2-compact GN13 and broth microdilution methods, respectively.Categorical agreement ( CA ) rates of disc diffusion and Vitek2-compact GN13 methods were determined by using broth microdilution meth-od as the reference method.The genes encoding ESBLs were screened in Escherichia coli (E.coli), Kleb-siella pneumoniae (K.pneumonia), Klebsiella oxytoca (K.oxytoca) and Proteus mirabilis (P.mirabilis) strains by using PCR analysis and gene sequencing.Disc diffusion and Vitek2-compact GN13 methods were used for the phenotypic confirmatory test of ESBLs and the sensitivity, specificity, positive predictive value and negative predictive value of the two tests were evaluated.Results The CA values of disc diffusion and Vitek2-compact GN13 methods for the 10 antibiotics were all >90% as compared with broth microdilution method.The major error (ME) rate for ertapenem was 3.2%and the very major error (VME) rates for am- picillin-sulbactam, ceftazidime and cefepime tests were all 2.2% by using Vitek2-compact GN13 method. The sensitivity, specificity, positive predictive value and negative predictive value of disc diffusion and Vitek2-compact GN13 methods in the phenotypic confirmatory test of ESBLs were 96.7%(29/30), 100%(20/20), 100%(30/30) and 95%(19/20), respectively.Conclusion Both disc diffusion and Vitek2-compact GN13 methods could be used for testing the antimicrobial susceptibility and the detection of ESBLs in Enterobacteriaceae clinical isolates with the advantage of accuarcy.Attention should be paid to the posibil-lity of oaurance of ME and VME when testing ertapenem, ampicillin-sulbactam, ceftazidime and cefepime by using Vitek2-compact GN13 method.
9.Surveillance of antimicrobial resistance of Haemophilus influenzae in Peking U-nion Medical College Hospital during 2008 and 2011

Objective To investigate the antimicrobial resistance of Haemophilus influenzae in Peking Union Medical College Hospital for rational clinical treatment.Methods A total of 223 strains of H .influenzae were collected from patients from Jan-uary 2008 to December 2011.The antimicrobial susceptibility was tested by Kirby-Bauer method.Production of beta-lactamase was detected using nitrocefin disks.WHONET 5.6 software was used to analyze the data of susceptibility testing.Results Tri-methoprim-sulfamethoxazole and tetracycline were the two antimicrobial agents to which the H .influenzae strains were most resistant.Theβ-lactamase positive ampicillin resistant strains accounted for 15.9 %,andβ-lactamase negative ampicillin resist-ant strains accounted for 8.9%.Conclusions H .influenzae is mainly isolated from respiratory specimens.Majority of the anti-microbial agents still show good antibacterial activity against H .influenzae strains.However,H .influenzae isolates are highly resistant to trimethoprim-sulfamethoxazole and tetracycline.
10.Observation and nursing of pregnancy-induced hypertension syndrome combined with obstructive sleep apnea hypopnea syndrome

Objective To analyze the clinical efficacy of nursing intervention on patients who were suffering from pregnancy-induced hypertension syndrome combined with obstructive sleep apnea hypopnea syndrome(OSAHS). Methods From April 2010 to December 2011,58 patients with pregnancy-induced hypertension combined with OSAHS were selected.They were randomly divided into the experimental group and the control group with 29 cases in each group.The control group received routine care and drug treatment,based upon this,the experimental group received pertinent nursing of OSAHS.The treatment effect was compared between the two groups. Results The total effective rate of the experimental group was significantly higher than the control group. Conclusions Pertinent nursing of OSAHS for patients with pregnancy-induced hypertension combined with OSAHS played a positive role in nursing of these patients,it can significantly decrease the blood pressure of patients.
