1.Review on classification of curative effect evaluation standard to modified Huangqi-Guizhiwuwu Tang on diabetic peripheral neuropathy

There are lack of specific treatment for diabetic peripheral neuropathy (DPN)right now.However, TCM intervention highlights the distinct advantage for the disease.Modified Huangqi-Guizhiwuwu Tang is one of the important recipes for that.But there are no uniform standards curative effect evaluation for it, and some clinical trials do not have some new evaluation standards.We summary the clinical research literatures in recent years and analyze the existing problems, therefore, provide new recommendations as views for medical researchers in the design of clinical trials.
2.Effects of Chinese Herbal Compounds on Levels of Blood Glucose, Froctosamine and Insulin of Diabetic Rats

Objective To observe the effects of Chinese herbal compounds (alkaloid of Coptidis Rhizoma 40%, total saponin of panax notoginseng 35%, and polysaccharide of Ophiopogonis Radix 25%) on levels of blood glucose, fructosamine, and insulin of diabetic rats, and explore mechanism of action.Methods Ten rats were randomly chosen as blank control group, and other rats were used to establish diabetic rat models by STZ intraperitoneal injection. Successfully molding rats were randomly divided into Chinese herbal compounds high, medium, low dose groups, metformin group, and model group, 10 rats in each group. Each rat received gavage for continuous 4 weeks. Then rat blood was collected by cutting head and taking caudal vein. FBG was detected by glucose oxidase method;insulin level was detected by radio-immunity double antibody method;fructosamine content was detected by fructosamine method.Results Compared with the blank control group, model group the levels of FBG, froctosamine content increased, while fasting insulin level were lower after 4 weeks'medication. Compared with the model group, the levels of FBG, froctosamine content in Chinese herbal compounds high and medium dose groups and metformin group were lower, while fasting insulin level increased after 4 weeks'medication (P<0.05).Conclusion TCM herbal compounds can increased the basic insulin level by decreasing the contents of FBG and fructosamine.
3.Expression of HMGB1, MMP-9 and VEGF-C in papillary thyroid carcinoma

Objective To study the expression of high mobility group box 1 (HMGB1),matrix metalloproteinase-9(MMP-9) and vascular endothelial growth factor-C (VEGF-C)gene in papillary thyroid carcinoma (PTC) and their significance.Methods Expression of HMGB1,MMP-9,VEGF-C proteins and HMGB1 mRNA was detected by technology of immunohistochemistry(SP) and in situ hybridization in 58 cases of PTC tissues,20 cases of thyroid adenoma tissues,25 cases of nodular goiter tissues and 10 cases of normal thyroid tissues.Results Expression of HMGB1,MMP-9,VEGF-C proteins and HMGB1 mRNA in PTC tissues was much higher than that in the 3 non-cancerous tissues (P < 0.05).The positive expression rate of HMGB1,MMP-9 proteins and HMGB1 mRNA was closely associated with the thyroid carcinoma size and lymph node metastasis (P <0.05).There was also closely association between the positive expression rate of VEGF-C protein and lymph node metastasis(P < 0.05).The positive expression rate of HMGB1,MMP-9,VEGF-C proteins and HMGB1 mRNA had no association with patients' sex or age(P > 0.05).The expression of HMGB1,MMP-9 and VEGF-C proteins had positive correlation with each other(P < 0.05).The expression of HMGB1 protein and HMGB1 mRNA was also positively correlated with each other(P <0.05).Conclusion Expression of HMGB1,MMP-9,VEGFC proteins and HMGB1 mRNA in PTC is correlated with the progress,invasion,metastasis of PTC,by detecting them may help to predict the clinical progress and prognosis.
4.Influence to the curative effect of THA with different methods to reconstruct limb length for Crowe Ⅳ type developmental dysplasia of the hip

Objective To explore the differences of the joint function and the patient's satisfaction between two different methods to reconstruct limb length for unilateral Crowe Ⅳ developmental dysplasia of the hip (DDH).Methods The clinical controlled study were used.21 cases with unilateral DDH Crowe Ⅳ treated with total hip arthroplasty were divided into 2 groups randomly.The patients in the first group were reconstructed the limb with the compensatory length and those in the second group were did with the real length.The patients were followed up 10 years.Two independent sample t-test was used to compare these two operations.Harris scores and SF-36 scores were used in the test.Survival analysis with Kaplan-Meier method was involved in the survival of the prosthesis and then test with Log-rank test.X-ray films of different period were compared to confirm the prosthetic loosening.Results Seventeen patients were followed up 8-10 years.There were no significant differences both in Harris score and most of the SF-36 scores at the median follow-up of 10 years.The item mental healthy of SF-36 expressed significant difference.Five patients required revision.There were 10 cases suffered with polyethylene wear,6 cases with severe osteoporosis in greater trochanter,and 3 cases with osteolytic reaction.Conclusion There are no significant differences between the two surgical method in the survival rate of prosthesis and the joint function in the median follow up.The satisfaction of the control groups is lower than the trial groups both at the early stage after the operation and the median follow up.The revision rate between the two groups is similar but the reason is different.
5.Effect of video-assisted thoracoscopic surgery in thoracic disease

Objective To investigate the effect of video-assisted thoracoscopic surgery( VATS) in thoracic disease,and the feasibility to carry out VATS for basic hospital. Methods The data of VATS treatment were collected to compare the differences between study group and control group,and evaluate the the feasibility to carry out VATS for basic hospital. Results The operation time was (100. 75±22. 72) min, blood loss was (54. 27±26. 21) mL,postoperative drainage was (920. 67±171. 99) mL. The postoperative complications were fewer,post-operative hospital stay was shorter,incision time was shorter(P=0. 000) and pain scores was lower(P=0. 000) in study group than that in control group. Basic hospital has the capacity to conduct this technical. Conclusion VATS is feasible to carry out in basic hospital.
6.Comparative study of routine clinical imaging examinations in early diagnosis of micro-hepatocellular carcinoma

Objective To investigate the early diagnostic ability of conventional imaging methods in micro-hepatocellular carcinoma(M HCC) in order to improve the imaging diagnostic procedure for M HCC.Methods The data of 40 cases of M HCC(52 lesions)and 21 cases of regenerative nodules(35 lesions)confirmed by clinical results were collected continuously.All the patients were examined with ultrasonography (US),plain CT combined with multi-phase enhancement scans,and plain MRI combined with diffusion weighted imaging(DWI).To compare the diagnostic efficacy of three different imaging methods,the imaging data were analyzed retrospectively by χ2test and receiver operating characteristic curve(ROC).Results There was a significant difference in the M HCC detection rate between US and CT,and between US and MRI,respectively(P<0.05).There was no significant difference in the MHCC detection rate between CT and MRI(P>0.05).The sensitivity of US,CT and MRI in the detection of MHCC was 61.5%,80.8% and 92.3%,specificity was 88.6%,91.4% and 97.1%,positive predictive value was 88.9%,93.3% and 98.0%,and area under the ROC curve was 0.751, 0.861 and 0.947,respectively.Plain MRI combined with DWI had the highest diagnostic efficacy,among the three imaging methods. Conclusion US can be used as a primary screening method for M HCC in high risk population,and plain CT combined with multi-phase enhancement scans can be the first choice for early diagnosis of MHCC.For highly suspected the cases with positive AFP but negative findings on US and CT,plain MRI combined with DWI are recommended to rule out the possibility.

8.HPLC characteristic fingerprint of Moghania philippinensis root.

OBJECTIVETo study HPLC characteristic fingerprint of Moghania philippinensis and provide a reliable method for scientific evaluation and quality control.
METHODThe chromatographic separation was performed on a Kromasil C15 column (4.6 mm x 250 mm, 5 microm). The mobile phase consisted of acetonitrile and water containing 0.3% acetic acid in gradient elution. The column temperature was set at 40 degrees C and the detection wavelength was at 254 nm.
RESULTThe developed HPLC fingerprint method above was applied to analyze 23 batches of crude herbs of Moghania philippinensis. The total 22 peaks were selected as the characteristic peaks. And five of them were identified as genistein, genistin, 5,7,3',4'-tetrahydroxy-6,8-diprenylisoflavone, 5,7,4'-trihydroxy-6,8-diprenylisoflavone and flemiphilippinin E by matching their retention time, UV spectra data with those of the authentic chemical reference substances.
CONCLUSIONThe method is simple, accurate and reproducible. It is suitable for the quality control of M. philippinensis.

9.Overview on quantitative analysis of multi-components by single-marker.

10.Everted intestinal sac method for quick finding absorption ingredients of Wuzhuyu decoction.

OBJECTIVETo establish a method for quick finding the absorption ingredients of Wuzhuyu decoction in order to select the index to control its quality.
METHODThe absorption of three concentration of Wuzhuyu decotion was investigated with the in vitro-everted intestinal sac model. The intestinal bag fluid of jejunum and ileum were collected in different time and the eight ingredients, which were evodiamine (Ev), rutaecarpine (Ru), limonin (Li), ginsenoside-Rb1, -Rg1, -Re (Rb1, Rg1, Re), isorhamnetin-3-O-beta-D-glucosyl(6''-->1'")-alpha-L-rhamnoside (Irs)and 6-gingerol (6-Gi), were detected by HPLC as the represent constituents in samples.
RESULTEight ingredients except Ru in samples could be detected, but Ev could not be detected in high concentration samples. The ratios between absorption ingredients were different from in Wuzhuyu decotion.
CONCLUSIONThe in vitro-everted intestinal sac canc absorb the ingredients of Wuzhuyu decotion selectivity. Compare with the ileum, the jejunum can provide the more absorption information and faster, the best test time is 60-90 min.
