1.Analysis of nine cases of acute thallium poisoning.

In this study nine cases of thallium poisoning in a series of homicidal poisoning were analyzed in order to provide more information concerning thallium poisoning. It was found that the most common clinical feature of thallium poisoning was peripheral neuropathy and paraesthesia was more common than amyasthenia. Understanding of these clinical characteristics of thallium poisoning was helpful to early identification and differential diagnosis. Since the early administration of Prussian Blue, as a specific antidote for thallium poisoning, can substantially improve the prognosis, it is of great importance to establish a correct and early diagnosis.
2.Effect of a pedicle screw augmentation with bone cement perfusion on the long segment fixation and fusion in elderly patients with spinal deformity treated by osteotomy

Objective To investigate the effect of pedicle screw augmentation with bone cement perfusion on the long segment fixation and fusion in the elderly patients with spinal deformity treated by osteotomy.Methods The cohort data of the older patients aged ≥70 years with spinal kyphosis were retrospectively analyzed in Beijing Hospital,Department of Orthopedic Surgery,Division of Spine Surgery from January 2012 to June 2014.32 cases with complete clinical date were included in this study.The kyphosis of all patients was secondary to thoracolumbar osteoporotic compression fracture.All patients were treated by posterior thoracolumbar spinal osteotomy with long segment fixation and fusion.The patients with fusions including sacral vertebral body fusion or the T score of BMD were less than-5 were excluded.Some patients with neurological compression symptoms also underwent selective decompression in spinal canal and nerve root foramen.All patients were divided into two groups:with (n=14)versus without(n=18)screw augmentation with bone cement perfusion(augmentation vs.non-augmentation/control group).The follow-up time was no less than 12 months.The data of age,gender,operative time,blood loss volume and preoperative bone mineral density were compared between two groups.Cobb angle of postoperative spinal surgery region,lordosis angle of postoperative lumbar,VAS and ODI improvement were analyzed.Data of surgical fixation and fusion segments were recorded.Pedicle screw loosening was compared between two groups.Results In the augmentation group,the pedicle screw loosening didn't occur.But control group showed screw loosening in 7 cases,a euphoric zone around screw in 4 cases,pedicle screw pullout in 2 cases,and vertebral cutting phenomenon with euphotic zone around screw in 1 case.There were no significant differences in ODI improvement rate and surgical satisfaction between the two groups.Conclusions When operative procedures of surgeon are in precision and skillful,bone cement augmentation technique for pedicle screw can reduce the occurrence of pedicle screw loosening in older patients during osteotomy and long segment fixation and fusion operation for spinal kyphosis.
3.Early predictive factors for intestinal necrosis in acute superior mesenteric artery occlusive diseases

Objective To explore the early predictive factors of intestinal necrosis in patients with acute superior mesenteric arterial occlusive disease and its significance for the decision of exploratory laparotomy.Methods This retrospective study enrolled 29 patients diagnosed with acute superior mesenteric artery embolism or thrombosis in Peking University People's Hospital between July 1995 and June 2015.Results 12 patients developed intestinal necrosis.Patients with intestinal necrosis had a poorer prognosis than those who did not develop intestinal necrosis (x2 =14.867,P =0.000).In univariate analysis,the early predictive factors for intestinal necrosis were D-Dimer ≥ 600 μg/L (x2 =11.455,P =0.002),INR≥1.2 (x2 =3.948,P =0.047),pH values <7.4 (x2 =8.191,P =0.004),BE < -1.0 mmol/L (x2 =8.191,P =0.004),blood lactate ≥ 2.2 mmol/L(x2 =7.535,P =0.006),BUN ≥ 6 mmol/L (x2 =10.076,P =0.002),CK ≥ 80 U/L (x2 =8.191,P =0.004),LDH ≥ 210 U/L (x2 =13.079,P=0.000),AST ≥25 U/L (x2 =10.076,P =0.002),SIRS (x2 =10.076,P =0.002).Multivariate logistic regression analysis found no independent predictive factors of intestinal necrosis in patients with acute superior mesenteric arterial occlusive diseases.Conclusion Intestinal necrosis in acute mesenteric arterial occlusive diseases indicates a poor prognosis.Coagulation abnormalities,liver or kidney dysfunction,metabolic acidosis and SIRS necessitates an immediate exploration.
4.Clinical Efficacy of Foam Sclerotherapy Combined with Melilotus Extract Tablets for Varicose Veins of Lower Extremities

Objective To compare the short-term clinical effect of the foam sclerotherapy combined with Melilotus Extract Tablets vs foam sclerotherapy alone in treating varicose veins of the lower extremities.Methods A retrospective cohort study was carried out in 120 cases receiving foam sclerotherapy in the Invasive Technology Department of Panyu District Central Hospital.Seventy-two patients in trial group wore graduated compression stockings and received oral use of Melilotus Extract Tablets after the operation,while 48 cases in the control group only wore graduated compression stockings after the operation.The follow-up for the two groups lasted for 3 months.We recorded the relief of symptoms (pain,soreness and distention) and physical signs (edema,hyperpigmentation,ulcer) after the operation.The grading levels of clinical-etiology-anatomic-pathophysiologic classification system (CEAP) for lower limb venous disease,venous clinical severity scores (VCSS) and disease quality-of-life questionnaire CIVIQ scores were observed before and after the operation.Results (1) Three months after the operation,the illness state of the two groups was improved to certain degree,and the differences of CEAP grading levels were insignificant(P > 0.05).(2) VCSS and CIVIQ scores of the two groups were improved after the operation (P < 0.01 compared with those before the operation),and the trial group had better effect on improving CIVIQ scores than the control group (P < 0.01).(3) The successful rate of the trial group was 88.89%,and that of the control group was 81.25%,the difference being insignificant (P > 0.05).(4) Time for obvious relief of lower limb symptom in the trial group was shorter than that in the control group (P < 0.05),but the time for hyperpigmentation and ulcer disappearing as well as the medical expenses in the trial group did not differ from those in the control group (P > 0.05).Conclusion Foam sclerotherapy combined with Melilotus Extract Tablets is effective,safe and economical for the treatment of varicose veins of the lower extremities,and its efficacy is superior to foam sclerotherapy alone.
5.Clinical effect of long-segmental fixation and fusion on degenerative scoliosis in the elderly

Objective To investigate the surgical curative effect of long-segment fixation and fusion on the degenerative scoliosis (DS) in the elderly.Methods The clinical data of 27 patients with DS undergoing surgical treatment from January 2011 to December 2013 in our department in Beijing Hospital were retrospectively analyzed.All patients suffered from degenerative scoliosis accompanied with lumbar canal stenosis, lumbar spondylolisthesis, lumbar vertebrae lateral slip in coronal surface, spinal vertebral rotation deformity and kyphosis.The decompression osteotomy and fixed segment, blood loss volume and operation time were recorded.The differences in scoliosis angle (Cobb angle), lumbar lordotic angle, top vertebral rotation degree (Nash-Moe vertebral rotation grade), apical vertebral offset distance, visual analogue scale (VAS) score and Oswestry disability index (ODI) score were compared between pretreatment versus the last follow-up.Results The selective 1-5 segment decompression and 5-10 segment fusion were selected in a total of 27 patients.Some patients were treated with Smith-Petersen osteotomy (SPO).All of 27 cases were treated with a lamina and inter-transverse fusion, among which 25 cases took inter-vertebral fusion additionally.The time of postoperative follow-up was more than 1 year.The Cobb angle, lumbar lordotic angle, apical vertebral offset distance, VAS and ODI scores were improved after treatment as compared with pretreatment [(11 6) vs.35 6 , (32±10) vs.(17±9) , (16.3±8.2) mm vs.(32.2±9.8) mm, (3.3±1.6) vs.(7.3±2.1), (18±14) vs.(33±14), t=2.469, 2.313, 2.331, 2.362, 2.395, P=0.021, 0.030, 0.029, 0.026, 0.023 respectively].The satisfaction survey result was excellent in 19cases, good in 5 cases, fair in 3 cases.Complications were found in 9 patients, among whom 2 cases had screw loosening in the internal fixation, 2 cases had adjacent segment degeneration, 1 case had pulmonary infection, 1 case had urinary tract infection, 2 eases had spinal fluid leakage, 1 case had epidural hematoma.No serious complications and deaths were observed.Conclusions Under strictly controlling the therapeutic indications, the limited decompression and long-segmental fixation and fusion have good therapeutic efficacy on degenerative scoliosis in elderly patients.
6.Investigation on Resources of Medicinal Plant of Euodiae Fructus

Objective To understand the distribution condition of resources of medicinal plant of Euodiae Fructus;To provide references for resource conservation and standardized plantation of Euodiae Fructus. Methods Literature search, interview survey and field survey were conducted to investigate the resource conditions of the main producing areas of Euodiae Fructus. Samples were collected and TKW of the medicinal plant was weighted. Results Currently, the main producing areas of Euodiae Fructus are provinces of Jiangxi, Guizhou, Hubei, Hunan and part of provinces and places of Guangxi, Anhui, Zhejiang, Shanxi, and Chongqing. Commercial Euodiae Fructus mainly comes from private investors. The wild resources of Euodiae Fructus are few. The TKW of the medicinal plant is within 5.34-45.62 g, with great differences. The national annual output of cultivated Euodiae Fructus is around 3500 t. The production and marketing is in balance. The price for Euodiae Fructus is relatively stable. Conclusion The plantation of Euodiae Fructus is dispersing, with small scale, low degree of standardization and intensification, and great quality differences. Standardized, large-scale, and industrialized plantation base should be established, and the assessment system for medicine quality should be perfected, with a purpose to ensure the qualified and stable production of Euodiae Fructus. Meanwhile, wild resource production should be enhanced to maintain the biological diversity and promote the sustainable development of Euodiae Fructus.
7.Effect of video-assisted thoracoscopic surgery in thoracic disease

Objective To investigate the effect of video-assisted thoracoscopic surgery( VATS) in thoracic disease,and the feasibility to carry out VATS for basic hospital. Methods The data of VATS treatment were collected to compare the differences between study group and control group,and evaluate the the feasibility to carry out VATS for basic hospital. Results The operation time was (100. 75±22. 72) min, blood loss was (54. 27±26. 21) mL,postoperative drainage was (920. 67±171. 99) mL. The postoperative complications were fewer,post-operative hospital stay was shorter,incision time was shorter(P=0. 000) and pain scores was lower(P=0. 000) in study group than that in control group. Basic hospital has the capacity to conduct this technical. Conclusion VATS is feasible to carry out in basic hospital.

9.Removal of plumbum ion from the extract of Lonicera japonica by gamma-mercaptopropyl-modified silica gel.

OBJECTIVETo remove plumbum ion from the extract of Lonicera japonica by gamma-mercaptopropyl-modified silica gel (MPS).
METHODThe reference plumbum ion solution was added into the extract of L. japonica as the tested samples. The static and dynamic adsorptions were investigated to evaluate the influent factors on the removal of plumbum ion in the extract. The solid content and HPLC profiles of the extract were determined to evaluate the process changes.
RESULTMPS showed excellent adsorption charateristics to remove plumbum ion from the extract of L. japonica, with fast adsorption rate and more than 80% removal efficiency. The optimum parameters were as follows: 6 times sample volume, 10 times of height to diameter, and 6 BV x h(-1) of flow velocity, at the temperature of 25 degrees C. During the removal process, obvious changes of solid content and HPLC profiles were not observed.
CONCLUSIONMPS could be used to remove plumbum ion from extract of L. japonica.

10.Surgical site infection associated complications in colorectal cancer patients

Objective The purpose of this study is to investigate the factors associated with incisional surgical site infection (SSI) in colorectal cancer. Methods We retrospectively analyzed impacting factors of SSI including age, gender, BMI, diabetes mellitus, operation, albumin levels, hemoglobin levels, tumor staging, and length of stay post-operation in 327 colorectal patients from 2001 to 2005. Results The incidences of SSI and fat liquefaction were 2. 4%(8/327) and 12. 2%(40/327) respectively. The incidences of SSI in patients with obesity and diabetes mellitus increased significantly (16. 8% vs. 10. 0% ,28. 6% vs. 13. 7%). The length of stay in patients with SSI prolonged significantly (29.0 d vs. 15.8 d). Conclusion Obesity and diabetes mellitus are the most important factors associated with SSI in colorectal cancer patients.
