1.The committed differentiation of bone marrow mesenchymal stem cells into tenocytes

Objective To investigate the feasibility of committed differentiation of rhesus bone marrow mesenchymal stem cells (MSCs) into tenocytes induced by BMP12. Methods MSCs were transfected with pTARGETTM bearing BMP12 gene by electroporation. The transfected cells were identified by morphological observation and molecular biological measure. Results Under the observation of light microscope, the morphological features of transfected cells changed significantly compared to parental MSCs. RT-PCR data showed the transfected cells had the mRNA expression of BMP12 and collagen Ⅰ, but without that of collagen Ⅲ. 98.39% of the transfected cells were CD44+ and negative for HLA-DR. Conclusion BMP12 could induce MSCs into tenocytes, and bone marrow MSCs might be the optional seed cells for tendon tissue engineering.
2.A preliminary evaluation of VISER testing in the diagnosis of ED

Objective To evaluate VISER as a non invasive monitoring of penile cavernosal pressure for the diagnosis of ED. Methods With the use of AVSS and papaverine ICI,39 cases,including 32 patients of ED and 7 controls,were tested with VISER device. Results 7 controls and 13 ED cases (40%) have achieved a valid erection by AVSS alone while another 13 ED cases required papaverine ICI (10 mg) for successful erection whereas 6 of ED cases failed what so ever. The VISER testing showed the erection data and peak data indexes, especially in total energy of erection and peak, were higher in controls than ED groups except the average duration of erection being longer in ED papaverine group. Conclusions VISER is an accurate system for measuring the changes of intracavernosal pressure in dynamic state during penile tumescence. The use of eye glasses TV monitor can help enhance the effectiveness of AVSS and lower the dosage of vasoactive drugs.VISER testing is non invasive,accurate and easy to perform and might serve as a screening test for ED patients in the near future.
3.Clinical Observation of Nasal Continuous Positive Airway Pressure Ventilation in the Treatment of Neonatal Respiratory Failure

Objective To explore the clinical efficacy of nasal continuous positive airway pressure ventilation and conventional mechanical ventilation in the treatment of neonatal respiratory failure (NICU).Methods 68 cases of neonatal respiratory failure in our hospital from June, 2014 to June, 2016 in neonatal intensive care unit, according to the random number table method were divided into two groups: The observation group involved 34 patients with nasal continuous positive airway pressure, the control group 34 patients with conventional mechanical ventilation treatment.The changes of arterial blood gas analysis, clinical effect and complication were compared between the two groups.Results The PCO2 of the two groups was significantly decreased (P<0.05), and the observation group was higher than the control group (P<0.05).There was no significant difference in PO2 between the two groups (P>0.05).However, pH value was significantly increased, and the observation group was smaller than the control group (P<0.05).The total effective rate was 94.12% in the observation group and 67.65% in the control group (P<0.05).The time of ventilation and incidence of comorbidities in the observation group was significantly shorter than that in the control group (P<0.05).Conclusion Nasal continuous positive airway pressure ventilation for treatment of neonatal respiratory failure can more significantly improve the efficacy than conventional mechanical ventilation, shorten the time on the machine and reduce the relevant complications.So it is worth promoting.
4.Application of PBL in theory teaching of pediatrics

Objective To promote teaching qualities of pediatrics,problem-based learning (PBL)and lecture-based learnin(gLBL)were contrasted,and effects of the new teaching method were evaluated.Methods 58 medical students of English class Grade 2004 were chosen and divided into two groups,namely,PBL group,LBL group,each taught through the corresponding method in the chapter Rickets of Vitamin D Deficiency.At the end of the course,an objective exam was taken and the scores,checkout rate and excellent rate were used to assess the teaching quality.Results The scores and checkout rate of case analysis of the students in the PBL group are higher than that in the LBL group.Conclusion The approach of PBL can elevate the students'clinical thinking capability.
5.DISTRIBUTION AND MORPHOLOGIC CHARACTERISTICS OF ACETYCHOLINESTERASE——CONTAINING NEURONS IN THE RAT BRAIN

Distribution and morphologic features of AChE-containing neurons were observed by the technique of AChE regeneration. There are three kinds: AChE-staining cells--heavily stained cells, medium stained cells and lightly stained cells. Most heavily stained cells are larger multipolar cells. They are located mainly in striatum, basal forebrain, hypothalamus, substantia nigra, locus coeruleus, red nucleus, ventral tegmental nucleus, parabrachial nucleus, pontine tegmental nucleus and the motor nuclei of cranial nerves. The results of AChE-staining were compared with the date of ChAT immunohistochemistry. The relationship between AChE and cholinergic neurons as well as the nature of AChE-containing neurons were discussed.
6.Clinical Efficacy of Foam Sclerotherapy Combined with Melilotus Extract Tablets for Varicose Veins of Lower Extremities

Objective To compare the short-term clinical effect of the foam sclerotherapy combined with Melilotus Extract Tablets vs foam sclerotherapy alone in treating varicose veins of the lower extremities.Methods A retrospective cohort study was carried out in 120 cases receiving foam sclerotherapy in the Invasive Technology Department of Panyu District Central Hospital.Seventy-two patients in trial group wore graduated compression stockings and received oral use of Melilotus Extract Tablets after the operation,while 48 cases in the control group only wore graduated compression stockings after the operation.The follow-up for the two groups lasted for 3 months.We recorded the relief of symptoms (pain,soreness and distention) and physical signs (edema,hyperpigmentation,ulcer) after the operation.The grading levels of clinical-etiology-anatomic-pathophysiologic classification system (CEAP) for lower limb venous disease,venous clinical severity scores (VCSS) and disease quality-of-life questionnaire CIVIQ scores were observed before and after the operation.Results (1) Three months after the operation,the illness state of the two groups was improved to certain degree,and the differences of CEAP grading levels were insignificant(P > 0.05).(2) VCSS and CIVIQ scores of the two groups were improved after the operation (P < 0.01 compared with those before the operation),and the trial group had better effect on improving CIVIQ scores than the control group (P < 0.01).(3) The successful rate of the trial group was 88.89%,and that of the control group was 81.25%,the difference being insignificant (P > 0.05).(4) Time for obvious relief of lower limb symptom in the trial group was shorter than that in the control group (P < 0.05),but the time for hyperpigmentation and ulcer disappearing as well as the medical expenses in the trial group did not differ from those in the control group (P > 0.05).Conclusion Foam sclerotherapy combined with Melilotus Extract Tablets is effective,safe and economical for the treatment of varicose veins of the lower extremities,and its efficacy is superior to foam sclerotherapy alone.
7.Influence to the curative effect of THA with different methods to reconstruct limb length for Crowe Ⅳ type developmental dysplasia of the hip

Objective To explore the differences of the joint function and the patient's satisfaction between two different methods to reconstruct limb length for unilateral Crowe Ⅳ developmental dysplasia of the hip (DDH).Methods The clinical controlled study were used.21 cases with unilateral DDH Crowe Ⅳ treated with total hip arthroplasty were divided into 2 groups randomly.The patients in the first group were reconstructed the limb with the compensatory length and those in the second group were did with the real length.The patients were followed up 10 years.Two independent sample t-test was used to compare these two operations.Harris scores and SF-36 scores were used in the test.Survival analysis with Kaplan-Meier method was involved in the survival of the prosthesis and then test with Log-rank test.X-ray films of different period were compared to confirm the prosthetic loosening.Results Seventeen patients were followed up 8-10 years.There were no significant differences both in Harris score and most of the SF-36 scores at the median follow-up of 10 years.The item mental healthy of SF-36 expressed significant difference.Five patients required revision.There were 10 cases suffered with polyethylene wear,6 cases with severe osteoporosis in greater trochanter,and 3 cases with osteolytic reaction.Conclusion There are no significant differences between the two surgical method in the survival rate of prosthesis and the joint function in the median follow up.The satisfaction of the control groups is lower than the trial groups both at the early stage after the operation and the median follow up.The revision rate between the two groups is similar but the reason is different.
8.Evaluation of clinical efficacy of high risk prostate cancer with the treatment of continuous and intermittent androgen deprivation

Objective To compare the clinical efficacy between continuous and intermittent androgen deprivation in high risk prostate cancer.Methods Sixty-four patients with high risk prostate cancer were treated from January 2008 to April 2009,36 cases who accepted goserelin and bicalutamide were taken as intermittent hormonal therapy (intermittent treatment group),while 28 cases who accepted bilateral orchiectomy in addition to flutamide were regarded as continuous hormonal therapy (continuous treatment group).The comparison of tumor specific mortality,time of prostate specific antigen (PSA) to nadir,tine to PSA recurrence,serum testerone and quality of life score were assessed between the two groups.Results In continuous treatment group and intermittent treatment group,follow-up period was (26.4 ± 10.3) and (28.1 ± 8.7) months,the time of PSA to nadir was (3.8 ± 2.1 ) and (4.0 ± 3.6) months,the time to PSA recurrence was (20.1 ± 12.3) and (24.5 ± 14.6) months,respectively.There was no significant difference between the two groups.At the time of 18,24 and 30 months after therapy,serum testerone was 0.85,0.88,0.89 μg/L in continuous treatment group,while 1.21,1.36,1.48 μg/L in intermittent treatment group,respectively (P < 0.05 ).Similarly,quality of life score was 38.7,40.5,39.8 scores in continuous treatment group,while 49.2,51.4,52.3 scores in intermittent treatment group at the time of 12,18 and 30 months after therapy,respectively (P < 0.05 ).Conclusions Clinical efficacy could not been found between continuous and intermittent endocrinic therapy of prostate cancer.During intermittent,quality of life seems to be better and increases in accordance with serum testerone recurrence at given time.

BACKGROUND: The increasing prevalence of diabetes mellitus has been become one of the diseases which threaten the heath of human being in the 21st century. Islet transplantation is considered to be the most effective approach to cure type Ⅰ diabetes mellitus. However, lack of donor tissue limits the application of this therapy. However, recent progress of stem cell research shows that stem cell therapy may be a potential means to solve this problem.OBJECTIVE: To take activin A and all-trans retinoic acid (AR) in inducing the differentiation of bone marrow mesenchymal stem cells (MSCs) and explore its possibility DESIGN: A randomized controlled experiment.SETTING: Institute of Biotechnology, Academy of Military Medical SciencesMATERIALS: This experiment was conducted at the Institute of Biotechnology, Academy of Military Medical Sciences from November 2004to June 2005. Six male Sprague-Dawley rats, with body mass of 150-160g, were provided by the Experimental Animal Center of Academy of Military Medical Sciences.METHODS: Femoral bone marrow of the rats was extracted under aseptic condition. Bone marrow mesenchymal stem cells (MSCs) were isolated with density gradient centrifugation. Passaged MSCs were randomly divided into 4 groups: high concentration of glucose (HG), AR, beta-mercaptoethanol (ME) and negative control groups. MSCs were induced to differentiate into IPCs with conditional medium containing high concentration glucose, activin A, RA and ME etc. After induction, phenotypes of differentiated cells were examined by immunocytochemistry and reverse transcription-polymerase chain reaction (RT-PCR).MAIN OUTCOME MEASURES: Expression of insulin and glucagon of differentiated cells were examined by immunocytochemistry. Insulin-1 mR-NA expression of differentiated cells was detected by RT-PCR.RESULTS: After bone marrow mesenchymal stem cells were induced,there were scattered insulin-and glucagon-positive cells in the HG group,many insulin-and glucagon-positive cells in the AR and ME groups, and these cells formed insulin-like structure. The expression of insulin-1mRNA could be observed in the HG, AR and ME groups. Insulin-and glucagonpositive cells and the expression of insulin-1mRNA were not observed in the negative control group.CONCLUSION: We adopt an induction scheme based on AR and other matured factors, and successfully make bone marrow mesench.ymal stem cells induce and differentiate into insulin positive reaction cells and form insulin-like structure, but its induction efficiency needs further improvement.
10.Discussion on the operative time of two stage soft ureteroscope lithotripsy for hemorrhagic embolism after percutaneous nephrolithotomy

Objective To investigate the the operative time of two stage soft ureteroscope lithotripsy for hemorrhagic embolism after percutaneous nephrolithotomy(PCNL).Methods The clinical data of 1 patient with massive hemorrhage after PCNL who treated with superselective renal artery embolization and ureteral soft lens were analyzed.Results The patient with postoperative bleeding after PCNL was treated with superselective renal artery branch embolization,after 30d embolization,the holmium laser lithotripsy under the soft ureteroscope for ureteral calculi was performed,and the renal pelvis mucosa smooth,no bleeding and scar formation were intraoperative visible.After operation,the stone was removed basically,and the double J tube was removed 2 weeks after operation.The patient had no special discomfort and the renal function was normal.Conclusion Postoperative 14-30d is a relatively safe time to perform flexible ureteroscopic lithotripsy for super selective renal artery embolization in the treatment of patient with massive hemorrhage after PCNL.
