1.A preliminary study on relationship between early awaking and serun cortisol in depression disorder

Objective To explore biological elements of early awaking in depression.Methods The plasma concentration of cortisol was measured by immunoradioassay in the morning in 59 depressive patients,and how many hours the depressive patients awake earlier than normal was evaluated.Results The plasma concentration of eortisol in the morning of early awaking group Was hisher than no early awaking group[(377.32±14.54)μm/L vs(324.15±16.44)μm/L,P<0.05].There Was rectilinear correlation between the hours that the pafients awake earlier than normal and the plasma concentration of cortisol in the moming(r=0.353.P=0.006.n=59).There was linear regression between the hours that the patients awake earlier than normal and the plasma concentration of cortisol in the morning(β=0.006,P=0.006,n=59).Conclusion The high plasma concentration of eortisol in the morning probably leads to early awaking in depression.
2.A comparative study of efficacy between pregabalin and gabapentin in treatment of patients with diabetic peripheral neuropathic pain

ObjectiveTo compare the efficacy and safety of pregabalin and gabapentin in treatment of patients with diabetic peripheral neuropathic pain.MethodsSixty patients with diabetic peripheral neuropathic pain were allocated into pregabalin group(30 cases ) and gabapentin group (30 cases) for a 4-week period treatment by random digits table method.The efficacy was measured with visual analogue scale (VAS) score,while the side effects were evaluated.ResultsVAS score in both groups was significantly lower at the end of 1st,2nd or 4th week after treatment than that before treatment [pregabalin group:( 4.05 ±0.93),(2.73 ±0.72),(2.06 ±0.58) scores vs.(7.45 ±0.82) scores; gabapentin group:(5A2 ±0.88),(2.93 ± 0.80),(2.19 ± 0.64) scores vs.(7.68 ± 0.84) scores] (P < 0.01 ).VAS score in pregabalin group was lower than that in gabapentin group at the end of 1st week significantly (P< 0.01 ).Not only there was no significant difference in VAS score at the end of 2nd or 4th week between two groups,but also in response rates at the end of 4th week (P > 0.05 ).The rate of adverse reaction in pregabalin group was significantly lower than that in gabapentin group [16.67% (5/30) vs.36.67%( 11/30)] ( P < 0.01 ).ConclusionPregabalin is as effective as gabapentin in treatment of patients with diabetic peripheral neuropathic pain,but it has faster effect and less adverse events than gabapentin.
3.Designing A New Chinese Mandarin Version of Speech Sample Text for Perceptual Assessment of Voice Disorders

Objective To design a new Chinese mandarin version of speech sample text for perceptual assessment .Meth-ods The speech sample text included all phonemes and limit number of words .The correlation analysis of the constituent rati-os of all phonemes was performed between the speech text and the ones in Chinese language reported by the institute of Acous-tics Chinese Academy of Sciences using Pearson's correlation coefficient and Spearman correlation coefficient .And their differ-ences were tested by paired sample t test and Wilcoxon signed rank test .Legibility was alsotested .Results Our speech sarrple text contained 21 vowel ,38 consonant and 4 tones .The proportions of vowel ,consonant and tone in the speech text had high correlation with those in Chinese language (for proportions of vowel ,the Pearson's correlation coefficient was 0 .908 and Spearman correlation coefficient was 0 .775 ,P<0 .01 ,respectively ;for proportions of consonant ,the Pearson's correlation co-efficient was 0 .802 and Spearman correlation coefficient was 0 .808 ,respectively ,P<0 .01 ;for proportions of tone ,the Pear-son's correlation coefficient was 0 .967 and Spearman correlation coefficient was more than 0 .999 ,respectively , P< 0 .01) . There was no difference in the proportions of vowel ,consonant and tone between them(P>0 .2) .The value of legibility was 2 , indicating legibility of the speech sample text was good .Conclusion The new Chinese mandarin version of speech sample text can be used in perceptual assessment .The value for perceptual assessment still needs further clinical research .
4.Influences of different tube voltages on the dose and image quality of a full field digital mammography

Objective To evaluate the effect of different tube voltages on the radiation dose and image quality of a full field digital mammography system.Methods Both experiments were performed under manual exposure mode.In one experiment,the tube voltages were kept constant at 25,28 and 31 kVp,and the CIRS 015 phantom was exposed at three target/filter combinations molybdenum/molybdenum (Mo/Mo),molybdenum/rhodium (Mo/Rh),and rhodium/rhodium (Rh/Rh) and at tube current-exposure time products of 32-140mAs.The entrance skin exposure (ESE) and average glandular dose (AGD) were recorded,the signal-to-noise ratio(SNR) and contrast-to-noise ratio (CNR) and figure of merit (FOM) were measured to evaluate the influences of different tube voltages on dose and image quality at same target/filter combination.The univariate of randomized completed block-design was used for statistics.In second experiment,the phantom were exposed using Mo/Rh combination when the tube current-exposure time product was kept constant at 56 mAs,and the tube voltage varied between 23 and 33 kVp in 1-kVp increments.The ESE and AGD were recorded,the SNR,CNR and FOM were measured for plotting the curves against tube voltages.Results At Mo/Mo combination,the AGD,ESE,SNR,CNR and FOM of 25,28 and31 kVp were(1.25 ±0.56) mGy,(6.46±2.86) mGy,71.52±8.37,1.91 ±0.26,3.21 ± 0.68; (1.94 ±0.85) mGy,(9.18±4.07) mGy,144.46 ± 11.31,2.41 ±0.28,3.37 ±0.96 and (3.01 ±1.38) mGy,(12.60±5.59) mGy,128.89 ±15.29,2.47 ±0.31,2.31 ±0.76 respectively; at Mo/Rh combination were (1.23 ±0.55) mGy,(5.26 ±2.33) mGy,67.31 ±4.11,1.82 ±0.19,3.01 ± 0.82; (1.86 ±0.84) mGy,(7.57 ±3.34) mGy,139.54 ± 12.16,2.30 ±0.25,3.23 ±0.92 and (2.81 ±1.24) mGy,(10.48 ±4.62) mGy,127.77 ±15.14,2.59 ±0.31,2.67 ±0.68; and at Rh/Rh were(1.09 ±0.48) mGy,(4.89 ±2.16) mGy,67.46 ±2.23,1.48 ±0.72,3.08 ± 1.69; (1.75 ± 0.78) mGy,(6.88 ±3.03) mGy,137.74 ± 14.65,2.37 ±0.26,3.62 ± 1.07 and (2.59 ± 1.13) mGy,(9.32 ± 4.12) mGy,117.91 ± 19.05,2.51 ± 0.31,2.74 ± 0.84.Both experiments indicated that,for a given target/filter combination,the AGD,ESE and CNR increased,but the ESE/AGD decreased with the tube voltage increasing; The first experiment indicated both SNR and FOM of 28 kVp were higher than that of 25 kVp and 31 kVp; the second experiment showed both SNR and FOM decreased with tube voltages increasing.The differences of AGD、ESE、ESE/AGD、SNR、CNR and FOM among the three tube voltages were significant (F =4.391-528.848,P < 0.05) ; but the difference of CNR between 28 and 31 kVp at Mo/Mo and Rh/Rh had no statistical significance (P > 0.05) ; and the differences of FOM between 25 and 28 kVp at the three target/filter combinations were not statistically significant (P > 0.05).Conclusion For a breast with 4.2 cm thickness and 50% adipose 50% glandular composition,different tube voltages have significant effects on the radiation dose and image quality.

Objective Respectively applying the treatment of biventricular pacing and right ventricular septal pacing in atrioventricular block,to compare the heart function influence of two kinds of pacing mode on pacemaker dependent patients, to provide evidence for the physiological pacing mode selection?Methods Enrolled 20 patients from January 2012 to March 2013 who should be placed in pacemakers, their primary disease was the second degree,high or third degree atrioventricular block,giving them three chamber pacemaker ( right atrial + biventricular ) each?Randomly divided into right ventricular septum pacing group ( group A, n=10) and biventricular pacing group( group B,n=10)?Twelve months later,each group crossed into the each other group and continued following?up for 12 months?After 24 months to obtain all the data to do the statistical analysis,including patients'6 min walking distance(6MWD),the Minnesota Heart Failure Quality of life score (MLHFQ),plasma N?terminal pro brain natriuretic peptide precursor(NT?proBNP),left ventricular ejection ejection fraction(LVEF),left ventricular diastolic end diastolic diameter(LVEDD),left ventricular contraction end diastolic diameter(LVESD),left ventricular twelve segmental 14W time standard deviation(Ts?12SD),left ventricular twelve segmental 14W time maximum delay(Ts?dif),the paced QRS qrsd?Results Compared with group B,the 6MWD and LVEF of 12,24 months after treatment of group A were significantly increased( ( 242?58 ±37?56) m vs?(347?42±36?59) m vs?(340?67±24?99) m;(39?97±5?84)% vs?(57?92±10?01)% vs?(60?50±10?06)%;P<0?05),QRSd and NT?proBN were significantly decreased((139?25±10?43) ms vs?(114?25±10?07) ms vs?(110?83±11?08) ms) ms;( 2 857?84±236?48) ng/L vs?( 2 144?26±301?43) ng/L vs? 6.Imaging study of maxillary sinus in normal adults using cone-beam CT

[ABSTRACT]OBJECTIVETo estimate different dimensions of maxillary sinus measured on cone-beam computerized tomography (cone-beam CT), and determine the differences in side and gender.METHODScone-beam CT image data from 200 normal adults was selected and analyzed by Invivo Dental software. The craniocaudal diameter, anteroposterior diameter, width and volume of maxillary sinus were measured as well as the thickness of the orbital floor. Statistical analysis was performed with SPSS 11.5.RESULTSThere were no statistically significant differences in the craniocaudal diameter, anteroposterior diameter, width and volume of maxillary sinus between sides (P>0.05, respectively). Significant differences were noted in the craniocaudal diameter and volume of bilateral maxillary sinus and the width of right maxillary sinus based on gender (P<0.05, respectively). There were no significant differences in the width of left maxillary sinus, the anteroposterior diameter of bilateral maxillary sinus and the osseous thickness of the orbital floor (P>0.05, respectively).CONCLUSIONOur findings indicate that cone-beam CT can be used to measure different dimensions of maxillary sinus and provide imaging anatomical data for clinical treatment.
7.Analysis on the Causes of Clinical Return of PIVAS Finished Infusions in Our Hospital and Discussion on Intervention Strategies

OBJECTIVE:To provide reference for reducing the clinical return of finished infusions. METHODS:Interventions for clinical return of PIVAS finished infusions in our hospital were introduced,the return situation of finished infusions before(dur-ing 2013-2014)and after(during 2015-2016)intervention was compared,and intervention effects were evaluated. RESULTS:Our hospital intervened it by rationally selecting and using syringes,strengthening visible particle check of finished infusion before dis-tribution;optimizing the order of drug configuration,reasonably arranging the drug infusion sequence;strengthening the clean-up in work area before drug infusion,correctly using disinfectant;strictly implementing intravenous infusion associative operation, playing the role of pharmacists;cultivating good work habits,and strengthening teamwork,etc. Compared with before interven-tion,the return numbers of finished infusion were decreased by 41.2%,and reported loss amount reduced by 45.7%. The return caused by visibility particles,finished infusion oxidation discoloration after too long storage time,finished infusion leakage,contin-uous infusion of drugs compatibility,heterodyne error,improper drug configuration method in packaging to the ward,improper use of disinfectant before infusion decreased by 25.3%,46.9%,39.4%,77.8%,73.9%,75.0%,100%,respectively. CONCLU-SIONS:Intervening the return of finished infusion can reduce the return numbers and drug waste.
8.Optimization of Preparation Technology of Rutin Nanoemulsion by Central Composite Design-Response Surface Method

Objective:To optimize the preparation technology of rutin nanoemulsion. Methods:The pseudoternary phase diagram of rutin nanoemulsion under the condition of different Kmwas drawn. With the drug loading and the particle size as the independent var-iables,and the percent of oil and the weight ratio of surfactant to cosurfactant as the dependent variables, the blank formula of rutin nanoemulsion was optimized by central composite design-response surface method. Results:The optimal formula was as follows:the ra-tio of ethyl oleate, cremopher RH40,1,2-propanediol and water was 7: 13: 5: 25. The average particle size of the prepared rutin nanoemulsion was 26.51 nm,and the drug loading was 8.97 mg·ml-1. Conclusion:The central composite design-response surface method can obtain a good formula of rutin nanoemulison,and the model has a good predictive function.
9.The effect of body weight supported treadmill training and muscle tone on motor function of lower extremi-ties of stroke survivors

Objective To observe the effect of body weight supported treadmill training ( BWSTT) on func-tion of lower extremities in stroke survivors, and analyze the influence of quadriceps femoris muscle tone on the thera-peutic effect of BWSTT. Methods Thirty-six stroke survivors were randomly divided into a BWSTT group and a conventional therapy group ( CT group) according to a random number table, each of 18. Both groups were given rou-tine rehabilitation training, while the BWSTT group was additionally provided with BWSTT for 4 weeks. Before and af-ter the treatment, Fugl-Meyer assessment for lower extremity (FMMS-LE), Berg balance scale (BBS) and Holden walking function rating scale ( Holden) were used to assess the motor function of lower extremities, balance function and walking ability of both groups. Meanwhile, the patients in BWSTT group were further divided into a mild-spasm subgroup and a seriously-spasm subgroup according to the modified Ashworth scale assessment result of quadriceps muscle, and the results of their efficacy indexes were further compared. Results No significant difference was found in the average FMMS-LE, Berg and Holden scores between the BWSTT group and CT group before the treat-ment ( P>0.05) . However, significantly improvement was found in all the above measurements of both groups after the treatment ( P<0.05) . Further comparison found that the FMMS score of the lower extremities and Holden grading in the BWSTT group improved more significantly compared with the CT group ( P<0.05) , while no significant differ-ences were found in the Berg score between the two groups ( P>0.05) . The change of FMMS (△FMMS) and Holden (△Holden) after treatment in the mild-spasm subgroup increased more significantly compared with those in the seri-ously-spasm subgroup, but no significant differences were observed regarding the change of Berg (△Berg) between the two subgroups. Conclusions BWSTT can significantly improve the motor function of lower extremities, balance function and walking ability of stroke survivors. However, hypertone of quadriceps femoris might have adverse effects on the therapeutic effect of BWSTT.
10. Screening of adult Ph-like acute lymphoblastic leukemia by multiplex real-time quantitative PCR

Objective:
To investigate the feasibility of multiplex real-time RT-PCR with fluorescent probes in early screening of Ph-like acute lymphoblastic leukemia (ALL) and analyze the clinical feature and prognos.
Method:
A total of 118 adult B-ALL patients diagnosed between October 2010 and March 2016 were enrolled in this study. Multiplex RT-PCR was used to detect the Ph-like ALL related fusion gene and CRLF2 expression in 58 BCR-ABL and MLL rearrangement negative patients. The clinical features, treatment response and prognosis were analyzed in Ph-like fusion gene positive and/or CRLF2 over-expression patients.
Result:
Among 58 patients, 9 patients (9/58, 15.5%) showed Ph-like ALL related fusion genes positive and 10 patients (10/58, 17.2%) showed CRLF2 over-expression. There were statistical differences in age, WBC count, immunophenotypes, cytogenetics and risk stratification among Ph-like fusion gene positive or CRLF2 over-expression patients, Ph+ patients, MLL+ patients and B-other patients. The 2-year overall survival rates were 65%, 47%, 64% and 74% respectively among these four groups (
