1.A comparative study on the close reduction of arytenoid dislocation under indirect and direct laryngoscope.

To assess the curative effects of different reduction techniques on the dislocation of cricoarytenoid joint caused by intubation, indirect laryngoscope (IL) and direct laryngoscope (DL) were utilized for the closed reduction of the displaced arytenoid under local anesthesia. 23 patients who underwent the reduction for dislocated arytenoid under IL or DL from January 1991 to June 2001 were reviewed. The data were collected on the duration of the laryngeal injury, times of receiving reduction, side-effects after the treatment and the period for voice to return to normal. The relationship between the duration of the laryngeal lesion and the period of the voice rehabilitation was examined. 13 patients received the reduction under IL and 10 patients under DL. Except the times of the reduction, which showed significant difference, no differences were found between IL group and DL group in the course and the period of voice rehabilitation, as well as sore throat after the manipulation. The patients' voice recovery was positively related to their course of disease in both IL and DL group. It is concluded that the recovery of normal voice is obviously affected by the duration of arytenoid dislocation. The reduction under IL is as effective as under DL in the treatment of arytenoid dislocation. Reduction by DL is better suit the patients with long time course of disease.

2.Clinical center-based health management and quality of life of patients with asthma

Objective To evaluate the effects of clinical center-based health management on quality of life (QOL) of patients with asthma. Methods A total of 642 patients who had received standard antiasthma treatment over 3 years in our clinical center from September 2005 up to now were randomly assigned to the good compliance group ( group A, n = 326 ) and bad compliance group ( group B, n = 316 ). A questionnaire was used to estimate the awareness about asthma,medical therapy and disease control. Levels of QOL were compared at 6 months and 1 and 3 years after health education. Results In group A,uncontrolled,partly controlled, and completely controlled asthma were found in 21%, 47%, and 32% participants, respectively. Significant difference of QOL was shown between the 2 groups after the intervention ( P < 0. 05 ). At 6 months, forced expiratory volume in one second ( FEV1 ), percentage of predicted FEV1 ( FEV1 % pre) ,and peak expiratory flow rate ( PEF)were significantly improved in group A ( all P <0. 05 ),although acute attack and hospitalization were largely reduced. In comparison with baseline,6 months and 1 year,self-reported QOL, limitation of motion, stimulus avoidance and response, and disease concerns were significantly changed at 3 years. Conclusions Clinic center-based health management could benefit disease control and QOL of patients with asthma.
3.Determination of Isoniazid and Acetylisoniazid Concentration in Human Plasma by HPLC

OBJECTIVE:To establish a HPLC method for determination of the plasma concentrations of isoniazid(INH)and its metabolite acetyl-isoniazid(AcINH).METHODS:Trichloroacetic acid was added to plasma to precipitate protein,then the supernatant was divided into 2 parts,distilled water was added to the first part;while HCl was added to the second one,after incubated at 80℃for 1 hour,1%cinnamaldehyde was added to perform precolumn derivatization.The column was Hypersil BDS C 18 ,the mobil phase was eluted with0.02mol/L NaH 2 PO 4 (pH=4.0)-acetontrile(69∶31)and detected wave-length was 340nm,the flow fate was1ml/min.RESULTS:INH detectable concentration showed a good linear correlation in the range of 0.57~145.88?mol/L,the average recoveries of INH and AcINH were 101.1% and 101.5% respectively,the average RSD within or between day was less than 10%.CONCLUSION:The present method is applicable to monitoring of INH plasma concentration and the study of metabolism capacity of N-acetyltransferase.
4.Revelations of Outpatient Satisfaction Survey on Hospital Management

Objective:To help hospitals at different levels develop and provide patients with high-quality medical services.Methods: Primary data were gathered through face-to-face surveys with questionnaires employed.Totally 195 questionnaires and 134 valid questionnaires were collected.Analysis of variance,independent-sample test,and cross-table combining z-test were used to complete data analysis.Results: Research outcome indicates there is no significant difference in outpatients' satisfaction level among hospitals at different levels,similar to outpatients' loyalty level.There are different affecting factors on outpatients' recommendation and satisfaction.Conclusion: Hospitals should be clear of patients' satisfaction level,evaluate patients' loyalty objectively,and make great effort on patients' concerning aspects.
5.Influence of Personal Factors on Outpatient′s Satisfaction of Medical Service

Objective:To explore the influencing factors on outpatients′ satisfaction of medical service.Methods: 195 samples coming from 5 provinces: Shandong,Hebei,Liaoning,Heilongjiang and Jilin province,and Tianjin City were all from the outpatients′ pupulation within a year,and were investigated with questionnaires.The data were collected by face-to-face survey,and totally 134 valid questionnaires were gathered.We use the following methods to complete data analysis: independent sample t-test,and analysis of variance,analysis of correlation,expecting to find the personal factors to affect outpatient satisfaction.Results: research outcome indicates that gender has no effect on outpatient′s satisfaction;while indexes as age and education level have significant influence.
6.Effects of wild jujube seed decoction on learning memory and levels of free radical and NOS in the brain of the pathogeny model rats with sleep deprivation

Objective To observe the effects of wild jujube seed decoction (WJSD) on learning memory and the levels of malondialdehyde (MDA) and the activities of supemxide dismutase (SOD) and nitric oxide synthase (NOS) in the brain of the pathogeny model rats with sleep deprivation (SD). Methods Levels of learning memory and MDA, and activities of SOD and NOS in rats' brain were assayed after SD induced by made-self multiple platform method ( MMPM), senescence induced by D-galactose, and Yin and blood-deficiency induced by eyclophosphamide and hydrocortisonum. Results After 48 h SD, the levels of learning memory was lower in the model group( ( 108.9 ± 12.5 ) s, ( 89 ± 11.5 ) s, ( 0 ) ) than those in the environmental control group ( ( 47.3 ±4.6)s,(9±1.4)s,(6.5 ±1.2))(( t=4.36,3.18,2.07, P<0.01==. However, the observation of the groups treated with WJSD, the levels of learning memory showed a tendency in returning to normal level (P < 0.05 = ,and MDA, and activities of SOD and NO of the high dose of WJSD showed a tendency in returning to normal leve1 (P < 0.05 = ,and the low dose of WJSD showed a tendency in returning to normal level too(P < 0.05 =. Conclusion WJSD can improve the disability of learning and memory of the pathogeny rats model, and its one of mechanism maybe involve the reduction of neural cell damage with free radical and NO.
7."Effect of application of Omaha system intervention in physical, psychology and social field of ""housebound"" in the elderly"

Objective To investigate the effect of Omaha system intervention on social psychology of the elderly,and to provide evidence for effective intervention in elderly with homebound state.Methods 218 cases with homebound conditions were divided into intervention group and control group by random digits table with each group 109 cases.The intervention group accepted intervention including health education,treatment procedures,case management,supervision of the four aspects of 1-3 times a week according to Omaha intervention system for a period of 3 months.The control group was the blank control.And Omaha evaluation form was used to evaluate social psychological and physical effect before and after the intervention.Results 3 months after the intervention,except physiology in the field of neuromusculo-skeletal function of two groups had no significant difference (t=2.03,P>0.05),scores in the intervention group in the field of physiology (like the circulation) were higher than the control group,the difference was statistically significant [(1 1.20±2.03) vs.(8.72±1.36),P<0.05].As for psychology (like mental health),scores of intervention group were higher than that of the control group [(12.53±1.42) vs.(8.63±1.33),t=20.12,P< 0.05].Conclusions Omaha system can effectively improve the quality of life of elderly with homebound state and maintain their physiological,social and psychological function.
8.Cloning and synthesis of a new A-superfamily conotoxin Bt14.10

Objective To clone a new conotoxin Bt14.10 from Conus betulinus derived from the South China Sea, synthesize the peptide , and to determine linkage of its disulfide bridges .Methods The genomic DNA was extracted from C.betulinus venom duct while the Bt14.10 sequence was cloned using primers designed based on the untranslated region and intron.The peptide was then synthesized using solid-phase method and folded into the target product whose disulfide bridge connection was further determined by two-step oxidative folding .Results A novel conotoxin designated as Bt 14.10 (CAHSVPGMHPCKCNNTC-NH2) was obtained,the disulfide connectivity of which was C1-C3,C2-C4.Conclusion Bt14.10 is a new A-superfamily conotoxin and has a distinct loop spacing pattern between cysteines in A-superfamily conotoxins.
9.The establishment of positive and negative evaluative conditioning reflex of neutral facial expression

Objective To examine the effect of positive and negative evaluative conditioning (EC) on neutral faces.Methods The experiment consisted of three phases:baseline phase,conditioning phase,and re-evaluative phase,in which 41 college students participated,watching sequences of neutral faces (CS) pairing to either a positive stimulus (USpos) or a negative stimulus (USneg).Their emotional experiences (valence and arousal) and physiological reactivity (eyeblink startle reflex and skin conductance) were recor ded.Results (1) In the re-evaluative phase,CSpos was rated significantly more positive than CSneg and CSneut,while CSneg was rated significantly more negative than CSneut (CSpos (5.05± 1.24),CSneg (3.73± 1.48),CSneut (4.46± 1.04),P<0.05).(2)In the re-evaluative phase,the mean startle eyeblink response magnitude(T score) to CSpos was significantly smaller than the responses elicited by CSneg and CSneut (CSpos (45.04±5.56),CSneg (51.44±9.30),CSneut (54.52± 10.60),P<0.01).Conclusion The findings suggest that neutral faces can acquire valences and approach motivation through EC.
10.Study on distributions of low wall shear stress in common carotid arteries of patients with intima-media thickening

Objective To measure wall shear stress (WSS) in common carotid arteries of normal subjects and patients with intima-media thickening and analyze the spatial distribution of low wall shear stress by quantitative analysis of blood flow shear stress visualization software.Methods Color Doppler flow images of common carotid arteries in normal subjects (control group) and patients with intima-media thickening (study group) were extracted from DICOM files.Two-dimensional distribution images of WSS,three-dimensional distribution images of WSS and the value of WSS in the common carotid arteries were obtained by this new algorithm.Then,the data and images of WSS were analyzed.Results WSS in the common carotid arteries of the control group were ranged from 4 to14 dyn/cm2.WSS in the common carotid arteries of the study group were ranged from 2 to 6 dyn/cm2.Compared to mean WSS values [(6.91 ±1.20) dyn/cm2] in common carotid arteries of the control group,mean WSS values [(2.87 ± 0.59)dyn/cm2] in common carotid arteries of the study group was significantly lower (t =17.828,P =0.000).Conclusions Low WSS areas have been emerged in local common carotid arteries in the patients with intima-media thickening.The reduction of WSS caused by local hemodynamic change might be related to common carotid artery intima media thickening.
