1.Research in application value of outpatient mobile infusion system in the department of infectious outpatient

Objective To observe the effect of the outpatient mobile infusion system in the department of infectious outpatient night infusion.Methods From July to December in 2012,520 patients in department of infectious outpatient infusion in our hospital were selected and were divided into the observation group (using outpatient mobile infusion system)and the control group (adopting conventional infusion) with 260 cases in each group.The infusion effect was compared between two groups.Results The average occupancy per patient care time in the observation group was less than the control group,the difference was statistically significant.The satisfaction degree in the observation group was higher than that of the control group,the difference was statistically significant.There was no nursing error occurred in both of the observation group or the control group.Conclusion The use of outpatient mobile infusion system can significantly improve the nurses work efficiency,ensure nursing safety,improve quality of care and patients' satisfaction degree.
2.Content Determination of Astragaloside Ⅳ in Qizhi Jiangtang Granules by HPLC-ELSD

OBJECTIVE:To establish HPLC-ELSD method for content determination of astragaloside Ⅳ in Qizhi jiangtang granules. METHODS:The separation was performed on Hypersil ODS2(250 mm?4.6 mm,5 ?m)column with acetonitrile-water(32 ∶ 68)as mobile phase. The flow rate was 1.0 mL?min-1. Injection volume was 10 ?L. The column temperature was 25 ℃. The temperature of drift tube for measurement was 110 ℃ and flow rate of N2 was 3.12 SLPM. RESULTS:Regression equation was A=1.699 C+6.181. The linear range of astragaloside Ⅳ was 0.259~1.295 mg?mL-1(r=0.999 6,n=5) with lowest detectable limit of 5.18 ?g?mL-1. An average recovery rate were 97.75%(RSD=2.17%). CONCLUSION:The method is simple,rapid and reliable for quality control of Qizhi jiangtang granules.
3.Clinical study of the application of phage amplified biologically assay in the detection of mycobacterium tuberculosis in sputum

Objective To assess the application of the phage amplified biologically assay (PhaB) in the detection of the mycobacterium tuberculosis (TB) in sputum.Methods To determine the TB patients in the 53 suspected patients we detected 53 sputum samples by PhaB method,BACTEC MGIT960 rapid culture method,and conventional methods (direct auramine smear microscopy and concentrated auramine smear microscopy) individually.Results TB positive samples detected by PhaB,BACTEC MGIT960 rapid culture,direct auramine smear microscopy and concentrated auramine smear microscopy were 24,22,11 and 17 respectively.Thus the sensitivity,specificity,positive predictive value and negative predictive value were 86.4%,83.9%,79.2% and 89.7%,when the PhaB is combined with concentrated auramine smear microscopy,the sensitivity,specificity,positive predictive value and negative predictive value were increased up to 90.9%,83.9%,80.8% and 92.9%.Conclusion PhaB method is a simple and rapid method to detect the Mycobacterium Tuberculosis in sputum with a relatively higher sensitivity and specificity than the conventional methods.When combined with concentrated auramine smear microscopy,PhaB method is even more sensitive.Thus the PhaB method is accessible and applicable in the clinical use of the TB detection.
4.Analysis of the Risk Factors for Hypertension in Patients with Chronic Kidney Disease Stage 5

Objective To analyze the risk factors for hypertension in patients with chronic kidney disease stage 5(CKD5) . Methods The basic information of 390 CKD5 patients complicated with hypertension was collected for univariate analysis ,in‐cluding gender ,age ,primary disease ,dialysis method ,body mass index(BMI) ,complications(hyperlipidemia ,high uric acid ,car‐diac insufficiency) ,level of education ,parathyroid hormone(PTH)level.Univariate variables that showed statistical significance were then subjected to the multivariate analysis(Logistic regression)to identify the risk factors for hypertension in CKD5 pa‐tients.The defined daily dose(DDD)that satisfied the criteria interms of different stages was evaluated.Results Overall hyper‐tension control rate was 22.8%.Univariate analysis showed that the following variables were significantly associated with hy‐pertension in CKD5 patients :>40 years old ,male ,diabetic nephropathy ,hypertensive nephropathy ,hemodialysis ,hyperlipemia , high uric acid level ,and high PTH level(P<0.05).Logistic multivariate analysis showed that diabetic nephropathy ,hyperlipi‐demia ,high PT H level were the independent risk factors for hypertension in patients with CKD5.In hypertension segmented standard ,there was no difference in the DDD between stage 0 and 1(P>0.05) ,and DDD at stage 2 and 3 was increased signifi‐cantly when compared with that at 0 and 1 standard(P<0.05).Conclusion Overall hypertension control rate is very low in pa‐tients with CKD5.Diabetics ,hyperlipidemia ,high PTH level are independent risk factors for hypertension in patients with CKD5.
5.Effect of Magnetic Stimulation on Sleep Disorder of Patients with Sequela of Brain Damage

Objective To evaluate the effect of magnetic stimulation on sleep disorder of patients with sequela of brain damage.Methods 60 patients with sequela of stroke or brain injury were divided into the control group and treatment group with 30 cases in each group.All patients in two groups were treated with regular rehabilitation training,and treatment group was added with magnetic stimulation,7±2 MT,20~30 min every time,once or twice a day.The period of treatment was 12 weeks.Results After treatment,the sleep disorder of patients of the treatment group improved significantly(P<0.05).Conclusion Magnetic stimulation could improve sleep disorder of patients with sequela of brain damage.
6.Effects of Magnesium Sulfate on Hemodynamics and Arterial Blood Gases in Patients With Chronic Pulmonary Heart Disease

Objective: Our aim was to evaluate effects of magnesium sulfate(MgSO4) on pulmonary hypertension in patients with chronic pulmonary heart disease(CPHD). Methods: Eight patients with CPHD undergone right-heart catheterization. The parameters of hemodynamics and arterial blood gases in patients were measured before and after MgSO4 intravenous infusion(2% MgSO4,40 mg/min). Results: Mean pulmonary artery pressure(mPAP), pulmonary vascular resistance(PVR),mean systemic artery pressure(mSAP), and total systemic vascular resistance(TSVR) significantly decreased(P<0.05) at 30 minutes after MgSO4 infusion. The decreased percentage of mPAP and PVR at 60 minute after MgSO4 infusion was 13.37% and 18.70% respectively(P<0.05), but mSAP and TPVR decreased only by 8.15% and 7.43% respectively. The decreased degree of mPAP and PVR at 60 minutes after MgSO4 infusion significantly correlated with mPAP(r=0.712, P<0.05)and PVR(r=0.802, P<0.01) before MgSO4 infusion. No unfavorable effect of MgSO4 on other parameters of hemodynamics and arterial blood gases was found. Conclusion: Magnesium sulfate can attenuate pulmonary hypertension in patients with CPHD but may cause transient systemic hypotension.
7.Effect of Extract from Baihua Qianhu on the Plasma Endothelin-1 in Patients with Hypoxic Pulmonary Hypertension

Objective: Our aim was to study the effect of endothelin-1 (ET-1) on the development of chronic hypoxic pulmonary hypertension and mechanism of QF-8 (the extract of baihua qianhu, a Chinese traditional medicine) in lowering pulmonary hypertension. Methods: The indexes of hemodynamics and the levels of plasma ET-1 in systemic vein, pulmonary artery, and systemic artery in 11 patients with pulmonary hypertension secondary to chronic obstructive pulmonary disease (COPD) were measured with right-heart catheterization and radioimmunoassay. Results: The ET-1 level in patients was significantly higher in systemic vein than that in 15 controls (P<0.01), pulmonary artery (P<0.05), and systemic artery (P<0.01). The levels of plasma ET-1 in patients had significant negative correlation with PaO2 and positive correlation with pulmonary vascular resistance index (PVRI). After taking QF-8, mean pulmonary artery pressure (mPAP) and PVRI significantly decreased, meanwhile, the levels of plasma ET-1 in systemic vein, pulmonary artery, and systemic artery also decreased by 25%, 21%, and 10%, respectively. Conclusion: Abnormality in synthesis or secretion of ET-1 may play a role in the development of chronic hypoxic pulmonary hypertension. The effect of QF-8 on pulmonary hypertension in patients with COPD may be related with its inhibition on the synthesis or secretion of ET-1.
8.Analysis of the demographic and blood donation behavior of repeat blood donors and first-time donors——based on the data of blood donors in Zhejiang province from 2006 to 2015

Objective To compare with the characteristics of different demographic and blood donation behaviors of the first blood donors and the repeated blood donors,to analyze the related factors influencing the repeated blood donation behavior,to provide the evidence to develop the recall strategy for the retention of the first-time donors strategies.Methods Use methods such as the composition ratio of descriptive analysis,and logistic regression analysis,Retrospectively analyzed the data of 3 226 571 cases of the whole blood donors in Zhejiang province from 2006 to 2015.from BIS2.0 Results ZheJiang repeated blood donors in 2006-2015 is accounted for 30.8%,men (57.8%),the proportion of aged 25 above is higher than the first blood donors;71.7% of men in the repeated blood donors are 60-79 kg,52.2% of women repeated blood donors are 50 to 59 kg;40% of the repeat donors blood for the first time donate 400 mL;71.6% of the repeated blood donors to donate again in 0.5-2 years,and of these,40.8% back in 0.5-1 year.Conclusion The main factors on the demographic aspects that influence the repeated blood donation is occupation,cultural degree,the quantity of blood donation for the first time.The characteristics of the precise recall people are as follows:Age 26 to 45 years old,stable career,donate 400 mL for the first-time,weight 70-89 kg of male,weight 55 kg above of women.The better recall intervention Interval is preferred to 0.5-2 years,and 0.5-1 year is the best.
9.Human Tumor Cells Apoptosis Induced by Dihydroartemisinin and Its Molecular Mechanism

OBJECTIVE:To study the apoptosis of human leukemic cells induced by Dihydroartemisinin and its molecular mechanism.METHODS:Human leukemia K562 cells were treated by Dihydroartemisinin.The inhibitory effect on cell proliferation was assayed by MTT.Fluorescence microscopy was applied to observe the presence of apoptosis.The expression of caspase-3 was assayed with reverse transcription-polymerase chain reaction(RT-PCR).Levels of mitochondrial and cytoplasmic cytochrome C were determined using Western blot.RESULTS:After treatment with Dihydroartemisinin for 48 hours,the IC50 values of human leukemia K562 cells were 8? 10-5mol? L-1 detected at a wavelength of 570nm by MTT.Distinct morphology changes of cell apoptosis such as karyopyknosis and conglomeration were observed by Hoechst33342/PI staining.RT-PCR assay showed the expression of Caspase-3.Western-blot detection showed the decrease of mitochondrial cytochrome C concentration but the positive expression of cytoplasmic cytochrome C concentration.CONCLUSION:Dihydroartemisinin could inhibit proliferation and induce apoptosis of human leakemic K562 cells,this may partially attributed to the promotion of the delivery of cyt-c and the activation of caspase-3.
10.The clinical features and identification of autonomic nervous mediated syncope and pseudo psychogenic syncope in children

Objectives To analyze and compare the clinical features of autonomic nervous mediated syncope (NMS) and pseudo psychogenic syncope (PPS) in children.Methods Clinical features were retrospectively analyzed in children with syncope complaint not caused by cardiac syncope, situation of syncope, drug-induced syncope, and neurogenic and metabolic diseases that caused brief loss of consciousness, and the clinical features of similarities and differences between NMS and PPS were compared. Results In 106 children with syncope were included, there were 85 cases (80.2%) of NMS, 13 cases (12.3%) of PPS, and 8 cases (7.5%) of unexplained syncope. PPS was induced by recent mental stimulation; the syncope was more frequent; the score of orthostatic intolerance was higher; the omen was rare; the duration of attack was longer. NMS was induced by long time standing, movement and body position change; the omen was mainly dizziness, blurred vision and gastrointestinal symptoms; the duration of attack was short (<5 min).Conclusions NMS is the most common cause of syncope in children, while PPS is an important cause of transient consciousness of non-syncope. There are similarities in clinical features between NMA and PPA and differential diagnosis is needed.
