1.Organophosphorus pesticide-induced immunotoxicity and its updated mechanisms

Organophosphorus pesticides(OP) are used as insecticides in agriculture throughout the world.The main toxicity of OP is neurotoxicity,which is caused by the inhibition of acetylcholinesterase.OPs also affect the immune response and suppress antibody production,T cell proliferation and the activity of natural killer(NK) cells and cytotoxic T lymphocytes(CTL).However,there have been few studies on the mechanism of OP-induced immunotoxicity,especially the mechanism of OP-induced inhibition of the cytolytic activity of killer cells.This study updates the mechanisms of pesticide-induced immunotoxicity,including its direct and indirect effects on the immune system.
2.Clinical analysis of 68 young patients with gastric cancer

Objective To analyze the incidence and pathological characteristic of gastric cancer in young patients and to explore the possibility of screening and preventing hereditary gastric cancer. Methods 68 young patients with gastric cancer under 35 years of age were retrospectively analyzed about their sex, endoscopes datum, clinic representation,pathology and expression of c-erbB-2 protein. Results The hemoglobin was lower in the female group (94. 5 ±21.7) g/L than male group(126. 2 ± 12. 8) g/L(t =4. 502 ,P <0.05) ;The malignant pathology was significant difference(P <0.05) in young female group than the male' s;The expression of c-erbB-2 protein has no significant difference(P > 0.05). Conclusions Gastric cancer in young patients was diagnosed later, misdignosed more,and move highly malignant. The prognosis is poor. Those phenomena was obviously in female.
3.Clinical observation on the curative effect of teprenone combined with esomeprazole for reflux esophagitis

Objective To investigate the curative effect of Teprenone combined with esomeprazole for reflux esophagitis. Methods 96 cases of reflux esophagitis comfirmed by endoscope were obtained and divied into observation group and control group randomly. Both groups were offered with esomeprazole and motilium. Teprenone was added to the observation group and sucralfate was added to the control group. Clinical effective rate,healing rate,adverse reactions and one-year follow up were observed closely after the course of 8 weeks. Results The relief time of clinical symptom and total effective rate in the observation group were(9.3 ±3.5)d and 95.8% respectively,which were significantly higher than those in the control group(P <0.05). Endoscopic effective rate was 93.8% in observation group and 81.2% in control group respectively and statistically significant difference was observed (P <0. 05). Recurrence rate after one year in the observation group was 8. 3% ,which was significantly lower than in the control group (P < 0.05). One case of headache and nausea occurred in both groups but no severe adverse reaction was observed. Conclusion Teprenone combined with esomeprazole were more effective and more likely to approach the complete cure for reflux esophagitis.
4.Clinical significance of EGFR mutations in non-small cell lung cancer

Objective:To investigate the mutations of the epidermal growth factor receptor (EGFR) gene and its clinical signifi-cance in non-small cell lung cancer (NSCLC). Methods:The EGFR gene mutations of exons 18 to 21 in NSCLC were detected by us-ing the ADx-ARMS? detection kit method. Results:The total mutation percentage in exons 18 to 21 of the EGFR gene was 45.8%(98/214) in NSCLC. These mutations predominantly occur in exons 19 and 21. EGFR gene mutation percentages were found in exons 18 (0.93%, 2/214), 19 (22.0%,47/214), 20 (2.3%, 5/214), and exon 21 (20.6%, 44/214) in the NSCLC. Two NSCLC cases were identified to have double EGFR gene mutations of exons 19 and 21. EGFR gene mutations were more frequently observed with adenocarcinoma histology (50.3%, 93/185) than with squamous cell carcinoma (17.2%, 5/29) (P=0.001). EGFR gene mutations occur more frequently in NSCLC cases in women than in men (P=0.002). EGFR gene mutations were significantly higher in NSCLC with lymphatic metastasis (66.7%) than in NSCLC without lymphatic metastasis (39.5%) (P<0.05). However, no evident association was found between EGFR gene mutations and age, as well as tumor grade and clinical stage of NSCLC (P>0.05). Conclusion:NSCLC, especially lung adenocar-cinomas, has exhibits frequent EGFR gene mutations in China. EGFR gene mutations in exons 19 and 21, combined with the clinical pathological features of lung cancer, can serve as the molecular marker to evaluate the efficacy of EGFR TKI for NSCLC patients.
5.Clinical significance of c-erbB-2 and nm 23 expression in gastric cancer

Objective To investigate the relationship between the expression of c-erbB-2 and rim23, and their clinical significance in gastric cancer. Methods The expression of c-erbB-2 and nm23 were detected by using immunohistochemical (SP) method in 85 surgi-cally removed specimens from patients with gastric cancer. Results The expression rate of c-erbB-2 and nm23 were 74.1% (63/85),52.9% (45/85). c-erbB-2 was positively correlated with the location, size and depth of invasion of gastric cancer, but not related with the histological types and lymph node metastasis. The expression rate of nm23 in patients without lymph node metastasis (85.7%) and in stage Ⅰ+Ⅱ (72.7%) was higher than that in patients with lymph node metastasis (42.2%) and in stage Ⅲ + Ⅳ (46.0%). The depth of inva-sion and clinical stage between the patients with c-erbB-2 over-expression and nm23 low-expression were positively related(P<0.01 or P<0.01). Conclusion c-erbB-2 promoted growth and invasion of gastric cancer, and nm23 restrained metastasis of gastric cancer. Both c-erbB-2 and nm23 could be served as predictors for biological behavior of gastric cancer.

Objective To establish the method of identifying MRSA with Taqman-fluorescence quantitative PCR basing on mecA/nuc/fem B three gene combined detecting.Methods Taking the coagulase positive MRSA,which isolated from the clinical samples and confirmed by VITEK 2 compact microbial analyzer,as the research obj ect,designed mecA/nuc/fem B specific PCR primers and Taqman fluorescent probe by bio-software PrimerPremier 5 and Designer Beacon 7,FAM,HEX and ROX markers were used to label the fluorescent probe at 5’,and the end of 3’was labeled with BHQ1,detected by fluo-rescence quantitative PCR instrment.Results ①1 g/dl gel electrophoresis results showed that the primer’s specificity of mec A/nuc/fem B were good,and molecular weight of the amplification band consistent with the expected molecular weight and no non-specific amplification band.②Three genes were obtained specific amplification in a single tube single channel and single tube multiple channel detection in PCR,and the three gene amplification effect in a single tube single tube single chan-nel and multichannel PCR similar.Conclusion Successfully established a method of multi channel Taqman-probe fluores-cence quantitative PCR identification of MRSA,mec A/nuc/fem B combined detection can effectively differentiate coagulase negative and positive MRSA,improve the accuracy of identification.

Objective To investigate the recent decade (1999 to 2009) incidence of committed suicide and attempted suicide of the Jinuo nationality in Jinuo Mountain and study the clinical characteristics of it. Methods 10 610 subjects at 44 Jinuo villagers committees were investigated by the method of door-to-door inquiry under the help of clues and collected the data of larithmics and medical history. Results 32 committed suicide patients were found in total, and the average incidence rate per year in the recent decade was 30. 16/100 000. 5 attempted suicide patients were found and the average incidence rate per year in the recent decade was 4.71/100 000. Conclusion The Jinuo committed suicide incidence rate is higher than nationwide level. The attempted suicide incidence rate is lower than nationwide level.
9.Causes of Fra-1 high expression in breast cancer cells and molecular mechanism of Fra-1 high transcription activity

Objective To explore the causes of Fra-1 high expression in breast cancer cells and molecular mechanism of Fra-1 high transcription activity .Methods Two human breast cancer cell lines MDA231 and MCF-7 and 1 human umbilical vein endothelial cell ECV304 were selected as the research objects .Real time fluorescent quantitative reverse transcription -polymerase chain re-action and Western blotting were used to detect the Fra-1 mRNA and protein expression ;the Fra-1 dual fluorescence reporter vector was constructed ,and the Fra-1 promotor transcription activity was detected ;the Fra-1 promotor short-cut reporter vector and mutation reporter vector at related loci were constructed ,and the transcription activity of short-cut reporter vector and mutation reporter vector was detected ;the binding situation of specific probe marked by biotin with activator protein-1(AP1) was observed by using the gel migration block test .Results The relative expression of mRNA and protein and promotor whole length reporter vector transcription activity of Fra-1 in human breast cancer cell lines MDA231 and MCF-7 cells were higher than that in human umbilical vein endothelial cell MCV304 ,the difference was statistically significant (P<0 .05);in constructed series Fra-1 promotor shortcut reporter vectors ,only the activity of pGL3B-256 was obviously decreased ,the difference was statistically significant (P<0 .05);the activity of AP1 mutation reporter vector pGL3B-M2 and SP1 and AP1 dual mutation reporter vector pGL3B-M3 was significantly lower than that of wild repoter vector pGL 3B-M1 ,the difference was statistically significant (P<0 .05);the gel migration block test found that the binding of specific probe marked by biotin with AP1 had obvious blocking effect .Conclusion In in vitro cultured breast cancer cell lines MDA231 and MCF-7 ,the trans-acting factor AP1 up-regulates its expression by activating Fra-1 gene promoter .
10.Omental transposition to mediastinum improves the outcome of postoperative intra-thoracic infections of Ivor-Lewis surgery.

OBJECTIVETo study the efficacy of pedicled greater omentum transposed to mediastinum in prevention against postoperative in-hospital intrathoracic complications after esophagectomy.
METHODSClinical data of 148 consecutive patients with esophageal cancer undergoing Ivor-Lewis surgery in our department from January 2010 to May 2014 were retrospectively reviewed. Among them, 84 patients with omental transposition(transposition group) and 64 patients without omental transposition(non-transposition group) were compared.
RESULTSPostoperative hospital stay was shorter in patients with omental transposition compared to those without omental transposition(P<0.05). Intrathoracic infection rate was significantly lower in transposition group(33/84, 39.3%) than that in non-transposition group(36/64, 56.2%), and as was the combined sepsis rate[19/33, 57.6% vs. 31/36, 86.1%, P<0.05]. No significant differences were found in the morbidity of anastomotic leakage, wound infection, cardiac complication and mortality during hospitalization. The median postoperative hospital stay was significantly shorter in transposition group than that in non-transposition group(13.0 vs. 16.5 days, P<0.05).
CONCLUSIONOmental transposition to mediastinum can reduce the development and severity of intrathoracic infection and shorten hospital stay in patients undergoing esophagectomy.
