1.Expression and Clinical Significance of NGF and Syncytin - 1 in Pregnant Women with Severe Preeclampsia

Objective To study the expression levels and clinical significance of nerve growth factor(NGF)and Syncytin-1 in ser-um and placental tissues of pregnant women with severe preeclampsia(SPE).Methods A total of 90 pregnant women who underwent ce-sarean delivery in the Department of Obstetrics,the Affiliated Hospital of Xuzhou Medical University in December 2021 to June 2022 were selected,and they were divided into 30 cases of early-onset SPE(early-onset group),30 cases of late-onset SPE(late-onset group),and 30 cases of normal women in the same period(control group).The maternal blood and placental tissues of the three groups were collected,and the expression levels of NGF and Syncytin-1 in each group were detected by enzyme-linked immunosorbent assay(ELISA)and immunohistochemical(IHC)staining,and their clinical significance was analyzed.Results The results of the ELISA showed that compared with healthy pregnant women in the third trimester,the expression of NGF and Syncytin-1 in the serum of pregnant women with SPE was reduced,and the early-onset group was lower than those in the late-onset group(P<0.05).The results of IHC staining showed that compared with healthy pregnant women in the third trimester,the expression levels of NGF and Syncytin-1 in pla-cental tissues were reduced in pregnant women with SPE,early-onset group was lower than those in the late-onset group(P<0.05).Conclusion The expression of NGF and Syncytin-1 in serum and placental tissues of the three groups of pregnant women was succes-sively reduced,and the expression was successively reduced in normal pregnant women,late-onset SPE and early-onset SPE,which may be involved in the occurrence and development of SPE by affecting the normal development of the placenta.
2.Research progress of home self-management support methods for cancer pain patients

Pain is one of the most common and difficult to control symptoms of cancer patients, which seriously affects the patient's physical and mental health as well as quality of life. Patients have problems such as low level of knowledge about cancer pain, poor initiative and participation in pain management, and insufficient self-management ability. Therefore, it is necessary to carry out home self-management of cancer pain patients, strengthen the education of cancer pain knowledge and response, and give full play to the subjective initiative of patients, so that they can have a deep understanding of cancer pain. This article reviews the home self-management support methods for cancer pain patients, with a view to providing references for further support and improvement of home self-management for cancer pain patients.
3. Evaluation of the effectiveness of nasal feeding registration form based on energy intake optimization design for patients with oral cancer

Objective:
To evaluate the effect of a new kind of nasogastric feeding registration form based on energy intake in perioperative nutrition support for patients with oral cancer.
Methods:
According to the specification of nasal feeding and summarizing the ingredients in common foods, the form was optimized and applied. By the cluster sampling method, the knowledge about energy intake of 12 nurses were investigated before and after the use of the form. The accuracy of energy intake assessment of 5 patients by 12 nurses respectively and the time required for single registration was compared before and after the use. Meanwhile, the knowledge about nasal feeding of 18 patients before the use and 17 patients after the use was compared.
Results:
The mean score of knowledge about energy intake of nurses was 57.50±17.77 before the use, which was lower than that after the use (89.00 ±6.18)
4.Self-management and the influencing factors of discharged patients with cancer after 125I seeds implantation

Objective? To investigate the status of self-management of discharged patients after radioactive 125I seeds implantation and analyze the possible influencing factors,so as to provide reference for postoperative continuous nursing guidance. Methods? From March to April 2018, convenient cluster sampling was used to select out of hospital tumor patients who met the inclusion criteria after 125I seed implantation in the Department of Radiation Oncology of Peking University Third Hospital within half a year. The self-designed Radiation Protection Knowledge and Self-management Behavior Evaluation Questionnaire was used to collect data through telephone follow-up and medical record review. In the self-designed part of the questionnaire, 2 particle group doctors and 2 deputy directors and 1 associate professor was invited to evaluate the content validity of the questionnaire. The total number of questionnaires was 72 and 72 valid questionnaires were collected. SPSS 21.0 statistical software was used for data entry and analysis. Results? After 125I seed implantation, the patients′ knowledge score of radiation protection outside hospital was (3.33±1.22) with a full score of 5; the self-management score of radiation protection was (27.40±9.47) with a full score of 44; the total self-management score of patients with lead-free clothing was higher than that of patients with lead-free clothing (t=5.51,P<0.001); the self-management level of radiation protection of medical insurance patients was higher than that of self paid patients (t=2.15,P<0.05); family economic pressure was the influencing factor of self-management of radiation protection (F=-4.41,P<0.05); the knowledge of radiation protection was positively correlated with self-management behavior of radiation protection (r=0.544,P< 0.001). Multivariate linear regression analysis showed that, the family economic incorne, the total score of radiation protection knowledge, and whether they have their own lead clothing were the main influencing factor of discharged patients with cancer after 125I seeds implantation (P<0.05). Conclusions? The knowledge of radiation protection outside the hospital and the ability of self-management of radiation protection of the patients after 125I seed implantation are medium level. The important factors influencing the self-management of radiation protection of the patients after seed implantation are whether they have their own lead clothing, family economic income, and the cognitive level of radiation protection. In order to achieve better self-management level of radiation protection, medical staff should educate patients about the hazards and protection of radiation, improve patients′ awareness of radiation protection, and encourage patients to buy lead clothing.
5.Current status and analysis of core competence of clinical nurses in national poverty country hospitals in Linfen, Shanxi Province

Objective:To explore the current status and influencing factors of core competence of clinical nurses in national poverty country hospitals in Linfen, Shanxi Province, so as to provide a basis for nursing managers to establish the nurses training program.Methods:From November 1 to November 30, 2018, we selected clinical registered nurses in three national poverty country hospitals (A、B、C) in Linfen, Shanxi Province, as subjects by convenience sampling. The self-designed General Information Questionnaire and Competency Inventory for Registered Nurse Scale (CIRN) were used to investigate the core competence of clinical nurses. Data analysis was carried out with the one-way ANOVA and multiple linear regression. A total of 156 questionnaires were collected and 130 of them were valid with 83.3% for the valid rate.Results:Among 130 clinical nurses in national poverty country hospitals in Linfen, Shanxi Province, the total score of core competence was (131.20±31.61) with a minimum of 33 and a maximum of 212. Univariate analysis showed that in terms of hospital factors, dimension scores of critical thinking and scientific research, clinical nursing, education and consultation and the total score of core competence of B hospital were higher than those of A hospital; dimension scores of critical thinking and scientific research, education and consultation of C hospital were higher than those of A hospital; those differences were statistical differences ( P<0.05) . In the respect of employment mode factors, dimension scores and total score of core competence of temporary workers were lower than those of formal recruitment and contract workers; dimension scores of critical thinking and scientific research, interpersonal relationship, law and ethical practice as well as the total score of core competence of formal recruitment nurses were lower than those of contract nurses with statistical differences ( P<0.05) . In terms of department factors, the score of professional development dimension of Pediatric Ward was lower than those of other wards except for Internal Medical Ward with statistical differences ( P<0.05) . Multivariate regression analysis showed that two influencing factors, the employment mode and department, entered the regression equation of total score of core competence of nurses which could explain 22.5% of the total variation. Conclusions:Clinical nurses of national poverty country hospitals in Linfen in Shanxi Province have the intermediate level of core competence and are competent at nursing work which are affected by the hospital, employment mode and department in varying degrees. Nursing managers should pay more attention to core competence training of nurses, update and perfect training program so as to improve core competence of nurses.
6.Research progress on the cultivation of clinical nurses' nursing informatics competency

Nursing informatics competency is the comprehensive ability of knowledge, attitude and skills exhibited by clinical nurses in various nursing informatics activities, which has become the basic condition for engaging in clinical work, and is a necessary professional core ability and necessary quality. At present, the nursing informatics competency of clinical nurses in China needs to be improved, and a training system in line with national conditions is urgently needed to guide the cultivation of clinical nursing informatics talents. This study summarizes the relevant research on the cultivation of clinical nurses' nursing informatics competency at home and abroad, understands the current status of clinical nurses' informatics competency training, so as to provide a reference for the cultivation of clinical nurses' nursing informatics competency and the construction of training programs.
7.GB7 acetate,a galbulimima alkaloid from Galbulimima belgraveana,possesses anticancer effects in colorectal cancer cells

GB7 acetate is a galbulimima alkaloid obtained from Galbulimima belgraveana.However,information regarding its structure,biological activities,and related mechanisms is not entirely available.A series of spectroscopic analyses,structural degradation,interconversion,and crystallography were performed to identify the structure of GB7 acetate.The MTT assay was applied to measure cell proliferation on human colorectal cancer HCT 116 cells.The expressions of the related proteins were measured by Western blotting.Transmission electron microscopy(TEM),acridine orange(AO)and monodansylcadaverine(MDC)staining were used to detect the presence of autophagic vesicles and autolysosomes.A transwell assay was performed to demonstrate metastatic capabilities.Oxygen consumption rate(OCR)and extracellular acidification rate(ECAR)assays were performed to determine the mitochondrial oxidative phosphorylation(OXPHOS)and glycolysis activity of HCT 116 cells.The data showed that GB7 acetate suppressed the proliferation and colony-forming ability of HCT 116 cells.Pretreatment with GB7 acetate significantly induced the formation of autophagic vesicles and autolysosomes.GB7 acetate upregulated the expressions of LC3 and Thr172 phosphorylated adenosine 5'-monophosphate(AMP)-activated pro-tein kinase α(p-AMPKα),which are key elements of autophagy.In addition,GB7 acetate suppressed the metastatic capabilities of HCT 116 cells.Additionally,the production of matrix metallo-proteinase-2(MMP-2)and MMP-9 was reduced,whereas the expression of E-cadherin(E-cad)was upregulated.Furthermore,GB7 acetate significantly reduced mitochondrial OXPHOS and glycolysis.In conclusion,the structure of the novel Galbulimima alkaloid GB7 acetate was identified.GB7 acetate was shown to have anti-proliferative,pro-autophagic,anti-metastatic,and anti-metabolite capabilities in HCT 116 cells.This study might provide new insights into cancer treatment efficacy and cancer chemoprevention.
8.Clinicopathological features of verrucous hemangioma

Objective:To investigate the clinicopathological features, and differential diagnosis of verrucous hemangioma (VH).Methods:Twenty-eight VH cases diagnosed from 2005 to 2020 in Henan Provincial People′s Hospital, Zhengzhou, China were analyzed retrospectively. Immunohistochemical studies were used to detect diagnostic markers. The mutation status of PIK3CA (exons 9 and 20) was detected using fluorescence PCR.Results:There were 13 males and 15 females in 28 cases, with the male to female ratio of 1.0∶1.2. There were 25 patients under the age of 18 years. The age range was from 10 months to 56 years (mean, 9.7 years; median, 4.5 years). There were 17 cases occurred in the lower extremities, 7 in the upper extremities and 4 in the trunk. All 28 cases were irregular red patches on the skin, which grew slowly. Some of them were thickened with uneven surface, which was light pink or red-white. Skin lesions of the 7 cases ranged from dark red and reddish brown, with a rough and hard surface. Satellite foci were present. Microscopically, 28 cases had a wide range of pathological features. Dilated, malformed vessels were observed from dermal papilla to deep soft tissue. Among them, the dermal papillary layer was mainly composed of many proliferating and expanding thin-walled capillaries and cavernous blood vessels. Thin-walled small vessels were found in the dermal reticular layer and subcutaneous fascia layer, with no obvious endothelial cell proliferation, occasional papillary hyperplasia, and lobular distribution of the malformed vessels in the fascia layer mixed with the fibroadipose tissue. There was epidermal papillary hyperplasia with hyperkeratosis and parakeratosis, lengthening and mutual fusion of epithelial horns. Immunohistochemistry showed that CD31, CD34, ERG and WT-1 were diffusely and strongly positive. The expression of GLUT-1 was present in superficial dermal vascular endothelial cells, but undetectable in the deep layer. The PIK3CA tests of 13 cases showed that no somatic mutations were found in exons 9 and 20. Twenty-five patients were followed up for 5 months to 10 years. Seven patients underwent multiple surgical resections and plastic surgeries due to the large size, and 8 patients had recurrence.Conclusions:VH is a rare congenital vascular malformation and more commonly occurs in infants and children. It tends to appear in limbs, especially lower limbs and distal limbs. Its morphology and immunophenotype are characteristic and should be distinguished from other vascular malformations and the resolution phase of infant hemangiomas. In about one third of the cases, postoperative recurrence may occur and long-term follow-up is often required.
9.Status quo of nursing informatics competencies of head nurses in a Class Ⅲ Grade A hospital in Beijing

Objective:To understand the current situation of nursing informatics competencies of head nurses in a Class Ⅲ Grade A hospital and to provide more scientific evidence for the training program of informatics competencies of nursing managers.Methods:This study was a cross-sectional study. In March 2021, a cluster sampling method was used to select 127 head nurses from Peking University Third Hospital as research subjects. The General Information Questionnaire and Nursing Informatics Competency Scale were used to conduct an on-site questionnaire survey. A total of 127 questionnaires were distributed in this survey, 110 valid questionnaires were recovered, and the effective recovery rate was 86.6%.Results:The total score of nursing informatics competencies of 110 head nurses was (133.54±18.23) . The results of multiple linear regression analysis showed that learning computer-related skills, participating in nursing information-related learning or training, and the department in which the department was located were the main factors affecting the nursing informatics competencies of head nurses ( P<0.05) . Conclusions:The nursing informatics competencies of head nurses is generally at the middle and upper level, but the nursing information operation ability is low. There are differences in the informatics competencies of head nurses in different departments. Information training and learning can improve the nursing informatics competencies. It is suggested that hospitals should pay attention to the construction of nursing information in departments and strengthen nursing information. Competency course training to enhance the nursing informatics competencies of head nurses.
10.Selection of perioperative venous access for simultaneous repair and reconstruction of oral cancer after radical resection

Objective:
To explore the characteristics of perioperative venous therapy in patients undergoing simultaneous repair and reconstruction of oral cancer after radical resection, and prevention of femoral vein catheterization complications, to provide clinical evidence for venous therapy.
Methods:
A retrospective analysis was perfomed to assess the choice of venous access and its effects in 95 patients undergoing simultaneous reconstruction due to oral cancer during the perioperative period.
Results :
In total, 95 patients underwent successful indwelling femoral vein catheterization with double lumens, and a midline catheter and peripheral intravenous indwelling needles were used to complete the intravenous therapy after surgery. Among the femoral vein complications, 1 case was complicated with lower extremity venous thrombosis (incidence was 1.1%), and 17 cases were complicated with puncture point bleeding (incidence was17.9%). Two cases of puncture site bleeding were noted among 28 cases using a midline catheter. Local phlebitis and infiltration were the most common complications of peripheral venous catheters. The difference in catheter-related complications among different types of catheters was statistically significant (P < 0.05). The complication rate of the midline catheter was lower than that noted with femoral vein catheterization and peripheral intravenous indwelling needles.
Conclusion
Based on the treatment characteristics of patients, proper venous catheters should be established during the perioperative period. The application of a femoral vein catheter during the operation combined with the use of a midline catheter and peripheral venous indwelling needles after the operation can satisfy intravenous therapy needs in patients undergoing simultaneous repair and reconstruction for oral cancer, and the midline catheter can effectively reduce venous catheter-related complications.
