1.Intraocular pressure and anterior chamber angle changes after phacoemulsification in different types of acute angle-closure glaucoma

Objective To evaluate the relationship between intraocular pressure (IOP) and anterior chamber angle in patients with different types of acute angle-closure glaucoma after phacoemulsification and intraocular lens implantation. Design Retrospective case series. Participants 173 cases of acute angle-closure glaucoma coexisting cataract (173 eyes) in Mingren Eye Hospital from January, 2005 to June, 2007. Methods All patients with acute angle-closure glaucoma underwent phacoemulsification and intraocular lens implantation, and were examined with high-frequency ultrasound biomicroscopy (UBM). Anterior chamber depth (ACD), intraocular pressure (IOP) and anterior chamber angle (ACA) were measured. Follow-up was 1-48 weeks after surgery. Main Outcome Measures Intraocular pressure, phacoemulsification complications, re-open rate of anterior chamber angle. Results At 8th week after surgery, the rate of IOP control in pupillary block group, non-pupillary blocking group, and multimechanism group was 88.9%, 52.0%, 83.4%, respectively and there was a statistically significant between these groups (?2=7.13, P=0.022). At 48th week, the rate of IOP control was 54.2%, 33.3%, 35.8%, respectively(?2=12.56, P=0.003). Fifty patients were follow up with UBM, and 66.7% in pupillary block group, 33.3% in non-pupillary blocking group and 33.3% in multimechanism group in 48 weeks postoperatively showed evidence of the widened anterior chamber angle. No specific postoperative complications were found in this study compared with those with phacoemulsification without glaucoma. Conclusions The use of phacoemulsification and intraocular lens implantation for acute angle-closure glaucoma coexsiting cataract can get best IOP-controlled rates in pupillary block group. It is related to the degree of widened anterior chamber angle.
2.Comparison of hidden blood loss in two kinds of internal fixation for femoral intertrochanteric fractures

BACKGROUND:Hidden blood loss is an important risk for the intertrochanteric fracture patients, especial y the elderly patients, which can cause anemia in patients after internal fixation and can affect wound healing and patient recovery. OBJECTIVE:To compare the perioperative hidden blood loss and the risk factors of proximal femoral anti-rotation intramedul ary nail internal fixation and dynamic hip screw fixation for the treatment of femoral intertrochanteric fracture. METHODS:We selected 70 cases of femoral intertrochanteric fracture patients who treated with proximal femoral anti-rotation intramedul ary nail and dynamic hip screw fixation, including 21 patients with the age ≥ 80 years and 49 patients with the age<80 years;28 patients with the body mass index>30 kg/m2 and 42 patients with the body mass index ≤ 30 kg/m2;30 patients received anti-rotation intramedul ary nail internal fixation and 40 patients received dynamic hip screw fixation. The perioperative blood loss was calculated with Gross formula according to the changes of height, body mass index and the hematocrit before and after fixation. RESULTS AND CONCLUSION:The mean total blood loss was 936 mL, the mean dominant blood loss was 237 mL and the mean hidden blood loss was 699 mL. The hidden blood loss was accounted for 74.7%in total blood loss. The dominant blood loss in the dynamic hip screw fixation group was higher than that in the anti-rotation intramedul ary nail internal fixation group, and the hidden blood loss was lower than the anti-rotation intramedul ary nail internal fixation group. The total blood loss and the hidden blood loss of the elderly patients were higher than those of the non-elderly patients;there was no significant difference between male and female patients, obesity and normal patients. The results indicate that hidden blood loss is the major reason for total blood loss of femoral intertrochanteric fracture after internal fixation. The hidden blood loss of anti-rotation intramedul ary nail internal fixation is larger than that of dynamic hip screw fixation, and elder is the risk factor for hidden blood loss.
3.Treatment and outcomes of 81 extremely low birth weight infants

Objective To summarize the characteristics of treatment and outcomes of extremely low birth weight infants (ELBWI).Methods The clinical data of 81 cases of ELBWI admitted to Bayi Children's Hospital Affiliated to General Hospital of Beijing Military Command from October 1st.2008 to August 31st.2011 were retrospectively analyzed.The survival rate and mortality of ELBWI with different gestational age and birth weight were compared with Chi-square test.Results Of 81 ELBWI,43 were male and 38 were female; the mean gestational age at birth was (28.4 ±2.1) weeks; the average birth weight was (903.5 ± 95.4) g.Complications of them included respiratory distress syndrome (68/81,84.0%),patent ductus arteriosus (47/81,58.0%),retinopathy of prematurity (41/81,50.6%),intraventricular hemorrhage (23/81,28.4%),bronchopulmonary dysplasia (17/81,21.0%),sepsis (15/81,18.5%),periventricular leukomalacia (2/81,2.5%) and necrotizing enterocolitis (1/81,1.2%).Among 81 infants,96.3% (n =78)received oxygen inhalation with the average time was 19.1 d (median 11 d,0-121 d) ; 70.4% (n=57)received pulmonary surfactant; 27.2% (n =22) received nasal continuous positive airway pressure;34.6% (n =28) received normal frequency mechanicalfrequency ventilation.All 47 ELBWI with patent ductus arteriosus received ibuprofen treatment,among which 6 cases received arterial canal ligation after failure of ibuprofen treatment.20 out of 41 cases of retinopathy of prematurity received laser surgery.Total parenteral nutrition was required in 74 infants(91.4%),the average age of beginning enteral feeding was 5.2 d (median 6 d,0-17 d)and achieved full gastrointestinal feeding time was 31 d (median 28 d,7-65 d).The survival rate of ELBWI with gestational age of ≤26,-28 and >28 weeks was 25.0% (4/16),72.0% (18/25) and 77.5% (31/40),respectively; the corresponding mortality was 12.5% (2/16),8.0% (2/25) and 2.5% (1/40),respectively.The survival rate of ELBWI with birth weight ≤800,-900 and >900 g was 33.3% (4/12),58.3% (14/24) and 77.8% (35/45),respectively; and corresponding mortality of them was 16.7% (2/12),8.3% (2/24) and 2.2% (1/45),respectively.Conclusions The outcome of ELBWI is closely related to gestational age and birth weight.Professional and sophisticated treatment might improve the survival rate and general outcome of ELBWI.
4.Incidence and risk factors of retinopathy of prematurity——analysis of 2185 premature infants

Objective To determine the incidence and risk factors of retinopathy of prematurity (ROP) in preterm infants.Methods Fundus examinations were performed by RetCam Ⅱ ophthalmoscopy on 2185 premature infants (birth weight ≤ 2000 g or gestational age≤34 weeks)admitted into the neonatal intensive care unit of Beijing Bayi Children's Hospital from January 1st 2009 to December 31st 2010.According to the results,all infants were divided into ROP group and nonRO P group.Two-sample t test and Logistic regression analysis were used to investigate the risk factors of ROP.Results Among 2185 premature infants,287 (13.1 %) cases were diagnosed with RO P.According to International Classification of RO P,34 cases (11.9 %) were in zone Ⅰ,147 cases (51.2%) in zone Ⅱ,and 106 cases(36.9%) in zone Ⅲ.And there were 117 cases (40.8%) with stage 1 lesion,142 cases (49.5%) with stage 2 lesion,28 cases (9.7%) with stage 3 lesion,and no stage 4 or 5 lesion was identified.Thirty-six cases (12.5 %) were accompanied by additional diseases.Logistic analysis showed that small gestational age (OR=0.859,95%CI:0.770-0.958,P=0.006),low birth weight (OR=0.729,95%CI:0.6340.838,P=0.000),long duration of oxygen supplement (OR=2.221,95%CI:1.904-2.592,P=0.000),assistant ventilation (OR=3.104,95%CI:2.0964.956,P=0.000),apnea (OR=1.767,95%CI:1.103 2.831,P=0.018) and=anemia (OR=2.242,95%CI:1.641-3.604,P=0.000) were independent risk factors of ROP.Conclusions The incidence of ROP in premature infants is high.Small gestational age and low birth weight,long duration of oxygen supplement,assistant ventilation,apnea and anemia are risk factors of ROP.Preventive measures should be taken against these factors.
5.Study on variations of mtDNA from high and low metastatic mouse hepatocarcinoma cell sublines

Objective To investigate the variations of mtDNA from high and low metastatic mouse hepatocarcinoma cell sublines Hca-F and Hca-P, and the relationship between mutations of mtDNA and carcinogenesis. Methods The variations of D-loop, ND3 and tRNA Met+Glu+Ile gene fragments of mtDNA from Hca-F and Hca-P cells were analyzed by PCR-RFLP and sequencing techniques. Results No amplification fragment length polymorphism and restriction fragment length polymorphism were observed in tRNA Met+Glu+Ile , ND3 and D-loop of mtDNA from the 2 cell sublines. Sequence difference between these 2 cell sublines were found in mtDNA D-loop region by sequencing. Conclusions Genetic alteration of mtDNA non-coding region in tumors, which may reflect the environmental and genetic influences operative during tumor progression, can be linked to their tumorigenic phenotype.
6.Study on 4977 bp deletion mutation of mitochondrial DNA in non-small lung cancer

Objective To study the 4 977 bp deletion of mitochondrial DNA in lung cancer, paraneoplastic tissue and normal lung tissue from non-lung cancer subjects and its significance in the development of cancer. Methods Lung cancer tissues and paraneoplastic tissues from 37 non-small lung cancer patients, and normal lung tissues from 20 patients without lung cancer were analyzed by long PCR technique. Results Mitochondrial DNA 4 977 bp deletion was detected in 54.1%(20/37) of lung cancer tissues, 59.5%(22/37) of paraneoplastic tissues and 30.0%(6/30) of normal lung tissues. The correlation between 4 977 bp deletion and age, smoking was present in our data. Conclusion Mitochondrial DNA 4 977 bp deletion, which may reflect the environmental and genetic influences during tumor progression, is not specific to lung cancer and unlikely to play an important role in carcinogenesis.
7.Analysis of RECQL4 gene variant in a child with Rothmund-Thomson syndrome.

OBJECTIVE:
To explore the genetic basis for a child with Rothmund-Thomson syndrome (RTS).
METHODS:
The child has featured poikeloderma, short stature, cataract, sparse hair and skeletal malformation. Peripheral blood samples of the child and her family members were collected and subjected to whole exome sequencing. Candidate variants were verified by Sanger sequencing.
RESULTS:
The child was found to harbor compound heterozygous variants of the RECQL4 gene, namely c.1048_1049delAG and c.2886-1G>A, among which c.2886-1G>A was unreported previously. According to the ACMG guidelines, the c.1048_1049delAG was predicted to be pathogenic (PVS1+PM3_Strong+PM2), while the c.2886-1G>A was predicted to be likely pathogenic (PVS1+PM2).
CONCLUSION
The compound heterozygous variants of the RECQL4 gene probably underlay the pathogenesis of RTS in this patient. Above finding has enriched the mutational spectrum of the RECQL4 gene.

8.Analysis of fertility characteristics and trends of registered population in a neighborhood in Jing’ an District,Shanghai from 2013 to 2022

ObjectiveTo analyze the fertility characteristics of the registered population in a neighborhood in Jing’an District, Shanghai from 2013 to 2022, and to provide a reference for population development planning and allocation of public health resources. MethodsData on newborns and puerperae registered in the neighborhood were collected through the Shanghai Birth Medical Information System and Public Security Department information system. Statistical analysis was conducted using Excel 2010, and the Joinpoint regression model was used to describe the changes. ResultsFrom 2013 to 2022, the crude birth rate, general fertility rate, and total fertility rate showed a declining trend, with average annual percent change (AAPC) values of -7.62%, -6.99%, and -4.54%, respectively. The average age of first childbirth and the age of childbearing among women gradually increased, with AAPC values of 1.24% and 1.28%, respectively. In the past 10 years, the total number of newborns showed a downward trend, with an AAPC value of -6.15%. After the implementation of the two-child policy, the proportion of second children showed an increasing trend, with an AAPC value of 7.37%. ConclusionThe fertility level of the population in the neighborhood is declining. The two-child policy has not significantly improved fertility rates, and the age of childbearing in women continues to rise. Diverse measures are needed to encourage childbirths and promote healthy births.
9. Study on the relationship between Toll like receptor 4 expression and apoptosis in periventricular leukomalacia rat model induced by hypoxia-ischemia

Objective:
To investigate the correlation between Toll like receptor 4 (TLR4) expression and apoptosis in periventricular leukomalacia (PVL) rat model induced by hypoxia-ischemia.
Methods:
One hundred and forty three-day-old sprague-dawley (SD) rats, which were divided into experimental group (ischemia-hypo-xia group) and control group (sham operation group) randomly, were used to establish a hypoxic model by ligating the right common carotid artery and inhaling gas mixtures with 60 mL/L oxygen and 940 mL/L nitrogen.The rats were killed 6 h, 12 h, 24 h, 3 d, 7 d after model reproducing and the brain tissues were used for the following experiments.The pathological changes and apoptosis of brain tissues were detected by way of hematoxylin and eosin (HE) staining and terminal deoxynucleotidyl transferase dUTP nick end labeling (Tunel) assay respectively, and TLR4 expression was detected by adopting immunohistochemistry and reverse-transcription polymerase chain reaction(RT-PCR). The data were analyzed by using the SPSS 19.0 software.
Results:
TLR4 expression in the modeling rat brain commenced to increase in 6 hours (0.541±0.069, 0.166±0.058)and reached the peak in 3 days(1.932±0.161, 0.300±0.039), and then began to decline in 7 days (1.242±0.109, 0.220±0.025) post hypoxia-ischemia.Compared with the control group, there were statistical significances at 6 h, 12 h, 24 h, 3 d and 7 d (all
