Chinese Journal of Practical Nursing 2012;28(1):66-68
Objective To acknowledge the psychological authority status under the foundation of autho0rization management,in order to analyze the effectiveness of authorization management. Methods 364 clinical nurses were investigated with Psychological Empowerment Scale and the results were performed statistic analysis through descriptive statistical method and variance analysis. Results The average score of clinical nurses was(3.22±0.70),which was at the moderate point. The four dimensional scores listed in descending order were self efficient, career significance, autonomy and job influence.Clinical nurses of different ages, different ranking, different working period, different marriage status had statistical outstanding variance. But nurses of different education background showed no significant difference in score of each dimension. Conclusions The nursing management shall not only focus on the authorization management itself, but also on the psychological authority status of clinical nurses.Through an efficacious incentive,we can enhance the psychological recognition and experience on authorization and activate the working motivation, maximize the working initiative and thus promote the innovation and expansion of nursing work.
2.Progress in treatment of clinical stage N2 non-small cell lung cancer
Journal of International Oncology 2010;37(6):454-457
Although the optimal treatment protocol for clinical N2-stage non-small cell lung cancer (NSCLC) is still uncertain, the recommended standard treatment protocol goes to radical concurrent chemora diotherapy. Concurrent chemoradiotherapy after induction chemotherapy is not superior to standard concurrent chemoradiotherapy. Neoadjuvant concurrent chemoradiotherapy followed by pulmonary lobectomy can improve overall survival ratio comparaed with preoperative neoadjuvant concurrent chemoradiotherapy. The role of concurrent chemoradiotherapy followed by consooldation chemotherapy is still controversial. Gefitinib or erlotinib failed to bring about the survival benefit from the maintenance treatment after concurrent chemoradiotherapy. Prophylactic cranial irradiation play a role merely in decreasing the risk of brain metastases, but not in the improvement of overall survival.

With the development of phamacogenomic and molecular biology, it has been revealed that the curative effect of non-small-cell lung cancer (NSCLC) is closely related to tumor molecular markers. Predictive biomarkers in NSCLC patients, including ERCC1, BRCA1, hMLH1, hMSH2, RRM1, β-tubulin, TS,EGFR and K-ras, can predict individual patient's response to chemotherapy or targeted therapy, making individualized therapy be possible.

Chinese Journal of Perinatal Medicine 2021;24(2):131-134
This paper reports a woman diagnosed with citrullinemia type I (CTLNⅠ) in puerperium who was unfortunately died later. The 28-year-old patient (G1P1) delivered a live girl at 39 +2 gestational weeks and was transferred from a local hospital to Henan Provincial People's Hospital on January 11, 2020, due to "a 3-day paroxysmal confusion accompanied by dizziness 4 days after delivery". Intermittent confusion, elevated blood ammonia, and citrulline concentration, and encephaledema were presented 10 h after delivery, and the patient eventually died of cerebral hernia on the day of self-discharge. Two pathogenic mutations of the ASS1 gene were found by genetic testing, including c.422t>G (p.val141gl; HET) and c.431c>G (p.pro144arg; HET) and confirmed the diagnosis of CTLNⅠ. CTLNⅠ is a life-threatening disease that could be easily overlooked and misdiagnosed and was difficult to treat. It most often occurs in newborns and infants, whilst it is rare during pregnancy and postpartum. The possibility of this disease should be considered in patients with neurological system symptoms and elevated blood ammonia during pregnancy or puerperium.

This study investigated the feasibility and effects of organ bath to be used for detection of bronchial function of non-heart-beating donor (NHBD) lung after 1-h warm ischemia. Sixteen Swedish pigs were divided into two groups randomly: heart-beating donor (HBD) group and NHBD with 1-h warm ischemia (NHBD-1 h) group. The bronchial rings whose lengths and inner diameters were both 1.5 mm were obtained from isolated left lungs of all the pigs. Acetylcholine, arachidonic acid natrium and papaverine were used to test and compare the contractile and relaxant function of bronchial smooth muscles and epithelium-dependent relaxation (EpiDR) response between HBD and NHBD-1 h groups. The results showed that there was no significant difference in the values of bronchial precontraction between HBD and NHBD-1 h groups (5.18+/-0.07 vs 5.10+/-0.11 mN, P>0.05). No significant difference in the values of EpiDR responses between HBD and NHBD-1 h groups (1.26+/-0.05 vs 1.23+/-0.07 mN, P>0.05) was observed either. During the process of EpiDR induction, the rings had no spontaneous relaxation in two groups. In addition, papaverine solution completely relaxed the bronchial smooth muscles of all bronchial rings. It was concluded that after warm ischemia for 1 h, the contractile and relaxant abilities of bronchial smooth muscles, and the epithelium-dependent adjustment both kept intact. Organ bath model could be a liable and scientific way to evaluate the bronchial function of NHBD lung.

Objective This study aims to comprehensively investigate the molecular characteristics of the predominant circulating Dengue virus type 1 (DENV-1) during the 2019 Dengue fever outbreak in Xishuangbanna, providing an essential insight to support the prevention and control of dengue fever in the local area. Methods A Dengue virus type 1 (DENV-1) strain, designated as JHS45, isolated from the blood of a febrile patient in Xishuangbanna in 2019, underwent a process of inoculation and cultivation in C6/36 cells. Second-generation sequencing was employed to capture the viral genetic sequence. Bioinformatics software, including CLC, was used for assembling the sequencing data. Sequentially, sequence alignment, construction of a phylogenetic tree, and analysis of amino acid sites were conducted using software such as Lasergene and MEGA6.1. Results Cytopathic effects of JHS45 appeared in C6/36 cells after 6 days. After sequencing and assembly, a 10 687-nucleotide (nt) long sequence of the JHS45 virus was obtained (GenBank accession number: OR593353). Genetic evolutionary analysis revealed that the JHS45 virus formed an evolutionary branch with DENV-1 genotype I viruses prevalent in Xishuangbanna, Guangzhou, Henan, and Zhejiang in China during 2019, as well as the DENV-1 genotype I virus prevalent in Thailand in 2013, with nucleotide homology of 97.6% to 99.9% and amino acid homology of 99.1% to 100%. Further analysis revealed that the JHS45 strain shared a smaller evolutionary branch with the DENV-1 genotype I viruses prevalent in Xishuangbanna (MW386863) and Guangzhou (MW261839) in 2019, showing the highest homology with nucleotide and amino acid homology of 99.9% and 100%, respectively. Amino acid differential site analysis between the JHS45 strain and the DENV-1 prevalent in Xishuangbanna since 2015 revealed 40 amino acid differential sites in the coding region of the JHS45 virus, primarily concentrated in the NS3 and NS5 regions of non-structural proteins. Conclusion The comprehensive analysis of the JHS45 strain's whole genome sequence indicates it is a DENV-1 genotype I virus. The genetic evolutionary relationship between this Xishuangbanna dengue fever outbreak is closely related to the prevalent virus strains in Xishuangbanna, Guangzhou, Henan, and Zhejiang. These findings provide a robust scientific foundation for monitoring dengue fever outbreaks, conducting virus evolution studies, and shaping effective prevention and control strategies, not only within Yunnan Province but also on a broader scale throughout China.

Objective To evaluate the curative effect and toxicities of platinum-based double regimens for patients aged ≥ 65 with advanced non-small-cell lung cancer (NSCLC) and identify the prognosis factors. Methods 70 patients aged ≥65 with staged ⅢA-Ⅳ NSCLC, who received platinum-based double regimens as first line treatment, were emrolled.Response rates and toxicities were evaluated.Progression free survival (PFS) and overall survival (OS) were estimated using the Kaplan-Meier method. Cox regression analysis was used to identify the potential prognosis factors. Results The median chemotherapy cycles was 3.The overall response rate was 41.5 % (22/53), and there was no difference between patients aged <70 and ≥70 (χ2 =1.945, P =0.378).The median PFS and OS were 6.0 months and 12.5 months. The chemotherapyrelated hematologic toxicities were common.Multivariate analysis revealed that performance status, numbers of metastasis, chemotherapy cycles were significant independent predictive factors for OS. Conclusion In elderly advanced NSCLC, platinum-based doublets show inspiring efficacy, but with more adverse events, and could not be all well tolerated. It should be personalized. Patients with poor performance status and multiple organs metastasis are hard to benefit from combined chemotherapy.Three to six cycles of chemotherapy is the optimal duration for patients who could be well tolerated.

Chinese Journal of Perinatal Medicine 2018;21(11):777-779
Ovarian vein thrombosis (OVT) is a rare, insidious and dangerous disease mostly occurs in the postpartum period. Early diagnosis and timely treatment are very important and may avoid serious complications. Here we reported a woman who had a fever and left back pain three days after vaginal delivery. Enhanced CT suggested that thrombosis occurred in the left ovarian vein, and even the renal vein was involved. Thrombus susceptibility gene detection revealed that the plasminogen activator inhibitor-1 gene was 4G/5G heterozygous genotype and the 5,10- methylenetetra-hydrofolate reductase gene was C/T heterozygous genotype. Anticoagulant treatments with low-molecular-weight heparin and rivaroxaban were effective.
