1.Capsaicin provocation test in chronic cough in clinic

Chronic cough in children is one of the most frequent reasons for consultation in clinic. The high cough sensitivity in most patients with chronic cough is difficult for treatment. Capsaicin,as the most com-monly used material for the test of cough sensitivity,is widely used in clinic. Capsaicin provocation test is more mature in foreign country,while in our country there is a few scholars applying it with the adult who suffered chronic cough,but few with children. This article reviews the process,chemical structure,nerve conduction,clini-cal operation,influence factors and safety of capsaicin provocation test.
2.Plasma brain natriuretic peptide/N-terminal pro-brain natriuretic peptide ratio reflects the severity of pulmonary infection in patients with heart failure

Eighty two heart failure (HF) patients were enrolled in the study,including 52 cases with pulmonary infection (infectious group) and 30 cases without pulmonary infection (noninfectious group).Plasma brain natriuretic peptide (BNP),N-terminal pro-brain natriuretic peptide (NT-proBNP) levels were measured and the correlation of plasma BNP,NT-proBNP and NT-proBNP/BNP with inflammatory markers (WBC count,CRP,PCT) and left ventricular ejection fraction (LVEF) was analyzed.Results show that NT-proBNP/BNP ratio in infectious group was significantly higher than that in noninfectious group (t =10.764,P < 0.01).In infectious group,NT-proBNP/BNP was positively correlated with inflammation markers (P < 0.01),but not with LVEF (P > 0.05) ; NT-proBNP and BNP levels were not correlated with inflammation markers and LVEF (P > 0.05).In noninfectious group,NT-proBNP/BNP was not correlated with inflammation markers and LVEF (P > 0.05),NT-proBNP and BNP were positively correlated with CRP and negatively correlated with LVEF (all P < 0.01),not correlated with WBC count or PCT (P > 0.05).Multiple linear regression analysis showed that NT-proBNP/BNP ratio had positive correlation with CRP in infectious group.The results reveal that in heart failure patients complicated with pulmonary infection,neither NT-proBNP nor BNP reflects the severity of heart failure or infection; but the NT-proBNP/BNP ratio can indicate the severity of infection independently.
3.The effects of different pre-hospital intervention of treatment modes on the prognosis of patients with severe multiple trauma

Objective To investigate the effects of different treatment modes on the prognosis of patients with severe trauma.Methods The general data of 396 patients with severe trauma [injurey severity scores (ISS) ≥25] in our hospital emergency for treatment from January 1,2008 to January 1,2012 was collected.The trauma patients were divided into study group and control group.In the study group,the trauma patients were cared by emergency physician of our hospital for pre-hospital treatment during transportation by ambulance since January 1,2010.In the control group,the trauma patients were served with traditional pre-hospital emergency care by the 120 and 999 before January 1,2010.The injury severity score,medical care and outcomes were recorded in trauma database and the efficiency and quality of medical care were compared between two groups.Results The emergency treatment time,length of hospital stays,ICU admission rate,prehospital mortality rate,long-term (6 months) disability rate,and complication rate in the study group were lower than those in the control group,presenting (78.23 ± 21.57) min vs.(96.45 ± 35.14) min,(23.55±12.46) dvs.(28.67±20.72) d,8.1% (18/222)vs.65.5% (114/174),13.3% (34/256) vs.21.6% (48/222),4.1% (9/222)vs.9.2% (16/174),8.1% (18/222)vs.18.4% (32/174),in which the differences were statistically significant (P < 0.05).Hospital mortality in the study group was also lower than that in the control group,showing 8.1% (18/222) vs.12.6% (22/174),but there was no statistically significant difference (P < 0.05).There was no significant difference in time from occurrence of injury to receiving treatment between the two groups.Conclusion Emergency physicians-cared mode had advantages to improve treatment success rates and reduce mortality in patients with multiple trauma compared with the current conventional emergency mode.It is a good alternative emergency mode.
4.The clinical evaluation of piperacillin and sulbactam sodium in treatment of respiratory, urinary tracts and orther infections in 579 patients

Objective To evaluate the clinical efficacy and safety of piperacillin and sulbactam sodium combinations in the treatment of common infections. Methods This was a multi-centre, prospective and open study. All subjects from 57 wards caught common infection like respiratory ( RTI) or urinary diseases ( UTI). The dosages of piperacillin and sulbactam sodium combinations 2. 5 g injection were determined according to indications:for adult, 2. 5 g or 5 g per time, 2 time/day; for severe or obstinate infection, 2.5 g or 5 g per time, 3 time/day. General information, clinical response pre- and posttreatment, infected locus, drug recipe and protocol, prognosis and adverse reaction were recorded. Results Data of 579 cases were collected with 388 males and 191 females. The average age was (66. 8 ± 17. 0) years. There were 500 patients who were suffering with RTI, with 362 cases of pneumonia, 102 of acute exacerbation of chronic bronchitis, and 36 of other infections. There were 50 cases with UTI, with 31 of simple urinary tract infection, and 19 of complex urinary tract infection. In addition, there were 9 cases of combined RTI and UTI, and 20 of other infections including peritonitis. The average duration of antimicrobial for RTI and UTI was (8. 65 ± 3. 78 ) days and (7. 45 ± 3. 46) days respectively with the total efficacy rate was 92. 6% and 98. 0% respectively for RTI and UTI. The incidence of adverse events was only 0.86% (5 cases), including nausea, rash, itching, ALT elevation and suspected drug induced fever in each one. Conclusion Piperacillin and sulbactam sodium compound had high clinical efficacy and safety in the treatment of common infections including RTI and UTI.
5.Observation of the efficacy of allergen blocking nasal spray in patients with rhinitis sicca.

OBJECTIVE:
To observe the efficacy of allergen blocking nasal spray (ABNS) , combined with seawater spray (SS) and myrtol standardized capsules (MSS), in patients with rhinitis sicca.
METHOD:
Patients diagnosed as rhinitis sicca were randomly divided into 2 groups: ABNS+SS group and SS group. In ABNS+SS group, patients used ABNS and SS four times per day and MSS twice per day. In SS group, patients used the same medicine except ABNS. Both group were observed for 14 days. The visual analog scale (VAS) of dryness, the endoscopy evaluation score (EES) and the tolerability of the spray were compared between the 2 groups.
RESULT:
There were 30 patients included in each group. The VAS and EES were improved in both groups after 2 weeks' treatment. The improvement of VAS and EES in ABNS+SS group was significantly superior to SS group (VAS: P < 0.01, EES: P < 0.05). There were only 2 patients complaint of slight discomfort when using ABNS, which did not disturb the treatment.
CONCLUSION
There is good efficacy of allergen blocking nasal spray (ABNS), combined with seawater spray (SS) and myrtol standardized capsules (MSS), in patients with rhinitis sicca.

6.Efficacy evaluation of specific immunotherapy with standardized dermatophagoides pteronyssinus extract for allergic rhinitis accompanied with asthma

Objective:To evaluate the efficacy of specific immunotherapy (SIT) with standardized dermatophagoides pteronyssinus extract for allergic rhinitis (AR)accompanied with asthma.Method:One hundred and fifty-five patients(40 AR with asthma, AR & asthma) in accordance with the inclusion criteria of SIT, were allocated to receive standardized Dermatophagoides pteronyssinus extract (SIT group, n=89) or medical treatment(control group, n=66). AR with or without asthma was observed separately. Symptom and medicine scores, quality of life were recorded and analyzed before and after 1 year treatment. Side effects were registered. Subjective evaluation of symptoms was made by the patients.Result:Rhinitis and asthma symptom scores, medicine scores and quality of life were greatly improved in SIT group of AR & asthma after 1 year, which were not significant changed in control group except for medicine scores. The subjective evaluation of symptoms was also significantly improved in SIT group. In patients of AR without asthma, the symptom scores, medicine scores and quality of life were both improved. The SIT group improved greater than that of control group.Conclusion:The clinical efficacy of patients with AR & asthma was not good with simple medical treatment, while great clinical efficacy could be acquired with SIT.
7.Relationships between changes of coagulation-fibrinolytic system paratmeters and recent dissolution of thrombus after treatment in patients with pulmonary embolism

Objective To discuss the relationships between regular pattern changes of plasma fibrinogen (Fib),D-dimer and fibrinogen degradation products (FDP) levels and the recent dissolution of thrombus in patients with pulmonary embolism (PE) in 14 days after treatment.Methods A prospective study was conducted.PE patients admitted to Departments of Respiratory Disease in 4 hospitals from January 2015 to March 2016 were enrolled and all of them were treated with thrombolysis and/or anticoagulation after admission.The computed tomographic pulmonary angiography (CTPA) was examined pre-treatment and 14 days post-treatment in PE patients.The pulmonary artery obstruction index (PAOI) was assessed according to the Mastora scoring method to estimate the thrombus load.The plasma Fib,D-dimer and FDP levels were measured before and on 1,2,3,5,7 and 14 days after treatment,and the relationships between the change regularities of these parameters and PAOI were also analyzed.Results A total of 42 PE patients were enrolled.The curve change of coagulation-fibrinolytic system parameters in 14 days after treatment showed that the Fib level was raised to its peak on the 3rd day after treatment and then decreased (g/L:4.24 ± 1.45 vs.3.83 ± 1.56),representing that its curve change was in accordance with the quadratic model (P =0.095).After treatment,the D-dimer and FDP levels were kept declining,they were reached the valley on 14th day [D-dimer (mg/L):1.58 ± 1.38vs.8.84 ± 6.35,FDP (mg/L):4.23 ± 3.63 vs.23.41 ± 16.54],and their curve changes were in accordance with the cubic model (F was 32.190 and 34.326,respectively both P =0.000).The PAOI variation before and 14 days after treatment [(18.77 ± 14.22)%] was not correlated with Fib variation [(1.20 ± 0.93) g/L,r =-0.194,P =0.219],but was positively correlated with D-dimer variation [(7.29 ± 7.10) mg/L] and FDP variation [(19.29 ± 18.67) mg/L,r was 0.556 and 0.460,respectively;P was 0.020 and 0.002,respectively].Conclusions The D-dimer and FDP levels are kept falling in PE patients after treatment,suggesting that the pulmonary artery embolus is being dissolved.
8.The expression of serum IL-10,12,13,16 in patients with allergic rhinitis and vasomotor rhinitis.

OBJECTIVE:
To study the role of serum IL-10, 12, 13, 16 in patients with allergic rhinitis and vasomotor rhinitis.
METHOD:
The serum levels of IL-10, 12, 13, 16 were measured by ELISA in 30 cases of allergic rhinitis, 25 cases of vasomotor rhinitis and 20 healthy people.
RESULT:
The level of IL-12 in allergic rhinitis was (170.33 +/- 90.58) ng/L, which was significantly lower than that of normal controls [(376.69 +/- 140.70) ng/L, P < 0.01]. The levels of IL-13 and IL-16 in allergic rhinitis were (408.51 +/- 189.68) ng/L and (151.53 +/- 63.56) ng/L, which were significantly higher than those of normal controls [(151.92 +/- 85.08) ng/L, (60.65 +/- 32.45) ng/L, P < 0.01]. There were no significant difference of levels of IL-10, 13, 16 between vasomotor rhinitis and normal controls, while the level of IL-12 in vasomotor rhinitis was lower than that of normal controls [(196.03 +/- 96.31) ng/L vs. (376.69 +/- 140.70) ng/L, P < 0.01]. It was suggested that IL-10 had positive correlation with IL-12 (r = 0.73, P < 0.01), and IL-13 had positive correlation with IL-16 (r = 0.94, P < 0.01).
CONCLUSION
The imbalance of IL-12, IL-13 and IL-16 play crucial roles of regulation in the onset and developing of allergic rhinitis. Further research is needed on the role of IL-12 in vasomotor rhinitis.

9.Efficacy evaluation of polysaccharide nucleic acid-fraction of BCG on vasomotor rhinitis.

OBJECTIVE:
To investigate the efficacy and safety of polysaccharide nucleic acid-fraction (BCG-PSN) in the treatment of vasomotor rhinitis.
METHOD:
Sixty patients were randomly divided into BCG-PSN group (n = 30) and control group (n = 30). The patients in BCG-PSN group were administered with BCG-PSN 1.0 mg twice a week for two months, and intranasal azelastine was used if needed. The patients in control group were administered with intranasal azelastine solely twice a day, which could be decreased with the symptom relief. Follow-up was 6 months. Symptom and medication scores were recorded. Side effects were registered.
RESULT:
The symptom and medication scores of BCG-PSN group were significantly lower than that of control group (P < 0.01) after BCG-PSN treatment. There was no significant difference in symptom score between the two groups at 6 months after BCG-PSN treatment (P > 0.05), while the medication score of BCG-PSN group was still much lower than that of control group (P < 0.01). No serious adverse events were reported in BCG-PSN group except for local pain on the injection place in one patient.
CONCLUSION
BCG-PSN is effective and safe in the treatment of vasomotor rhinitis.

10.Age features of the allergens in allergic rhinitis patients of different age in Hubei area.

OBJECTIVE:
To investigate the distribution of positive allergens in patients with allergic rhinitis and the discrepancy among different age group.
METHOD:
Skin prick tests were done to 2 808 allergic rhinitis patients, the major allergens of local area were tested with standard allergens. Data was analyzed with age and allergens.
RESULT:
Dermatophagoides pteronyssinus and dermatophagoides farinae had the highest positivity among all allergens (91.03%, 86.47%). Other important allergens were cockroach (19.55%), ragweed (15.63%) and mugwort (14.13%) subsequently. The distribution of major allergens was variable among different age group. The positivity of mold mixture I was much higher in 3-9 years old group than other age group. Positive percentage of ragweed and mugwort allergens were increased with age growing. Positive ratio of cockroach which was higher in the elder than that in the younger, was highest in 30-39 years old group. Ratio in patients with single allergen positive decreased with age growing.
CONCLUSION
House dust mite was the most important allergen of the local area, the distribution of positive allergens was variable in different age group.
