1.The Relationship between the Blood Pressure in Subjects with Impaired Glucose Relation and the Tirst-phase Insulin Secretion

Objective To investigate the relationship between the blood pressure in subjects with impaired glucose relation and the first-phase insulin secretion.Methods The oral glucose tolerance test was performed in 126 subjects who were divided into normal blood pressure group higher blood pressure group and hypertension group.The beta-cell function was calculated by HOMA-IR and △I30/△G30.Results ①The BMI and WHR were significantly higher in hypertension group.The serum level of lipids was changed in higher blood pressure and hypertension groups.② The fasting plasma insulin level,60、120 minutes insulin level after oral 75 game glucose were significantly higher in hypertension group.③ The HOMA-IR was significantly higher in higher blood pressure and hypertension groups,with more higher in hypertension group.④ The △I30/△G30 was significantly decrease in hypertension group.There are no different in other groups.Conclusion The higher blood pressure,the more severe insulin resistance,the more decrease first-phase insulin secretion.
2.Nursing of patients with inhalation injury during bedside fibrobronchoscope

Objective To explore the nursing method during bedside fibrobronehoseope to patients with inhalation injury and ensure the safety of the procedures. Methods 58 patients with inhalation injury with bed-side flbrobronehoseope treatment from 2005 to 2008 were retrospectively analyzed. Results 3 eases showed a-cute respiratory distress syndrome, 3 cases died of respiratory failure, 2 patients died of multiple organ failure,dyspnea, cough symptoms improved in others after sputum aspiration. Conclusions To master the preoperative,intraoperative, postoperative nursing methods during fibmbronehoscope can improve clinical efficacy.
3.Acellular porcine aorta matrix as a novel tissue engineered vascular scaffold

BACKGROUND: The key of tissue engineered blood vessel research depends on the appropriate scaffolds. The porcine vascular tubes have frequently been used as candidates for tissue engineering vessel construction, but high immunogenicity and poor mechanical strength limit its application for tissue engineering scaffolds.OBJECTIVE: To prepare a novel tissue engineered vascular scaffold with good mechanical properties and biocompatibility using acellular porcine aorta matrix.METHODS: Porcine aorta was decellularized and modified by thermal cross-linking to improve the mechanical strength and biodegradation properties and to prepare acellular porcine aorta matrix scaffolds. Hematoxylin-eosin staining of histological sections and biomechanical tests were performed to assess the decellularization effects and the mechanical strength of the vascular matrix respectively. Vascular endothelial cells from human umbilical cord veins were isolated and seeded on the acellular matrix scaffolds and cultured in vitro. The biocompatibility was evaluated. RESULTS AND CONCLUSION: After the porcine aorta was treated by 1% Triton X-100 solution for 84 hours, the vessel was fully decellularized, and the architecture of matrix was well preserved. The acellular vascular matrix demonstrated improved biomechanical properties after modification by thermal cross-linking under vacuum at 120 °C for 12 hours. Its tensile strength reached 1.70 MPa. After 7 days of in vitro culture, the seeded endothelial cells formed a typical vascular endothelial layer structure on the surface of acellular matrix, as observed by scanning electron microscopy. Results proved that acellular vascular matrix of porcine aorta could maintain the morphology and structure of natural vessels, its mechanical strength could be greatly improved after successive freeze-drying and thermal cross-linking. A good compatibility between the acellular matrix and endothelial cells of umbilical vein is also achieved.

Objective To observe the effect of Sijunzi Decoction(SD) and Huangqi Sijunzi Decoction(HSD) on mitochondrial succinate dehydrogenase,cytochrome oxidase activity of liver in rats with spleen asthenia,thus to reveal the pathogenesis mechanism of spleen asthenia from energy metabolism,and to clarify the therapeutic mechanism of spleen-invigorating and Qi-tonifying herbs.Methods Forty SD rats were randomly divided into 4 groups:normal control group,spleen-asthenia model group,SD(10.5 g/kg) group and HSD(18 g/kg) group,10 rats in each group.Except that the normal control group,the rats in the other groups were fed with half-full diet once every other day and with gastic gavage of Xiaochengqi Decoction 60 g/kg every day to establish animal model of spleen asthenia.After 15 days,the model rats were administered the corresponding drugs according to the experimental design for 4 continuous weeks.The changes of mitochondrial succinate dehydrogenase,cytochrome oxidase activity of liver were measured,and the general status of the rats were observed.Results Compared with the normal control group,the level of mitochondrial succinate dehydrogenase and cytochrome oxidase activity in liver of spleen asthenia group were decreased obviously(P

Objective Research on the relation between the content of thiol group in chitosan-thioglycolic acid conjugates and in vitro adhesion. Methods Prepared chitosan-thioglycolic acid conjugates with different content of thiol group,and determined the swelling behavior,instant detachment force between test disc and mucosa,recorded the maximum detachment force(MDF),and calculated the total work of adhesion(TWA)of these samples.Results The relation between the content of thiol groups and in vitro adhesion of the chitosan-thioglycolic acid conjugates was positive correlation, the adhesion of the polymer increased along with the content of thiol group,but the rate decreased.Conclusion The adhesion of chitosan-thioglycolic acid conjugates with higher content of thiol gyoup was far more stronger than chitosan.
6.Intrapartum operation on fetuses with birth defects and its outcome

Objective To discuss the value of intrapartum operation in management of birth defects and the prognosis. Methods From August 2008 to November 2009, 11 fetuses were identified with birth defects through 3D color Doppler ultrasound and confirmed by MRI and fetal karyotype in the Maternal Fetal Medicine Center, Affiliated Shengjing Hospital, China Medical University including three lymphangiomas,two congenital diaphragmatic hernias (CDH), one sacrococcygeal teratoma, three omphalocele and two gastroschisi. All the above identified birth defects were indications for surgery. All fetuses were born abdominally and received intrapartum operations, including three intrapartum fetal operations with placental infusion (two repairs of CDH, one sacrococcygeal teratoma resection), six ex-utero intrapartum treatment (EXIT; two repairs of omphalocele, two repairs of gastroschisi, two lymphangioma resection) and two surgeries in house (one omphalocele repair and one lymphangioma resection). Both the mothers and fetuses were regularly followed up. Results ( 1 ) Operations: the average operating time for the three intrapartum fetal operations was 89 minites, 5.5 minites for the six EXIT, during which EXIT was performed first,followed by blocking the umbilical circulation and neonatal surgery, and 37 minites for the two surgeries in house. All neonates survived except for one death from severe CDH at 3.5 hours after the operation. The average blood loss for cesarean section and fetal operation was 275 ml. All mothers recovered soon without fever or infection and were discharged three to five days after the operation. (2) Follow-ups: the ten survived neonates were followed up at 1 - 18 mouths at the pediatric clinics and all were growing and developing normally except for one baby with gastroschisi suffered from enteral torsion and feeding intolerance showed lower weight than babies at the same age, but catched up to normal at four months old after posture therapy. One baby with mild CDH developed pulmonary infection at two months after operation with 1/4 pneumothorax on chest X-ray, and were hospitalized for two weeks. At six months old, patent ductus arteriosus was diagnosed in the same baby and chest X-ray was normal. The baby with omphalocele was complicated with ventricular septal defect before operation and the cardiac function was normal during followups for one year. The baby with sacrococcygeal teratoma was reported to have no automatic micturition, but recovered to normal at one month of age. Conclusion Babies with certain birth defects can be managed through intrapartum operation with better outcomes.
7.Preparation of ibuprofen/EC-PVP sustained-release composite particles by supercritical CO2 anti-solvent technology.

Ibuprofen/ethyl-cellulose (EC)-polyvinylpyrrolidone (PVP) sustained-release composite particles were prepared by using supercritical CO2 anti-solvent technology. With drug loading as the main evaluation index, orthogonal experimental design was used to optimize the preparation process of EC-PVP/ibuprofen composite particles. The experiments such as encapsulation efficiency, particle size distribution, electron microscope analysis, infrared spectrum (IR), differential scanning calorimetry (DSC) and in vitro dissolution were used to analyze the optimal process combination. The orthogonal experimental optimization process conditions were set as follows: crystallization temperature 40 degrees C, crystallization pressure 12 MPa, PVP concentration 4 mgmL(-1), and CO2 velocity 3.5 Lmin(-1). Under the optimal conditions, the drug loading and encapsulation efficiency of ibuprofen/EC-PVP composite particles were 12.14% and 52.21%, and the average particle size of the particles was 27.621 microm. IR and DSC analysis showed that PVP might complex with EC. The experiments of in vitro dissolution showed that ibuprofen/EC-PVP composite particles had good sustained-release effect. Experiment results showed that, ibuprofen/EC-PVP sustained-release composite particles can be prepared by supercritical CO2 anti-solvent technology.
8.Study of anti-cancer traditional Chinese medicine using systems biology technology

This article focuses on the research of molecular mechanism of brain tumor treatment using the Jinlong capsule via system biology technology. Methods:Human Genome U133 Plus 2.0 gene chip was used to detect the genes of samples, in-cluding the brain tumor tissues of nude mice after Jinlong capsule intervention and those of blank control group mice. Differentially ex-pressed genes were identified based on fold change between the two groups. To identify the upstream regulators of the response signa-tures, the differentially expressed genes were subjected to interactome analysis by one-step overconnectivity test and multi-step hidden node algorithm. A set of genes preferentially connected to differentially expressed genes via direct interactions and pathways (called to-pologically significant genes) was generated. Concurrent pathway enrichment analysis on key pathways, processes, and functional units of both differentially expressed genes and topologically significant genes was performed to identify the most likely signaling pathways connecting regulators and effector genes. Finally, condition-specific networks (called causal network) were built to model molecular events by using a set of manually annotated protein interactions, pathways, and proteins and a toolkit of algorithms and filters on Meta-Core platform. Results:A total of 37 differentially expressed genes have been identified between Jinlong capsule-treated sample and ve-hicle sample with fold change of 2. Connection analysis identified 106 topologically significant genes. The main feature of the causal network is stimulation of neural cell specific genes that regulate normal cell physiology, particularly developmental processes and apop-tosis. Another important effect of the Jinlong capsule is its inhibition of the gene markers of interferon response, suggesting signaling in-hibition, followed by de-activation of immune response. Conclusion:Jinlong capsule exerts an anti-neoplastic effect by inducing stimu-lation of neural cell and by inhibiting interferon signal transduction.
9.Anti-tumor activity of HER-specific peptide vaccines-CKL9 and YL20

Aim To explore whether the polypeptide vaccines CKL9 and YL20 can induce immune response and anti-tumor effect on HER-2 (+) tumors in vitro and in vivo, and to provide suggestions for clinical use.Methods The proliferation of specific lymphocytes and cytotoxic T lymphocyte activity(CTL) stimulated by CKL9 and YL20 were studied with CCK-8 assay and LDH assay, and the antitumor activity of CKL9 and YL20 was evaluated in vivo.Results The lymphocyte proliferation was promoted by incubation with CKL9 and YL20, and the relative increase of cells was 11.1% and 16.7% respectively at the concentration of 50 mg·L-1 of CKL9 and YL20.The LDH assay confirmed the CTL effect induced by CKL9 and YL20 on HER2-positive tumor cells, not on HER2-negtive tumor cells.With an effector-target ratio of 80 ∶1, the inhibition of tumor cell by cytotoxic T lymphocyte stimulated by CKL9 and YL20 could reach 89.8% and 84.3%, respectively.The HER2(+) tumor cell N87 transplanted in Babes mice was inhibited by pre-immune polypeptide CKL9 and YL20.Conclusion The HER2-specific polypeptide vaccines CKL9 and YL20 could induce persistent specific CD4 and CD8 T cell immune and inhibit the growth of HER2 positive tumor cells.
10.Therapeutic drug monitoring of mizoribine in renal transplant recipients

Mizoribine(MZR), as an orally prescribed immunosuppressive agent, has been applied in the prevention of rejection after kidney transplantation.MZR requires individual dosing due to the variation of bioavailability.However, therapeutic drug monitoring (TDM) of MZR is not well developed in China, as compared to other clinically used immunosuppressive agents.To our knowledge, this is the first TDM review of MZR.Pharmacokinetic characteristic, concentration determination methods and sample selection of MZR were summarized, also the rational therapeutic window was proposed.Furthermore, gene polymorphism and population pharmacokinetics of MZR were estimated.This review will provide reference for TDM-based individual dosing of MZR in renal transplant recipients.
