1.Value of serum iodine, urinary iodine and thyroid function in differentiating painless thyroiditis

Objective To discuss the difference of levels of serum and urinary iodine and thyroid function in painless thyroiditis patients and other thyroiditis patients.Methods The patients who were preliminary diagnosed as having higher level of serum thyroid hormone were divided into Graves' disease group,subacute thyroiditis group and painless thyroiditis group.Then all the patients were divided into another two groups according to the level of autoantibody.The value of serumal thyroid hormone and serumal iodine and urinary iodine were measured and the relationship between the groups and the value were observed.Results There was close relationship between serum T 3,T 4,urinary iodine and the types of thyrotoxicosis.Conclusion The morbidity of painless thyroiditis is not uncommon.Clinical situation,serumal iodine,urinary iodine and serumal thyroid hormone may be useful for clinical diagnosis and differential diagnosis of painless thyroiditis.
2.Effects of different concentrations of glucose on the expressions of osteoprotegerin,the ligand of osteoprotegerin and related cytokines in MG63 cells

Objective:To observe the regulative effects of different concentrations of glucose on the expressions of osteoprotegerin(OPG),the ligand of osteoprotegerin(OPGL) and the related cytokines[tumor necrosis factor related apoptosis inducing ligand(TRAIL),macrophage colony-stimulating factor(M-CSF) and transforming growth factor ?(TGF-?)] in osteosarcoma MG63 cells.Methods:The expressions of OPG,OPGL,M-CSF,TRAIL and TGF-? mRNA was examined by reverse transcriptase(RT)-PCR.Results:High concentration glucose up-regulated the expression of OPGL,M-CSF and TRAIL but down-regulated OPG and TGF-? expression in the MG63 cells.Conclusion:One of the key pathogenetic factors of diabetic osteoporosis is that high concentration glucose leads to the down-regulated expression of OPG and TGF-? but the up-regulated expression of some bone-resorbing cytokines such as OPGL,M-CSF and TRAIL in osteoblasts,then stimulates osteoclast differentiation and activity,which potentiates bone resorption and bone loss.
3.Clinical analysis of 61 cases of drug eruptions complicated with drug-induced liver dysfunction

Objective To investigate whether the sort of drugs causing drug eruptions complicated with drug-induced liver dysfunction are the same as those causing drug eruptions. Methods Sixty-one cases of drug eruption complicated with drug-induced liver dysfunction in 261 cases confirmed as drug eruptions from January 1998 to March 2003 were analyzed retrospectively. Results The major hepatotoxic drugs in these cases were antivirus drugs (60%, 9/15), antituberculosis drugs (66.67%, 8/12), zyloric (55.56%, 5/9), and some traditional Chinese medicines (31.58%, 6/19). Conclusion The sort of drugs causing drug eruptions complicated with drug-induced liver dysfunction are the same as those causing drug eruptions, which should be taken into consideration in clinical medication.
4.STUDIES ON LOCALIZATION OF GONADOTROPIN-RELEASING HORMONE RECEPTORS IN RAT LIVER

Objective To detect the existence of gonadotropin-releasing hormone receptors(GnRH-R) in rat liver,and to supply morphological evidence for studying the functional significance of the GnRH in hepatocyte. Methods Immunohistochemical SABC method and in situ hybridization with a digoxigenin labeled probe were used to study the localization of GnRH-R in the livers of five rats. Results The rat hepatocytes were found with the GnRH-R immunoreactivity and GnRHR mRNA hybridization signals.The GnRH-R immunoreactive substance was distributed in the cytoplasm of all positive cells,with immuno-negative nuclei.The GnRHR mRNA hybridization signals were also detected in the cytoplasm of all positive cells but not in the nuclei.Hepatocytes in different parts of liver lobules showed different intensity of GnRH-R immunoreactivity and GnRH-R mRNA hybridization signals.Conclusion The rat hepatocyte may express GnRH-R.The hepatocytes in different parts in liver lobules exhibit different levels of GnRH-R expression.
5.Relationship between total sermn bilirubin and carotid atherosclerosis in type 2 diabetes mellitus

Ultrasonography examinations were performed for carotid atherosclerosis in 548 patients with type 2 diabetes mellitus (T2DM). Blood glucose, glycosylated hemoglobin, total serum bilirubin, uric acid, total cholesterol ( TC ), triglycerides ( TG), high-density lipoprotein (HDL), low-density lipoprotein (LDL) were measured. Total serum bilirubin level in T2DM patients without carotid atherosclerosis was (13.0±7. 1 )μmol/L, while that was (10. 9 ± 4. 9 )μmol/L in patients with carotid atherosclerosis (P <0. 01 ). The logistic analysis showed that total serum bilirubin level was negatively related to the presence of carotid atherosclerosis ( P < 0. 05 ), which indicated that higher level of total serum bilirubin might be a protective factor for macroangiopathy in T2DM.
6.Establishment and preliminary evaluation of a rapid detection method for germ tube-specific antigens of Candida albicans

Objective To establish a double-antibody sandwich enzyme linked immunosorbent assay (DAS-ELISA) for the rapid detection of germ tube-specific antigens of Candida albicans,and to evaluate its specificity and sensitivity.Methods A DAS-ELISA was established with the monoclonal antibody McAb03.2C1-C2 as the primary antibody,and horseradish peroxidase (HRP) labeled McAb03.2C1-C2 as the secondary antibody.The established assay was used to detect germ tube-specific antigens of Candida albicans in sera from 5 patients with systemic Candida albicans infection and from 6 rabbit models at 12,24,48,72hours,on week 1,2 after infection with Candida albicans.Results A good liner relationship was observed between the absorbance value at 495 nm and antigen concentrations when the titer of McAb03.2C1-C2 was 1 ∶ 4000 and the concentration of coated antigen varied from 1.25 to 40 μg/ml.The specificity and sensitivity of the DAS-ELISA were 95% and 92% respectively in the detection of germ tube-specific antigens in the rabbit models.The results of detection with DAS-ELISA in serum specimens from the patients were consistent with those with the routine method.Conclusions A DAS-ELISA is primarily established for the rapid detection of germ tube-specific antigens of Candida albicans,and has shown a satisfactory sensitivity and specificity in the animal model experiment.
7.Blood glucose control in a patient with diabetes mellitus after facial composite allotransplantation

A facial allotransplanted patient presented hyperglycemia with blood glucose ranged 14. 3 -33. 3 mmol/L after receiving immunosuppressive drugs and glucocorticoids. To control the blood glucose level, the patient was treated with two subcutaneous doses of 10 U human neutral protamine hagedorn (NPH) insulin, and the fasting glucose level came down to 3. 6 - 9. 4 mmol/L. Then the continuous subcutaneous infusion of insulin aspart ( Novo Industri) was administrated (from 96 to 21 U/d) , and the fasting blood glucose levels were 3. 9 -4. 6 mmol/L. With oral administration of Metformin and Repaglinide, the fasting blood glucose was maintained to 4. 3 -5.9 mmol/L. With these medications, the blood glucose level of the patient was under good control and the acute and chronic complications of hyperglycemia were effectively prevented.
8.Value of Modified Response Evaluation Criteria in Solid Tumors response in predicting the prognosis of patients with unresectable hepatocellular carcinoma patients after transarterial chemoembolization

ObjectiveTo investigate the association of Modified Response Evaluation Criteria in Solid Tumors (mRECIST) response with the prognosis of patients with unresectable hepatocellular carcinoma (HCC) after transarterial embolization (TACE). MethodsA retrospective analysis was performed for the clinical data of 190 patients with unresectable HCC who were consecutively admitted to Department of Liver Disease and Digestive Interventional Radiology, The First Affiliated Hospital of Air Force Medical University, and treated with TACE from January 2010 to December 2014. The mRECIST criteria were used to evaluate imaging response after TACE; the patients with complete response (CR) or partial response (PR) were enrolled as response group(n=89), and those with progressive disease (PD) or stable disease (SD) were enrolled as non-response group(n=101). The Kaplan-Meier method was used to calculate median survival time, and the log-rank test was used for comparison between groups; the Cox regression model was used to identify the influencing factors for prognosis. ResultsAccording to the mRECIST criteria, 39 patients (20.5%) achieved CR, 50 (26.3%) achieved PR, 67 (35.3%) had SD, and 34 (17.9%) had PD. The objective response rate based on mRECIST was 46.8% for the whole population. The response group had a significantly longer survival time than the non-response group, and the median survival time was 29.9 (95% confidence interval [CI]: 25.0-34.8) months for the response group and 7.5 (95% CI: 5.7-9.3) months for the non-response group (P<0.001). The multivariate analysis showed that mRECIST response (hazard ratio [HR]=2.02, P<0.001), hepatitis B (HR=4.03, P<0.001), and portal invasion (HR=2.12, P=0.008) were independent risk factors for survival. ConclusionThe mRECIST response has a certain value in predicting the prognosis of patients with unresectable HCC after TACE.

Objective Recombinant human parathyroid hormone(1-34) [ rhPTH(1-34)] is the unique anabolic substance acting on the skeleton. The efficacy and safety of long-term administration of rhPTH(1-34) in Chinese postmenopausal women have not been evaluated. This study compared the clinical efficacy and safety of rhPTH(1-34) with elcatonin for treating postmenopausal women with osteoporosis in 11 urban areas of China. Methods A total of453 postmenopausal women with osteoporosis were enrolled in an 18-month, multi-center, randomized, controlled study. They were randomized to receive either rhPTH(1-34) 20 μg(200 U) daily for 18 months, or elcatonin 20 U weekly for 12 months. Lumbar spine ( L1-4) and femoral neck bone mineral density (BMD), fracture rate, back pain as well as biochemical markers of bone turnover ( serum bone-specific alkaline phosphatase was measured by radioimmunoassay; C-telopeptide/ creatinine ( CTX/ Cr) measured by quantitative sandwich enzyme-linked immunosorbent assay) at 6, 12, and 18 months. Adverse events were recorded. Results rhPTH(1-34) increased lumbar BMD more significantly than that did by elcatonin at 6 months( M6), 12 months (M12), and 18 months(M18; 4. 3% vs 1. 94% , 6. 8% vs 2. 72% , 9. 51% vs 2. 86% , P<0. 01). There was only a small but significant increase of femoral neck BMD at M18(2. 64% , P<0. 01) in rhPTH(1-34) groups. There were greater increases in bone turnover markers in the rhPTH(1-34) group than in the elcatonin group at M6, M12, and M18[serum bone-specific alkaline phosphatase(BSAP) 93. 67% vs -3. 56% , 117. 78% vs -4. 12% , 49. 24% vs-5. 81% , P<0. 01; urinary CTX/ Cr 250% vs -29. 5% , 330% vs -41. 4% , 273 % vs -10. 6% , P<0. 01]. rhPTH (1-34) showed similar effect of pain relief as elcatonin. The incidence of clinical fractures was 5. 36% (6 / 112) in elcatonin group and 3. 23% ( 11 / 341 ) in rhPTH ( 1-34 ) group ( P = 0. 303 ). Both treatments were well tolerated. Hypercaluria(9. 38% ) and hypercalcemia(7. 04% ) in rhPTH(1-34) group was transient and caused no clinical symptoms. Pruritus(8. 21% vs 2. 68, P=0. 044) and redness of injection site(4. 40% vs 0, P=0. 024) were more frequent in rhPTH(1-34). Nausea / vomiting(16. 07% vs 6. 16% , P = 0. 001) and hot flushes(7. 14% vs 0. 59% , P<0. 001) were more common in elcatonin group. Conclusion rhPTH(1-34) treatment was associated with greater increases in lumbar spine BMD and bone formation markers. It could increase femoral BMD after 18 months treatment. rhPTH(1-34) could ameliorate back pain effectively. The results of the present study indicate that rhPTH(1-34) is an effective, and safe agent in treating postmenopausal women with osteoporosis.
10.A randomized, multicenter, active-controlled trial to compare the efficacy of recombinant human parathyroid hormone (1-34) with that of elcatonin in postmenopausal women with osteoporosis in China

Objective To compare the clinical efficacy and safety between recombinant human parathyroid hormone ( rhPTH) ( 1 -34) and elcatonin in the treatment of postmenopausal women with osteoporosis in China.Methods This 6 month, multicenter, randomized and controlled study enrolled 205 postmenopausal women with osteoporosis.They were randomized to receive either rhPTH (1 -34) 20 μg (200 U) daily or elcatonin 20 U weekly.Lumbar spine (L1-4 ) and femoral neck bone mineral density (BMD) and biochemical markers of bone turnover were measured. In the meantime adverse events were recorded. Results The results showed that both rhPTH ( 1 -34) and elcatonin increased L1-4 BMD significantly at the endpoint of the study, but femoral neck BMD did not change significantly.From baseline to endpoint, BMD of L1-4 and femoral neck in the rhPTH( 1-34) group increased by 5.51% (P <0.01) and 0.65% (P >0.05), but BMD of L1-4 and femoral neck in elcatonin group increased by 1.55% (P <0.05) and 0.11% (P>0.05).Moreover, the rhPTH(1-34) group had better improvement in L1-4 BMD than the elcatonin group at 3, 6 months, but there was no difference of BMD in these two groups with regard to femoral neck.There were greater mean increases of the bone markers in the rhPTH( 1-34) group than those in the elcatonin group at 3, 6 months [serum bone-specific alkaline phosphatase ( BSAP) 36.79% vs 0.31% ; 92.42% vs -0.17% ; the ratio of urine N-telopeptide of type I collagen and creatinine (NTX/Cr) 48.91% vs -5.32% ; 68.82% vs - 10.86%].Both kinds of treatment were well tolerated and there were no differences between the two groups in the rates of adverse events and serious adverse events.Conclusion It is concluded that rhPTH (1 -34) has more positive effects on bone formation than elcatonin as shown by the greater increments of L1-4 BMD and bone formation markers and the less occurrence of adverse events as well as no significant change in hepatic, renal or hemopoietic function.
