1.Comparison of the effects of autotransfusion and homologous transfusion on milieu interieur and secretion of erythropoietin

Objective To compare the effects of autotransfusion and homologous transfusion on milieu interieurand secretion of erythropoietin (EPO) .Methods Thirty patients aged 8-21 yr undergoing surgical correction of kyphoscoliosis were randomly divided into two groups of fifteen: group A and group B. In group A patients received homologous transfusion during operation, while in group B shed blood was collected from operative field and after filtration RBCs were separated by a differential centrifugation and washed in normal saline and reinfused. Anesthesia was induced with fentanyl 2 ?g? kg-1 , propofol 1 mg ? kg-1 and vecuronium 0.1 mg ? kg-1 and maintained with inhalation of isofiurane and 50% N2O in oxygen and intermittent iv boluses of vecuronium and fentanyl. Blood routine, blood gases, electrolytes, plasma osmotic pressure and activated clotting time (ACT) were measured before operation, 2 h after skin incision and at the end of surgery. Blood EPO concentration was measured before operation, at the end of surgery and 24 h after operation. Results MAP and HCO3- were significantly lower during operation in group A than those in group B. Platelet count was significantly lower at the end of operation in group B than in group A but still within normal range. The EPO level increased at the end of operation and returned to preoperative level 24 h after operation in both groups, and there was no significant difference in EPO levels between the two groups. Conclusion Autotransfusion of the washes RBCs is helpful in maintaining the patients milieu interieur. There is no difference in EPO secretion and erythropoiesis between autotransfusion and homologous transfusion.

3.Management of Non-medical Prescribing in Britain and Its Enlightenment to China

OBJECTIVE:To systematically investigate the management mode of non-medical prescribing in Britain and its enlightenment to the establishment and implementation of non-medical prescribing in China. METHODS:By retrieving domestic and foreign literatures,non-medical prescribing in Britain was introduced systematically in terms of development history,types, core elements,implementation status and effect;the suggestions were put forward for the development of non-medical prescribing in China. RESULTS & CONCLUSIONS:Britain implemented non-medical prescribing since 1994. After years of practice and a series of legislation and revision,independent prescribers (including nurses and pharmacists) have owned the statutory right to prescribe any medicine for patients by 2012. In Britain,non-medical prescribing is divided into independent prescribing which mainly exists in community pharmacies and is limited to specific diseases,supplementary prescribing which exists in hospitals or clinic and needs to be signed with doctors. The management institutions are mainly composed of one state administrative department (British Department of Health) and three professional associations (British Nursing and Midwifery Council,The Royal Pharmaceutical Society of Great Britain,The Pharmaceutical Society of Northern Ireland). There are detailed and strict stipulations on the qualification and corresponding responsibilities of the prescribing authority. The British government has provided legal protection for the development of the non-medical prescribing model,and the number of non-medical prescribers is increasing. The implementation of this model has increased the patients'access to medical services. Drawing lessons from the development of non-medical prescribing management in Britain,our country needs to improve legislation,provide legislative protection for the implementation of non-medical prescribing,expand the scope of practice of nurses and pharmacists,establish and improve the training program of non-medical prescribing professionals to promote the development of non-medical prescribing model in China.
4.Application of mutual management model in recovery of multi-burn family members

Objective To discuss the effects of mutual management model on the recovery of multi-burn family members. Methods The multi-burn family members who were hospitalized the 180th Hospital of PLA from February 2010 to October 2014 were chosen as study objects, and 68 patients who were discharged to the hospital from February 2010 to March 2012 were chosen as the control group, while 70 patients who were discharged to the hospital from April 2012 to October 2014 were chosen as the observation group. The two groups of patients were hospitalized for the same treatment, and the patients in control group received general guidance for rehabilitation, while the patients in observation group were administered with the application of mutual management model being discharged, and the members of the family monitored, helped and encouraged each other; they received follow-up survey at 1 month, 6 months, 1 year and 3 years after being discharged, and the effects of mutual management on improving the quality of life and returning to the job (work and study) were analyzed. Results The quality of life after 6 months discharged in two groups had statistical significance (F=424.679,P<0.01), the quality of life in observation group were higher than that of control group in body, psychology and social function, and status of material life (t=5.115、4.088、4.314、4.760;P<0.01). The length of returning to job or school was shorter than that of control group (χ2=8.101,P<0.05). Conclusions The mutual management model could effectively utilize the resources, and the patients managed with each other, took care each other and encourage each other, so as to improve the quality of life.
5.Mutation analysis of the CHD7 gene in patients with congenital heart disease.

OBJECTIVETo investigate the germline mutations of the CHD7 gene and their roles in patients with congenital heart disease (CHD).
METHODSGenomic DNAs extracted from peripheral blood were subjected to screen mutations in CHD7 gene by denaturing high performance liquid chromatography (DHPLC) followed by DNA sequencing of aberrant peaks in 67 CHD patients and 100 healthy control. Case-control study and bioinformatic analysis were utilized to explore the potential functional roles of the variations detected.
RESULTSSeven kinds of single nucleotide substitution were detected in the CHD patients in different introns of the CHD7 gene. Among them, IVS11+ 127A to G and IVS12+ 21T to G were rare variations and the allele frequencies of both were 0.0075; while IVS2+ 34G to A, IVS4+ 39G to A, IVS12-5T to C and IVS16+ 51C to A were the single nucleotide polymorphisms and the allele frequency was 0.2635, 0.2156, 0.1505 and 0.3636 respectively. The frequency of IVS12-5T to C in the CHD group was significantly lower than that in the control group (5.42%versus 9.57%, P< 0.05). The variant of IVS14-35C to G was only detected in patients with CHD. Bioinformatic analysis showed that IVS12-5T to C might increase exon splicing ability comparing with the wild-type sequence.
CONCLUSIONThe CHD7 gene mutation may not be the main reason for sporadic congenital heart disease, whereas the single nucleotide polymorphism of IVS12-5T to C might play a protective role in the onset of this disease.
