Objective To summarize our experience on laparoscopic appendectomy combined with high ligation of the vaginal process in children.Methods From January 2004 to May 2007,totally 326 patients with appendicitis underwent laparoscopic appendectomy in our hospital.During the operation,by inguinal exploration,83 cases of vaginal process deformity (17 bilateral and 69 unilateral lesions) were confirmed.High ligation of the vaginal process was performed on these patients.Results The operation was successfully done in all of the 83 cases,none of them showed postoperative complications.The cases were followed up for 6 to 12 months,during which only one patient had recurrent inguinal hernia.Three patients developed latent hernia but receined no treatment. Conclusions Laparoscopic appendectomy combined with high ligation of the vaginal process is an safe,effective,and low-cost procedure,leading to quick recovery and less pain for children with appendicitis complicated with vaginal process deformity.

OBJECTIVETo analyze the changes of patients with type 2 diabetes in different stages in glucagon (GC) and free fatty acid (FFA) in fasting, OGT and L-Arg experiments, and discusses the role of pancreatic alphabeta cells in diabetes pathogenesis by studying the relations among indexes such as glucagon (GC), free fatty acid (FFA) and blood glucose (BG), insulin, insulin homeostasis model (HOMA) and glucose metabolism hormone secretion curve, in order to provide theoretical basis for the treatment of diabetes.

METHODStudy objects were divided into the T2DM group (45 cases), the IGT group (28 cases) and the NGT group (30 cases) for an OGTT experiment and then an L-Arg experiment on the next day. Under the fasting state, their blood glucose (FBG), insulin (F), glucagon (FGC), free fatty acid (FFA) were detected to calculate HOMA-beta, insulin sensitivity index (ISI) and HOMA-IR of different groups. Meanwhile, efforts were made to calculate different time quantum detected in OGTT and L-Arg experiments and area under the curve AUC(BG), AUC(INS) and AUC(GC).

RESULTObvious overall differences were observed in FFA and FGC of the three groups. FGC of each group was negatively correlated with HOMA-beta and ISI. Among all of the 103 study objects, FGC was positively correlated with FBG and HOMA-IR and negatively correlated with HOMA-beta and ISI, with no correlation with FINS; FFA was positively correlated with FBG, HOMA-IR and negatively correlated with FINS, HOMA-beta, ISI. FGC and FFA were positively correlated in the T2DM group and the IGT group, but with no statistical correlation in the NGT group. The sequence of the three study objects was T2DM > IGR > NGT in AUC(GC) in the OGTT experiment and T2DM > IGR > NGT in in AUC(GC) in the L-Arg experiment, with the significant positive correlation between AUC(GC) and AUC(BG) and significant negative correlation with AUC(INS).

CONCLUSIONGlucagon and free fatty acid of T2DM and IGT patients increased, which was positively correlated with blood glucose and HOMA-IR and negatively correlated with INS, HOMA-beta and ISI. The increase in glucagons of T2DM and IGT patients indicated inappropriate secretion of pancreatic alphabeta cells among patients with type 2 diabetes.

Objective To evaluate the clinical effect of single-site umbilical laparoscopy in the treatment for inguinal incarcerated hernia in children. Methods Retrospective reviews were conducted for the clinical data of the 105 children with inguinal incarcerated hernia during January 2010 to August 2014. According to different surgical approaches, data were divided into single-site laparoscopic surgery group (SLS, n=56) and open surgery group (OS,n=49). The follow-up period was 6-36 months. Results Two groups of patients were successfully completed surgery. The average operative duration was significantly longer in OS group (33.73±4.41)min than that of SLS group (26.14±5.17) min (P<0.01). The average operative blood loss was (3.18 ± 2.90) mL for OS group versus (1.73 ± 0.88) mL for SLS group (P<0.01). The hospitalization duration was (4.22±1.16) d for OS group versus (3.50±0.97) d for SLS group (P<0.01). The postoperative complications were 24.5%(12/49) for OS group and 8.9%(5/56) for SLS group. There was no recurrence of hernia in two groups. Conclusion Single-site umbilical laparoscopy is safe and efficacious for treatment of inguinal incarcerated hernia in children, which offers a new choice of treatment for occult inguinal hernia.

Objective To observe the curative effect of Rougan Granules on rats hepatocirrhosis induced by carbon tetra- chloride(CCl_4)with 10 % alcohol and high fat feed.Method The rat model was established by gastric garage of CCh (CCl_4:salad oil=1:1)1 mL/kg twice a week for two weeks,and with 10 % alcohol as drinking water,high fat feed as food.After administration of CCl_4,the rats were divided into 5 groups:the model group,high-,medium-and low-dosage of Rougan Granules groups,the eolchicine group.And normal control group was also set up.After adminis- tration for 6 weeks,hyaluronic acid,typeⅣcollagen and typeⅢprecollagen in serum were measured and liver histopathol- ogy examination was done.Result The contents of hyaluronic acid,typeⅣcollagen and typeⅢprecollagen in model rats were increased significantly,while Rougan Granules could decrease those increase significantly.Liver histopathology exami- nation showed medium or high fatty degeneration of hepatic cell,obvious hyperplasia of fibrous tissue with fibrous distance broaden and pseudolobuli formed in model group,but in Rougan Granules group the general pathological changes of liver were less.Conclusion Rougan Granules have an effect of experimental anti-hepatocirrhosis in rats.

Objective To dosimetrically compare three delivery techniques of VMAT,IMRT and 3D-CRT in the treatment of accelerated partial breast irradiation (APBI).Methods Twenty patients with T1/2N0M0breast cancer were treated with VMAT.These cases were subsequently re-planned using static gantry IMRT and 3D-CRT technology to evaluate dosimetric differences.Dosimetric parameters including dose conformity index (CI),dose volume histogram (DVH) analysis of normal tissue coverage,dose parameters of PTV and normal tissues were evaluated,the delivery parameters including MU and delivery time were also analyzed.Results The IMRT and VMAT plans provided lower maximum dose,better mean dose and more conformal target dose distributions than the 3D-CRT plans (F =14.86,8.57,18.23,P ＜0.05).The volume of ipsilateral breast receiving 5 Gy for VMAT technique was significantly less than that of3D-CRTor IMRT(F=5.83,P＜0.05).The ipsilateral lung volume receiving 20 Gy (V20),5 Gy(V5) and the 5％ volume dose (D5) of IMRT were superior to those of 3D-CRT and VMAT(F =16.39,3.62,4.81,P ＜ 0.05).The low volume dose distributions of D5 in contralateral lung for IMRT was better than that of VMAT and 3D-CRT(F =3.99,3.43,P ＜ 0.05).The total mean MUs for VMAT,3D-CRT and IMRT were 621.0 ± 111.9,707.3 ± 130.9 and 1161.4 ± 315.6,respectively (F =31.30,P ＜ 0.05).The average machine delivery time was(1.5 ± 0.2)min for the VMAT plans,(7.0 ± 1.6)min for the 3D-CRT plans and (11.5 ± 1.9)min for the IMRT plans.Conclusions VMAT and IMRT techniques offer improved dose conformity as compared with 3D-CRT techniques without increasing dose to the ipsilateral lung.In terms of MU and delivery time,VMAT is more efficient for APBI than conventional 3D-CRT and static beam IMRT.
