2.The Changes of T Cells Subpopulations and Immunoglobulin and Their Relationship in Children Patients with Simple Nephrotic Syndrome.

Objective To search for the changes of T cells subpopulations and immunoglobulin and their relation-ship in children patients with simple nephrotic syndrome. Design Case-control research. Patients aud Participants 39 patients with simple nephrotic syndrome were divided into two groups:the incipient group and relapse group (6 cases were determined at the incipient and relapse time) .Thereare 28 patients in incipient group, 19 males and 9 females, at the age of 2 to 10 years old. There are20 patients in relapse group, 12 males and 8 females, at the age of 3 to 13 years old. There are 35health children in control group, 21 males and 14 females, 2~13 years old. Interventions T cells subpopulations were determined by indirect immunofluorescence of OKT linesmonoclonal antibodies. The serum IgG was determined by routine simple agar immunodiffusion tests. Results and Conclusions The CD_3~+ and CD_4~+ cells are of no change in the children patients withsimple nephrotic syndrome, and the CD_8~+ and CD_(10)~+ cells are obviously increased, the Values of CD_4~+/CD_8~+ are obviously lower than those in the control qroup, there are no difference between the incipientand relapse groups. The levels of serum IgG were decreased in the 85.3% children patients, IgM were inc-reased in 29.4% of that. The values of CD_4~+/CD_8~+ have positive correlation and negative correlationwith the levels of serum IgG and IgM respectively.
3.Effect of Sling Exercise Therapy on Rehabilitation after Total Knee Arthroplasty

Objective To observe the effect of sling exercise therapy (SET) on rehabilitation after knee arthroplasty. Methods 46 patients after knee arthroplasty were randomly divided into experimental group (n=23) and control group (n=23). The control group received continuous passive motion (CPM) training, while the experimental group received SET. They were evaluated with range of motion (ROM) of the knee, Hospital for Special Surgery Knee Score (HSS) and posture control ability before, 15 days and 3 months after operation. Results 15 days after operation, the maximal passive knee flexion was 6.0° more in the SET group than in the CMP group (95%CI=0.9°~11.2°; P<0.01), and the maximal active knee flexion was 5.7° more in the SET group than in the CMP group (95%CI=0.4°~9.1°; P<0.01); however, there was no significant difference in the maximal active knee extension, the maximal passive knee extension, HSS and posture control ability between 2 groups (P>0.05). 3 months after operation, there was no significant difference in all the items between 2 groups (P>0.05). Conclusion Compared with CPM, SET might be more effective on increasing knee ROM in the short-term, and could be used as an effective intervention for patients after knee arthroplasty at the early stage.
4.Hemorrhage after duodenopancreatectomy

Objective To study the sources and the relationship between the management and the outcome of hemorrhage after cephalic pancreatoduodenectomy.Methods The clinical data of 370 patients who underwent pancreatic resection at the Lihuili Hospital and the Second Affiliated Hospital of Zhejiang University were retrospectively analyzed.Results Postoperative bleeding occurred in 35 patients with 11 deaths.Among those intraabominal bleeding occurred in 14 cases and gastrointestinal hemorrhage occurred in 22,with one case suffering from both.Bleediug developing within 72 hours after operation in 12 cases (early-stage group),which was caused by improper intraoperative homeostasis.In other 23 cases,bleeding 72 hours after operation(later stage group)was caused by the erosion following pancreatic and/or bile leakage.Relaparotomy was performed in 13 cases and endoscopic homeostasis was performed in 3. Relaparotomy or endoscopic homeostasis was superior to that of conservative therapy in the early-stage group (P0.05).Pancreatic or bile leakage was identified as the significant risk factors for the postoperative bleeding.Conclusions In order to prevent the postoperative hemorrhage and to reduce the mortality of pancreatic resection,skillful techniques,expeditious homeostasis,proper management of stump pancreas and the prevention of pancreatic and bile leakage are essential.
5.Strong Expression of Recombinant Human Morphogenetic Protein-4 in Escherichia coli and its Bioassay in vivo

Objective:To produce rhBMP-4 with bioactivity in E.coli. Methods: The full-length human BMP-4 gene was mutated by PCR without changes in amino acid sequence, then the synthesized gene was cloned into plasmid pET-3c, transducted into BL21(DE)plysS, and induced by adding IPTG to a final concentration of 1.0 mmol/L. The protein product was purified using ion-exchange chromatography method and then renaturated, bioactivity was checked by C2C12 differentiation in vitro and mouse ectopic bone formation in vivo. Results: A 438 bp gene fragment encoding mature peptide of hBMP-4 was cloned , the protein product was mostly in the form of inclusion body, after renaturation, the engineering protein shows better bioactivity. Conclusion:The mutant strategy can enhance the expression of bioactive rhBMP-4 in E.coli expression system.
6.Human umbilical vein vascular endothelial cell injury induced by fluoride in vitro

Objective To study the effect of different concentrations of fluoride on cultured human umbilical vein vascular endothelial cells(HUVEC). Methods Different doses of sodium fluoride (NaF) were added to HUVEC culture medium, fluoride concentrations were 0(control), 100,400,700,1000,2000 μmol/L, respectively,6 re-set hole in each group. After continuous culture for 48 h, cells and culture medium were collected. Cell morphology was studied by Wright-Giemsa staining; cells apoptosis was determined by acridine orange fluorescence staining; cell activity was measured by methyl thiazolyl tetrazolium (MTT) assay; superoxide dismutase (SOD),glutathione peroxidase(GSH-Px) activity, malonaldehyde(MDA) content, induced nitricoxide synthase(iNOS), and endothelia nitricoxide synthase(eNOS) activity in cell culture medium were determined by spectrophotometry; cell iNOS mRNA and eNOS mRNA expression were detected by RT-PCR; intercellular adhesion molecule-1 (ICAM-1)and vascular cell adhesion molecule-1 (VCAM-1) levels were detected by double antibody sandwich ELISA method.Results With increased dose of fluoride, HUVEC cells decreased, the structure changed. In 400 - 2000 μmol/L group, the SOD activity[(6.627 ± 0.213), (6.668 ± 0.152), (5.935 ± 0.122), (4.755 ± 0.182)kU/L] was lower than those of the control group[(7.457 ± 0.398)kU/L, P < 0.05 or < 0.01], GSH-Px activity[(481.284 ± 43.785),(492.223 ± 16.474), (382.762 ± 25.167), (293.687 ± 24.881 )kU/L] was also lower than those of the control group [(585.078 ± 47.323)kU/L, P < 0.05 or < 0.01], MDA level[(0.609 ± 0.011 ), (0.646 ± 0.016), (0.852 ± 0.013),(1.188 ± 0.045)nmol/L] was higher than those of the control group[(0.512 ± 0.027)nmol/L, P < 0.05 or < 0.01];iNOS activity[(3.604 ± 0.115), (3.615 ± 0.075), (3.848 ± 0.103), (4.275 ± 0.079)kU/L] also was higher than those of the control group[(2.798 ± 0. 136)kU/L, all P < 0.01], iNOS mRNA expression increased, eNOS activity [(5.539 ± 0.079), (5.503 ± 0.064), (5.226 ± 0.142), (4.809 ± 0. 107)kU/L] decreased compared to those of control group[(5.996 ± 0.155)kU/L, P < 0.05 or < 0.01], eNOS mRNA expression decreased; ICAM-1 levels [(0.852 ± 0. 102), (0.886 ± 0.061 ), (0.961 ± 0.158), (1.418 ± 0. 167)μg/L] increased compared to those of the control group[(0.687 ± 0.046)μg/L, P < 0.05 or < 0.01], VCAM-1 levels[(2.719 ± 0.197), (2.946 ± 0.167),(3.173 ± 0.225 ), (3.613 ± 0. 153 ) μg/L] was higher than those of the control group [(2.375 ± 0.067 ) μg/L, all P <0.01]. Conclusions High concentrations of fluoride reduce the activity of antioxidant enzymes, which leads to metabolic disorders of nitric oxide and abnormal cytokines expression, thereby inhibiting vascular endothelial cell growth, structural change and induced apoptosis. This is an important factor in high fluoride-induced vascular endothelial injury.
7.DNA microarray analysis of protective mechanism of buyang huanwu decoction on focal cerebral ischemia/reperfusion in rats.

OBJECTIVETo analyse the effects of buyang huanwu decoction (BYHWT) on differentially expressed genes during cerebral ischemia/reperfusion in rats with DNA microarray.
METHODcDNA microarray chips containing 512 cDNAs were made by Biostar Genechip Inc. Sprague-Dawley rats were subjected to 2 h of middle cerebral artery occlusion (MCAO) with an filament. Saline or BYHWT was given p.o. after onset of cerebral ischemia and brains were removed after 24 h of recirculation for mRNAs isolation. A differential measurment of mRNAs from post-ischemic and BYHWT treated animals was performed with microarray.
RESULTUp-and down-regulated genes were 69 and 80 in ischemic group. Up-and down-regulated genes were 25 and 6 in BYHWT treated group.
CONCLUSIONBYHWT regulates the differential expression genes after focal brain ischemia/reperfusion in rats, due to its mechanism of protecting cerebral ischemia/reperfusion injury.

8.Onset of adrenal ganglioneuroblastoma in an adult after delivery.

A case of adrenal ganglioneuroblastoma is presented here. This adrenal ganglioneuroblastoma was found in a 27-year-old female 7 months after delivery. CT clarified that the tumor originated retroperitoneally and was large in size (11.4 cm x 9.4 cm). The tumor was surgically removed together with pancreatic body and tail, left kidney and spleen, and pathological diagnosis was adrenal ganglioneuroblastoma-intermixed. Adrenal ganglioneuroblastoma is extremely rare in adults, with only about 9 cases documented including this case.

9.Noninvasive assessment of left anterior descending and right coronary artery flow reserve by transthoracic Doppler echocardiography in patients with coronary artery disease.

OBJECTIVETo assess the feasibility of evaluation of CFR for LAD and right coronary artery (RCA) as well as diagnostic accuracy for patency of each vessel by transthoracic Doppler echocardiography (TTDE).
METHODS65 consecutive patients (age 58 +/- 14, ejection fraction 49% +/- 8%, 48 men and 17 women) were studied for CFR by TTDE with adenosine infusion (140 microg.kg(-1).min(-1)). LAD flow was interrogated nearby the apex in modified 2-ch apical view. RCA flow was interrogated at mid posterior wall on distal part of the posterior descending branch (PDA) in modified 2-ch apical view for PDA.
RESULTSFlow for LAD was detected in all patients (4 with use of contrast agent) and for RCA in 55 patients (5 with contrast agent). Two patients had an occluded RCA. Feasibility of RCA flow detection was 55/63 (87%). In all patients maximal flow of each branch was detected in less than 2 min of adenosine infusion (140 mg.kg(-1).min(-1)). Total time for both CFR estimation was 7.3 +/- 1.6 min. Selecting 2.0 or 1.8 as CFR cut-off value for diagnosis of coronary stenosis (> or = 50% in diameter) or severe stenosis (> or = 70%) was found fitted for both LAD and RCA with similar diagnostic performance (sensitivity 89%/93%, specificity 86%/84%, area under curve 0.89/0.92 respectively, in stenosis diagnosis and sensitivity 88%/89%, specificity 90%/83%, area under curve 0.94/0.95 respectively in severe stenosis diagnosis).
CONCLUSIONSCFR evaluation of both distal LAD and PDA is of high feasibility and can provide high diagnostic yield for relevant vessel patency.

10.Effect of Combination of Glycyrrhizae Radix et Rhizoma and Euphorbia Pekinensis Radix in Resisting Hepatocellular Carcinoma ascites in Mice

OBJECTIVE To investigate the effect of the combined administration of herbal pair Euphorbia Pekinensis (DJ) and Glycyrrhiza glabra (GC) at different doses or ratios on hepatocellular carcinoma (HCC) ascites in mice.METHODS A total of 120 male Kunming mice were randomly divided into normal control, model, DJ-GC groups [ratios 1∶0.5, 1∶1, 1∶2, 1∶4], DJ-alone group and GC-alone group.HCC ascites model was constructed by inoculating H22 cells intraperitoneally.HCC ascites mice in the drug treatment groups were intragastrically administrated with corresponding doses of Euphorbia Pekinensis and Glycyrrhiza glabra for 12d, respectively.Then, the therapeutic effects of herbal pair DJ and GC acting on HCC acites was analyzed in terms of body weight, ascites volume, abdominal circumference and the index of liver and kidney.After that, the expressions of AQP2 and V2R were detected by Western Blot, and the level of ALT, AST, BUN, CR were detected by biochemical method.RESULTS When compared with model group, the DJ-alone group and DJ-GC 1∶2 group could efficiently reduced the ascites volume, abdominal, body weight (P<0.05~0.01) and the expression of aquaporin-2 and renal vasopressin receptors-2 in the kidney tissues (P<0.05~0.01).DJ-GC 1∶2 group could enhance the effect of DJ-alone group (P<0.05~0.01), while the DJ-GC 1∶4 group could reduce the effect of DJ-alone group (P<0.05~0.01).CONCLUSION The therapeutic effects of DJ on HCC were altered in accordance with its combination with GC at different ratios by being enhanced or weakened, which might be partially related to their regulation on aquaporin-2 and renal vasopressin receptors-2 signal axis.
