Objective To explore the relationship between the timing of functional training and its therapeutic effects on patients with cerebral infarction. Methods One hundred and fifty-six patients with cerebral infarction of internal carotid artery system were randomly and equally divided into three groups (groups A, B and C), and the functional training was initiated within 7d, 7~30d or over 30d after cerebral infarction in groups A, B, C, respectively. OBJECTIVE:
To verify the relationship between THI and VAS scores in patients with sudden hearing loss and tinnitus in a prospective study.
METHOD:
Forty-four patients classified their tinnitus according to VAS and THI before and after treatment, and both scores were compared through the Pearson's correlation coefficient test and Nonparametric tests.
RESULT:
There was a correlation between VAS and THI scores in patients with sudden hearing loss and tinnitus. Tinnitus assessment scores were significantly reduced after treatment. Decreasing of THI and VAS scores were not depended on frequency and loudness of tinnitus and severity of deafness, but therapeutic effect of sudden sensorineural hearing loss.
CONCLUSION
THI and VAS scores are useful for assessment of tinnitus in patients with sudden hearing loss and tinnitus.

3.Failure in the Prophylactic Use of Antibiotics and Antibiotic Resistance of Pathogens

OBJECTIVE:To provide reference for the rational clinical prophylactic use of antibiotics. METHODS:The cases in whom the clinical prophylactic use of antibiotics between 2006 and 2007 in our hospital showed failure in efficacy were monitored to analyze the cause of failure,meanwhile the clinical samples were sent for culture,isolation,identification and drug susceptibility tests as well as drug resistance analysis. RESULTS:Cephalosporins,Quinolones,Penicillins,Cefhalosporins + enzyme inhibitor were more commonly used antibiotics in our hospital,with drug resistance rates at 57.21%,65.14%,68.63%,and 18.81% respectively. Among the total 240 cases monitored,the prophylactic use of antibiotics totaled 877 times,of which,388 (44.24%) were non-indicated drug use,459 (52.34%) showed drug resistance,286 (32.61%) involved improper drug choice,and 42 (17.50%) showed dual infection. CONCLUSION:The clinical prophylactic use of antibiotics in our hospital is far from perfect; therefore,it is urgent to tighten control on the standard prophylactic use of antibiotics.
4.Multi-factor analysis of early pulmonary infections after renal transplantation

Objective To find out the risk factors of early pulmonary infections after renal transplantatioa Methods The data were collected from 96 patients receiving renal transplantation between Oct. 2006 and Oct 2010, including 48 cases of early lung infection after renal transplantation as infection group, and 48 patients receiving immunosuppressive regimen at the same period as-control group. Taking the factors of lung infecition as variables, t test or chi-square test was used in univariate analyses whereas logistic regression was used in multivariate analyses. Results Single factor analysis showed that induction therapy, albumin levels, dose of steroid in 1 month after operation,family income and prophylactic SMZ treatment were related to lung infectioa Analysis of multiple variables logistic regression revealed that induction therapy, albumin levels, dose of steroid in 1 month after operation and prophylactic SMZ treatment were related to lung infection. Conclusion The correlation analysis indicated that induction therapy and dose of steroid in 1 month after operation have positive correlations with pulmonary infection, while albumin levels and prophylactic SMZ treatment have negative correlations with pulmonary infection.
5.Data base establishment and data mining on regular pattern of TCM beauty prescription

ObjectiveTo explore the regular pattern of TCM beauty prescription.MethodsBased on the ACCESS database platform,we established TCM beauty prescription database and analyzed the information in detailed.ResultsThe total number of beauty prescription in the database was 1821.We focused on analyzing the frequency of medicine application in ancient beauty prescriptions,medicinal diet,acne,chloasma and freckles prescriptions.Statistics showed that dahurian angelica root,indian bread,medlar,honey,black sesame,baical skullcap root,and Chinese angelica were the most commonly used seven kinds of Chinese herbs.ConclusionThe main parts of Chinese medicines for making beauty were those herbs which can nourish yin and blood,supplement yang and qi; and the main parts of Chinese medicine for treating skin diseases were those herbs which can clear heat and detoxicate.
6.Study of angiotensin-Ⅱactivating NF-κB in human pulmonary microvascular endothelial cells through classical pathway

Objective To investigate the activation of nuclear factor-κB(NF-κB),which was stimulated by angiotensin-Ⅱ(AngⅡ)through classical pathway in human pulmonary microvascular endothehal cells(HPMEC).Method The experiment was divided into two groups:in Ang Ⅱ group,HPMECS were incubated with 10-6mol/L AngⅡ for 0,0.5,1,2 and 4 hours,respectively;in losartan group,HPMEC was pretreated with 10-6mol/L losartan(inhibitor of AngⅡ type 1 receptor)for 1 hour,and then stimulated with 10-6mol/L AngⅡ for 2 hours,and the nucleax protein and the cell plasma protein were prepared by lysis and centrifugation.Electrophoretic mobility shift assay(EMSA)was used to detect the NF-κB DNA binding activity.The inhibitor of κBa(IκBα)was detected by Western blotting.The data were expressed as(x±s)and analyzed with one way analysis of variance.A P value less than 0.05 indicated significant difference.Results Compared with the activity of NF-κB at 0 h (100.0±25.1)after AngⅡstimalation,the activity increased significantly at 0.5 hour(144.5±16.1,P<0.05),and reached peak value at 2 hours(270.1±27.2,P<0.05).The concentration of IκBα at 0 hours was 44.4%±2.1%,decreased markedly at 0.5 hours(38.9%±3.6%,P<0.05),and to the lowest level at 2hours(32.6%±2.3%,P<0.05).The activity of NF-κB(115.4±10.7)and the concentration of IκBα(43.6%±3.7%)in losartan group had ilo significant difference with AngⅡ group at 0 h(P>0.05).The activity of NF-κB and the concentration of IκBα in losartan group had significant difference with AngⅡ group at 2hours.Conclusions NF-κB can be activated through classical pathway,which stimulated by AngⅡ in HPMEC.

8.Appropriate compatibility of propofol and sevoflurane for posterior lumbar interbody fusion of patients with mild cognitive impairment

Objective To investigate the appropriate compatibility of appropriate compatibility of sevoflurane and propofol for patients with mild cognitive impairment (MCI) undergoing posterior lumbar interbody fusion in order to protect their cognitive function.Methods Eighty patients, 41 males, 39 females, aged 65-75 years, BMI 17-26 kg/m2, ASA physical status Ⅰ or Ⅱ, scheduled to undergo elective posterior lumbar interbody fusion, were to be scored according to Montreal cognitive assessment (MoCA), mini mental state examination (MMSE), dementia scale (CDR) and daily living ability scale (ADL) to identify patients with MCI before the surgery.They were randomly assigned to 4 groups (n=20 each) using a random number table: TCI propofol 2.0-2.5 μg/ml group (group P), TCI propofol 1.2 μg/ml+sevoflurane 0.6 MAC group (group PS1), TCI propofol 0.6 μg/ml+sevoflurane 0.9 MAC group (group PS2), 1.0-1.5 MAC sevoflurane group (group S).MoCA and MMSE were used to evaluate the cognitive function of patients 1 d before the operation (T0), after patients become wide-awake (T1), 3 d and 7 d after operation (T2 and T3).Apolipoprotein J (ApoJ) concentration related to cognitive function in blood samples, which were drawn at T0-T3 would be measured with ELISA method.Results Compared with T0, the scores of MMSE and MoCA in four groups decreased significantly (P<0.05) at T1, the scores of MMSE and MoCA in group S decreased significantly (P<0.05) at T2;compared with T1, the score of MMSE in the four groups increased significantly at T2, T3 (P<0.05).The scores of MMSE at T1, T3 in group S decreased significantly compared with groups P, PS1 and PS2 (P<0.05).The scores of MoCA at T2, T3 in group S decreased significantly compared with groups P, PS1 and PS2 (P<0.05).Compared with T0, the concentration of plasma ApoJ in the four groups increased significantly at T1 (P<0.05).Compared with T1, the concentration of plasma ApoJ in the four groups decreased significantly at T2 and T3 (P<0.05).Compared with group PS1, the concentration of plasma ApoJ at T1, T3 increased significantly in groups S and group PS2 (P<0.05).Conclusion TCI propofol 1.2 μg/ml combined with 0.6 MAC sevoflurane group is the appropriate compatibility of sevoflurane and propofol for patients with MCI undergoing posterior lumbar interbody fusion,because it has less negative influence on cognitive function and lower concentration of plasma ApoJ.
9.Human immunodeficiency virus-associated mild cognitive function decline: a preliminary study of the combination of diffusion tensor imaging and resting-state functional magnetic resonance imaging

Objective The purpose of this study was to use diffusion tensor imaging (DTI) and resting-state functional magnetic resonance imaging (rs-fMRI) alone or in combination to observe the distribution of white matter lesions and cortical malfunctional areas in human immunodeficiency virus (HIV) infected patients with mild cognitive decline and to explore the relationship between the DTI and the rs-fMRI methods.Methods Twenty-six HIV infected patients with mild cognitive impairment and 30 healthy volunteers were selected by Montreal Cognitive Asessment (MoCA) scale evaluation.DTI data and rs-fMRI data were obtained,fractional anisotropy (FA) value images were obtained with voxel based analysis and the resting-state default mode network (DMN),functional connectivity images were obtained with cingulate gyrus as a seed point.Overlay images were obtained with FA,DMN and Ch2 standard images.Results Compared with the control group,the white matter FA values were significantly decreased in the left precuneus(t=4.0499,P<0.005) and right precuneus (t=5.1553,P<0.005),right superior frontal gyrus(t=5.1517,5.1484,P<0.005),right middle frontal gyrus (t=4.1444,P<0.005),right precentral gyrus (t=3.7395,P<0.005),right occipital lobe (t=7.2236,P<0.005),and right inferior parietal lobule (t=4.1450,P<0.005) in acquired immunodeficiency syndrome (AIDS) patients.In resting-state default mode network,areas significantly related to cingulate gyrus seed point included the left cingulate gyrus (t =32.78,P<0.005),left precuneus (t =4.51,P<0.005),left superior frontal gyrus (t =14.33,4.53,P<0.005),left middle temporal gyrus (t =10.01,5.72,P< 0.005),left inferior temporal gyrus (t =5.99,P<0.005),left parahippocampal gyrus (t =7.63,P<0.005),right posterior cingulate (t =34.81,P<0.005),right precuneus (t=32.09,P<0.005),right superior frontal gyrus(t =14.12,P<0.005),right middle frontal gyrus (t=17.71,P<0.005),right superior temporal gyrus (t=14.59,P<0.005),and right middle temporal gyrus (t=11.83,P<0.005); while areas not significantly related to the cingulate gyrus seed point included the left precuneus (t =5.39,P<0.01),left anterior cingulate gyrus (t =3.66,P<0.01),left cerebellar tonsils (t =7.51,P<0.01),right superior parietal lobule (t=4.44,P<0.01),right parahippocampa gyrus (t =3.69,P<0.01),and right cerebellar tonsil (t=6.15,P<0.01).Overlayed images showed that the white matter FA value of the left precuneus were decreased and the functional activitis of the corresponding cortex were significantly decreased; while the white matter FA values of the left precuneus,right precuneus,right superior frontal gyrus,right middle frontal gyrus were decreased without affection of the functional activity of the corresponding cortex in AIDS patients.Conclusion White matter nerve fiber disconnection of multiple brain regions and its corresponding cortical function decline with compensatory activity co-participated in the pathogenesis of AIDS mild cognitive decline.
