Objective To explore the influence of Azithromycin on helper T lymphocyte cell(Th)1/Th2 in peripheral blood of children with bronchitic asthma.Methods Twenty-four asthmatic children and 20 healthy children were selected.Peripheral blood mononuclear cells (PBMC) were isolated from venous blood and made into cells suspension in aseptic condition.0.2 mg/L,0.1 mg/L,0.05 mg/L and 0 mg/L Azithromycin were added into the cultures in asthmatic group.The control group was not interfered with Azithromycin.The supernatant was collected after 48 h.The levels of IFN-?,IL-4 and IL-10 in the supernatant were determined by enzyme-linked immunosobent assay(ELISA).SPSS 11.5 software was used to analyze data.Results 1.The level of IL-4 produced from PBMC of asthmatic group was significantly higher than that of control group(P0.05).2.Azithromycin 0.1 mg/L more promoted the secretion of IL-4 than the other 3 concentrations(Pa0.05).3.Azithromycin 0.2 mg/L and 0.1 mg/L more increased the level of IL-10 than the control group(P0.05).Conclusions The routine drug level of Azithromycin(0.1 mg/L) had no effects on the imbalance of Th1/Th2 of asthmatic children,but could modulate the immunological function by up-regulating the level of IL-10.

diabeticfoot ulcer of rats induced by streptozotosin,and to find the significance of free radical scavenging and the antioxidative function.MethodsA total of 60 male wistar rats were randomized into the control group (20 rats) and diabetic model group (40 rats).The model group was induced to diabetic foot ulcer using streptozotosin tail intravenous injection,and the control group was injected equal citric acid-citric acid sodium buffer fluid instead.A rectangular full-thickness skin of dorsum of foot in the two groups was removed from model rats to construct diabetic foot ulcer.Serum zinc levels of the two groups were monitored,and immunohistochemical technique was used to detect the MT expression.ResultsSerum zinc levels in the model group( 1.17 ±0.09) was significantly lower than control group( 1.48 ±0.07),and the difference was statistically significant ( t =12.94,P ＜0.01 ).The MT expression in control group was low,while it significantly increased in diabetic model group,and the difference was statistically significant ( x2 =12.78,P ＜ 0.01 ).ConclusionsThe zinc levels and MT expression of diabetic foot ulcer rats may be associated with free radical scavenging and the antioxidative function.The increased expression of MT may play an important role in preventing the further development of diabetic foot ulcer disease.

[ ABSTRACT] AIM:To explore the effect of dexmedetomidine ( DEX) on the CCAAT/enhancer-binding protein-homologous protein ( CHOP) pathway during lung ischemia-reperfusion ( I/R) in mice.METHODS:C57BL/6J male mice were randomly divided into sham operation group ( sham group) , lung ischemia/reperfusion group ( I/R group) , ischemia/reperfusion +normal saline group ( I/R+NS group ) and ischemia/reperfusion+dexmedetomidine group ( I/R+DEX group) .Dexmedetomidine was infused intraperitoneally with 25 μg/kg for 30 min prior to the ischemia period in I/R+DEX group, the normal saline was administrated with the same volume of dexmedetomidine in I/R+NS group.After fini-shed the 3 h-reperfusion period , the left lung tissues were harvested to determine lung wet/dry weight ( W/D) , the total lung water content ( TLW) , and index of quantitative evaluation for alveolar damage ( IQA) .Morphological observation and terminal-deoxynucleotidyl transferase mediated nick end labeling ( TUNEL) were applied to evaluate the structure changes and the apoptosis index (AI) of the lung tissues.The expression of CHOP and glucose-regulated protein 78 (GRP78) at mRNA and protein levels in the lung tissues was detected by Western blot and RT-PCR.RESULTS:Compared with sham group, the W/D, TLW, IQA, AI, the mRNA and protein expression of CHOP and GRP78 obviously increased, and the left lung tissues structure were damaged more obviously both in I/R group and I/R+NS group.Compared with I/R group, the W/D, TLW, IQA, AI and the protein and mRNA expression of CHOP in I/R+DEX group decreased, the injury of the left lung tissue structures induced by I/R in I/R+DEX group were also alleviated .CONCLUSION:DEX alleviates the
lung I/R injury, which may be related to inhibition of apoptosis mediated by CHOP pathway.

Objective To analyze gene mutations of a Niemann-Pick disease type C (NPC) proband,and carry out prenatal diagnosis for the family.Methods The coding regions of NPC1 gene in the proband (late-infantile form) and white blood cell (WBC) in peripheral blood of its parents were amplified by polymerase chain reaction and direct DNA sequencing in both directions was performed.The sequencing results were compared with human NPC1 gene sequence (NM_000271) in GenBank,and sequences of mutated exons were determined.Direct sequencing was used on 50 normal Chinese individuals' DNA samples (control) to exclude mutation's single nucleotide polymorphism (SNP).An inter-species alignment of homologous NPC1 proteins was performed using ClustalX 1.81 software.During the second pregnancy of the proband's mother,the amniotic fluid was obtained at 18 weeks of gestation and the amniocytes were cultured for gene mutation analysis.Neonate's DNA of WBC in peripheral blood was also extracted for NPC1 gene analysis.Results Mutation analysis of NPC1 gene revealed two novel heterozygous mutations (c.2284-2287 delCTCT and p.V959G) in the proband,which originated from her father and mother,respectively.These two mutations were absent in the control,suggesting that these mutations were not SNP.While comparing with the amino acid in NPC1 protein of human,mouse,rat,rabbit,cat and pig,it revealed that p.V959 belonged to a conservative amino acid region and the missense mutation of p.V959G may perturb the function of NPC protein.Neither mutation was found in DNA from amniotic fluid or from the cultivated amniocytes in the second pregnancy,suggesting a normal fetus.c.2284-2287 delCTCT and p.V959G mutation were not found in NPC1 gene analysis of WBC in peripheral blood of the neonate,which was consistent with the prenatal diagnosis.Conclusions PCR-direct sequencing could be used as genetic diagnosis for NPC proband and prenatal diagnosis for its family.The mutation p.V959G may be correlated to late infantile form of NPC.

Objective Establish a method for measuring the activities of Galactocerebrosidase (GALC), α-Glucosidase(GAA), α-Galactosidase (GLA) and α-L-Iduronidase (IDUA) in dried blood spots specimen by tandem mass spectrometry ( MS/MS ).Methods A total of 2175 dried blood spot samples forinborn errors of metabolism in neonatalscreening center of Shanghai Xinhua hospital were collected in July and November, 2013.And twenty dried blood spot samples from patients withlysosomalstorage disorders( LSDs) of Shanghai Xin Hua Hospital were collected from September 2012 to January 2014.The extraction of DBS was incubated with enzyme substrates and internal standards.After liquid-liquid and solid-phase extraction, the extraction solution was dried under nitrogen and reconstituted.Then enzyme reaction products and internal standards were analyzed by MS/MS.Linearity, precision, accuracy and the limit of detection were evaluated.2175 dried blood spot samples were detected to establish the normal reference range for the activities of four enzymes according to 0.5th to 99.5th percentiles.20 specimens from patients withLSDs were detected to verify the reference range inclinical judgment.Results The intraassay and interassay precisions ranged from 1.7%to 11.8%, and the intraassay and interassay accuracies ranged from 85%to 115%.The linear coefficients for measured concentration of enzyme products/internal standards and theoretical concentration were 0.997-0.999.The limits of detection forGALC, GAA, GLA and GLA were 0.03 μmol/(L· h), 0.09 μmol/(L· h), 0.12 μmol/(L· h) and 0.16 μmol/(L· h) .The normal reference values for GALC, GAA, GLA and GLAwere 0.51-8.51μmol( L· h) ,1.99-22.22μmol/( L· h),1.68-41.59 μmol/(L· h) and 2.36-19.21 μmol/(L· h).The enzymes of 20 patients with LSDs were remarkably decreased compared to the normal range.The Krabbe, Pompe, Fabry, MPSⅠpatients can be effectively detected by this MS/MS method.Conclusions A MS/MS method for measuring GALC, GAA, GLA and IDUA enzyme activities in DBShas been established.

Objective． An internal fixation apparatus— — distraction reduction fixation system(DRFS) was designed to satisfy the clinical needs for spondylolisthesis. Methods. Since 1996, 53 patients were treated with DRFS. Among them, 35 had spondylolisthesis, 12 had lumbar canal stenosis accompanied with instability, 2 had vertebral tumors and 4 suffered from spinal fracture. The average age was 53.6 years old (ranged 24～ 72yrs). The mean time for follow-up was 30.6 months (16 ～ 44 months). Results． The slip rate was 0.15± 0.10 before operation, and decreased to 0.09± 0.07 after operation. Entire slip reposition was achieved in 19 cases (54.3％ ). The change in height of the intervertebral space within the fixation segments was 0.7± 0.17. Conclusion． DRFS achieved better results for spondylolisthesis less II degree and no other adverse effects were found. Compared with other foreign and domestic techniques, it had advantages in less implants, less operation gears required and ease to utilize in operation. It was proved to be an ideal internal fixation apparatus.

OBJECTIVETo evaluate the reliability of Bio-Oss particles as bone grafts in combination with membrane-technique.

METHODS6 small biopsy specimens were retrieved without causing iatrogenic damage in 6 patients (implant sites). The Donath's hard tissue section and micro grinding technique was adopted, and Leval Laczko staining method was used.

RESULTSThe histological results showed Bio-Oss particles were easily distinguished from the newly formed bone, but new bone was formed among different particles of Bio-Oss, no connective tissue was found. Intimate contact between newly formed bone and particles of the graft was present.

CONCLUSIONSBio-Oss appears to be highly biocompatible and osteoconductive. It can be used success fully as a bone substitute in combination with guided bone regeneration membrane technique.

OBJECTIVETo determine the long-term clinical outcome and predictability of the maxillary sinus augmentation procedure.

METHODSSeventy-four patients underwent 100 sinus bone graft procedure and 206 implants were placed. The patients were followed up for 1-9 years, with an average of 3.46 +/- 1.65 years after implant placement.

RESULTSThe 5-year cumulative survival rate (CSR) of implants was 92.28%. Only 8 implants failed and no sinusitis occurred.

CONCLUSIONSSinus bone graft is an adjunctive procedure with a predictable outcome in patients with severely atrophied posterior maxilla.

OBJECTIVETo evaluate the result of bone regeneration due to using PRP in combination with beta-TCP in bone defect adjacent to oral implantation.

METHODSTen patients (6 males, 4 females, with an average age of 49.6 years) participated in this study. Seven of them underwent maxillary sinus augmentations, and 3 underwent GBR for peri-implant bone defects. PRP + beta-TCP was used in 4 cases and beta-TCP in other 6 as control. X-ray examinations were carried out prior to operation and in 1 week, at 3 months, 6 months after operation. After 4 - 6 months, 3 biopsy specimens were obtained at the time of the second stage operation in each group.

RESULTSBone grafts healed well without any infection in all cases. Radiographs showed that bone grafts integrated together with the bone. The histological result showed that new bone was formed among particles of beta-TCP in both groups, but in PRP + beta-TCP group denser and better arranged woven bone was observed, and more new bone was formed into the micropores of the particles.

CONCLUSIONSThe result in this study implied that PRP in combination with beta-TCP can improve bone regeneration in bone defect adjacent to oral implantation.

OBJECTIVETo evaluate the clinical result of alveolar vertical distraction osteogenesis for implant.

METHODS19 cases with severe vertical alveolar defects (more than 10 mm defect) underwent vertical distraction procedure before implant placement. 15 cases were male and 4 cases were female. The alveolar bone defects were caused by tumor resection in 10 cases, by trauma in 8 cases, and due to congenital defect in 1 case. The vertical defects in all cases were over 10 mm. X-ray examination were taken prior to operation and in 1 week, 2 week, 4 week, the end of active distraction and before removing distractor. Implants were placed in 17 cases after removing distractors. The prosthesis were finished 6 month later.

RESULTS11 cases out of 19 had implant prosthesis. 6 cases had implant in the jaw but waiting for the prosthesis. Altogether 65 dental implants were placed, 2 cases did not receive further implant treatment because of cost problem. The study showed that new bone formed after distraction in all 19 cases. Average gained height of new bone was 13 mm.

COMPLICATIONSUnexpected mandible fracture in 2 cases, infection in 1 case.

CONCLUSIONSAlveolar vertical distraction is a good alterative for severe alveolar defects. The distractor should be further developed and modified.
