1.Effect of understanding of stroke patients for illness on direction of rehabilitation

ObjectiveTo investigate the understanding of stroke among stroke patients and promote the direction of rehabilitation.MethodsA survey was carried out among 80 stroke patients by questionnaire.Results42.5% of those patients knew nothing about the risk factor of stroke and the rate of right behavior related to maintain health was relatively low. The rate of satisfaction with rehabilitative direction was only 66.3% though 98% patients had experienced that direction.ConclusionIntensive and continuous direction of rehabilitation is necessary for the stroke patients. Comprehensive health education should be given to those patients to ensure their best self care and improve their quality of living.

3.Present status in studying immunotherapy for acute leukemia and its perspective--Editorial.

One of the important approaches for further prolonging remission duration and eradicating minimal residual disease in acute leukemia is immunotherapy. Four kinds of immunotherapy for acute leukemia are under investigation: (1) monoclonal antibodies, among them, Mylotarg (cytotoxic antibiotic calicheamicin linked to CD33 Mab) is given for the treatment of refractory or relapsed acute myeloid leukemia and molecular relapse in acute promyelocytic leukemia with good results, Campath-1H (antiCD52 Mab) is administered in the treatment of prolymphocytic leukemia and Rituximab (anti-CD20 Mab) in B-PLL with high complete remission rates. Other Mabs under preclinical and clinical trials include anti-IL-2 receptor Mab for the treatment of acute T lymphocytic leukemia, anti-220 kD Mab-6G7 for acute leukemias, recombinant immune toxin BL22 (anti-CD22) for hairy cell leukemia and Mabs labeled with radio-isotopes for different types of acute leukemias; (2) adoptive cellular immunotherapy using cytokine-induced killer cell, alloreactive NK cells, allogeneic or autologous leukemic-specific CD8(+) cytotoxic T lymphocytes, and other immune effector cells; (3) cytokines and other immune modulators comprising IL-2, IL-12, GM-CSF, CD40L, FLT-3L and thalidomide and its derivatives; (4) leukemia vaccines of several different formulations including antigen-specific, leukemia cell-based, leukemia antigen-pulsed dendritic cell (DC) and leukemia-derived DC vaccines, the latter two formulations are more attractive. In conclusion, up to now, the most effective example of immunotherapy in acute leukemia is provided by the administration of Mabs, and the majority of other approaches in immunotherapy for acute leukemia although promising, need further studies.

5.Histopathological analysis of 40 patients with bilateral breast cancer

Objective To find an easy and proper way to differentiate metastatic breast cancer from second primary breast cancer by analyzing the histopathological characteristics of 40 patients with bilateral breast cancer.Methods According to the Chaudary histological criteria,the histopathological and biological characteristics of 16 cases of synchronous bilateral breast cancer and 24 cases of metachronous bilateral breast cancer were evaluated.The histopathological and biological characteristics included Nottingham histological grade,immunohistochemistry (IHC) results of the estrogen receptor,progesterone receptor and expression of Her-2.Results The average age of the 40 patients at first diagnosis was 41 years old (range,26-68 years old).The average time interval between first and second diagnosis of tumors was 34 months (range,7-209 months) in metachronous cancer.The concordant histopathological type was found in 93.8 % (15/16) of synchronous cancer patients and 58.3 % (14/24) of metachronous cancer patients (P =0.02).The concordance rates of tumor stage was 68.8 % (11/16) in synchronous cancer patients,while it was 25.0 % (6/24) in metachronous cancer patients (P =0.03).For progesterone receptor status,the concordance rates were 87.5 % (14/16) and 54.2 % (13/24) in synchronous and metachronous cancer patients respectively (P =0.03).There was no statistically significant difference in terms of estrogen receptor status and Her-2 expression (P > 0.05).Conclusions Without considering the limitation of Chaudary criteria and the number of patients,it seems to be an easy and proper way to differentiate metastatic cancer from second primary cancer in the patients with bilateral breast cancer by combining the histopathological type,tumor stage and progesterone receptor status.The synchronous cancer is closer to same clonal lesion (metastatic lesion).

7.Effect of sevoflurane on hippocampal CaMK Ⅱ/CREB signaling pathway in aged rats

Objective To evaluate the effect of sevoflurane on hippocampal calcium/calmodulindependent protein kinase Ⅱ (CaMK Ⅱ)/cyclic adenosine monophosphate response element-binding protein (CREB) signaling pathway in aged rats.Methods Sixty pathogen-free healthy male Sprague-Dawley rats,aged 18 months,weighing 600-750 g,were divided into 2 groups (n=30 each) using a random number table:control group (group C) and sevoflurane group (group Sev).Group Sev inhaled 2% sevoflurane in the mixture of 50% air and oxygen (2 L/min) for 4 h.Group C inhaled the mixture of 50% air and oxygen (2 L/min) for 4 h.Morris water maze test was performed on 6 days before anesthesia and 1 day after anesthesia.The escape latency,swimming distance,frequency of crossing the original platform and time of staying at the platform quadrant Ⅱ were recorded.On 1,3 and 7 days after anesthesia,the rats were sacrificed,and the hippocampus was obtained for determination of the expression of CaMK Ⅱ,phosphorylated CaMK Ⅱ,CREB and phosphorylated CREB by Western blot.Results Compared with group C,the escape latency and swimming distance were significantly prolonged,the frequency of crossing the original platform was decreased,and the time of staying at the platform quadrant Ⅱ was reduced on 5th day of training and 1 day after anesthesia,and the expression of CaMK Ⅱ,phosphorylated CaMK Ⅱ,CREB and phosphorylated CREB was down-regulated after anesthesia in group Sev (P< 0.05).Conclusion Sevoflurane leads to cognitive decline through inhibiting hippocampal CaMK Ⅱ/CREB signaling pathway in aged rats.
8.Comparative study on determination of sperm DNA integrity by sperm chromatin dispersion test and by acridine orange staining test

Objective To analyze and compare the accuracy and utility of determining sperm DNA integrity by sperm chromatin dispersion (SCD) test and acridine orange staining (AO) test.Methods The level of DNA fragmentation was determined by SCD test and AO test in 32 adult healthy fertile men and 27 idiopathic oligozoospermia (IO) patients.Sperm nuclei with large DNA dispersion halos or with mediumsized halos were normal and nuclei with small-sized halos or no halo were abnormal.The normal sperm DNA was double strands and stained by AO as green.The damaged sperm DNA was single strand,which were stained as green for native DNA and red for denatured DNA.Results The percentage of sperm nuclei with large halos,medium-sized halos,small-sized halos and no halo evaluated bv SCD test in IO patients were (49.9±13.8)%,(11.5±5.4)%,(11.9±6.1)%and(26.7±10.0)%,and they were(73.2±6.2)%,(14.7±6.3)%,(6.8±2.9)%and(5.3±2.2)%in healthy control group,respectively.There was significant difference between two groups(t=8.576,P<0.01;t=2.083,P<0.05;t=4.284,P<0.01;t=11.823,P<0.01).The percentage of sperm DNA fragmentation was (38.6±12.1)%in IO patients and was (12.1±5.2)% in fertile men,respectively.A statistically significant difference was found between IO patients and healthy control group under SCD test (t=11.995,P<0.01).AO test showed no significant differences between IO patients and healthy control group (t=1.626,P>0.05).The percentage of sperm DNA fragmentation evaluated by AO test in IO patients was (45.5 ±13.8)%,and it was (39.8±13.3)%in healthy control group.Conclusions Sperm DNA fragmentation may lead to male infertility.The SCD is effective in testing the sperm DNA fragmentation as a screening procedure to determine semen quality during basic infertility investigation for clinical use.
9.Diffusion tensor imaging and 1H-MR spectroscopy study on radiation-induced injury of the brain after nasopharyngeal carcinoma radiotherapy

Objective To investigate the changes of brain tissue in bilateral temporal lobes at different stages after nasopharyngeal carcinoma radiotherapy by diffusion tensor imaging(DTI)and 1H-MR spectroscopy(1H-MRS).Methods DTI and 1H-MRS were performed in 48 patients with nasopharyngeal carcinoma.in which conventional MRI revealed normal findings after radiotherapy.Twenty-four healthy controls were enrolled in this study and underwent the galne MR scanning.After the image processing and spectral analysis,apparent diffusion coefficient(ADC),fractional anisotropy(FA) and 3 eigenvalue λ1,λ2,λ3 of DTI and the NAA/Cho,NAA/Cr,Cho/Cr of 1H-MRS were measured in bilateral temporal lobes.Forty-eight NPC patients were divided into 3 groups [ less than 6 months(16 cases),6 to 12 months (6 cases)and more than 12 months(26 cases)after radiotherapy]according to different stages of radiationinduced injury of brain.each group's DTI and 1H-MRS data were measured respectively and one-way ANOVA was applied to analyze each parameter. Results The FA value of each test group(less than 6 months.6 to 12 months and more than 12 months)and the control group were 0.445±0.017,0.460±0.016,0.461±0.025,0.473±0.023 respectively.The ADC values of each group were(8.51±0.43)×10-4.(8.48±0.34)×10-4,(8.40±0.33)×10-4,(8.68±0.57)×10-4mm2/s respectively.And the maximum eigenvalue λ1 of each group were(1.251±0.065)× 10-3,(1.293±0.051)×10-3,(1.317±0.074)×10-3,(1.350±0.091)× 10-3mm2/s.The three indicators were significantly difrerent among groups(F=10.873,3.399,9.750 respectively,P<0.05).λ2,λ3 values showed no significant difference among the groups.The NAA/Cho of 1H-MRS of each groups were0.910±0.112,0.972±0.101,1.060±0.095,1.261±0.105 respectively,and the NAA/Cr were 1.212±0.236,1.208±0.183,1.228±0.236,1.435±0.225 respectively.Both of them had significant differences among groups(F=52.840,8.176 respectively,P<0.01).Cho/Cr showed no significant difference among the groups.Conclusions DTI combining with 1H-MRS play a certain guiding role on monitoring and evaluating of radiation injury of brain tissue in nasopharyngeal carcinoma after radiotherapy.It Can provide a scientific basis for the dynamic monitoring of radiation brain injury.
10.Effects of phellodendron and its main components on the cell membrane fluidity

AIM: To investigate the effect of phellodendron and three kinds of its main components, which have asuppressive effect on the immune system, on the membrane fluidity of normal murine splenocytes. METHODS: The fluidity ofmembrane lipid regions of splenocytes was determined by the fluorescence polarization technique using 1, 6 - diphenyl - 1, 3, 5- heatriene (DPH) as a fluorescence probe. RESULTS: The results showed that the water extract of phellodendron and one of itsmain components (palmatine) increased the cell membrane fluidity in the inactive state, but the other two components, berberineand jatrorrhizine, decreased the cell membrane fluidity. After activated by ConA, all of them can decrease the cell membrane flu-idity. CONCLUSION: These results suggest that their immunosuppressive function might be due to decreasing the cell membranefluidity.
