Objective To optimize the extraction conditions of volatile oil from Angelica Sinensis and Cassia Bark in Wenyang purgation granules.Methods The volatile oil was extracted from Angelica Sinensis and Cassia Bark by the method of steam distillation light oil device with diethyl ether extraction. The yield of the volatile oil was chosen as the evaluation index. The time of dip in water, the ratio of water to herbal medicine material and the time of distillation were used as the main factors. The optimum extraction conditions were investigated by the L9(34) orthogonal design. Results The optimal conditions for extraction process of volatile oil from Angelica Sinensis and Cassia Bark in Wenyang purgation granules were as follows:the time of dip in water was 3 h;the ratio of water to herbal medicine material was 10∶1;the time of distillation was 6 h. Conclusion The optimized conditions of extraction process are stable and feasible.

4.An analysis of risk factors for brain metastases after prophylactic cranial irradiation for limited-stage small-cell lung cancer

Objective To evaluate the high?risk factors for brain metastases after prophylactic cranial irradiation ( PCI), and to provide a basis for personalized treatment. Methods A retrospective analysis was performed in 188 patients with limited?stage small?cell lung cancer who received PCI in our hospital from 2005 to 2010. The Kaplan?Meier method was used to calculate the cumulative rate of brain metastases. The log?rank test and the Cox model were used for the univariate and multivariate analyses of the potential factors for the cumulative incidence of brain metastases, respectively. Results In the 188 patients, 31 ( 16?5%) had brain metastases. The 1?, 2?, and 3?year cumulative incidence rates of brain metastases were 4%, 15%, and 20%, respectively. The univariate analysis showed that staged Ⅲ disease before treatment, elevated levels of tumor markers, incomplete remission after chemoradiotherapy, and local?regional relapse were risk factors for high incidence of brain metastases ( P= 0?044, 0?037, 0?005, 0?007) . The multivariate analysis revealed that incomplete remission after chemoradiotherapy and local?regional relapse after chemoradiotherapy were risk factors for high incidence of brain metastases after PCI ( P= 0?003, 0?040 ) . Conclusions Patients with incomplete remission or local?regional relapse after chemoradiotherapy have high incidence of brain metastases after PCI. For those patients, a frequent follow?up of the central nervous system plus salvage cranial irradiation might provide an alternative to PCI.
5.Reconstruction of Spinal Alignment with Total Laminonectomy and Cervical Transpedicular Screws Techniques Applied in Acute Multi-segments Spinal Cord Injury

Objective To assess the efficiency of total laminectomy and transpedicular screw placement techniques and a thoroughly decompression with a three column fixation device on the early cervical spine cord injury.Methods 27 patients were operated successfully with total cervical laminectomy through transpedicular screw techniques.Precise measurements of all cervical spines were made by postoperative CT scan of pedicle dimensions,angulation,and offset relative to the lateral mass boundaries.And meanwhile,cortical integrity and neurovascular injury were then assessed by obtaining postoperative computed tomography scans of each patient.Results Using manual techniques,220 pedicles were instrumented and all the cases were followed up between three to twenty-four months.The postoperative gradations were obviously better than the preoperative evaluation.Regardless of the technique used,the vertebral artery was the structure most likely to be injured.Conclusion Insufficient correlation between different surgeons' assessments of surface landmarks attests to the inadequacy of screw insertion techniques in the cervical spine based on such specific topographic.At the early stage of cervical spinal cord injury,using total laminonectomy decompression through cervical screws techniques to reconstruct spinal alignment could improve the prognosis of patients.
6.Characteristics of the Patients with Post-stroke Depression:208 Cases Report

Objective To investigate the characteristics of the patients with post-stroke depression (PSD).Methods208 cases of patients with stroke were analyzed retrospectively. Results and ConclusionThe PSD occurred more frequently within the first month but less on the first week (P<0.001). The incidence of PSD was higher in female than in male (P<0.001).Frontal, temporal, parietal cortices lesion significantly increased the incidence of the PSD compared with in other area(P<0.01). The more severely the neural function was damaged, the higher the incidence of the PSD was. The muscle strength of patients with PSD recovered poorer than those without PSD. There was significant difference in the incidence of the PSD between the middle age group and aged group (P<0.01).
7.Significance of Determination of Argyrophilic Proteins in Nucleolar Organizer Regions of Blood T Lymphocytes before and after Complete Remission in Children with Acute Leukemia

Objective To detect the argyrophilic proteins in nucleolar organizer regions(Ag-NORs) that express rDNA and rRNA proliferation of T lymphocytes before chemotherapy and after complete remission(CR) in children with primary acute leukemia(AL).Methods The argyrophilic granules area of NOR/nuclear area(I.S%) of T lymphocytes was detected by image analysis system in peripheral blood of 42 patients before chemotherapy and after CR and 30 normal children.Results I.S% in the patients before chemotherapy(5.06%?1.36%) were significantly lower than those in the healthy donors(7.51%?1.06%)(t=8.238 P0.05).Conclusion These results suggest that decrease of Ag-NORs expresses the evidence for tumour induced suppression of immune function of T cells in children with AL prior to treatment.
8.Effect of"Five in One"mode on managerial improvement of medical equipment in operation room

Objective To explore the influence of the comprehensive and continuous management on the quality improvement of medical equipment in the operation room.Methods The"Five in One"mode,which consisted of organization scheme, regulations implementation, operation training, utilization supervision and maintenance, was applied in an observation group,and a control group was established with conventional management mode.The two groups were compared in the times per week of medical equipment utilization, maintenance and hidden risks elimination, the incidence rates of failures and adverse events as well as the medical staffs'satisfaction.Results The observation group had the times per week of medical equipment utilization and hidden risks elimination significantly more than those of the control group, while the times in a week of medical equipment maintenance,failures and adverse events less statistically(P<0.05).The medical staffs' satisfaction in the observation group was enhanced obviously when compared with that in the control group (P<0.05). Conclusion The new"Five in One"mode is effective in solving the problems that occur when the old decentralized mode is adopted for medical equipment management in the operation room. 9.Relationship between thymus output function in CML patients and their bcr-abl mRNA levels.

The study was purposed to analyze the relationship between the content of T-cell receptor excision DNA circles (TREC) and bcr-abl mRNA levels in CML patients and to evaluate the prognostic significance of recent thymic output function detection in patients with chronic myelogenous leukemia (CML). Quantitative detection of TREC and bcr-abl fusion gene transcripts in peripheral blood from 15 CML patients were preformed by real-time PCR. The change of bcr-abl levels in 6 patients was followed-up for two years. The results showed that there was no significant correlation between TREC and bcr-abl mRNA levels in peripheral blood from CML patients for the first attack. Patients who had higher TREC at diagnosis had a larger reduction of bcr-abl after 2 years of follow-up. While out of 2 patients who underwent haemopoietic stem cell transplantation (HSCT), one patient with higher level of TREC before transplantation was confirmed to express undetectable level of TREC by three consecutive detections after transplantation, other one patient was identified to express low level of bcr-abl. It is concluded that high thymic output function in CML patients can be beneficial for killing the residual CML cells.

10.Diagnostic value of MRI in testicular tumor.

OBJECTIVETo study the MRI manifestation of testicular tumor and the value of MRI in the diagnosis of the disease.
METHODSWe retrospectively analyzed 23 cases of pathologically confirmed testicular tumor, and observed the morphological characteristics, signals and surrounding conditions of the tumor using plain and enhanced MRI scanning.
RESULTSOf the 23 cases, seminoma was identified in 7, mixed germinoma in 3, teratoma in 3, endodermal sinus tumor in 2, epidermoid in 1, Leydig cell tumor in 1, leucoma in 1, nonspecific inflammatory mass in 3, and tuberculosis in 2. MRI revealed the precise locations and specific characteristics of
CONCLUSIONBased on MRI findings and clinical manifestation, most testicular tumors can be diagnosed correctly.
