OBJECTIVETo investigate the relationship between chronic pyelonephritis (CPN) and immune function, and the therapeutic mechanism of Yishenkang granule (YSKG).
METHODSOne hundred and twenty patients of CPN were divided into 3 groups randomly, the YSKG group (treated with YSKG), the control A group (treated with Sanjin tablet) and the control B group (treated with western medicine). Serum levels of immunoglobulin (Ig), complement 3 (C3), interleukin-2 (IL-2), peripheral T-lymphocyte subsets, and urinary secretory immunoglobulin A (sIgA) were determined before and after treatment with monoclonal antibody assay, agar diffusion method, radioimmunoassay (RIA), and radioimmuno-equilibrium method, and compared with normal control.
RESULTSThere were disorders of T-lymphocyte subsets in CPN, lowering of serum Ig, C3 and urinary sIgA, and increase of blood IL-2 (P < 0.05 or P < 0.01). These abnormalities could be normalized after YSKG treatment.
CONCLUSIONFunctional disorders of cellular and humoral immunity exist in CPN patients of chronic stage, YSKG could correct the immune functional disorder, control the recurrence of CPN effectively and alleviate the immunopathological damage.

Objective To study the role of lipase in inflammation in patients with acute pancreatitis. Methods Acute pancreatitis patients (n =200) were enrolled in the study. The patients were examined by computerized tomography and the severity of AP is determined by Blathazat Score. We assess the muhivariate-adjusted association of amylase and lipase with inflammatory markers of AP.We identified that lipase was more specifically correlated with AP progression than amylase, lactate dehydrogenase and creatitine kinase. Results We demonstrated that in AP patients,a doubling of lipase excretion was associated with 6.8% increase of white blood cells (95% CI,3.06%-10.5%,P < 0.01),10.3% (95% CI,5.7%-14.9%,P < 0.01) increase of neutrophil number but 14.8% (95% CI,2.3%-27.3%,P <0.05) decrease of lymphocytes in the blood, respectively.By contrast, amylase has no association with these cells. Both amylase and lipase had no relationship with triglyceride level in AP patients. Conclusions Lipase serves as an indicator for the severity and treatment of AP.

Objective To investigate the efficiency and safety of articulating spacer for severe infected knee arthroplasty in patients with medical comorbidities and local sinus tracts. Methods Ten consecutive patients with medical comorbidities (rheumatoid arthritis, diabetes mellitus, etc) or local sinus tracts, who were complicated with late infected TKA, were included in the study. All the patients underwent twostage revision using articulating spacers. All of the patients were debridement thoroughly and followed by implantation of an antibiotic-loaded cement articulated spacer. Two-stage revisions were not followed untill the infection were controlled. The hospital for special surgery (HSS) knee scoring system and range of motion were used to evaluate the outcomes. Results One patient underwent knee fusion because the infection was not controlled after first-stage surgery. The other 9 patients had no evidence of infection. The mean follow-up was 50 months (range, 24-90 months), no recurrent infection developed for these 9 patients. The mean modified HSS score had improved from 48 points (range, 32-63) before the resection surgery to 79 points (range,62-91) at the end of the spacer period. At the latest follow-up, the modified HSS score averaged 89 points (range, 74-95). The good and excellent rate was 0, 80% and 100%, respectively. The average range of motion had increased from 13°-70° preoperatively to 8°-93° prior to the revision. And at the latest follow-up, the range of motion averaged 3° to 110°. Conclusion The delayed two-stage revision using an articulating spacer is effective in the treatment of chronically infected TKA characterized by simple, good reproducible, high rate of infection control, better joint function after surgeries

Objective To investigate the early effect of medium-dose ionic irradiation on the expression of Fos protein in the rat brain. Methods Fos protein was observed in rat brains at times ranging from 24 hours to 4 weeks after hemispheric irradiation (single-fraction maximal dose of 20Gy) with the immunohistochemical technique. Results Compared with that of the un-radiated rats, the expression of Fos protein in the irradiated brain decreased distinctly 24 hours and 1 week after irradiation. However, the quantity of Fos immunopositive cells increased gradually afterwards. At four weeks after radiation, expression of Fos protein recovered progressively in medulla oblongata and pons,in which Fos immunopositive cells were more than those in control group. In contrast, expression level of Fos protein in mesencephalon, diencephalons or telencephalon was still less compared with that of the un-irradiated rats. Conclusion The result suggested that the neuronal activity might be inhibited in certain nuclei of the rat brain in early stages after hemisphere irradiation, and this inhibitory phenomenon was more obviously in higher neural centers.

Journal of Preventive Medicine 2018;30(3):221-225
Objective To examine the independent effect of depression on activities of daily living of patients with mild cognitive impairment (MCI). Methods Data were obtained through the face-to-face interview. Totally 154 elderly patients that were screened from 552 old people by the Montreal Cognitive Assessment (MoCA) were diagnosed as MCI. They were investigated with the Geriatric Depression Scale, and instrumental ability of daily living (IADL) were measured by the Functional Activities Questionnaire. The effect of depression on IADL of patients with MCI was analyzed by logistic regression analysis. Results The prevalence of depressive symptoms among Chinese elders with MCI was 31.82%. The scores of Medical Outcomes Study Social Support Survey, and Functional Activity Questionnaire of depression group and non-depression group were significantly different (P<0.05) . Logistic regression analysis showed that scores of Functional Activity Questionnaire was positively associated with the scores of Memory Inventory for the Chinese (OR=1.36, 95% CI: 1.15-1.61) and depression (OR=6.19, 95% CI:2.07-18.55) . Conclusion The prevalence of depression in elderly patients with MCI was high. Depression would significantly reduce the IADL of the old patients with MCI. Medical workers should be aware of the extent and impact of depressive symptoms in MCI and take active measures in evaluating and treating the depressive symptoms.

Objective:To evaluate the radiographic and clinical outcomes of pre-operative Halo-gravity traction (HGT) and posterior correction surgery in treating patients with neurological deficits secondary to severe focal angular kyphosis of the upper thoracic spine. Methods:A total of 16 patients (11 males, 5 females) with neurologic deficits secondary to severe focal angular kyphosis of the upper thoracic spine undergoing preoperative HGT and posterior correction operation from January 2010 to December 2019 were retrospectively analyzed. The average age was 12.9±5.6 years (range 6-27 years). The standing X-ray of whole spine was taken at pre-, post-traction and post-operation. The Cobb angles of main curve at both sagittal and coronal planes were measured accordingly. The neurologic function at pre-traction, post-traction and post-operation was assessed according to the American Spinal Injury Association (ASIA) grading. The complications during HGT, operation and post-operative follow-up were recorded for each patient. Results:The average values of focal kyphosis and scoliosis were 96.1°±16.0° (71°-128°) and 75.5°±20.5° (40°-107°) at pre-traction respectively. The spinal cord function graded by ASIA criteria at pre-traction was B in 1 patient, C in 6 and D in 9, respectively. The correction rates of focal kyphosis and scoliosis were 32.8%±15.0% (18.0%-65.9%) and 22.9%±8.0% (14.1%-38.6%) after traction, which were further improved to 45.4%±14.9% (29.0%-69.0%) and 33.6%±8.6% (23.3%-49.3%) at post-operation without significant correction loss during 35.6±14.2 (24-72) months follow-up. After traction, the spinal cord function improved to grade D in 4 patients and grade E in 12 patients. At the last follow-up, the spinal cord functions were grade E in 15 patients and grade C in 1 patient. No neurologic monitor events occurred during operation. One patient suffered from transient left brachial plexus after operation. Further, proximal hook loosening was observed in 2 patients during follow-up. The spinal cord function was ASIA grade C pre-operatively in one patient, who recovered to ASIA grade E after operation and significantly deteriorated to ASIA C at 4 years follow-up.Conclusion:The correction of spinal kyphoscoliosis was satisfactory in this cohort. Preoperative HGT followed by posterior spinal correction surgery is an effective and safe procedure in treating neurological deficits secondary to focal angular kyphosis in the upper thoracic spine.
