2.Effect of spleen-invigorating prescription on dendritic cell function in patients with chronic hepatitis B of TCM Pi-deficiency syndrome type.

OBJECTIVETo observe the effect of Spleen-invigorating Prescription (SIP) on dendritic cell function in patients with chronic hepatitis B.
METHODSA total of 60 patients with chronic HBV of Pi-deficiency syndrome type were enrolled and randomized to 2 groups, 30 in each group. Patients in the control group were given intramuscular injection with human interferon alpha 1b, 3 times a week, while those in the treated group were given orally with SIP twice a day, the therapy lasted for 6 months. Dendritic cells (DCs) were isolated from peripheral blood and cultured, then the expression of surface markers, HLA-DR, CD86, CD80, CD40, CD14 and CD11c were detected before and after treatment by flow cytometry, and the function of DCs was also evaluated by mixed lymphocyte reaction (MLR) determination once before treatment and once after treatment.
RESULTSThe expressions of DCs' surface CD86, CD80, CD40 and CD11c in the treated group were higher (P < 0.05, P < 0.01) and the changes of stimulating index, IFN-gamma and IL-12 were superior in the treated group to those in the control group (P < 0.05).
CONCLUSIONSSIP can significantly improve DC's function, so, one of mechanisms of SIP in improving clinical efficacy may be the regulation of immune function.

3.Isolation and identification of impurities from raw material of clindamycin phosphate.

Four impurities were isolated from raw material of clindamycin phosphate (CP), and their structures have been determined. LC-MS was used to determine the molecular weights of the impurities in the raw material of CP. Reversed-phase preparative HPLC was used to prepare them, and their chemical structures were identified by HR-MS and NMR. The four unknown impurities were determined as clindamycin-B-phosphate (1), clindamycin-2,4-diphosphate (2), 3',6'-dehydro clindamycin phosphate (3), epi-clindamycin phosphate (4). Impurity 1 has been included in BP and EP, while 2, 3 and 4 have not. The impurities 2, 3, 4 are first separated from raw material of CP.

4.Studies on absorption kinetics of scopoletin in rat stomachs and intestines.

OBJECTIVETo study absorption kinetics of scopoletin in rat stomachs and intestines.
METHODRats was cannulated for in situ recirculation. UV and HPLC methods were used to determine the concentrations of phenolsulfonphthalein and scopoletin, respectively.
RESULTThe absorption rates in rat stomachs at 2 h after administration was 76.31%; The absorption rates at colon, duodenum, ileum and jejunum were 46.25%, 40.54%, 38.21%, 32.77%, respectively. The absorption rate constant (Ka) at concentrations of 10.0144, 20.0288-40.0576 mg x L(-1) in intestine were 0.6434, 0.6137, 0.5970 h(-1), respectively. The Ka of scopoletin at pH of 6.0, 6.8 and 7.4 in intestine were 0.6217, 0.6033, 0.6137 h(-1), respectively.
CONCLUSIONThe concentrations and pH values of scopoletin solution had no distinctive effect on the absorption kinetics. The absorption of scopoletin was a first-order process with passive diffusion mechanism. Scopoletin was well absorbed at stomachs and intestines in rats. Colon was the best absorption site of scopoletin, which suggest that a sustained-release preparation should be suitable for this compound.

5.Effect of anesthesia before mode on emergency response for the patients undergoing time selective operation

Objective To investigate the selective operation patients in operation room in anesthesia before nursing management model on the stress influence research.Methods 236 patients undergoing elective operation were randomly divided into intervention group and control group with 118 cases in each group.Intervention group patients,preoperative in anesthesia recovery in anesthesia before nursing management,and corresponding humanistic care nursing interventions,in 5-10 minutes before anesthesia by nurse access implementation of the operation,in the humane care of itinerate nurse performed under anesthesia operation.The control group with traditional nursing management in operation room,after access arrangement corresponding operation for waiting operation anesthesia.Heart rate,blood pressure,blood glucose and psychological condition were compared and analyzed between the two groups.Results In the preoperative 3d SAS of the intervention group and the control group were (42.3 ± 3.8 ) score and ( 50.6 ± 3.4 ) score,and they had significant difference (t =8.63,P <0.01 ); blood glucose,heart rate,systolic blood pressure of the intervention group before anesthesia were (4.7 ±0.3) mmol/L,(78.3 ±7.4),times/min,(16.9 ±0.3) kPa; in the control group they were ( 5.3 ± 0.4) mmol/L,(95.3 ± 7.6) times/min,( 18.3 ± 0.6) kPa,and the differences were statistically significant (t =14.10,7.82,8,12,respectively,P < 0.01 ).Conclusions Preoperative intensive care management and related nursing intervention pattern for patients with operation can reduce the patient' s emotional response; improve the follow-up operation and treatment compliance.Besides,it also enable patients to maintain good physical and mental condition and improve the patient' s preoperative safety,prevent nursing adveese events.
6.Quality of life and major dimensions of health status among the elderly in the central and western poor areas of China

Objective To analyze the quality of life and the main dimensions of health status among the elderly living in the central and westem poor areas of China.Methods Cluster sampling was used and 1145 elderly aged above 60 were sampled from 4 provinces in western areas of China.Economic status,performance of health,self reported situation and satisfaction on health were investigated.We conducted comparative dimensions of health analysis across age and gender.Results Among the elderly,58.6% of them had monthly income less than 250 Yuan RMB and 14.1% of them with monthly income between 250-400 Yuan RMB.As for the sources of income,50.1% of the elderly relied on their children while 43.7% of them supported themselves.Among the elderly aged 60-,70-and 80-,68.54%,54.63% and 38.46% of them had no difficulty in mobility while 75.73%,68.52% and 53.85% had no difficulty taking care of themselves,67.98%,56.02% and 35.90% of them had no difficulty in managing daily activities,55.51%,51.39% and 38.46% of them had no pain or discomfort and 69.66%,59.26%,51.28% of them did not show any signs on anxiety or depression.Men aged over 80 had better status than women,however,there was no significant difference for the age groups 60-or 70-on the mobility and self-care between genders.37.98%,31.02% and 28.21% of the elderly in the age groups 60-,70-or 80-,self-reported to have had good health,with males more than females (P<0.05).76.74%,75.00% and 79.49% of the 60-,70-or 80-elderly groups self-reported having satisfactory life style.The aged 60-elderly did not show significant gender difference on self-reported satisfaction with life but the male elderly aged 70 and above reported having more satisfied life than the females (P<0.05).Factors as marriage status,gender,chronic disease,ability of mobility and daily activities played positive promotion roles on health status of the elderly.Conclusion Sources of income was limited among the elderly.The overall self-reported health status was poor but males were more likely to show positive results on self-assessment.Proportions of health-related quality of life indicators among elderly declined with age,and the performance of life function of the elderly women were inferior to men.Elderly living in the rural areas presented a higher proportion on ‘satisfaction of life'.
7.Association of aldosterone synthase (CYP11B2) gene -344T/C polymorphism with essential hypertension in Mongolian nationality.

OBJECTIVETo explore the relationship between the -344T/C polymorphism of aldosterone synthase (CYP11B2) gene and essential hypertension in Chinese Mongolian population.
METHODSBy cluster-sampling method, a total of 1575 Mongolian people in Tongliao city of Inner Mongolia were included in this study. And 417 subjects were normotension, 596 subjects were prehypertension and 562 subjects were essential hypertension. A survey was conducted to collect data by personal interview using a standard questionnaire, meanwhile fasting blood samples were drawn. Height, weight, waist circumference, blood pressure, blood-fat indexes and fasting plasma glucose were measured. The variant genotypes of CYP11B2 were identified by PCR assays. The relationship between the -344T/C polymorphism of CYP11B2 gene and essential hypertension were analyzed by multinomial logistic regression model.
RESULTSCrude prevalence of prehypertension among Mongolian people was 37.84% (596/1575) and hypertension was 35.68% (562/1575). The age-standardized prevalence of prehypertension was 38.57% and hypertension was 31.53%. The frequency of the T and C allele was 0.66 (481/728) and 0.34 (247/728) for normotension group, 0.69 (696/1042) and 0.33 (346/1042) for prehypertension group, 0.71 (706/998) and 0.29 (292/998) for hypertension group. The multiple logistic models showed CYP11B2 variant genotypes were associated with prehypertension (TT/CC, OR = 1.33, 95%CI: 0.87 - 2.01; TC/CC, OR = 1.74, 95%CI: 1.13 - 2.67; TC + TT/CC, OR = 1.49, 95%CI: 1.01 - 2.22); CYP11B2 variant genotypes were associated with hypertension (TT/CC, OR = 1.70, 95%CI: 1.07 - 2.70; TC/CC, OR = 1.59, 95%CI: 0.98 - 2.50; TC + TT/CC, OR = 1.66, 95%CI: 1.06 - 2.58).
CONCLUSIONCYP11B2 gene -344T/C polymorphism were associated with essential hypertension in Chinese Mongolian population.

8.Influence of genetic polymorphisms in drug metabolism enzymes and transporters on pharmacokinetics of different fluvastatin formulations

OBJECTIVE The purpose of the present study was to investigate the impact of fluvas-tatin formulation on the pharmacokinetics-genetic polymorphis relationship. METHODS We compared the difference between the pharmacokinetics of fluvastatin as an extended-release (ER) 80 mg tablet and an immediate-release(IR)40 mg capsule in terms of drug metabolism enzyme and transporter ge-netic polymorphisms. In this open-label, randomized, two-period, two-treatment, crossover study, ef-fects of BCRP, SLCO1B1, MDR1, CYP2C9, and CYP3A5 polymorphisms on the pharmacokinetics of fluvastatin were analyzed in 24 healthy individuals.Each treatment duration was 7 days with a washout period of 7 days between the crossover.Serum concentration of fluvastatin was evaluated using high-performance liquid chromatography-tandem mass spectrometry. RESULTS The SLCO1B1 T521C genotype had no statistically significant effect on IR 40 mg capsule of fluvastatinafter single or repeated doses.However,for the ER 80 mg tablet,the SLCO1B1 T521C genotype correlated with the AUC0-24of repeat doses (P=0.01). The CYP2C9*3 genotype correlated with the AUC0- 24after the first dose IR 40 mg capsule (P<0.05); however, the difference between CYP2C9*1/*1 and CYP2C9*1/*3 was not statistically significant after repeated doses. CONCLUSION The effect of SLCO1B1 T521C on fluvas-tatin exposure was observed and was more profound in ER and repeated dose administration than in IR and single dose administration.We recommend that formulation should be incorporated into future pharmacogenomics studies and clinical implication guidelines.
9.The prognosis of hepatitis E superinfection in hospitalized chronic hepatitis B patients in China: A systematic review and Meta-analysis

Objective To evaluate the prevalence and prognosis of hepatitis E virus ( HEV) su- perinfection in patients with chronic hepatitis B ( CHB) . Methods Electronic databases of Pubmed, CNKI,and Wanfang were searched,and references of the relevant articles for literatures associated with HEV superinfection in CHB patients in China published during 2008-2019 were screened. Studies which met the inclusion and exclusion criterias were selected and related data were pooled for analysis. The R 3. 5. 2 and Revman 5. 3 were used to measure heterogeneity with the I2 statistic and pooled incidence and odds ratio ( OR) . Results 28 literatures were eligible for inclusion. The results showed the rate of HEV superinfection in hospitalized CHB ( CHB-HEV) patients was 13. 6% ( 95%CI: 12. 5% -14. 9%) . The pooled incidence of liver failure in CHB-HEV patients was 34. 7% ( 95%CI: 29. 6%-40. 1%) ,which was higher than CHB patients ( OR= 10. 9,95%CI: 6. 8-17. 4) ,CHB patients superinfected with hepatitis A virus ( CHB-HAV) ( OR= 3. 8,95%CI: 2. 4-6. 2) ,and hepatitis E patients ( OR= 5. 1,95%CI: 3. 8- 6. 8) . The pooled mortality of CHB-HEV patients was 13. 8% ( 95%CI: 10. 3-17. 8) ,which was higher than CHB patients ( OR = 8. 5,95%CI: 4. 0-18. 3) ,CHB-HAV patients ( OR = 14. 4,95%CI: 4. 8- 42. 9) ,and hepatitis E patients ( OR= 4. 4,95%CI: 3. 0-6. 4) . Conclusion In China,HEV sueperinfection on chronic hepatitis B patients are common,and is an important cause of liver failure and death.
10.Operative cooperation in cochlear implantation with intraoperative CT scanning

Objective To explore the operative cooperation in cochlear implantation with intra-operative CT scanning.Methods Ten cases of cochlear implantation with intraoperative CT scanning between April 2010 and April 2011 were selected and summarized the characters of operations.Results The electrode arrays were placed adequately in all ten cases.The duration of operation was (2.3 ± 0.5)h,the times needed for scanning during operation was (1.4 ± 0.7) h,the duration of intraoperative CT scanning was (13.5 ± 6.7) min.Conclusions The complex cochlear implantation with intraoperative CT scanning can be smoothly finished with proper preoperative and scientific and reasonable cooperation.
