Objective To explore clinical characteristics in boys and girls with systemic lupus erythematosus(SLE) and provide refe-rences for clinician′s correct diagnosis and cognition.Methods The clinical characteristics and laboratory examination of 88 children with SLE were comparatively analyzed from Dec.1993 to Sep.2007 by a retrospective cohort study.There was a ratio of 1.04.9 between boys and girls with SLE,and the peak incidence was among adolescent girls(10-15 years old),accounting for 60.2% of all children with SLE.There was no difference in onset age between the 2 groups.Disease activity was evaluated using SLE disease activity index(SLEDAI).Clinical characteristics and laboratory examination were studied by Spearman rank correlation analysis.Results There were significant differences in the constituent ratio of onset symptom and clinical symptom between boys and girls with SLE.Initial manifestation in boys with SLE was mainly skin lesion while in girls with SLE was fever.There were no significant difference for the incidence of each organ lesion between boys and girls with SLE,but higher rate of anemia and hepatic disfunction in girls with SLE was found.There were up trends of splenomegaly,abnormal electroence phalogram,butterfly erythema occurred in girls with SLE,while gastrointestinal symptoms,serositis,eyes damage occured in boys with SLE.Nephrotic syndrome was the main type of renal damage in both groups(boys:40.0%,girls:45.7%).There were significant differences in the constituent ratio of detection rate in laboratory examination,but no significant difference of positive rate was found in any kind of item between boys and girls with SLE.There was some relationship between C3 and muti-organ lesion,double stranded DNA(dsDNA) and renal lesion through correlation analysis for girls with SLE.Conclusions There are some differences of initial manifestation between boys and girls with SLE,the clinical features and laboratory examination show no gender difference.C3 may be a indicator of the disease severity for girls with SLE.

Objective To evaluate the effects of dantrolene pretreatment on diaphragmatic function in septic rats .Methods Thirty adult male Sprague-Dawley rats , weighing 200-220 g , aged 9-10 weeks , were randomized into 3 groups (n=10 each) using a random number table :sham operation group (group S) ,spesis group (group CLP) and dantrolene group (group D) .The animals were anesthetized with pentobarbital sodium . Dantrolene 6 mg/kg was injected intraperitoneally (in dimethyl sulfoxide 500 μl ) .Sepsis was induced by cecal ligation and puncture 1 h later in CLP and D groups .The left and right diaphragm was rapidly excised at 24 h after cecal ligation and puncture . The left diaphragm was used to detect the systolic function including the single stimulation twitch , dmax/dt , dmin/dt , maximal force of tetanic contraction , force-frequency curves , and fatigue index .Results Compared with group S ,the single twitch myopalmus ,dmax/dt ,dmin/dt ,maximum peak tension and fatigue index were significantly decreased in group CLP ,the single stimulation twitch ,dmax/dt ,dmin/dt and maximal force of tetanic contraction were decreased , fatigue index was increased in group D , and the force-frequency curve was shifted downward in CLP and D groups ( P<0.05 or 0.01) .Compared with group CLP ,the single stimulation twitch ,dmax/dt ,dmin/dt ,maximal force of tetanic contraction and fatigue index were significantly increased in group D ( P< 0.05 or 0.01 ) .Conclusion Dantrolene pretreatment can improve diaphragmatic function in septic rats .

Objective:To study the distribution of allergens in our district and the correlation between skin prick test with antigen and nasal provoking test in patients with allergic rhinitis.Method:A total of 1 400 cases with allergic symptoms and signs underwent skin prick test with standardized aeroantigens provoking by AllergoPhamar Company. The patients allergic to Dermatophagoides with different scale of the skin prick test were taken nasal provoking test in order to evaluate the correlation between skin prick test and nasal provoking test.Result:One thousand and eighty-eight(78%) cases showed a positive result in skin prick test.The percent of patients with positive skin prick test were as follows: 462 cases of Dermatophagoides pleronyssinus, 428 cases of Dermatophagoides farinae. Positive perennial aeroantigens were 52.2%(568),positive seasonal aeroantigens were 35.7%(388). All the patients of the scale of skin prick test was +++ and ++++ were positive. There are 29 patients were positive with nasal provoking test which the scale of skin prick test was ++. There are 21 patients were positive with nasal provoking test,which the scale of skin prick test was +.Conclusion:The correlation between skin test with antigens and nasal provoking test in patients whose positive scale were +++ and ++++. There are false positive in the patients whose positive scale were + and ++. The specific immunotherapy should consider the scale of the skin prick test and eliminate the false positive before the therpy.

Objective To evaluate the clinical value of small doses of solifenacin to treat children of primary nocturnal enuresis. Methods Children (n=78) of primary nocturnal enuresis were randomly divided into treatment group(39 cases) and control group(39 cases). All children received behavioral intervention therapy, while children in the treatment group were given Solifenacin(5 mg/tablet, 1/3 tablet, 1 times a day for 3 months) orally besides behavioral therapy. Children were followed up for 3 months after the end of the treatment, and the curative effect and recurrence rate were compared between two groups. Results Compared with that of children in control group, cure rate of children in treatment group is higher at the end of treatment (89.7%vs 69.2%, P<0.05) as well as by 3 month of follow-up time (82.1%vs 46.2%, P<0.05). The re?currence rate of children in treatment group, by three months follow-up time, was lower than that of children in control group (8.6%vs 33.3%,P<0.05). Conclusion Small dose of Solifenacin in addition to behavioral intervention is safe and effec?tive in treatment of children with enuresis, which enhance cure rate and reduce recurrence.

Objective Through the 2003 -2012 epidemic encephalitis B cases analysis ,to study the prevalence of epidemic en-cephalitis B in Shaoguan ,and to provide the scientific basis for the prevention and control epidemic encephalitis B .Methods Active surveillance of suspected cases ,retrospective analysis of laboratory confirmed cases related materials were done .Results There were 47 cases who suffered from epidemic encephalitis B in Shaoguan from 2003 to 2012 ,and 2 death cases occurred .Epidemic en-cephalitis B incidence in years was (0 -0 .531) /10 million .There was a morbidity peak in June and July ,which accounted for 87 .23% of the total incidence ,20 cases occurred in June(42 .55% )and 21 cases occurred in July(44 .68% ) .Men suspected cases in 130 cases ,31 laboratory confirmed cases .Women suspected cases in 86 cases ,16 laboratory confirmed cases .The positive cases of male was more than female .In 47 epidemic encephalitis B cases ,0- <4 years old 6 cases ,4- <7 years old 22 cases ,7- <20 years old 17 cases ,≥ 20 years old 2 cases .Most of the patients were preschool children and students ,accounting for 82 .97% .There were 32 cases with no or uncertain history of vaccination ,accounting for 68 .08% of the total cases .15 cases had history of immunity ,ac-counting for 31 .92% of the total cases .Conclusion Prevention of epidemic encephalitis B should pay more attention to epidemic en-cephalitis B vaccination .Mosquito control work of rural and peri-urban areas should be strengthened .

ObjectiveTo study the effects of rehabilitation training on the expression of brain derived neurotrophic factor(BDNF) around the cerebral infarcted area of rats.Methods60 SD rats were randomly divided into rehabilitation group and immobilization group 24 hours after cerebral infarction.The rehabilitation group were given water maze training,rotating bar and rolling cage exercises everyday while the immobilization group were fixed in cages. Histochemistry was used to detect the BDNF expression around the cerebral infarcted area at the 1st,3rd,7th,10th and 14th day after infarction respectively.ResultsAt 1st day after the infarction, the expression of BDNF increased obviously around the infarcted area in both group. More BDNF positive neurons were found in the rehabilitation group than that in the immobilization group at 3rd day (P<0.01). BDNF positive astrocytes showed obvious increasing in both group. BDNF positive neurons decreased with time going, and the color became lighter at the same time .At the 7th day after infarction, there were only a few positive neurons, and seldom obvious positive neurons could be seen around the infarcted area at 10th and 14th day. Great deal of BDNF positive astrocytes were found at 3rd,7th,10th and 14th day, and the rehabilitation group showed more expression than that of the immobilization group(P<0.01).ConclusionsThe rehabilitation training may increase the expression of BDNF which might take an active part in the recovery of the central nervous system injury and the rebuilding of its function.

Objective To evaluate the diagnostic value and limitation of multichannel intraluminal esophageal impedance and pH (MII-pH) monitoring on the diagnosis of gastroesophageal reflux-related chronic cough (GERC).Methods The patients with suspicious GERC consecutively referred to our respiratory clinic between May 2010 and July 2011 underwent a MII-pH monitoring,and received anti-reflux drug therapy,irrespective of the laboratory findings.Chronic cough due to gastroesophageal reflux was determined when there was a favorable response to anti-reflux therapy.Then,the sensitivity,specificity,false positive and negative rate,total consistence,positively and negatively predictive value,the area under the curve of ROC and the Kappa value of the laboratory investigation were calculated for the diagnosis of GERC.Results During the research period,56 patients completed MII-pH monitoring.Among them,the abnormal reflux was found in 35 patients,and GERC was finally confirmed in 30 patients (85.7％) including 25 patients (83.3％) due to acid reflux and 5 patients (16.7％) due to non-acid reflux.In the remaining 21 patients with normal reflux episodes,6 patients (28.6％) could be explained by non-acid reflux for their cough because of a relatively predominant weakly acid reflux and favorable response to empirical anti-reflux therapy.For the diagnosis of GERC,MII-pH monitoring had the sensitivity of 83.3％,the specificity of 75.0％,false positive rate of 25.0％,false negative rate of 16.7％,total consistence of 80.4％,positive predictive value of 85.7％,negative predictive value of 71.4％,the area under the curve of ROC of 0.792 and Kappa value of 0.577 respectively.Conclusion MII-pH is a sensitive and reliable tool for the diagnosis of GERC due to its ability to detect both acid and non-acid reflux.

Objective To investigate the diagnosis and treatment of Waldenstrom's macrogiobulinemia(WM).Methods The clinical information of 15 patients with WM were analyzed retrospectively.Results 3 cases had achieved complete response(CR).9 cases had achieved partial response(PR).7 cases remained alive,one of them lived beyond 79 months.Conclusion The traditional diagnosis of WM is not enough,and immunophenotype described for WM diagnosis is more important.WM is an incurable disease.Treatment with plasmapheresis and chemotherapy appears highly effective and safe for WM.

Objective To investigate the diagnosis and treatment of sequential diffuse large B-cell lymphoma (DLBCL) after peripheral T-cell lymphoma (PTCL).Methods A case with sequential DLBCL after PTCL was reported,and the characteristics and responses of this case were analyzed.The previous literature was reviewed in order to explain the mechanism and prognosis of such type of disease.Results This patient was diagnosed as PTCL not otherwise specified (PTCL-NOS) definitely,but after a period of treatment,DLBCL was developed as a second tumor.The characteristics and onset interval were just similar to those described in the literature,in which the mechanisms were mentioned as common effects of tumor cell,microenviroment and therapies.This patient got effects through the initial treatment,but considering the poor outcome by former researchers,the prognosis needed to be closely followed up.Conclusion Sequential development of EBV-unrelated DLBCL after PTCL-NOS is very rare,and the mechanism,therapy and prognosis need further investigation.

Objective To investigate the patients' radiation dose received in digital radiography (DR) and provide basic data for developing diagnostic reference levels.Methods The patient' s ESD was estimated using the TLDs and DAP was measured by the dose-area product meter.The E values were then calculated by the DAP using Monte Carlo data and RefDose software.Measurements were made for twelve types of examination:skull PA,skull LAT,chest PA,chest LAT,abdomen AP,pelvis AP,cervix spine PA,cervix spine LAT,thoracic spine PA,thoracic spine LAT,lumber spine PA and lumber spine LAT.Results Both kV and mAs varied in the same type of examination for ESD,DAP and E(F =33.47,24.68,43.19,P ＜ 0.05).The dose each time for lumber spine LAT was the highest,reached 4.62 mGy in ESD and 2.26 Gy·cm2 in DAP,respectively.The E of abdomen AP averaged as 0.59 mSv,higher than that of lumber spine LAT.Even for the same type of examination,the dose from each equipment was different.Conclusions DR has the potential to reduce the patients' radiation doses.The guidance levels suitable for Chinese population should be established as soon as possible.
