1.Influence of Chinese Herbal Medicine on Survival Rate of Nasopharyngeal Cancer Patients After Chemoradiotherapy

Objective A retrospective study was carried out to investigate the survival rate of nasopharyngeal cancer patients after the combined treatment of chemoradiotherapy and Chinese herbal medicine. Methods Based on the will of the patients, 230 cases were divided into treatment group (N=107) and control group (N=123). Both groups received chemoradiotherapy, and the treatment group was additionally given oral use of Chinese herbal medicine over 180 doses per year, and the treatment lasted over 2 years. The survival rate was calculated by Kaplan-Meier estimator, and the Kaplan-Meier plot for the two groups was compared. Results (1) In the treatment group, the one-year, 3-year and 5-year survival rate was 99.1%, 86.4%, 72.5%, and was 95.9%, 73.5%, 58.3% respectively in the control group. The Kaplan-Meier plot showed that significant difference of patient survival was presented in the two groups ( P<0.01) . ( 2) The results of hierarchical group analysis of 230 nasopharyngeal cancer patients showed higher survival rate can be achieved in the T3-4 and N0 poorly-differentiated squamous carcinoma patients aged 50 years or more at the stage Ⅲ-Ⅳ in the treatment group than that in the control group, and the statistical differences were significant ( P<0.05 or P<0.01). Conclusion Chemoradiotherapy combined with Chinese herbal medicine can increase the survival rate, and prolong life span of the nasopharyngeal carcinoma patinets, in particular for the T3-4 and N0 poorly-differentiated squamous carcinoma patients aged 50 years or more at the stageⅢ-Ⅳ.
2.Pharmacokinetic-pharmacodynamic modeling of irbesartan in healthy Chinese adult volunteers

Aim To investigate the combined pharmacokinetic-pharmacodynamic(PK-PD) model of irbesartan in healthy Chinese male volunteers. Methods Eighteen healthy male volunteers received 300 mg irbesartan tablet orally. The plasma drug concentration (C_p) was determined by HPLC and pharmacologic effects, including SBP, DBP and HR, were measured simultaneously. The pharmakinetic and PK-PD model parameters were calculated. Results The pharmacokinetic profiles were best fitted a two-compartment open model. There was a hysteresis loop between effects and plasma concentrations. The relationship between effects and effect compartment concentrations of drugs could be represented by the Sigmoid-E_ max model. Conclusion We successfully eatablished the PK-PD model of irbesartan in healthy male volunteers. These findings may provide a more rational basis for patient-specific dosage individualization.
3.Etiology and treatment of neonatal pneumothorax

ObjectiveTo investigate the cause,diagnosis and treatment measures of neonatal pneumothorax.MethodsThe clinical data of 98 cases with neonatal pneumothorax in our hospital from Mar 2005 to Mar 2010 were reviewed retrospectively.ResultsThere were 27 premature neonates and 71 full-term neonates in the 98 cases.Ten cases were spontaneous pneumothorax and 88 cases were non spontaneous pneumothorax.After the treatnent of primary disease,pleural aspiration,chest drainage and mechanical ventilation,88 cases were cured,8 cases were improved and 2 cases died.ConclusionNeonatal pneumothorax is related with cesarean section,improper use of endotracheal intubation,meconium aspiration,premature hyaline membrane disease using pulmonary surfactant and ventilator therapy.Most pneumothorax is due to lung disease.The prevention of preterm birth and asphyxia can reduce the incidence of neonatal pneumothorax.
4.Research on characteristics and related factors of adverse reactions of anti-tuberculosis among Zhuang nationality patients with smear-positive pulmonary tuberculosis

Objective To understand the characteristics and related factors of adverse reactions (ADR) of anti-tuberculosis drugs among Zhuang nationality patients with smear-positive pulmonary tuberculosis (TB) and to provide basis to ADR control.Methods During Dec 2007 to May 2008,408smear-positive pulmonary TB cases of Zhuang nationality in 14 counties were selected by simple random sampling,and followed up for 6-8 months.Questionnaires were performed and liver/renal functions were tested during the research.Data were analyzed by chi square test and Logistic regression model.Results The incidence of ADR of Zhuang nationality people was 17.2%,which was higher than national level ( 12.62 % ).The incidence of allergic reaction was highest of all ADR ( 5.9 % ) and the incidences among different types of ADR were significantly different ( x2 =36.9,P=0.000).Serious ADR types were allergic reaction,gastrointestinal reaction,liver damage,nerve damage and kidney damage.And 24.3 % of the cases had two or three kinds of ADR.ADR occurred at the median of 21.5th day of anti-TB treatment,and the gastrointestinal reaction occurred the earliest (6th day).81.4 % of the ADR occurred in the first 2 months of anti-TB treatment.The rates of drug withdrawal and interruption were 11.5 % and 9.2 %,respectively.Liver damage and nerve damage were the main causes of treatment withdrawal and interruption.Multiple factor non-conditional Logistic regression analysis showed that the ADR was correlated with age and the risk of ADR in ≥60years old patients was 3.3 times of that in 14-34 years old patients (OR=3.275,P=0.006).Thus,ADR incidence was increased with aging.Conclusions The incidence of ADR is higher in Zhuang nationality patients than that in general patient population.ADR monitoring and intervention must be strengthened in high-risk groups during high-risk period to reduce the adverse effect of anti-TBtreatment.
5.Screening of invasion and metastasis related genes regulated by Stat3 in pancreatic cancer SW1990 cell

Objective To screen the genes related with signal transducers and activators of transcription 3 (STAT3) regulating pancreatic cancer invasion and metastasis by gene chips.Methods Human pancreatic cancer cell line SW1990 stably expressing low level of Stat3 was established by lentivirus transfection,while cells transfected with mock plasmid and cells without transfection served as control groups.The differences of invasion and metastasis related genes expression among the three groups were screened by gene chips.STAT3 mRNA and protein expression was measured by real-time PCR and Western blot.Three differentially expressed genes (MMP-7,IL-1β and IgTα7) were verified.ResultsThe expression level of STAT3 mRNA was 0.391 ± 0.037 after pancreatic cancer SW1990 cell trarsfected with STAT3 targeted lentivirus,which was significantly lower than those in mock plasmid group (1.002 ± 0.015) and nontransfected group ( 1.206 ± 0.042,P < 0.05 ) ; the expression level of STAT3 protein was 182.38 ± 65.32,which was significantly lower than those in mock plasmid group (223.40 ±58.40) and non-transfected group (212.33 ±53.69).Eight invasion and metastasis related genes of SW1990 lowly expressing Stat3 were upregulated,while 3 genes were down-regulated.By verification,the mRNA level of MMP-7 and IL-1β were lower than in control group transfected with mook plassmid(0.287 ± 0.115 vs 1.010 ± 0.124,t =19.45,P =0.000;0.490 ± 0.10 vs 1.002 ± 0.002,t =13.83,P =0.000),but the mRNA level of IgTα7 was not decreased (1.173 ±0.280 vs 0.998 ±0.003,t =4.236,P =0.094).Meanwhile,the protein level of MMP-7 was significantly down-regulated when Stat3 was knocked down.ConclusionsStat3 causes changes of expressions of many invasion and metastasis-related genes of SW1990,and MMP-7 may be the main target gene regulated by Stat3.
6.A new labdane diterpenoid glycoside from Nervilia fordii.

To study the chemical constituents of Nervilia fordii (Hance) Schltr., various chromatographic methods were used, including D101 macroporous resin, silica gel, ODS and preparative HPLC chromatographic techniques. A new labdane diterpenoid glycoside named as nervilifordoside A was isolated from 60% EtOH extract of Nerviliafordii. The structure of compound 1 was elucidated as 12, 17-epoxy-3-hydroxy-17-methoxy-labdan-13-en-16, 15-olide 3-O-alpha-rhamnopyranosyl-(1 --> 2)-O-beta-glucopyranoside on the basis of HR-MS, 1D and 2D NMR spectroscopic data as well as chemical methods.
7.Comparative Analysis of Four Different Operation Methods for Treatment of Complex Upper Ureteral Calculi

Objective To compare the effects of ureteroscopic lithotripsy (URL), minimally invasive percutaneous nephrolithotomy ( MPCNL) , retroperitoneal laparoscopic ureterolithotomy ( RLU) and open ureterolithotomy (UL) for the treatment of complex upper ureteral calculi. Methods The data of 281 patients with complex upper ureteral calculi from January 2005 to January 2013 were retrospectively reviewed. 48 patients of them received treatment of URL, 113 patients received MPCNL, 67 patients received RLU and other 53 patients received UL. Results Success rates of treatment at the first time were:URL 62.5% (30/48), MPCNL 92.9%(105/113),RLU 100%(67/67) and UL 100%(53/53) . The mean blood losses during the operation were:URL (9.2 ± 1.4) mL,MPCNL (72.5 ± 5.8) mL,RLU (43.1 ± 8.5) mL and UL (100.5 ± 9.2) mL. The average operation time of URL group was shorter than three other groups, and the difference was statistically significant (P< 0.05) . The average hospital stay of URL group was also shorter than three other groups ( < 0.05) . The difference was not statistically significant in complications among four groups after operation ( >0.05) . Conclusion Clinical characteristics of patients and individual require ment should be considered comprehensively before an individual treatment choice is made for the treatment of complex upper ureteral calculi.
8.Effect of long-term glucocorticoid administration on nondepolarizing muscle relaxants-induced neuromuscular blockade in rat diaphragma

Objective To evaluate the effect of long-term glucocorticoid administration on nondepolarizing muscle relaxants-induced neuromuscular blockade in rat diaphragma in vitro.Methods Forty adult male SpragueDawley rats,aged 8 weeks,weighing 180-220 g,were randomized into 2 groups (n =20 each):control group (group C) and dexamethasone group (group D).In group D,dexamethasone 0.6 mg/kg was injected intraperitoneally once a day for 14 consecutive days,while the equal volume of normal saline was injected instead of dexamethasone in group C.The left phrenic nerve-hemidiaphragm was removed at 24 h after the last injection to evaluate the effect of d-tubocurarine.Different concentrations (0-10 μmol/L) of d-tubocurarine were added.The d-tubocurarine concentration-effect curve was drawn.The half inhibitory concentration and 95 % confidence interval of d-tubocurarine were calculated.Right diaphragma was removed for determination of the expression of embryonic nicotinic acetylcholine receptor (nAChRγ) mRNA and adult nicotinic acetylcholine receptor (nAChRε) mRNA by RTPCR.Results Compared with group C,the concentration-effect curve was shifted to the right,the half inhibitory concentration was significantly increased,and the expression of nAChRγ mRNA and nAChRε mRNA was up-regulated in group D (P < 0.05).Conclusion Long-term glucocorticoid administration can weaken nondepolarizing muscle relaxants-induced neuromuscular blockade in rat diaphragma in vitro,and up-regulation of nAChR mRNA expression may be involved in the mechanism.
9.Effect of Xinmailing Solution on neuronal cell injury induced by glutamate

AIM: To detect the effects of Xinmailing Solution and MK-801 on injury of neuronal cell induced by glutamate. METHODS: Cultured neuronal cell injuried by glutamate was prepared and the content of malondialdehyde and nitrite in cell supernatant was measured. Morphology changes were also observed with discrepancy microscope at the same time. RESULTS: Xinmailing Solution and MK-801 attenuated cell injury induced by glutamate,and inhibited increase in malondialdehyde and nitrite in cell supernatant. CONCLUSION: Xinmailing Solution had a protective effect on neuronal cell at cell level.
10.Study on the effects of acidosis on transient receptor potential channel M7 (TRPM7) in mouse cardiac fibroblast

Objective To investigate the effects of acidosis on the current change of transient receptor potential channel M7 (TRPM7) and collagen production in mouse cardiac fibroblast (MCFs), and to explore the pathophysiological function of TRPM7 on the cardiac fibrosis. Methods (1) The model of MCFs was established and isolated. (2) MCFs was subcuhured. (3) Patch clamp technique was used to observe the current characteristics of TRPM7 in low PH solutions. (4) The influence of acidic condition on Ca2+ influx in MCFs was recorded by calcium fluorescent indicators. Results (1) There was a high level expression of TRPM7 in MCFs and the electrophysiological characteristics of TRPM7-like (TRPM7L) was similar to that of TRPM7. (2) Ca2+ influx was increased in acidic condition, and the F340/F380 ratio was increased from 1.0 to 4. 6 at pH 4.0 compared with pH 7.0 (t=2.72, P<0.01). Conclusions (1) TRPM7 is the molecular basis of the native TRPM7L in MCFs and TRPM7L plays an important role in Ca2+ influx. (2) The pathophysiological function of MCFs is influenced by regulation of Ca2+ influx mediated by TRPM7L in the condition of acidosis.
