Objective To understand the characteristics and related factors of adverse reactions (ADR) of anti-tuberculosis drugs among Zhuang nationality patients with smear-positive pulmonary tuberculosis (TB) and to provide basis to ADR control. Methods During Dec 2007 to May 2008,408smear-positive pulmonary TB cases of Zhuang nationality in 14 counties were selected by simple random sampling, and followed up for 6-8 months.Questionnaires were performed and liver/renal functions were tested during the research.Data were analyzed by chi square test and Logistic regression model. Results The incidence of ADR of Zhuang nationality people was 17.2%,which was higher than national level ( 12.62 % ).The incidence of allergic reaction was highest of all ADR ( 5.9 % ) and the incidences among different types of ADR were significantly different ( x2 =36.9,P=0.000).Serious ADR types were allergic reaction, gastrointestinal reaction, liver damage, nerve damage and kidney damage. And 24.3 % of the cases had two or three kinds of ADR.ADR occurred at the median of 21.5th day of anti-TB treatment, and the gastrointestinal reaction occurred the earliest (6th day).81.4 % of the ADR occurred in the first 2 months of anti-TB treatment. The rates of drug withdrawal and interruption were 11.5 % and 9.2 %,respectively.Liver damage and nerve damage were the main causes of treatment withdrawal and interruption.Multiple factor non-conditional Logistic regression analysis showed that the ADR was correlated with age and the risk of ADR in ≥60years old patients was 3.3 times of that in 14-34 years old patients (OR=3.275,P=0.006).Thus,ADR incidence was increased with aging. Conclusions The incidence of ADR is higher in Zhuang nationality patients than that in general patient population.ADR monitoring and intervention must be strengthened in high-risk groups during high-risk period to reduce the adverse effect of anti-TBtreatment.

OBJECTIVE:To establish a method for simultaneous determination of sodium valproate(VPA)and its metabo-lite 2-propyl-2-pentenoic acid (2-ene-VPA) in human plasma. METHODS:Plasma sample was extracted with cyclohexane and experienced derivatization with 2,4′-dibromoacetophenone using n-octanoic acid as an internal standard. RP-HPLC method was adopted. The determination was performed on Zorbax SB-C18 column with mobile phase consisted of acetonitrile-water(65∶35,V/V)at the flow rate of 1 mL/min. The column temperature was set at 35 ℃,and UV dectection wavelenth was set at 258 nm. The sample size was 20 μL. RESULTS:The linear range of VPA and 2-ene-VPA were 5.0-200.0,0.5-20.0 μg/mL(r=0.999 9, n=5). The limits of quantification were 5.0,0.5 μg/mL. RSDs of inter-day and intra-day were all lower than 5%. Method recov-eries were 95.99%-98.80%and 97.40%-98.17%,and extraction recoveries were 80.46%-86.23%and 80.45%-85.61%. The plas-ma concentrations of VPA in 10 epileptic children were 27.4-93.2 μg/mL,and those of 2-ene-VPA were 0.85-3.94 μg/mL,respec-tively. CONCLUSIONS:The method is simple, specific and suitable for plasma concentration determination and pharmacokinet-ic study of VPA.

Objective To explore the antidepressant effect and mechanism of Chaihu Shugan San (CHSGS) composition, a compound traditional Chinese herb medicine and its components. Methods Rats were randomly divided into 6 groups: normal control group, model control group, a CHSGS group, a component I group, a component II group and a fluoxetine control group. The depression model was replicated by chronic unpredictable mild stress and single house for 28 d. Behavioral scores of the rats were detected by Open-field test and sucrose solution consumption test, and ERK1/2 mRNA expression in the hippocampus tissue was assayed by real-time fluorescent quantitative PCR. Results ERK1/2 mRNA expression was down-regulated in the depression model group compared with the normal control group (P<0.01). Compared with the model group, ERK1/2 mRNA expression in the CHSGS group and fluoxetine group was both up-regulated (P<0.05 or P<0.01); and only ERK1 mRNA expression in the component I group was up-regulated (P<0.05). No significant difference existed between the component II group and the model group (P>0.05). Conclusion Isolated-living condition and chronic mild unpredictable stress can down-regulate the expression of ERK1/2 mRNA in the hippocampus tissue. CHSGS may exert an antidepressant effect through increasing the expression of ERK1/2 mRNA in the hippocampus, component I may play an important role in its antidepression effect, while compatibility of the use of component II can enhance the antidepressant efficacy.

To explore the feasibility of chemical and biological method in evaluation of the in vitro dissolution rate of Liuwei Wuling tablet (LWT), this experiment investigated the inhibitory effect of LWT dissolving solutions on LX-2 hepatic stellate cells in 0.1% SDS dissolution medium in different dissolving periods. From these results, the cumulative dissolution rate of LWT was obtained based on the cell inhibitory rate. The dissolution rates of deoxyschizandrin, phillyrin, and Specnuezhenide were determined by HPLC method. A novel approach of self-defined weighting coefficient had been created to establish the integrated dissolution rate model. Then f2 similar factor method was used to evaluate the relevance of these two methods. The results showed that f2 values for deoxyschizandrin, phillyrin, Specnuezhenide, and the integrated dissolution were 61, 43, 61 and 75 respectively, indicating that the dissolution of multi-component integration could fully reflect the biological potency of the whole recipe. The dissolution evaluation method for multicomponent integration based on biological activity is expected to be one of the effective means for in vitro dissolution test of LWT.
