1.Alteration of serum lipids in patients with acute leukemia and its clinical significance.

To explore the clinical significance on alteration of serum lipids in acute leukemia (AL) patients, the level of serum lipids was monitored in 86 AL cases by using of automatic biochemical analyzer. The results showed that TG was significantly higher (P < 0.05) while TC, LDL-C and HDL-C were obviously lower (P < 0.05) in AL patients than those in normal controls. After chemotherapy, TG decreased but TC, LDL-C and HDL-C were still higher (P < 0.05) as comparing to pre-treatment in complete remission cases. There were no changes of those parameters in non-remission patients. It is concluded that determination of serum lipids level in AL patients is a simple and important accessory index to evaluate curative effect and monitor patient's condition.

2.Create mathematical model and analysis of correlation between Chinese medicinal characteristics and immunoregulatory activity based on literature informatics.

OBJECTIVEIntends to create mathematical model and analysis of correlation between Chinese medicinal characteristics and immunoregulatory activity based on literature informatics.
METHODThe numbers of the Chinese medicines with immune effects were worked out within the framework of "The China Pharmacopeia" of 2005 edition, from the literature publicized since 1980. The correlation and mathematical model were figured out between Chinese medicinal characteristics including biological classification, different tastes, channel tropism as well as the parts used and immunoregulatory activity based on the statistical software SPSS.
RESULTThe results showed that the immunoregulatory activity was related to the five tastes of Chinese medicines, and the pungent medicines had less immune effect. The Chinese medicines of underground parts had more immune effect compared with other parts of the medicine. Medicines acting upon heart and kidneys were more powerful as for the immune effects (P <0.05). The coincidence was 74.7% between mathematical computing and original classification.
CONCLUSIONThere are correlations,between Chinese medicinal characteristics and immunoregulatory activity. The mathematical model based on these results can be used for immunopharmacology.

3.Preliminary clinical report of treatment for neurogenic bladder by sacral neuromodulation using a new tined-lead electrode

ObjectiveTo explore the effects of sacral neuromodulation using a new tined-lead electrode on neurogenic bladder.MethodsThe use of a new tined-lead electrode for sacral neuromodulation was evaluated in a study including 5 consecutive patients with neurogenic bladder.The tined leads were implanted at the S3 foramen under the X-ray screening.Subjects completed the recording of detailed voiding diary pre-and post-operation including fluid intake,voided volume,leaked volume,catheterized volume,frequency,accompanying symptoms and sensation.Vesicourethral function was assessed by video-urodynamics.ResultsUrinary frequency and voided volume were improved 22% and 49% respectively in one patient with spinal bifida.Urinary frequency,voided volume and residual volume were improved 0.7%,11% and 46% respectively in another one.Urinary frequency,voided volume and residual volume were improved 0.4%,18% and 44% respectively in the third one.Frequency of leakage and leaked volume were improved 36% and 54% respectively in the patient with brain trauma.Frequency of CIC and catheterized volume were improved 42% and 54% respectively,and indexes of urodynamics were improved 37%~45% in the patient with spinal cord injury.ConclusionA new tined-lead electrode for sacral neuromodulation provide a new alterative and minimally invasive procedure to treat neurogenic bladder.
4.Impact of psychological behavior interventions on psychologic status and self-efficacy of perioperative people with breast cancer

Objective To discuss the impact of psychological behavior interventions on psychologic status and self-efficacy of perioporafive people with breast cancer. Methods 37 patients with breast cancer were randomly divided into conventional care group and psychological support group. General duty nursing and health education were adopted to conventional care group. Both psychological support interventions and self-regulation training were adopted to psychological support group. Both groups were evaluated by Hamilton Anxiety Scale (HAMA), Hamilton Depression Rating Scale(HAMD) and General Perceived Self-Efficacy Scale (GSES) after been hospitalized and operated. Results There was significant difference in the scores of conventional care group and psychological support group evaluated by HAMA and HAMD before and after intervention(P <0.01). The score of psychological support group evaluated by GSES is higher than conventional care group(P <0.05). Conclusions Psychological behavior interventions can ameliorate psychological anxiety and depression of porioporative people with breast cancer, and can improve patients' self-efficacy.
5.Effect of individualized nursing intervention on the quality of life of patients undergoing colostomy

Objective To discuss the effect of systematic individualized nursing intervention on the quality of life of patients after colostomy. Methods A total of 66 patients undergoing colostomy were divided into the intervention group (n = 34) and the control group (n = 32) randomly. Patients in the control group were given routine nursing care while patients in the intervention group performed systematic individualized nursing intervention based on routine nursing care. Questionnaires including European Organization for Research and Treatment of Cancer (EORTC) and Quality of life Questionnaire Core-30 (QLQ-C30) were used to collect the data at the first day of admission, at the first month of post-operation and at the sixth month of post-operation. The results of two groups were compared. Results The results of follow-up survey after half a year illustrated that every items of the quality of life of the intervention group patients were higher than that of the control group cases. There were statistical significant difference between them (P <0. 01). Conclusions Systematic individualized nursing intervention can enhance the quality of life of patients undergoing colostomy remarkably.
6.Investigation of operation personnel hand hygiene cognition and compliance in operation room

Objective To investigate the realistic status of hand hygiene cognition and execution rate among the operation-room personnel,in order to provide the corresponding integrated manage strategy for surgical site infection reduction.Methods Investigation of hand hygiene cognition and execution rate were performed among the 114 operation-room personnel with 46 doctors and 68 nurses.Results Hand hygiene cognition scores and execution rate of circuit nurses respectively were (78.43 ± 7.71) and 71.39% (4 183/5 839),meanwhile (62.44 ± 9.82) and 47.81% (2 395/5 009) of the surgeon and the anesthesiologist,which emerged significant diversity between these two group (P < 0.01).Conclusions Integrated hand management should be performed to insure the hand hygiene compliance,resulting in minimizing the surgical infection.
7.The role of comprehensive intervention measures in improving the patho-gen detection rate of hospitalized patients before antimicrobial therapy

Objective To understand the pathogen detection of hospitalized patients before antimicrobial therapy in a hospital through implementation of comprehensive intervention measures,and provide reference basis for the de-velopment of targeted measures.Methods Hospitalized patients who received therapeutic antimicrobial agents in this hospital were selected as the research subjects.Patients who were hospitalized from January to May 2022 were selected as the pre-intervention group,comprehensive intervention measures were taken from June to October 2022,and those who were hospitalized from November 2022 to March 2023 were selected as the post-intervention group.The pathogen detection rate before antimicrobial therapy,sterile specimen detection rate,antimicrobial use rate,de-tection rate of key multidrug-resistant organisms of patients before and after the intervention were analyzed.Results Compared to before intervention,the proportion of pathogen detection rate before antimicrobial therapy(62.09%vs 74.04%),detection rate of healthcare-associated infection diagnosis-related pathogens(62.82%vs 92.73%),and sterile specimen detection rate(35.17%vs 41.06%)of hospitalized patients after intervention all increased signifi-cantly,with statistically significant differences(all P<0.05).After intervention,pathogen detection rate before the combination use of key antimicrobial agents was not statistically different from before intervention(93.33%vs 90.48%,P>0.05),while antimicrobial use rate was lower than before intervention(39.93%vs 44.95%,P<0.05).There was no statistically significant difference in the detection rate of key multidrug-resistant organisms be-fore and after intervention(all P>0.05).Conclusion Adopting scientific and rational intervention measures can improve the pathogen detection rate,provide a reference basis for the rational use of antimicrobial agents.There was no significant improvement in the pathogen detection rate before the combination use of key antimicrobial agents and the detection rate of key multidrug-resistant organisms,indicating that relevant measures still need to be further optimized.

9.Clinical study of double units unrelated cord blood transplantation in adult patients with hematological malignancies.

OBJECTIVETo observe the engraftment, survival and graft-versus-host disease (GVHD) after 2 units unrelated cord blood (UCB) transplantation for treatment of adult hematological malignancies.
METHODSAmong twelve patients with hematological malignancies, ten were in stable stage and 2 in advanced stage. Conditioning regimen was Bu/Cy or Cy/TBI in 11 cases, and 1 received nonmyeloablative regimen. Antithymocyte globulin (ATG) was administered in all patients. GVHD prophylaxis consisted of cyclosporine A (CsA), mycophenolate mofetil (MMF) and short course methotrexate (MTX). Each patient received 2 units UCB of HLA mismatched at 0 -2 loci. Median total nucleated cells (TNC) infused was 5.55 x 10(7)/kg [range (2.99 -8.18) x 10(7)/kg].
RESULTSOne patient showed primary graft failure. The other 11 patients showed neutrophil engraftment at a mean time of (21.6 +/- 5.1) days and platelet engraftment at (34.9 +/- 9.5) days. One patient showed secondary graft failure and died of leukemia relapse at 3 months after transplantation. Ten patients (83.3%) gained sustained engraftment. In 9 patients the UBC unit with larger TNC dose predominated engraftment, and only 1 patient showed the unit with smaller TNC predominated (P = 0.011). Acute GVHD was observed in 6 patients, including grade I in 5 and grade II in 1. Limited chronic GVHD was observed in 2 of 10 patients survived more than 100 days. Of the total 12 patients, eight were still alive in event-free status and 3-year event-free survival(EFS) was (66.7 +/- 13.6)%. Of the 10 patients in stable stage at the time of transplantation, the probability of EFS was (70.0 +/- 14.5 )%.
CONCLUSIONSTwo UBC units transplantation with HLA mismatched at 0 - 2 loci is feasible as a treatment modality for adult hematological malignancies, and the unit with larger TNC dose would predominate the engraftment.

10.Correlation of chemokine CCL-2/MCP-1 level in the plasma with aGVHD and idiophathic pneumonia syndrome after allogeneic hematopoietic stem cell transplantation.

The aim of this study was to investigate the relationship between the plasma levels of chemokine CCL-2/MCP-1 and acute graft-versus-host disease (aGVHD) and/or idiopathic pneumonia syndrome (IPS) after allogeneic hematopoietic stem cell transplantation (allo-HSCT). ELISA assays were used to detect the plasma level of CCL-2/MCP-1 of 22 patients who received allo-HSCT, including 14 patients without or with grade I, 8 patients with grade II - IV aGVHD, respectively. 8 out of 22 patients were also diagnosed with IPS clinically. The dynamic changes of the plasma levels of CCL-2/MCP-1 chemokine and its correlation with aGVHD and/or IPS were analysized retrospectively. The results showed that the plasma levels of CCL-2/MCP-1 in the patients with moderate and serious aGVHD (grade II - IV) significantly increased, as compared with that prior to allo-HSCT (p < 0.05). The plasma levels of CCL-2/MCP-1 in the patients with aGVHD and/or IPS were higher significantly than those without any of these complications (p = 0.001). The retrospective analysis indicated that the plasma levels of CCL-2/MCP-1 in the patients with IPS significantly increased (p = 0.006). It is concluded that plasma level of CCL-2/MCP-1 correlates with aGVHD and/or IPS, and plays a role in the pathogenesis of these complications.
