Objective:To discuss the role of incision for short-term outcomes of simultaneous resection in synchronous colorec-tal liver metastases (sCRLM). Methods:We reviewed the data of 37 patients who underwent simultaneous resection between January 2009 and December 2014 in our department and compared the short-term outcomes between Mercedes and midline incisions. Results:Mercedes and midline incisions were used in 19 and 18 patients, respectively. The two groups showed similarities in patient characteris-tics, major hepatectomy, surgery time, blood loss, and hilar block time. The midline group comprised more rectal cancer patients (P<0.001). The two groups did not differ significantly in complication incidence (47.4%vs. 16.7%, P=0.08) and postoperative stay time (22.1 ± 9.5 d vs. 17.2 ± 6.7 d, P=0.08). At body mass index (BMI)<25, the complication incidence (P=0.046) and postoperative stay time (P=0.051) were lower in the midline group than in the Mercedes group. Conclusion:Midline incision provided similar exposure in simultaneous resection for sCRLM and was better than Mercedes incision in rectal cancer patients. Patients with midline incision may attain better short-term outcomes if BMI is<25.

Objective To investigate the clinical efficacy and toxicity in the use of simultaneous integrated boost intensity modulated radiation therapy (SIB-IMRT) for inoperable locally advanced nonsmall cell lung cancer (LA-NSCLC).Methods Between February 2012 and July 2015,58 pathologically diagnosed inoperable LA-NSCLC patients treated with SIB-IMRT were analyzed.A radiation dose of 50-64 Gy was administered in 1.8-2.2 Gy/fraction (26-30 fractions) to the planning target volume (PTV).Simultaneously,60-70 Gy was administered in 2.0-2.35 Gy/fraction (26-30 fractions) to the planning gross tumor volume (PGTV).Results The median follow-up time was 28.0 months (ranging from 6.0 to 40.0 months). The median overall survival (OS) and progress-free survival (PFS) were 25.0 (95% CI:23.8-26.2) and 15.0 (95% CI:11.3-18.7) months, respectively. The 1-,2-year OS were 91.4% and 51.7%,respectively.The 1-,2-year PFS were 56.9% and 22.7%,respectively.None of the patients developed grade 4 or 5 pneumonitis and esophagitis. In addition,in the subgroup analysis, the patients with N3 have a higher incident of ≥ grade 2 esophagitis compared with N0-N2,the incident are 29.2% and 20.6%,respectively (P < 0.05).Conclusions SIB-IMRT is feasible and well-tolerated for inoperable LA-NSCLC patients. It remains to be further evaluated in a large sample size prospective clinical trial.

Nasopharyngeal cancer (NPC) is an Epstein–Barr virus-associated malignant tumor. Radiation alone or concurrent chemo-radiotherapy is the principal treatment method and can generally achieve excellent efficacy for NPC patients. However, the prognosis of locoregionally advanced disease or distant metastasis remains poor. Currently, immunotherapy has become a new treatment of sol-id tumors, and it is mainly activating the antitumor immune system of the body. The review aims to explore the progress of immuno-therapy, including adoptive immunotherapy, tumor vaccines, and immune checkpoint inhibitors in NPC treatment.

This study aimed at optimizing the extraction process of Bi Xie Fen Qing (BXFQ) drink using ultrasound-microwave cooperation with central composite design-response surface method in comparison with the traditional decoction process.Taking microwave frequency, amount of water, extraction time as the main detection factors, diosgenin, glycyrrhizic acid and the extract yield were tested as the evaluation indexes;and taking an integrated score as response value for response surface and contour, predictive analysis was carried out and the optimum extraction conditions were achieved. It was found that the optimum extraction process of BXFQ drink was identified:the microwave frequency was 434 W,water addition was 18.4 times, extraction time was 9.3 mins and the ultrasonic frequency was fixed at 50 W.Under the optimum process, the diosgenin extraction yield rate was 23.17% (mg· g-1),extraction yield rate of glycyrrhizic acid was 0.64% (g·g-1),and the extraction yield rate of extractum was 34.12% (g·g-1).All the indexes were superior to those of the traditional method. It is concluded that the composite design-response surface method is suitable for the extraction optimization of BXFQ drink with favorable predictability of the mathematical model. The optimized ultrasoundmicrowave cooperation was easy to operate with high extraction efficiency. It is suitable for industrialized production with the provision of a scientific reference for the modern formulation development of BXFQ drink.

Objective To investigate the effects of extracorporeal membrane oxygenation (ECMO) on survival of adult from acute respiratory distress syndrome (ARDS). Method We searched Pubmed, Embase, Cochrane Library, Web of Science databases to find relevant literatues on ECMO in treatment of ARDS, which are reported from January 1966 to June 2010. Meta analyses was performed. Results Three papers about randomized controlled trial (RCT) of evaluating ECMO in patients with severe ARDS were enrolled for analyses. Meta-analysis of the three randomized controlled trials revealed ECMO did not decrease the mortality of ARDS patients. However, the cumulative meta-analysis of randomized trials showed ECMO had a protective effect on patients with ARDS. The most recent observational studies suggested that ECMO significantly decreased the mortality of ARDS caused by H1 N1 viral pneumonia. Conclusions There is no evidence to prove the benefit of ECMO in patients with ARDS. However, ECMO should be considered to use in early stage of ARDS as a last rescue resort for potentially reversible severe acute respiratory failure. Further investigation of large sample of high quality RCTs is needed.

Objective To study the effect of infrasound on the changes of expression of NMDAR1 in hipp- ocampal cells. Methods Eighty-eight male Sprague-Dawley rats were randomized into eleven groups:control group,90 dB/1 d,7 d,14 d,21 d and 28 d infrasound exposed groups;130 dB/1 d,7 d,14 d,21 d and 28 d infra- sound exposed groups. All the animals in the test groups were put in an infrasound field with 8 Hz,90 dB or 130 dB for 2 hours daily.Immunohistochemistry methods were used to detect the changes of intracellular expression of NMDARI in hippocampal cells. Methods The expression of NMDAR1 in hippocampus after the rats were exposed to infrasound of 8 Hz,90 dB SPL showed a procedure from reducing on the 1st day to rising on the 7th and peaked on the 14th day, then to descending on 21st day and returning to the standard level on the 28th day.Exposure to infra- sound of 8 Hz,130 dB SPL induced opposite effects on the expression of NMDAR1 compared with 90 dB SPL,which showed a process of increasing, descending, reaching to the lowest, then ascending and returning to the normal. The lowest expression of NMDAR1 occurred on the 14th day in every observed hippocampal area. Conclusion 8 Hz, 90 dB/130 dB infrasound induced certain reversible reaction in the expression of NMDAR1 of hippoeampal cells in rats, which may disturb their learning and memory function.
