3.Investigation and analysis of needs of guidance on job hunting for nursing students

Objective To investigate the needs of guidance on job hunting for nursing students,provide the basis for relieving nursing students' pressure of finding jobs and improving the employment rate of nursing students.Methods A self-rated questionnaire was designed and used to investigate 158 nursing students about their needs of guidance on employment.Results One hundred and thirty-six nursing students (86.1%) thought their own skills (including operating skills,English level,awards,and level of using computer) were the key factors in job hunting; one hundred and thirty-one (82.9%) nursing students thought temperament and good looking were important factors; one hundreds and twenty-seven (80.4%) nursing students thought communication ability was very important.eighty-six (54.4%) nursing students thought social relations and job hunting skills may play important roles; eighty-four (53.2%) nursing students thought graduation school had impact.When it came to selecting jobs,one hundreds and thirty-two (83.5%) nursing students thought prospect of the job was the key factor; one hundreds and thirty-one (82.9%) nursing students thought it was salaries; one hundred and twenty (75.9%) nursing students thought it was working strength; one hundred and seven (67.7%) nursing students thought it was opportunities of continuing education; ninety-nine (62.6%)thought it was working atmosphere; eighty-three nursing students (52.5 %) thought it was stability of job.When it was concerning priority of different work places,one hundred and fifty (94.9%) nursing students chose third level of first-class hospital; one hundred and twenty-five (79.1%) chose second level of first-class hospital;eighty-one (51.2%) nursing students chose community medical institutions; eighty (50.6%) nursing students chose private hospital; sixty-nine (43.6%) nursing students chose private clinic.Conclusions The nursing students could focus on the improvements of their comprehensive quality,and have high expectation on their jobs,flexible thoughts and greater mobility.
4.Identification of a novel frameshift mutation of human androgen receptor gene in a patient featuring complete androgen insensitivity syndrome.

OBJECTIVETo identify potential mutation of human androgen receptor (AR) gene in a patient with complete androgen insensitivity syndrome (CAIS).
METHODSDNA sequences of 8 exons and exon/intron boundaries of the AR gene were amplified with PCR and directly sequenced.
RESULTSDNA sequencing has revealed a frameshift mutation due to deletion of nucleotide C at position 3507 in exon 6, which gave rise to a stop codon resulting premature termination for translation.
CONCLUSIONA novel frameshift mutation in exon 6 of AR gene probably underlies the disease in our patient.

5.Preliminary analysis on human telomerase reverse transcriptase gene expression in acute leukemia.

OBJECTIVETo explore the relationship between gene expression of human telomerase reverse transcriptase (hTERT) and its clinical characteristics in leukemia.
METHODSThe protocol of RT-PCR was used to detect the hTERTmRNA expressing levels in peripheral blood samples from leukemic patients under primary treatment(n=42), in complete remission(n=21), with recurrent leukemia (n=4); and from normal subjects (n=5), respectively.
RESULTSThe positive percentage of hTERTmRNA expression was 73.81% for the primary treatment cases, and 19.05% for the complete remission cases. All of the recurrent cases gave positive results. One of the normal controls presented low level of hTERTmRNA expression. The expressing level of hTERTmRNA in primary treatment cases was 0.64+/-0.21, in complete remission leukemia 0.31+/-0.16, in recurrent cases 0.84+/-0.09, and in normal controls 0.10.
CONCLUSIONThe activation of telomerase may be an essential factor in the development of leukemia and usually be the late event in its progression. As an indicator of leukemia cell, the detection of hTERT mRNA may be used in clinical analysis, disease monitoring and prognosis judgement.

6.Spectrum of phenylalanine hydroxylase gene mutations and genotype-phenotype correlation in the patients with phenylketonuria in Beijing area of China.

OBJECTIVETo identify the mutation spectrum and the distribution of minihaplotypes (STR/VNTR) of phenylalanine hydroxylase (PAH) gene and explore the correlations between genotype and phenotype of patients with phenylketonuria (PKU) in Beijing area of China.
METHOD(1) Fifty cases with PKU were involved in this study. PKU was identified by the Neonatal Screening Center of Beijing. All 13 exons and their flanking intronic sequences of PAH gene of these patients were amplified and then subjected to SSCP analysis and direct sequencing. (2) The distribution of polymorphic locus of short tandem repeat (STR) and variable number tandem repeat (VNTR) was analyzed by PCR and denaturing gel electrophoresis. (3) The correlations between genotype and phenotype were studied by analysis of the matching rate between the expected and observed phenotypes. The predicted phenotype was determined on the basis of the sum of the assigned values of the two mutant alleles.
RESULTS(1) A total of 34 different mutations were detected with the relative frequency of 95% among 50 PKU patients. The prevalent mutations in this study were: R243Q (20%), EX6-96A > G (11%), Y356X (9%), and V399V (7%). The next common mutations were R111X (5%), R413P (5%), R252Q (3%) and A434D (3%). Thirty-four detected mutations were distributed throughout the whole PAH gene, except exon 1, 8 and 13. Exon 7 and 11, with the mutant rate 34% and 19% respectively, seemed to be the hot mutant areas/regions of PAH gene. (2) The minihaplotypes (STR/VNTR) of 34 mutations were identified in this research. The STR and VNTR showed 8 and 3 alleles, respectively. Among them, 244 bp (44%) and 240 bp (34%) were the prevalent STR alleles. Meanwhile, the VNTR3 (83%) was the most common VNTR allele in PKU patients. (3) A better consistency (81.5%) between expected and observed phenotypes was revealed by analysis of correlation between genotype and phenotype. Especially in classic PKU, the consistency rate was up to 87.5%.
CONCLUSION(1) The frequency distribution of common PAH gene mutations in Beijing region was close to that of Tianjin and Yunnan regions, while it was different from that of Southern regions of China, such as Guangzhou, especially Taiwan. The PAH mutation with a highly heterogeneous trait was also demonstrated in this study. (2) STR and VNTR minihaplotype will prove helpful to trace the origins of PAH mutations and to analyze the genetic drift. However, the most minihaplotypes of the STR/VNTR are similar, so it is necessary to associate some other polymorphic loci with the STR/VNTR minihaplotype to analyze the different mutations. (3) The fact that a better consistency existed between phenotypes and genotype with most PKU patients suggested that the study of the genotype of PKU patients would be helpful to the individualized treatment and to genetic counseling for their families.

7.The use of antiparkinsonism drugs in 92 cases in Xi' an city

Objective To investigate the treatment status of antiparkinsonism in Xi'an. Methods Six general hospitals were randomly chosen in Xi' an and all Parkinson' s disease (PD) patients were interviewed by questionnaire from Jan. 2007 to Apr. 2007. Results 92 PD outpatients were enrolled in, including 48 males and 44 females, from 43 to 86 years old (mean 65.6±17.1) with duration of the disease from 0.2 to 27.8 years (mean 4.4 ±9.4). The preference of the drug use from the patients were: 40 (43.5%) preferred taking levodopa, 25 (27.2%) with amantadine and/or trihexyphenidyl, 14(15.2%) with levodopa and others, 4(4.4%) with dopamine agonist and others, 2 (2.2%) with other drugs, 7 (7.6%) with no treatment. There were 69 (75.0%) patients onset with resting tremor, 15 (16.3%) with bradykinesia, 6 (6.5%) with rigidity, and 2 (2.2%) with unknown symptoms. There was no startically significant difference in anti-PD drugs among the patients onset with different symptoms (P>0.05). 45 patients appeared the onset of disease before 65 years old and with no dementia, 47 onset after 65 with or without dementia. There was no significant difference of anti-PD drugs between the two groups (P>0.05). Most patients initiated anti-PD treatment with levodopa but few of them chose dopamine agonist. According to the classification of Hoehn & Yahr, 25(27.2%) belonged to Grade Ⅰ, 53(57.6%) to Grade Ⅱ ,8(8.7%) to Grade Ⅲ ,3(3.3%) to Grade Ⅳ and 3 (3.3%) to Grade Ⅴ. There was no significant differences of anti-PD drugs between different grades of the disease (P>0.05). 55.3% of the patients changed their anti-PD drugs randomly during the therapy, but with no relation to their gender, age, educational level, dementia, the number of family members, course of diseases, or the degree of Hoehn & Yahr, frequency and categories of medicine. Conclusion Anti-PD treatment in Xi' an did not strictly follow the standardized protocol, with few patients using dopamine agonist and over 50% of the patients changed their drugs randomly.
8.Effect of multi-disciplinary team combined with bundle management on prevention of multidrug-resistant organism infection in the intensive care unit

Objective To explore the effect of multi-disciplinary team (MDT) combined with bundle management on prevention and control of multidrug-resistant organism (MDRO) infection in the intensive care unit(ICU).Methods Patients who were admitted to the ICU in a tertiary first-class hospital from January 2013 to December 2015 were studied,MDT combined with bundle management has been applied in the prevention and control of MDRO infection in ICU since January 2014,continuous quality improvement program was performed one year later,isolation of MDROs from specimens of ICU patients before implementation(in the year of 2013),after implementation(in the year of 2014),and after continuous quality improvement(in the year of 2015) was compared.Results The infection rates of MDROs in ICU patients before implementation,after implementation,and after continuous quality improvement were 26.55% (154/580),17.13% (117/683),and 12.01% (77/641) respectively,showing a downward trend,with a significant difference (x2 =44.030,P<0.001);the total isolation rates of MDROs in ICU patients were 64.44%(154/239),63.59%(117/184),and 43.26% (77/178) respectively,showing a downward trend,with a significant difference (x2 =22.284,P<0.001).The main MDROs in ICU were multidrug-resistant (MDR) and pandrug resistant(PDR) Acinetobacterbaumannii (44.54%).Conclusion MDT combined with bundle management can decrease MDRO infection rate and isolation rate in ICU.
9.Clinical observation of propofol combined with flurbiprofen axetil for induced abortion anesthesia.

OBJECTIVE:
To observe the effect of propofol combined with flurbiprofen axetil for abortion anesthesia.
METHODS:
Eighty ASA I - II induced abortion patients were randomly divided into 4 groups. Group I was administrated 1 microg/kg with tramadol first, after 10 minutes with 2 mg/kg propofol. Group II was administrated with 1 mg/kg tramadol first, after 10 minutes with 2 mg/kg propofol. Group III was administrated flurbiprofen axetil 50 mg first, after 10 minutes 2 mg/kg propofol. Group IV was administrated propofol 2 mg/kg. The speed of intravenous injection of propofol was 100 mg/min in all groups. Induction time, recovery time, propofol dosage, HR, BP, SpO2, and side effect were recorded. The anesthesia effect was judged by operation doctors.
RESULTS:
Propofol consumption and awaken time in Group IV was more than those of other groups. The number of patients in Group I with minimum value of SpO2 in the operation (85% - 90% or SpO2<85%) was higher than that of other groups (P<0.05). The number of patients in Group I and II with the tongue falling backwards during anesthesia, with post-anesthesia nausea and vomit was higher than that of other groups (P<0.05). Incidences of post-anesthesia excitement or delirium were higher than other groups. Fewer patients in Group IV showed a good level anesthesia effect than other groups. The post-operative hypogastric pain of VAS score (0 - 2) in Group III was better than others.
CONCLUSION
Propofol combined with flurbiprofen axetil gives more efficient anesthesia for induced abortion patients in gynecology department. It can not only have satisfactory anesthesia effect, but also decrease adverse effects and obtain better depression effects to the post-operative hypogastric pain.

10.Application of pipe file registration card in urology pipe management

Objective To explore application effect of pipe file registration card in urology pipe management.Methods 120 patients who received treatment in July 2010 were divided into experimental group and control group randomly and each group had 60 patients.General guidance were trained in two groups.Pipe file registration cards were used in experimental group.Surveys and analysis were conducted on fluid flowing,safety,extubation,compliance behavior of patients,omissions replace number,satisfaction investigation results when they left the hospital.Results Numbers of patients with drainage,sense of security,smooth pipe,compliance behavior were 55,54,40 and 55 in experimental group,whereas 25,24,10 and 25 in control group,the difference was statistical significant ( x2 =6.91,7.16,13.11,6.91,20.02,respectively; P < 0.01 ).Conclusions The establishment of pipe file registration card can standardize nursing operation,form orderly work process,improve nursing quality and treatment effect,and improve patient satisfaction.
