1.Analysis of different methods of internal fixation of ankle fractures

Objective To investigate the treatment effects of different fixation for fracture and dislocation of ankle joint and to sum up the experience for clinical treatment.Methods One hundred and fifty patients with fracture and dislocation of ankle joint using 5 different internal fixation surgeries were involved with this research:simple cross Kirschner wire fixation,intramedullary fixation ordinary screws,cancellous lag screw fixation,4 holes common plate fixation,tension band fixation.Patients were followed up and evaluated by the American Orthopedic Foot and Ankle Society Score for scoring,and investigated the postoperative recovery and the occurrence of postoperative infections and complications in patients.Results After followed up from 12 to 24 months with an average follow-up time of 19 months,of 150 patients,there were 29 cases with excellent result,65 cases with good result,44 cases with general and 12 cases with poor result,the excellent and good rate was 62.6% (94/150).One case with nonunion,6 cases with malunion,7 cases with traumatic arthritis,10 cases with wound pain,the incidence of postoperative complications was 16.0% (24/150).There were 10 cases with shallow infection,5 cases with deep infection,and 11 cases with severe skin irritation; The incidence of postoperative infection was 17.3% (26/150).Conclusion Surgical treatment is the main treatment of fracture and dislocation of ankle joint,5 different methods of internal fixation of ankle fractures had good therapeutic effect.

Object To study the chemical constituents of the dried root of Fordia cauliflora Hemsl which is indigenous to Guangxi and commonly used in traditional medicine of Zhuang and Yao nationalities Methods The constituents were isolated with column chromatography and their structures were elucidated on the basis of physicochemical data and spectral analysis Results The three constituents were isolated and identified as 2 [3 methoxyphenyl] 4H furo[2,3 h] 1 benzoyran 4 one (Ⅲ), 3 methoxy 2 [3 hydroxyphenyl] 4H furo [2, 3 h] 1 benzopyran 4 one (Ⅳ) and pinnatin (Ⅴ) Conclusion The all of them are isolated from this plant for the first time Compound Ⅲ,Ⅳ are new and tentatively named as cauliflorin A and cauliflorin B
3.Clinical efficacy of Qizhengxiaotong plaster plus Xianling Gubao for osteoarthritis of knee

ObjectiveTo compare and observe the clinical effect of Qizhengxiaotong plaster and Votalin emulsion in curing osteoarthritis of knee (KOA) for outpatient treatment.MethodsIn 70 cases,the KOA patients are randomly divided into treatment group and control group,respectively given Qizhengxiaotong plaster plus Xianling Gubao and Votalin emulsion for one month.To observe the clinical effects of patients respectively pre and post treatment.ResultsAfter a course of treatment,Qizhengxiaotong plaster plus Xianling Gubao could improve the clinical symptoms and signs significantly.Its improvement for pain was the same as Votalin.But Qizhengxiaotong plaster plus Xianling Gubao was significantly better than Votalin in eliminating the swollen,tenderness and dysfunction.And there was no obvious difference between Qizhengxiaotong plaster plus Xianling Gubao and Votalin emulsion in total effective rate,evaluated by WOMAC Osteoarthritis Index Guide and VRS-5.The typical symptoms were improved after the therapeutic method.No adverse reaction was found during the treatment.ConclusionQizhengxiaotong plaster plus Xianling Gubao had obvious effect on the osteoarthritis of knee and could improve the patients' pain and limitation of activity.Hence,it was worthy to be generalized in the conservative treatment of the osteoarthritis of knee for out-patients.

5.Investigation of Cif Gene Expression between Mucoid and Non-mucoid Pseudonmonas Aeruginosa

Objective To compare the difference of the Cif gene expression and the inflammation markers (such as WBC and CRP et al)between mucoid and non-mucoid Pseudonmonasaeruginosa,combining with the clinical data.Methods Real-time RT-PCR was used to determine the expression level of Cif in Pseudonmonas aeruginosa and clinical data were collected to discuss the relationship among the number of WBC,the level of CRP and the Cif gene expression.Results The expression level of Cif gene in mucoid Pseudonmonas aeruginosa had a significantly higher than the non-mucoid Pseudonmonas aerugi-nosa,which was statistically significant (t=2.09,P<0.05).Specimen data collected from patients with the infection of mu-coid or non-mucoid Pseudonmonas aeruginosa showed that the number of WBC and the level of CRP were not significantly different (t=0.65,0.60,P>0.05).The data also showed that the cystic fibrosis transmembrane conductance regulator in-hibitory factor (Cif)gene expression level had little correlation with the number of WBC (a=0.095,R2=0.029).Conclusion The expression of Cif gene may works for the biofilm formation of mucoid Pseudonmonas aeruginosa,which maybe a new virulence factor to Pseudonmonas aeruginosa.
6.Studies on the anti-inflammatory molecular mechanism of chlorogenic acid extracted from Lonicera confusa DC in vitro

Aim To investigate the anti-inflammatory molecular mechanism of chlorogenic acid extracted from Lonicera confusa DC in vitro.Methods PM? of a rat was segregated.MTT assay was used to detect the effects of the chlorogenic acid on PM? cells growth activities. PM? was stimulated with LPS for a prolonged period,ELISA was used to detect the level of TNF-?,IL-6 and PGE2 in the supernatant;COX-2 activity was determined by the level of PGE2 in the supernatant.After stimulating PM? with A23187 for a short time,the 6-keto-PGF1? level in the supernatant was measured by radioimmunoassay to express COX-1 activity.Results Chlorogenic acid had no inhibitive effects between 31.25 mg?L-1 and 1000 mg?L-1.The level of TNF-?,IL-6 and PGE2 in drug groups was lower than that of LPS-induced group,and the difference was significant,in a dose-dependent manner.The concentration of 50 mg?L-1 group was ineffective in the expression of TNF-?.Low concentration chlorogenic acid inhibited the expression of 6-keto-PGF1?,while high-concentration induced it.Conclusions The anti-inflammatory effect of chlorogenic acid may be related to inhibiting TNF-?,IL-6 activity and affecting exogenous AA metabolism.
7.Application of cytometric bead array in rapid high-throughput detection of small molecules.

Cytometric bead array (CBA) is a new analytical technique, which can achieve real-time and rapid detection of targeted components in a small amount of sample. With many advantages of high throughput screening, high specificity and sensitivity, low cost, easy operation and good repeatability, this CBA technique has been widely used for the detection of various components in foods, agricultural products and environmental samples. Recently, it has got significant development in rapid detection of small molecules. This review briefly introduced the theory of CBA technique, summarized the application in the analysis of small molecules, such as mycotoxins, pesticide residues, shellfish toxins, and then prospected the application of trace small molecules detection in the complex matrices of traditional Chinese medicine and the development trend of it.

8.Mini-open anterior corrective surgery with assistance of thoracoscopy for thoracic idiopathic scoliosis

Objective To introduce the mini-open anterior correction under video assisted thoracosco- py for thoracic idiopathic scoliosis and report its clinical results.Methods From July 2001 to 2006,37 ca- ses of right thoracic idiopathic seoliosis were surgically corrected with mini-open anterior instrumentation. There were 4 males and 33 females with average age of 14.1 years and average Cobb angle of 56?.Fourteen cases with Lenke I A 14 cases,IB ++-++++ 14 cases,IC 9 cases.The Risser sign was ++-++++. The operative time,blood loss,instrumented levels,correction rate and loss of correction were analyzed. Results The operative time averaged 220 miu.The intraoperative blood loss averaged 320 ml.The average number of instrumented levels was 7.8.The postoperative Cobb angle was 16.8?on average with correction rate of 70%.With a follow-up of 18-36 months,the loss of correction averaged 4.6%,but no hardware complications.Conclusion The clinical results of mini-open anterior correction under thoracoscopy for tho- racic idiopathic scoliosis were satisfactory.It may minimize the complications of classical thoracotomic anterior correcion and mav reduce the high requirement for anesthesia,long operative time and high cost of thoraco- scopic anterior instrumentation.
9.The influence of the Risser sign on the post-operative sagittal profile of idiopathic thoracic scoliosis patients treated with the anterior instrumentation

Objective To analyze the influence of the Risser sign on the sagittal compensatory mode of the spine in idiopathic thoracic scoliosis after anterior spinal fusion.Methods Between June 2002 and November 2006,43 adolescent idiopathic scoliosis(AIS)patients(Lenke 1)undergone anterior correction with a minimum 2 years follow-up were included in this study.The patients were divided into three groups according to the Risser sign:group A(Risser 0),group B(Risser 1-3),and group C(Risser 4-5).The preoperative,postoperative and follow-up sagittal profile were evaluated by the following radiological parameters measured on the lateral radiograph,including the thoracic kyphosis,the lumbar lordosis,the thoracolumbar junction kyphosis,the distal junctional kyphosis,and the sagittal vertical axis.Results In group A,the thoracolumbar junction kyphosis significantly changed from-1.7° preoperative to 6.6° at the final follow-up,with an average increase of 8.3°.Similarly,in group B,the thoracolumbar junction kyphosis changed from -7.3° before surgery to 0.6° at the final follow-up,with an average variation of 7.9°.No obvious change of the thoracolumbar junction kyphosis was observed in group C.At the final follow-up,the average thoracic kyphosis in three groups was 21.2°,18.4° and 14.7°,respectively.No significance of the variation of the thoracic kyphosis was observed in the three groups,however,in group A and B,the thoracic kyphosis showed an ascending trend during the follow-up without significant statistical difference,in addition; the ratio of the thoracic kyphosis increased in group A was higher compared with group B and C.Conclusion For AIS patients with low Risser sign,the increased thoracic kyphosis,and the thoracolumbar junction kyphosis may be ascribed to the decompensation of thoracolumbar region caused by the reconstruction of sagittal alignment due to the continued growth of posterior elements of the thoracic spine.
10.Perioperative evaluation and treatment strategy for severe scoliosis with respiratory failure

Objective To investigate the preoperative evaluation of pulmonary function and protocol for scoliotic patients with respiratory failure. Methods From September 2000 to June 2008, sixteen patients suffered from respiratory failure were recruited into this study. There were two males and three females who were diagnosed as idiopathic scoliosis with a mean age of 13.5 years (range, 10-16 years). The other eleven patients were diagnosed as congenital scoliosis, including five males and six females with a mean age of 12.4 years (range, 10-14 years). The mean preoperative Cobb angle was 126.6°±15.5° with a mean height of 137.6 cm (range, 120-160 cm) and a mean weight of 32.5 kg (range, 18-40 kg). Decision was made depending on the severity of pulmonary impairment and spinal deformity. Preoperative treatment included respiratory training, noninvasive positive pressure ventilation therapy and halo-gravity traction. Results All patients received corrective surgery; ten patients were extubated tracheal tubes successfully 1 hour after surgery and six patients had to stay in intensive care unit (ICU) for 24 hours. Pulmonaryedema occurred in two cases and pneumonia in one case. The Cobb angle had improved to 72.0°±13.2° after surgery with a correction rate of 43%. All patients recovered well after surgery with no major cardiac or pulmonary complications.Conclusion Patients with preexisting respiratory failure can tolerate deformity correction operation with few complications after receiving a set of preoperative respiratory training modalities.
