1.Expression of drculatory and pulmonary angiotensinⅡin rats with acute lung injury

Objective To investigate the changes in circulatory and pulmonary monary angiotensinⅡin rats with acite lung injury(ALI)and explore the role of angiotensinⅡin ALI.Method Thirty S-D rats were randomly divided into control group(n=6)and ALI group(n=24).The ALI group was further divided into four subgroups of observation at various intervals,3,6,9 and 12 hours after administration of lipopolysaccharide (LPS)into the femoral vein(each n=6).The indices rate,blood gas analysis,wet weight/dry weight(W/D)ratio of lung lobes,and pathological changes were successively observed at 3,6,9,and 12 hours after injury.The content of angiotensinⅡin lung tissue and blood plasma were detected at above set intervals by radioimmunoassay.Data of these assays were analyzed by one-factor analysis of variance.Results Compare with the control group,pH and PaO2 of arterial blood in ALI group decreased significantly(P<0.05)at all intervals and PaCO2 of arterial blood in ALI group decreased significanfly(P<0.05).at all intervals and PaCO2 of arterial blood and lung W/D weight ratio increased significantly(P<0.05),and scores of lung histopathology denoted the lung injuried(P<0.01).After injury of lung,angiotensin Ⅱ content increased markedly in lung homogenate and blood plasma (P<0.05).Angiotensin Ⅱ content in blood plasma reached peak value at 9 hours,and content of angiotensin Ⅱ in lung homogenates kept on increasing at allintervals of observation.Conclusions A large amount of angiotensin Ⅱ releases into lung tissue and blood plasma during ALI,suggesting systemic and pulmonary rennin-angiotensin systems are activated.
2.Study on nursing practice environment among 22 public hospitals in Shanghai area

Objective To investigate the present situation and to analyze related factors of nursing practice environment among 22 public hospitals in Shanghai area.Methods We investigated 1 600 nurses from the 22 hospitals with general information questionnaire and the Chinese version of Practice Environment Scale,and also analyzed related factors by single and multiple factor analysis.Results 1 582questionnaires were taken back.49% were dissatisfied with their working environment,and 28.1% were evaluated to have nice practice conditions.Nurse concerned with workforce management and adequate resources were widespread.Logistic analysis indicated that hospital's level,the nurse's age,educational background and seniority were main factors influencing nurses' satisfaction.Nurse work in tertiary hospitals reported lower satisfaction.Conclusions More attention should be paid on improving nursing practice environment.
3.Feasibility of biomedical fibrin glue in laparoscopic repair of peptic ulcer perforation

Objective To investigate the feasibility of biomedical fibrin glue in laparoscopic treatment of peptic ulcer perforation.Methods A total of 126 patients with peptic ulcer perforation were randomly divided into two groups (63 in each group).Biomedical fibrin glue was used in treatment group,and routine treatment was used in control group.The total volume of drainage after operation,rate of intestnial fistula and adhesions,allergy reaction,time of drainage tube removal,and average hospotal stay time were observed in both groups.Results There was no allergic reaction in treatment group.Total volume of drainage in treatment group was(65.3?7.5)mL,and (110.2?9.6)mL in control group,with a significant difference between the two groups(P
4.Baizhu suppresses S180 sarcoma growth and inhibits Bcl-2 expression in tumor-transfected mice

Objective To study Baizhu's inhibitory effect on S180 sarcoma growth and its effect on Bcl-2 expression in tumor-transfected mice. Methods S180 cells were subcutaneously injected into 60 healthy Kunming mice. Meanwhile the mice were dealt with MTX or Baizhu, separately; the weight of tumor was measured in the following two weeks; the expression of Bcl-2 was detected by RT-PCR. Results The tumor's weight in Baizhu group was lower than that in model group, but higher than in MTX group (P<0.05), but Baizhu's inhibition was not associated with its dose (P>0.05), and Baizhu's rate of tumor growth inhibition was lower than that of MTX. Compared with that in control group, Bcl-2 expression was lower in Baizhu group obviously. Conclusion Baizhu can inhibit tumor growth and serve as an adjuvant drug in tumor therapy.

6.Analysis on the Utilization of Traditional Chinese Medicine Injection in Our Hospital from 2006 to 2008

OBJECTIVE: To analyze the utilization of traditional Chinese medicine (TCM) injections in our hospital in order to provide reference for clinical rational use of TCM injection. METHODS: The utilization of TCM injection in our hospital from 2006 to 2008 was statistically analyzed in terms of consumption sum, DDDs and its order. RESULTS: The consumption sum and DDDs of TCM injection in our hospital increased year by year, especially stasis-removing agents and antitumor drug. Over the 3 years the proportion of consumption sum of TCM injections in the total consumption sum were 57.3%, 59.4% and 55.9%, respectively. There was poor synchrony between consumption sum order and DDDs order. CONCLUSION: TCM injections are used rationally in our hospital. In addition, safety reevaluation of TCM injection should be improved.
7.The evaluation value of severity and prognosis of septic shock patients based on the arterial-to-venous carbon dioxide difference

Objective To assess the value of central venous-to-arterial carbon dioxide difference [ P( cv-a) CO2 ] in evaluation of disease severity and prognosis in patients with septic shock.Methods There were 45 consecutive resuscitated septic shock patients from April 2009 to October 2010 included immediately after their admission into our ICU.The patients were divided into low P(cv-a) CO2 group and high P(cv-a) CO2 group according to a threshold of 6 mm Hg ( 1 mm Hg=0.133 kPa).All patients were treated by early goal directed therapy (EGDT).The parameters of hemodynamics,lactate clearance rate,the Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ ) score,the sequential organ failure assessment (SOFA)score,6 h rate of EGDT achievement,the ICU mortality and 28 days in-hospital mortality were recorded for all patients.Results There were 30 patients in the low P(cv-a) CO2 group,and 15 in the high P(cv-a)CO2group.There were no significant differences between low P(cv-a) CO2 and high P(cv-a) CO2 patients in age,APACHE Ⅱ score and SOFA score (all P > 0.05 ).Compared with the high P(cv-a)CO2 group,the low P(cv-a) CO2 group had higher cardiac index ( CI ) and 24 h CI,higher delivery O2 ( DO2 ) and 24 h DO2,higher central venous oxygen saturation ( ScvO2 ) [ (74 ± 9) % vs (67 ± 8) % ],lower lactate [ ( 3.4 ± 2.1 )mmol/L vs (5.7 ± 4.5 ) mmol/L] and higher △SOFA score [ (0.7 ± 1.8 ) vs ( - 0.4 ± 1.1 ) ],lower 24 h SOFA score [ (7.8 ± 2.0) vs (9.8 ± 2.0 ) ],higher 6 h rate of EGDT achievement ( 83.3% vs 53.3 % )(P < 0.05 ),however,there were no differences in 28 days mortality and ICU mortality between the two groups ( P > 0.05 ).Conclusion P(cv-a) CO2 might be an indicator for predicting the severity of patients with septic shock and evaluating tissue perfusion.

9.Study on main pharmacodynamic effects for Schisandra lignans based upon network pharmacology.

In this study, we focused on the study of pharmacodynamic effects for 6 major bioactive lignans of Schisandra chinensis, namely deoxyschizandrin, schisandrin B, schisandrin C, schisandrin, schizandrol B and schisantherin A. A compound-gene-pathway network, which contained 124 related genes and 88 pathways, was constructed by collecting drug genes through mining relevant literatures and network pharmacology analysis. Based on the network analysis, 32 pathways and 80 related genes were associated with inflammation, which implied that anti-inflammatory might be the major pharmacodynamic effects of these compounds. All lignans except schizandrol B reduced LPS-induced NO production in RAW264.7 cells, which validated the anti-inflammatory hypothesis generated from network analysis. Furthermore, we investigated the effects of deoxyschizandrin, schisandrin C, schisandrin and schisantherin A on the secretion of inflammatory cytokines TNF-α, IL-1β, IL-6, PGE2 and protein expressions of iNOS, COX-2. As a result, deoxyschizandrin showed the strongest anti-inflammatory activity with inhibitory effect on all 4 inflammatory cytokines secretions and protein expressions of iNOS, COX-2. This study provided evidences for systematic exploration on the pharmacolgical actions and mechanisms of schisandra.

10.Drug Resistance Mechanism of Patients Infected with Aminoglycoside-resistant Acinetobacter Baumannii in Emergency Intensive Care Unit

Objective To investigate drug resistance mechanism of aminoglycoside-resistant Acinetobacter baumannii by detecting 16S rRNA methylase gene and three common genes of aminoglycoside-modifying enzymes in Acinetobacter baumannii infected patients at EICU. Methods The 48 Acinetobacter baumannii strains were collected,and antimicrobial susceptibility tests were performed by VITEK automicroscan. The MIC was detected by 2-fold agar dilution method,and genes were analyzed by polymerase chain reaction( PCR) . Results Among 48 strains,28 were highly resistant to aminoglycosides and 20 showed lower resistances. The 16S rRNA armA,APH(3')-I,ANT(3'')-Ia,AAC(6')-Ib genes were detected in 71. 43%,60. 71%,82. 14%, and 53. 57%of the 28 highly resistant strains,but only present in 0. 00%,0. 05%,0. 05%,and 0. 05%of the low-resistant isolates(P<0. 01). Conclusion The aminoglycoside-modifying enzymes and 16S rRNA methylase were frequently found in Acinetobacter baumannii clinical isolates,which is closely related to the high-level resistance to aminoglycoside antibiotics.
