Objective To explore the role of TRAIL receptors expression on peripheral blood mononuclear cell (PBMC) in the apoptosis of PBMC,and the relation of the expression of TRAIL receptors with the severity of liver damage in chronic hepatitis B patients.Methods The expressions of DR4,DRS,DcR1 and DcR2 in 55 patients and 30 cases of control were assayed by RT-RCR and floweytometery.The relationship of the expression of TRAIL receptors with the severity of liver damage were analized.Results The level of DcR1 in the PBMC of chronic hepatitis B patients was much lower than that of control group (P<0.05).There was close relation between the expression of DcR1 and liver damage (P<0.05).The level of DcR1 was negatively correlated with ALT but positively correlated with serum albumin.Conclusion The expression level of DcR1 on PBMC in chronic hepatitis B patients was down-regulated,which may contribute to the increased apoptosis of PBMC in chronic B hepatitis patients.The expression of DcR1 can reflect the degree of liver injury in chronic hepatitis B patients to some degree.

AIM: To explore the effect of HBV transfection on apoptosis induced by TWEAK and the regulatory effect of IFN-γ on the apoptosis process.METHODS: BEL-7402-pCDNA3/1.1HBV hepatoma cell line was used as cell model in this experiment, in which BEL-7402 was stably transfected with pCDNA3/1.1HBV and its corresponding empty vector BEL-7402-pCDNA3 was used as control. The effect of HBV transfection on TNF-like weak inducer of apoptosis(TWEAK) induced hepatoma cell apoptosis was determined by CCK-8. The apoptosis before and after HBV transfection induced by TWEAK and the effect of IFN-γ on apoptosis induced by TWEAK in HBV transfected hepatoma cells were detected by TUNEL and caspase 3 activity detection kit.RESULTS: BEL-7402-pCDNA3/1.1HBV and BEL-7402-pCDNA3 hepatoma cell line were successfully established and identified. Cell viability of hepatoma cells was increased after HBV transfection. The apoptosis rate was much higher after HBV transfection compared to the empty vector control and empty cell control. The apoptosis rate induced by TWEAK was much higher in BEL-7402-pCDNA3/1.1HBV cells when exposed to IFN-γ.CONCLUSION: After HBV infection, TWEAK might be involved in the clearance of HBV infected hepatocyte by apoptosis and inflammatory factor such as IFN-γ.
3.Simultaneous resection for synchronous colorectal liver metastases:incisions and short-term outcomes

Objective:To discuss the role of incision for short-term outcomes of simultaneous resection in synchronous colorec-tal liver metastases (sCRLM). Methods:We reviewed the data of 37 patients who underwent simultaneous resection between January 2009 and December 2014 in our department and compared the short-term outcomes between Mercedes and midline incisions. Results:Mercedes and midline incisions were used in 19 and 18 patients, respectively. The two groups showed similarities in patient characteris-tics, major hepatectomy, surgery time, blood loss, and hilar block time. The midline group comprised more rectal cancer patients (P<0.001). The two groups did not differ significantly in complication incidence (47.4%vs. 16.7%, P=0.08) and postoperative stay time (22.1 ± 9.5 d vs. 17.2 ± 6.7 d, P=0.08). At body mass index (BMI)<25, the complication incidence (P=0.046) and postoperative stay time (P=0.051) were lower in the midline group than in the Mercedes group. Conclusion:Midline incision provided similar exposure in simultaneous resection for sCRLM and was better than Mercedes incision in rectal cancer patients. Patients with midline incision may attain better short-term outcomes if BMI is<25.

5.The study of differential diagnostic value between pigmented villonodular synovitis and rheumatoid arthritis in active age with clinic and ultrasound

ObjectiveTo explore the value of differential diagnosis between pigmented villonodular synovitis (PVNS) and rheumatoid arthritis (RA) in active stage with clinic and ultrasound. MethodsWe analyzed retrospectively the characters of clinic and ultrasound of sixteen patients with PVNS and sixteen patients with rheumatoid arthritis inactive stage.ResultsIn aspects of patient’s age (T=185), pain scores (T=136) history of trauma (P=0.029), functionally uncompensated conditions (P=0.001) type (P=0.000) and sum (P=0.000) of involved joints、bright spot-echoes in hydrops (P=0.001) tendons nearby involved (P=0.001) and the Adler’s grade of Doppler ultrasound blood flow (P=0.009), the differences between PVNS and RA in active stage had statistical significance (P<0.05). Clinic characters: patients with PVNS were younger, often had a history of trauma. But patients with RA in active stage had a higher pain score and functionally uncompensated condition. Ultrasound characters: big and single joints were often involved in patients with PVNS, and tiny bright spot echoes could be detected in hydrops. Instead, big and small, multiple joints, tendons nearby were often involved in patients with RA, who had a higher ratio of gradeⅢ Doppler ultrasound blood flow. And in aspects of sex, the presence of hydrops and synovium thickness, the differences between the two had no statistical significance (P>0.05).ConclusionThe clinical characters including Patient’s age, pain scores, history of trauma, functionally uncompensated conditions, and the characters of ultrasound including type and sum of involved joints, bright spot echoes in hydrops, tendons nearby involved and the Adler’s grade of Doppler ultrasound blood flow, are helpful for the differential diagnosis of PVNS and RA in active stage.
6.Quantitative CT assessment for hepatic steatosis

Objective To evaluate the role of CT in quantitative assessment for hepatic steatosis.Methods We retrospectively assessed 215 liver grafts used in orthotopic liver transplantation (LT) from January 2006 to December 2009.Pathology and CT data were reviewed.The correlation between liver steatosis level and CT value of the liver were analyzed.The ratio of liver-spleen,liver-descending aorta,liver-inferior vena cava were calculated. Results There were 52 steatosis grafts diagnosed by pathologist and the steatosis level was from 2.78% to 56.8%.The CT value of the liver,the ratio of liver-spleen,liverdescending aorta,liver-inferior vena cava was 41.3 ± 7.5 HU,0.89 ± 0.16,1.02 ± 0.16,1.04 ± 0.15,respectively.There were significant linear negative correlation between the steatosis level and the CT value,the ratio of liver-spleen,liver-descending aorta and liver-inferior vena cava. The correlation index was -0.881,-0.817,-0.951 and -0.948,respectively. Conclusions The CT value of the liver,the ratio of liver-spleen,liver-descending aorta and liver-inferior vena cava could be used to quantitatively evaluate liver steatosis.The ratio of liver-descending aorta was the most sensitive index.
7.Effects of Adjuvant Medication Special Comments on Rational Use of 12 Vitamins for Injection in Our Hospital

OBJECTIVE:To provide reference for rational drug use and strengthen the management of adjuvant medication man-agement in the hospital. METHODS:1 080 and 860 discharged medical records of 12 Vitamins for injection were selected from our hospital during Jun.-Nov. 2014 (pre-special comment group,i.e. group A) and Jun.-Nov. 2015 (post-special comment group,i.e. group B),respectively. The application of 12 Vitamins for injection,hospitalization stay,drug cost and the incidence of ADR were compared before and after adjuvant medication special comment. RESULTS:After adjuvant medication special comments,the propor-tion of drug use of no indication,unsuitable solvent selection,irrational drug dosage,drug use of drug interaction and contraindica-tion,drug cost and the incidence of ADR in group B were all lower than in group A,with statistical significance(P<0.05). There was no statistical significance in hospitalization stay between 2 groups(P>0.05). CONCLUSIONS:Clinical pharmacists adopt adju-vant medication special comments to effectively standardize clinical application of 12 Vitamins for injection in our hospital.

OBJECTIVETo detect the expression of TRAIL receptors in fibroblasts of hypertrophic scar in the proliferative stage and explore its significance.
METHODS30 samples of hypertrophic scar were taken from 30 burn cases in the proliferative stage. 30 samples of normal skin were taken as the control. The expressions of TRAIL receptors in the fibroblasts of hypertrophic burn scar and the normal skin were assayed by semiquantitative RT-PCR and flowcytometry.
RESULTSThe expression level of DR5 in the fibroblasts of hypertrophic burn scar is much lower than the control (P < 0.05); the expression level of DcR1 in the fibroblasts of hypertrophic burn scar is much higher than the control (P < 0.05).
CONCLUSIONSThe down-regulated DR5 expression and elevated DcR1 expressions in the fibroblasts of hypertrophic burn scar may attribute to the apoptosis change induced by TRAIL and explain the apoptosis differences between the fibroblasts of hypertrophic scar and normal skin to a certain extent.

10.Effect of penehyelidine hydrochloride on TIPE2-TLR4-MyD88 signaling pathway in a rat model of traumatic acute lung injury

Objective To evaluate the effect of penehyelidine hydrochloride(PHCD)on tumor necrosis factor α-induced protein 8-like-2(TIPE2)-Toll-like receptor 4(TLR4)-myeloid differentiation fac-tor 88(MyD88)signaling pathway in a rat model of traumatic acute lung injury(ALI).Methods Thirty SPF healthy male Sprague-Dawley rats,aged 8 weeks,weighing 190-210 g,were divided into 3 groups(n=15 each)by a random number table method: sham operation group(group Sham),traumatic ALI group(group ALI)and group PHCD.ALI was induced by blunt chest trauma in ALI and PHCD groups.PHCD 2 mg/kg was intraperitoneally injected immediately after blunt chest trauma in group PHCD.The rats were sacrificed and lung tissues were removed at 8 h after the model was successfully established for exami-nation of the pathological changes and ultrastructure of lung tissues(with a light microscope or an electron microscope)and for determination of the wet to dry weight ratio(W/D ratio)and expression of TLR4 and MyD88 in lung tissues.Results Compared with group Sham,the W/D ratio was significantly increased,TIPE2 expression was down-regulated,and the expression of TLR4 and MyD88 was up-regulated in ALI and PHCD groups(P<0.05).Compared with group ALI,the W/D ratio was significantly decreased,TIPE2 expression was up-regulated,and the expression of TLR4 and MyD88 was down-regulated(P<0.05),and the pathological changes of lung tissues and ultrastructure were significantly attenuated in group PHCD.Conclusion The mechanism by which PHCD reduces traumatic AIL is related to activating TIPE2-TLR4-MyD88 signaling pathway in rats.
