1.Application of spiral CT three dimentional reconstruction on developmental dysplasia of the hip in children

Objective To evaluate the value of surface shaded reconstruction (SSD) of CT scanning on developmental dysplasia of the hip (DDH). Methods Unilateral DDH in 39 cases was examined with Picker 2000S spiral CT to perform SSD of the affected pelvis. All patients were females with the age ranging from 3.5 to 8 years old (average 4.25 years). Of 39 cases, 22 cases were complete dislocation the hip, 17 cases were subluxation of the hip. The measurement on SSD included of the depth and surface area of normal and affected acetabulum; the distances from the lowest point of the bottom of the acetabulum (O point) to the anterior end (A point) of the lateral margin of anterior acetabular column (OA), the middle point (B point) of the acetabular roof (OB) and the posterior end (C point) of lateral margin of the posterior acetabular column (OC). Results In normal hip, the medial and lateral margin of the acetabular roof was overlapping, and in DDH, a bony defect was found in the lateral acetabular margin in three-dimensional CT scanning. The depth of the dysplastic hip with subluxation and the normal hip was (21.25?1.25)mm and (23.89?1.56)mm respectively with significant difference(P
2.The diagnostic value of MRI in soft tissue tumor

Objective To explore the diagnostic value of MRI in tumor of soft tissue. Methods The MRI manifestations of 180 soft tissue tumors dating from October 2001 to August 2004 were retrospectively analyzed, they were all confirmed by pathology. All cases were examined with Picker 0.23 T MRI. Fast spin echo(FSE)T1 and T2-weighted sequences and short tau inversion recovery (STIR/TSHIRT) fat-suppressed sequences were commonly used. MR examinations were performed at least in two of coronal, sagittal, axial planes. Of 180 cases, there were 62 vascular tumors, 34 giant cell tumor of tendon sheath, 26 neural tumors, 23 lipomas, 18 fibrous tumors; 17 malignant tumors including 7 of malignant fibrous histiocytoma(MFH), 3 of malignant lymphomas, 2 of liposarcoma and one each of chondrosarcoma, myofibrosarcoma, melanoma, facial sarcoma and leiomyosarcaoma. 10 cases were of tumor-like lesions, including 2 of myositis ossificans, 8 of fat necrosis. GCT of tendon sheath (33/34 cases), hemangiomas (23/52 cases) were common in hands and feet. MFH often involved legs and upper arms(4/7 cases, 3/7 cases). The tumors in the groin were mainly malignant(4/5 cases). Results The ratio of benign to malignant tumors was 9∶1. Tumors showed isointensity or hypointensity in T1WI and hyperintensity or mainly hyperintensity but mixed signal in T2WI. The diameter of benign tumors was 1-18 cm, the malignant was 4-10 cm. The tumors were lobulated, oblong or irregular in shape. 12 invaded to bone, 28 involved to neurovascular system. Some body parts were easily involved because of different histologic component. Conclusion The MRI for soft tissue tumors is highly sensitive. The location and some characteristical signs are useful for diagnosis, whereas the size of the lesion, the degree of border smoothness and the signal intensity are not certain for the diagnosis.
3.DISTRIBUTION OF 〔~(14)C〕 BIMOLONE IN MICE BY WHOLE BODY AUTORADIOGRAPHY

Distribution of 〔14C〕 bimolane in mice was studied by means of whole body autoradiography. At 30min, and lh, 3h, 9 h, 24h after iv, the mice were briefly anesthetized with ether and frozen by immersing indry ice/acetone. The mice were processed for whole body auto-radiography by continual sections with LKB-2250 PMV big whole body automatic-manual frozen cutter. The exeriments showed that 〔24C〕 bimolane distributed rapidly into the various tissues after iv administrtion. The highest radioactivity was found in lung, kidney, gastrointestinal tract, then the tumor, skin etc, the lowest radioactivity was found in brain.
4.Helical CT three-dimensional and multiplanar reconstruction for evaluation of acetabular fractures

Objective To explore the clinical value of helical CT surface shaded display (SSD) and multiplanar reconstruction (MPR) images in diagnosing the acetabular fractures. Methods According to the anatomical basis of "Y" type cartilage in children acetabulum, adult acetabulum was divided into "two columns" the anterior and posterior column and "four walls"the anterior, posterior, medial and roof walls, so as to define the localization, type, interface and union of fractures accurately. Results From 1999 to 2001, 68 cases of acetabluar fractures were reviewed. Of 68 cases, 28 cases were diagnosed as complex double columns and three walls (anterior, posterior and interior wall respectively) fracture, 7 cases as complex single column (anterior column) and three walls (anterior, posterior and interior wall) fracture, 5 as complex single column (anterior column) and two walls (anterior and interior wall) fracture, 21 as simple posterior wall fracture, 7 as simple anterior wall. In this group, 21 complex double columns and three walls fractures, 7 complex single column and three walls fractures, 5 complex single column and two walls fractures, 15 simple posterior wall fractures, 3 simple anterior wall fractures had reduction and internal fixed with plates or absorbable screws; another 7 complex double columns and three walls fractures, 6 simple posterior wall fractures, 4 simple anterior wall fractures were treated with traction reduction because the separation and displacement of their fragments were not significant. Using ?2 statistical analysis,there was a significant difference between the X-ray film and SSD and MPR in demonstrating the numbers and types. Conclusion SSD and MPR reconstruction imagines are of instructive importance in sufficient evaluation, operative preparation and appropriate instrumentation of internal fixation. In the management of acbtabular fracture, plates are needed to stabilize the fracture involved the anterior or posterior column, while the fragments of anterior or posterior wall are only fixed with screws, small fragments could be removed in order to avoid forming loose body. Therefore, SSD and MPR are the regular and important method in preoperative examination of complex acetabular fracture and dislocation.
5.The effect of continuous thoracic paravertebral block analgesia guided by sonography on pulmonary func-tion after thoracotomy

Objective To investigate the effect of continuous thoracic paravertebral block anal-gesia guided by sonography on pulmonary function after thoracotomy.Methods Sixty patients,male 29 cases,female 31 cases,aged 18-60 years,BMI 1 6-28 kg/m2 ,ASA grade Ⅰ or Ⅱ,who had under-went thoracotomy were divided randomly into 2 groups,30 cases each:group G with general anesthe-sia and postoperative patient controlled intravenous analgesia (PCIA),whereas group GP with general anesthesia combined with continuous thoracic paravertebral block (CTPVB)and postoperative continuous CTPVB.CTPVB were performed before induction as the patient was conscious so that the effect of CTPVB could be tested by blocking range.Both resting and coughing visual analogue scales (VAS)were recorded at the points of 30 minutes after extubation (T1 ),2 hours after operation (T2 ),6 hours after operation (T3 ),24 hours after operation (T4 )and 48 hours after operation (T5 ). Forced vital capacity (FVC),forced expiratory volume in the first second (FEV1 )and maximal mid expiratory flow (MMF)were measured by spirometer and the three maximal values were recorded at time points of entry of operating room (T0 ),T4 and T5 .Blood gas analysis was employed at corre-sponding time points by a blood gas analyzer and oxygen inhalation was ceased 30 minutes before drawing blood from radial artery.PaCO 2 ,PaO 2 and alveolararterial oxygen difference (PA-a O 2 )were recorded.Adverse effects were observed.Results Compared with group G,VAS when resting and coughing in group GP at T1-T5 decreased significantly (P <0.05).Compared with T0 ,FVC,FEV1 , MMF and PaO 2 at T4 ,T5 in both groups decreased significantly (P <0.05),PA-a O 2 increased signifi-cantly (P <0.05 ).Compared with group G,PaO 2 in group GP at T4 ,T5 increased and PA-a O 2 in group GP at T4 ,T5 decreased significantly (P <0.05).Conclusion CTPVB guided by sonography had excellent effect.It can not only improve pulmonary function after thoracotomy significantly but also promote intrapulmonary oxygenation.
6.Study on correlation between depth of sedation and Narcotrend index in Midazolam target-controlled infusion

Objective To investigate the reliability of Narcotrend for monitoring the depth of sedation [the observer's assessment of alterness/sedation(OAA/S) scale] with midazolam and correlation between the depth of sedation and Narcotrend index.Methods 0 ASA Ⅰ-Ⅱ status patients scheduled for elective lower limb operations underwent spinal anesthesia,who were given target-controlled infusion of midazolam.Target plasma concentration was 50ng/ ml at first and increased by 10 ng/ml each grade until OAA/S scale became 1 point.Each target concentration infusion was maintained for 5min.Observe the patient the OAA/S scale to be 5 points.If the operation was not over yet,regulated the appropriate depth of sedation until the end of operation.The Narcotrend index of different OAA/S scale and heart rate,mean arterial pressure,respiration were recorded.Results In the deepening or recovery phase of sedation,OAA/S scales were correlated with Narcotrend index (Spearman' s r =0.786,0.652,all P < 0.05).Conclusion Narcotrend is a good index to guide target controlled infusion of midazolam,the index is closely related with the depth of sedation of midazolam.
7.Diagnostic value of radom spot albuminuria to creatinine ratio in women with preeclampsia

Objective To investigate the correlation between spot albuminuria to creatinine ratio (ACR) and 24 h urinary protein excretion in women with preeclampsia and determine the optimal cut-off values of spot ACR in mild preeclampsia and severe preeclampsia.Methods Twenty-eight women with mild preeclampsia and 22 with severe preeclampsia at Nanfang Hospital,Southern Medical University between October 2010 and June 2011 were recruited.Maternal serum cystatin,uric acid,mea nitrogen,creatinine and albumin levels were collected and analyzed.Twenty-four hours urinary protein excretion was measured with immunoturbidimetric assay and ACR with automatic analyzer DCA2000.The correlation between ACR and 24 hours urinary protein excretion was explored.And the optimal cut-off values of the spot ACR for mild and severe preeclampsia were determined with receiver operating characteristic curve.Results ( 1 )Maternal serum biochemical parameters:uric acid levels in mild and severe preeclampsia were (359 ± 114)μmol/L and (450 ± 132) μmol/L,while cystatin levels were ( 1.3 ±0.3) mg/L and ( 1.6 ±0.5) mg/L respectively.The differences were statistically significant ( P < 0.05 ).Serum urea nitrogen,creatinine and albumin in mild preeclampsia were(3.6 ± 1.6) mmol/L,(52 ± 38 ) μmol/L and ( 33 ± 3 ) g/L,while in severe preeclampsia were( 6.2 ± 3.1 ) mmol/L,( 78 ± 59 ) μmol/L and ( 29 ± 6 ) g/L respectively.There were no statistical significant differences ( P > 0.05 ).(2) Twenty-four hours urinary protein excretion and ACR:24 hours urinary protein levels in mild and severe preeclampsia was (700 ± 160) mg and (4800 ±2200) mg (P<0.05).ACR in mild and severe preeclampsia was (72.7 ± 12.4) mg/mmol and (401 ±245) mg/mmol respectively (P < 0.05 ).(3) There was a strong correlation between the spot ACR and 24hours urine protein excretion ( r =0.938 ; P < 0.05 ).( 4 ) The optimal spot ACR cut-off point for the diagnosis of preeclampsia:the optimal spot ACR cut-off point was 22.8 mg/mmol for 300 mg/24 hours of protein excretion in mild preeclampsia,the area under curve was 0.956,with a sensitivity,specificity of 82.4%,99.4% respectively.And the optimal spot ACR cut-off point was 155.6 mol for 2000 mg/24 hours of protein excretion in severe preeclampsia,the area under curve was 0.956,with a sensitivity,specificity of 88.6%,91.3% respectively.Conclusions Compared with 24 hours urinary protein excretion,the spot ACR may be a simple,convenient and accurate indicator of early diagnosis of preeclampsia.Spot ACR may be used as a replacement for 24 hours urine protein excretion in assessment of preeclampsia.The optimal spot ACR cut off points were 22.8 mg/mmol for mild preeclampsia and 155.6 mg/mmol for severe preeclampsia.

9.Effects of auricular point sticking on labor pain and anxiety

Objective:To observe the effects of auricular point sticking on pain and anxiety during the latent period of the first stage of labor in primiparas. Methods:Primiparas meeting eligibility criteria were recruited.The participants were randomized into an auricular point group,a placebo group,and a control group.The control group received daily care.The auricular point group received 120 min of auricular point sticking therapy.The placebo group received the same auricular plasters as the auricular point group but without pressing.Participants'pain,anxiety,and uterine contractions were measured at enrollment and 30,60,and 120 min of interventions. Results:Data from 78 participants were analyzed in this study.After uterine contraction was adjusted as a covariate,there was no significant difference among groups in the baseline anxiety,baseline pain,and anxiety at 30-min intervention(P>0.05),and no significant difference between the placebo group and the control group in each indicator at each time point(P>0.05).The anxiety scores of the auricular point group at 60 min and 120 min were lower than those of the placebo group and the control group(P<0.05).The pain in the auricular point group was less than that in the placebo group and the control group at 30,60,and 120 min of interventions(P<0.05). Conclusion:Auricular point sticking therapy can relieve anxiety and pain in women during the latent period of labor.Moreover,the effect is fast-acting.It can be used as a safe and effective complementary therapy.
10.Predictive value of random spot albuminuria to creatinine ratio in women with hypertensive disorders complicating pregnancy

Objective To determine the random spot albuminuria to creatinine ratio (ACR) of normal pregnant women , to track the pregnancy outcome , and to discuss the predictive value of ACR in women with hy-pertensive disorders complicating pregnancy (HDCP). Methods Except for 87 pregnant women suffering from HDCP, 2 038 pregnant women were enrolled in this study. ACR, routine examinations of blood and urine, blood biochemical, 24-hr urinary protein were determined. Results ACR, but not 24-hr urinary protein level,was sig-nificantly higher in women with HDCP. There was positive correlation between the ACR and 24-hr urinary protein quantitation. Age, gestational weeks, ACR, red blood cells, fasting plasma glucose, serum creatinine, total pro-tein were the independent risk factors for HDCP. The sensitivity , specificity and optimal cut off value of ACR for predicting HDCP were 0.78, 0.63, 1.46 mg/mmol. Conclusions There was positive correlation between ACR and 24-hr urinary protein quantitation , and ACR provided a more sensitive pathway for early predictionof HDCP.
