1.Comparative Study on Efficacy and Safety of Lobaplatin/Cisplatin Combined with Etoposide in the Treat-ment of Primary Small Cell Lung Cancer

OBJECTIVE:To compare the efficacy of lobaplatin/ cisplatin combined with etoposide in the treatment of primary small cell lung cancer. METHODS:60 patients with small cell lung cancer were randomly divided into observation group and con-trol group. Observation group was given 30 mg/m2 Lobaplatin injection,adding into 500 ml 5% Glucose injection,d1,by intrave-nous infusion for 1 h+100 mg/m2 Etoposide injection,adding into 500 ml 0.9%Sodium chloride injection,d1-3,by intravenous infu-sion for 1 h. Control group was given 25 mg/m2 Cisplatin injection,adding into 500 ml 0.9% Sodium chloride injection,d1-3,by in-travenous infusion for 1 h+Etoposide injection(the same dose with observation group). 3-week was regarded as a chemotherapy cy-cle. The clinical efficacy and toxicities in 2 groups were observed. RESULTS:There were more than 2 cycles for both groups. There was no significant difference in the total effective rate between 2 groups(P>0.05),and the incidences of nausea,vomiting and diarrhea in observation group were significantly lower than control group,the differences were statistically significant (P<0.05). CONCLUSIONS:Lobaplatin/ cisplatin combined with etoposide has similar efficacy in the treatment of primary small cell lung cancer,and lobaplatin has better safety.
2.Interventional Therapy By Trans Uterine Artery for Treatment of 42 Patients With Tubal Pregnancy

Objective To analyze the effect and safety of interventional therapy by trans uterine artery for treatment of tubal pregnancy. Methods Forty-two patients with tubal pregnancy were performed two side uterine arteries. MTX perfusion and embolization with gelfoam particles guided by DSA fluoroscopy. Results Forty patients obtained the decline of serum β-HCG value and 39 patients' ectopic mass absorption after treatment. Two patients occurred ectopic mass bleeding after interventional therapy and emergency surgical operation were done, but the amount of bleeding was low than 400ml. Conclusion Interventional therapy by trans uterine artery for treatment of tubal pregnancy could preserve the function of fallopian tubal, it was more benefit for patients who want normal pregnancy.
3.Effect of High Flow Oxygen Uptake on the Quality of Coronary Artery Imaging

Purpose To compare the difference of CT coronary artery imaging quality under highflow oxygen uptake and normal breathing, and to investigate the effect of high flow oxygen uptake on the quality of CT coronary artery imaging.Materials and Methods 132 patients underwent coronary CTA examination using 256-slice CT (Philips), among them 71 patients were supplied with highflow oxygen, and the other 61 were asked to breathe normally. Coronary arteries were post-processed and reconstructed on AW 4.4 workstation. Scanning completion rate, signal to noise ratio (SNR), contrast to noise ratios(CNR) and image quality score ofcoronary segment using these two prospective ECG-gating techniques were compared.Results The scanning completion rate and image quality score of the highflow oxygen uptake group were significantly superior to those of the normal breathing group (P<0.05). However, there was no statistically significant difference of SNR and CNR between the two groups (P>0.05).Conclusion Imaging quality of coronary CTA can be improved using highflow oxygen uptake with reduced patient radiation dose, thus worth being used clinically as a simple and practicable method.
4.Questionnaire survey of depression prevalence and associated risk factors in elder patients with low-vision in Beijing suburb

To explore the prevalence and risk factors of depression in low-vision elder patients aged 60-85 years residing at Beizhuang Township,Miyun county.The activity of daily living scale (ADL) and geriatric depression scale (GDS) were used for diagnosis and grade the severity of depression.The prevalence rate of depression was higher in low-vision patients (36.2%) than in the control group (15.5%,P <0.05).The need for assistance in daily activities was higher in low-vision patients than in the control group(x2 =13.57,P <0.01).Patients with good economic conditions,being cared by relatives and living with their spouse had lower levels of ADL and GDS (all P < 0.05).
5.The clinical effects of transcatheter hepatic arterial chemoembolization and systemic chemotherapy for treatment of colorectal cancer with hepatic metastasis

ObjectiveTo explore the clinical effect of transcatheter hepatic arterial chemoembolization and systemic chemotherapy on colorectal cancer with hepatic metastasis.Methods50 cases of colon cancer with hepatic metastasis according to treatment methods were divided into A,B group.A group treated by TACE plus intravenous chemotherapy,B group treated by intravenous chemotherapy alone.Side effects,changes in liver tumor size and survival rate were observed.ResultsThe incidence of side effects of two groups had no significant difference ( P > 0.05 ).A group of liver lesions smaller in size or stable is more than in group B( P <0.05 ).A group of 1 year,3 years survival rate is 84%and 64%respectively,which is higher than B group 56%and 36% ( all P < 0.05 ).ConclusionTranscatheter hepatic arterial chemoembolization and systemic chemotherapy is a safe and effective treatment of colon cancer with hepatic metastasis,it can obviously raise the survival rate.
6.Proximal femur locking plate fixation for the treatment of unstable femoral intertrochanteric fractures in elderly patients

Objective To explore the efficacy of proximal femur locking plate(PFLP) for the treatment of unstable femoral intertrochanteric fractures in elderly patients.Methods Data of 120 cases of patients with unstable femoral intertrochanteric fractures who were treated by PFLP were retrospectively analyzed.There were 56 males and 64 were females,aging from 62 to 78 year-old (average,69.4 year-old).85 cases were left hip,and 35 cases right hip.According to AO fracture classification,there were 12 cases of A2.2,23 A2.3,34 A3.1,29 A3.2,and 22 A3.3.According to Evans intertrochanteric fractures classification:there were 93 cases of type Ⅲ,19 type Ⅳ,and 8 type Ⅴ.Operation time,length of hospital stay,fracture healing time,postoperative complications and hip joint Harris scores were recorded.Results All patients were followed up for 8-14 months (average,12.4 months).Their operating time were 51-80 min (average,63.1 min);X-ray exposure were 8-15 times (average,11.6 times);intraoperative blood loss were 70-120 ml (average,92.7 ml);postoperative drainage volume were 50-100 ml (average,61.7 ml),and length of hospital stay were 7-14 d (average,10.2 days).The healing time ranged from 12 to 32 weeks (20.4 weeks average).14 cases had delayed union,and healing time was 8-10 months.In addition,4 cases had deep vein thrombosis (after thrombolysis therapy the vein was recanalized);2 had internal fixation broken,who were then treated with PFNA Ⅱ,and bone healed after 6 months;5 had hip varus,who were then treated via hip replacement,and no hip prosthesis fracture were found.No other complications occurred in the remaining patients.At the latest follow-up,according to Harris hip score:there were 38 cases with excellent results,71 good,9 fair,and 2 poor,and the rate of excellent and good was 90.8% (109/120).Conclusion PFLP for the treatment of unstable femoral intertrochanteric fractures has advantages of less invasion,less complication,reliable fixation and high fracture healing rate,thus it is suitable for unstable intertrochanteric fractures in elderly patients.
7.Extraction and characterization of antifungal substances produced by antagonistic Streptomyces S24.

Streptomyces S24 has broad spectrum resistance to the Aspergillus in food and feed, such as Aspergillus flavus, Aspergillus niger, Asperegillus alutacells and so on. We studied the adsorption and desorption properties of antifungal substance from Streptomyces S24 on macroporous resins, screened the best elution solution and also investigated some physical and chemical characters of antifungal substance by determining the antifugal activity using oxford plate assay system. According to the analysis results, AB-8 resin offered the best adsorption and desorption capacity for antifungal substance and its saturated absorption capacity was 7.0822 x 10(4) microg/g, the optimal elution solution was 85% acetone and the dynamic desorption rate could reach 93.82%. The antifungal substance was stable to heat and alkali, not sensitive to organic solvents, and sensitive to ultraviolet rays and acid. Based on its ultraviolet spectrometry, the antifungal substance was identified as heptaene macrolide antibiotic.

8.Construction and biological characteristic for the recombinant modified vaccinia virus ankara co-expressing modified GP5 and M protein of porcine reproductive and respiratory syndrome virus.

Modified ORF5 (MORF5) and ORF6 gene of PRRSV were cloned into two multiple cloning sites of MVA transfer vector pLR-gpt to construct the recombinant plasmid pLR-MORF5/ORF6. Homologous recombination between pLR-MORF5/ORF6 and the wtMVA on BHK-21 cell line was mediated with liposome by infecting the cell with 0.01 MOI wtMVA two hours before transfecting the recombinant plasmid into the cell. When the cytopathic effect (CPE) was obvious, virus was collected from the cell plate and the recombinant virus was selected with drug selecting medium (2% MXHAT). After 12 cycles of selection, rMVA with a selection marker Eco gpt was obtained and named as rMVAgpt-MGP5/M. By infecting BHK-Cre expressing Cre recombinant enzyme, the Eco gpt marker in rMVAgpt-MGP5/M was deleted and this rMVA was named as rMVA-MGP5/M. The insertion of MORF5 and ORF6 into the MVA genome was confirmed with PCR analysis and the expression of MGP5 and M protein was identified with Western blot and IFA. Through biological study on the recombinant MVA, no obvious difference was observed between rMVA-MGP5/M and the wtMVA regarding to the CPE and growth curve. The recombinant MVA constructed in this study could coexpress the modified GP5 and M protein and the expressed product had good immunocompetence. Furthermore, the insertion of the MORF5 and ORF6 into MVA genome had no obvious effect on the replication and biological characteristics of this virus.

9.Multivariate correlation analysis of femoral head necrosis after operation of femoral neck fracture

Objective To analyze the related factors of femoral head necrosis after internal fixation operation of femoral neck fracture.Methods Clinical data of 524 patients with femoral neck fracture who underwent internal fixation surgery between January 2012 and January 2015 were retrospectively analyzed.Taking the femoral head necrosis as an observable indicator,the clinical data of postoperative femoral head necrosis and those other patients who were treated at the same period were compared.Whether the clinical factors (gender,age,surgical reduction type,fracture type,quality of reduction,post-injury operation time,loading time after operation,and whether to withdraw internal fixation) were in correlation with postoperative femoral head necrosis was analyzed by univariate analysis.Then the statistically significant indicators were integrated into logistic regression analysis to determine the related factors of femoral head necrosis.Results According to inclusion and exclusion criteria,the study group consisted of 212 cases,including 94 males and 118 females;the follow-up time was 2-5 years,and the average follow-up time was 3.6±1.6 years.There were 46 cases with femoral head necrosis and 166 cases without femoral head necrosis after operation.The rate of femoral head necrosis was 21.7% (46/212).The femoral head necrosis group included 46 cases with 21 males and 25 females;20 cases below 60 years old and 26 cases above 60 years old;17 cases using open reduction and 29 cases using closed reduction;6 cases belong to type Ⅰ/Ⅱ and 40 cases belong to type Ⅲ/Ⅳ according to Garden classification;16 cases got satisfactory reduction while 30 cases with unsatisfactory reduction;30 cases below 48 h and 16 cases above 48 h on post-injury operation time;17 cases below 3 months and 29 cases above 3 months on post-operation loading time;22 cases with internal fixation removal and 24 cases without internal fixation removal.The femoral head without necrosis group included 166 cases with 73 males and 93 females;84 cases below 60 years old and 82 cases above 60 years old;69 cases using open reduction and 98 cases using closed reduction;120 cases belong to type Ⅰ/Ⅱ and 46 cases belong to type Ⅲ/Ⅳ according to Garden classification;160 cases got satisfactory reduction while 6 cases with unsatisfactory reduction;119 cases below 48 h and 47 cases above 48 h on post-injury operation time;70 cases below 3 months and 96 cases above 3 months on post-operation loading time;74 cases with internal fixation removal and 92 cases without internal fixation removal.Univariate x2 analysis suggested that fracture type and quality of reduction were associated with postoperative femoral head necrosis.Other clinical factors (gender,age,surgical reduction type,post-injury operation time,loading time after operation and whether to remove internal fixation or not) were not associated with postoperative femoral head necrosis.The multivariate Logistic regression analysis showed fracture type and quality of reduction were significantly related to postoperative femoral head necrosis.Conclusion Femoral head necrosis after internal fixation operation of femoral neck fracture is affected by many factors.The fracture type and quality of reduction are important factors affecting femoral head necrosis after internal fixation operation of femoral neck fracture.
10.Pathogenic infection spectrum revealed by metagenomics high-throughput next-generation sequencing in patients with hematological diseases after allogeneic hematopoietic stem cell transplantation

Objective:To investigate the infection spectrum revealed by metagenomics high-throughput next-generation sequencing (mNGS), and to provide a reference for infection diagnosis after allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods:A total of 64 patients who developed systemic or local infection symptoms after allo-HSCT in Hebei Yanda Lu Daopei Hospital from January 2018 to November 2018 were enrolled. Gene sequences of pathogenic microorganisms in blood, cerebrospinal fluid and bronchoalveolar fluid specimens were detected by using mNGS. The pathogenic microorganisms or suspected pathogens were determined based on the clinical manifestations of patients.Results:There were 97 samples of mNGS detection for 64 patients who underwent allo-HSCT. The most common gram-positive bacteria were staphylococcus haemolyticus (19 times) and staphylococcus (14 times), and the most common gram-negative bacterium was acinetobacter baumannii (8 times). The most common viruses were cytomegalovirus, EB virus and Torque teno virus (35, 22 and 23 times, respectively), and the most common fungi were malassezia globus (14 times) and candida parapsilosis (8 times). There were 3 mycobacterium tuberculosis complexes detected in 3 patients with acute myeloid leukemia who received allo-HSCT. Mycoplasma orale was detected in one patient's sputum, and none parasite was detected.Conclusion:mNGS can comprehensively reveal the infection spectrum of hematologic diseases after allo-HSCT, especially for pathogenic microorganisms that are rare or difficult to cultivate, and it can effectively help the diagnosis of clinically infectious pathogens.
