1.Comparison of open osteotomy model and closed fracture model in the rat femur

BACKGROUND:There is no doubt that animal model of fracture is the basis of fracture healing research.The fracture animal mode is divided into open osteotomy model and closed fracture model.There is no research about the difference between two kinds of fracture rat models.OBJECTIVE:To compare the modeling process between open osteotomy model and closed fracture model in rats,and to compare fracture healing using radiograph.DESIGN,TIME AND SETTING:The randomized,controlled animal experiment was performed at the Laboratory of Department of Orthopaedics,Second Affiliated Hospital,Soochow University from January to May 2007.MATERIALS:A total of 40 SPF male Sprague Dawley rats,weighing(220?30) g,were randomly divided into an open osteotomy model group and a closed fracture model group,with 20 rats for each group.Electric swing saw was a kind of self-made percussion device for closed fracture models.METHODS:All rats in the open osteotomy model group received open osteotomy to create transverse fracture of the middle femur,and fixed using Kirschner wire.Rats in the closed fracture model group received retrograde Kirschner wire fixation,and midshaft femoral closed fracture induced by model establishment stent.MAIN OUTCOME MEASURES:Mean model establishment time;Successful rate of model establishment;Radiography of fracture healing at 2,3,4 and 8 weeks after surgery.RESULTS:Success rate of open osteotomy model establishment was 90%.Average modeling time was 17 minutes.Rate of malunion and nonunion was 40%.Successful rate of each closed fracture models was 85%,with average modeling time was 12 minutes.There was no malunion and nonunion.At 2,3 and 4 weeks after surgery,X-ray score was lower in the open osteotomy model group compared with the closed fracture model group(P 0.05).CONCLUSION:Model establishment of open osteotomy takes a longer time,with a high rate of malunion and nonunion.In near future or medium-term,fracture healing is significantly lagged.Thus,open osteotomy is not suitable for the study of early or medium-term fracture healing.

Objective Effects of exercise training and Chinese Medicine Compound for Invigorating Qi and Tonifying the Kidney on testicular StAR and P450 SCC mRNA expression in rats were observed. Methods 50 Wistar rats were randomly divided into 4 groups: sedentary control group(SC, n=10), sedentary with medicine group (SM,n=10), training group(T, n=15) and training with medicine group(TM, n=15). After 6 weeks swimming training , serum testosterone level of rats were tested through radioimmunological method; using RT-PCR, we measured testicular StAR and P450 SCC mRNA levels of rats. 4.Tne bone density distribution of the sacroiliac joint measured by computer assisted tomography

Objective To find out the character of the its strain distribution. Methods We use five fresh cadaver sacroiliac joints of male adults. We fixed the samples in the CT. We chose the same point of the joints and measure the CT values . Result The bone CT values of the sacroiliac joints and the same slide distributes in a curve from the anterior of the joint to the posterior. Conclusion The density of the bones are directly related with the strains. So the strain distribution on the sacroiliac joint surface is similar to the CT value distribution. This is similar to the result we get with tri- dimentional finite elements study of the sacroiliac joint.
5.Clinical report on arthroscopic reconstruction of anterior cruciate ligament with LARS artifical ligament

[Objective]To explore the operating methods and rudimental clinical effects of reconstruction of anterior cruciate ligament(ACL) with the ligament advancement reinforcement system(LARS) artificial ligaments under arthroscope.[Method]Since June of 2006,15 patients(11 males and 4 females,with an average age of 26.8 years)with acute ACL rupture have been treated with the LARS artificial ligaments.The results of preoperative Arto-scan of all patients suggested discontinuation of ACL.The average score of Lysholm on knee joint was 50 points.The operations were completed under arthroscope.Bone tunnels of the femur were located at the central point of arc on the medial surface of ectocondyle of 140 degree.Bone tunnels of tibia were located between the medial and lateral condyle on the tibial plateau、2-4 mm to summit line of fossa intercondylica.The diameter of the bone tunnels was 7.5 mm while that of LARS ligament and hollow crushing screw were 8 mm.[Result]All 15 patients were available for follow-up with an average of 18 months.No postoperative complications such as infection,ligament rupture,ligament loosening occurred.The clinical results were graded as being excellent in 8 patients,good in 5 and fair in 2 according to the Lysholm's classification.The excellent and good rate was 86.6%.[Conclusion]The anatomical reconstruction and the stability of the knee joint can be obtained after ACL reconstruction with LARS artificial ligaments.The operation under arthroscope has advantages of less-injury,rapid recovery and satisfactory curative effect.
6.Repairing bone defects using human bone marrow mesenchymal stem cells transfected with insulin like growth factor-1 in nude mice

BACKGROUND:Insulin-like growth factor-1(IGF-1) is an important regulator in osteoblasts proliferation,differentiation and matrix synthesis,which participates in the process of fracture healing and repairing. OBJECTIVE:To investigate the potential bone regeneration effect of human bone marrow mesenchymal stem cells(MSCs) transfected into IGF-1 retroviruses vector on bone defects repairing. DESIGN,TIME AND SETTING:A controlled observation experiment was performed between March 2004 and May 2005 at the Laboratory Animal Center of Soochow University. MATERIALS:MSCs were transfected into IGF-1 retroviruses vector in vitro and combined with demineralized bone matrix(DBM) . METHODS:Fifteen BalB/C nude mice were prepared for 8 mm calvarial defects models and divided into 3 groups by number table method,with 5 animals in each group. DBM transfected with IGF-1 and DBM transfected with vector were implanted into the defect in MSCs cells transduced with IGF-1 and vector groups,respectively. There was no other intervention in the blank control group. The implants were harvested and evaluated by histological examination at 4 weeks after model preparation. MAIN OUTCOME MEASURES:①The mRNA expression of IGF-1 was analyzed by RT-PCR and Western Blot. ②The complexes of cells and DBM were analyzed by scanning electron microscope(SEM) at days 3 and 6 after co-culture. ③Gross observation of the calvarial defects models. ④Osteogenic activity of the implants was evaluated by hematoxylin-eosin staining at weeks 4 after model preparation. RESULTS:①There were mRNA and protein expression of IGF-1 in MSCs cells transduced with IGF-1 vector. ②SEM showed that there were plenty of cells adhered to surface of DBM,and grew into inner part at the 3 days after co-culture with IGF-1,which secreted much collagen fibers at days 6. The count of adherent cells and collagen fiber was smaller in the vector group at each time points. ③No obvious inflammatory reaction could be seen in each group at 4 weeks after model preparation. Compared with vector control,abundant new bone and blood vessel formation occurred in the calvarial defects treated with DBM/IGF-1,and no new bone formation in the blank control groups. ④Results of hematoxylin-eosin staining showed that grafts in the MSCs cells transduced with IGF-1 group was combined closely with calvarial defects,new bone and vessel could be found. There were few bone-like materials formation in vector group,and no defects in control group were repaired. CONCLUSION:The composites of IGF-1 transfected MSCs and DBM has good effect in repairing calvarial defects.
7.Effect of Zhuang Medicine Acupuncture on Brainstem Auditory Evoked Potential in Patients with Posterior Circulation Ischemic Vertigo

Objective To investigate the effect of Zhuang medicine acupuncture on brainstem auditory evoked potential (BAEP) in patients with posterior circulation ischemic vertigo (PCIV).Methods Eighty-five PCIV patients were randomly allocated to a treatment group of 45 cases and a control group of 40 cases. The control group received conventional basic therapy and the treatment group, Zhuang medicine acupuncture in addition. The peak latencies (Pg) of BAEP waveⅠ ,Ⅰ andⅠ and the interpeak latencies (IPg) of waveⅠ-Ⅰ,Ⅰ-Ⅰ andⅠ-Ⅰ were compared between the two groups after treatment.Results There were statistically significant post-treatment differences in waveⅠ andⅠ Pg and waveⅠ-Ⅰ andⅠ-Ⅰ IPg between the treatment and control groups (P<0.05).Conclusions Zhuang medicine acupuncture plus medication is an effective way to treat PCIV. It can improve BAEP indicators in the patients.
8.Experimental study on regeneration of articular cartilage defects in rabbits with bone marrow stromal cells

Objective To observe whether the full-thickness defects of articular cartilage at the knee joint of rabbits could be repaired by implantation of polyglycolic-acid(PGA) composites adhered with bone marrow stromal cells (BMSCs). Methods Culture-expanded rabbits’BMSCs were seeded onto porous PGA scaffolds. After a 72-hour co-culturing, the cell-adhered PGA was implanted into the articular cartilage defect at the intercondylar fossa of the femur. The rabbits were sacrificed 12 weeks later after the operation and the specimens were examined histologically for morphologic features, and stained immunohistochemically for type Ⅱcollagen. Results The specimens harvested from BMSCs-PGA composite demonstrated a hyaline cartilage formation. No obvious progressive degeneration sign was found in the newly formed tissue. The control groups showed no hyaline-like cartilage formation. Conclusion PGA composites adhered with in vitro culture-expanded autologous BMSCs can facilitate formation of hyaline-like cartilage in rabbits.
9.Optimization of Methanol Extraction Technology for Tripterine in the Fruit of Celastrus monospermus by Orthogonal Design

OBJECTIVE:To optimize the methanol extraction technology of tripterine in the fruit of Celastrus monospermus. METHODS:HPLC was adopted to determine the content of tripterine. Using extraction time,extraction times,solid-liquid ratio as investigation factors,extraction rate of tripterine as investigation index,orthogonal test was designed,and verification test was con-ducted. RESULTS:The optimal methanol extraction technology was as follow as 45% methanol extracting 2 h each time with sol-id-liquid ratio of 1:8 for 3 times. In the verification test,the average extraction rate of tripterine was 0.917 mg/g(RSD=2.85%, n=3). CONCLUSIONS:The optimized methanol extraction technology is stable and feasible with high extraction rate,and is suit-able for the extraction of tripterine in the fruit of C. monospermus.
10.Clinical valure of C-arm fluoroscopy in arthroscopic reconstruction of anterior cruciate ligament with LARS artificial ligament

Objective To discuss the operation skills and clinical effects of C-arm fluoroscopy in arthroscopic reconstruction of anterior cruciate ligament(ACL)with the Ligament Advancement Reinforcement System(LARS)artificial ligaments.Methods The study involved 36 patients with acute ACL rupture treated with the LARS artificial ligaments from June 2006.There were 25 males and 11 females,at age range of 22-51 years(average 28.3 years),involving 19 left knees and 17 right knees.The results of preoperative MRI of all patients suggested discontinuation of ACL,with average score of Lysholm on knee joint for 50.The operation was completed under arthroscope.While the locations of the femoral tunnel portal and the tibial tunnel exit were mainly determined by the C-arm fluoroscopy.The diameter of the LARS artificial ligament was 7.5 mm while that of the interference screw 8 mm.Results All 36 patients were followed up for a mean duration of 18 months(9-20 months).The average Lysholm Score was 52 preoperatively and 92 at the 12th week after operation.The clinical results were graded as excellent in 23 patients,good in nine and fair in four according to the Lysholm's classification,with excellence rate of 89%.Conclusions Arthroscopic reconstruction of anterior cruciate ligament with LARS artifical ligament under C-arm fluoroscopy takes advantages of convenient operation,accurate location and satisfactory clinical effect.
