1.Between "Right to Life" and "Medical Autonomy": Progressive Course of Hospice Palliative Care, Patient Autonomy and Euthanasia in Taiwan

With the development of medical technology, terminal stage patients rely excessively on medical equipment to prolong their lives, but this aggravates the pain of dying patients. The current situation drives the Taiwanese people to think deeply about the following issues: Does the quality of life before death outweigh the ineffective medical treatment at the end of life? Is the patient's autonomy higher than the right to life? This article started with several cases that triggered the discussion of euthanasia in Taiwan, such as the case of Mr. Fu Daren, a sports anchor in Taiwan, who wished to pass legislation on euthanasia. Meanwhile, this paper reviewed the revision process of the "hospice palliative care rules" in Taiwan, and introduced the spirit of "patient autonomy rules" to be implemented in 2019. From the perspective of the revision of the regulations, the current trend was further explored. The awareness of Taiwanese people on the right of self-determination in their final life was improved, and the pursuit of the quality of their final life was more than the extension of their lives.
2.Research on the effect of cognitive education combined with team psychological intervention on job burnout and mental health of obstetric nurses

Objective:To explore the effect of cognitive education combined with team psychological intervention on job burnout and mental health of obstetric nurses.Methods:Forty two clinical nurses working in the Second Affiliated Hospital of Hainan Medical College from October to December 2019 were selected. According to the random number method, the patients were divided into the control group and the observation group (21 people). The control group used the method of team psychological intervention, and the observation group used the method of cognitive education on the basis of the control group. The levels of job burnout, resilience and physical and mental symptoms were compared between the two groups before and after intervention.Results:There was no significant difference in the scores of job burnout, resilience and physical and mental symptoms between the two groups before intervention ( P>0.05). After intervention, the scores of job burnout and resilience in the observation group were 34.81 ± 4.87, 20.29 ± 2.64, 30.21 ± 4.23, 38.91 ± 5.45, 28.17 ± 3.94, 13.22 ± 1.72, which were higher than 30.55 ± 3.36, 18.35 ± 2.20, 27.33 ± 4.10, 34.58 ± 4.15, 24.59 ± 3.20, 11.42 ± 1.37 in the control group. The scores of physical and mental symptoms were 1.81±0.25, 2.01 ± 0.28, 1.83 ± 0.24, 1.09 ± 0.12, 1.11 ± 0.16, 1.27 ± 0.14, which were lower than those of the control group 2.12 ± 0.23, 2.32 ± 0.30, 2.21 ± 0.24, 2.09 ± 0.29, 2.07±0.27, 2.26±0.29. The differences between the two groups were statistically significant ( t values were - 11.348 - 25.390, P<0.01). Conclusions:The method of cognitive education combined with team psychological intervention for obstetric nurses can alleviate the job burnout of nurses, help to maintain mental health, and has good operability.
3.Effects of knockdown of Stat5 on invasion and epithelial mesenchymal transition of thyroid carcinoma TT cells

Objective To investigate the effects of knockdown of signal transducer and activator of transcription 5 (Stat5) on invasion and epithelial mesenchymal transition (EMT) of thyroid carcinoma TT cells.Methods Thyroid carcinoma TT cells were cultured and divided into Stat5-small interfering RNA (siRNA) group and NC-siRNA group,transfected with Stat5 siRNA and negative control NC-siRNA respectively.After transfection,the numbers of invasive cells were determined by Transwell model and the expressions of matrix metalloproteinase (MMP) family genes,EMT marker genes,angiogenesis genes in the cells were determined by fluorescent quantitative PCR.Results The numbers of invasive cells in Stat5-siRNA group were significantly lower than those in NC-siRNA group (42.39 ± 5.82 vs.64.41 ± 8.49,t =-4.784,P =0.001;51.23 ± 6.38 vs.78.54 ± 9.52,t =-5.329,P < 0.001;60.35 ± 8.35 vs.98.42 ± 11.25,t =-6.076,P <0.001) after the transfection of siRNA for 12 hours,18 hours and 24 hours.Twenty-four hours after transfection of siRNA,the mRNA expressions of MMP1,MMP2,MMP9,MMP13,N-cadherin,Vimentin,vascular endothelial growth factor (VEGF),basic fibroblast growth factor (bFGF),angiogenin (Ang)-1 and Ang-2 in Stat5-siRNA group were significantly lower than those in NC-siRNA group (0.42 ± 0.07 vs.1.03 ± 0.15,t =-8.240,P<0.001;0.35 ±0.06 vs.1.01 ±0.13,t=-10.307,P<0.001;0.29±0.05 vs.1.05± 0.18,t=-9.097,P<0.001;0.54±0.08 vs.0.98±0.12,t=-6.822,P<0.001;0.38 ±0.06 vs.1.04±0.18,t=-7.778,P<0.001;0.29±0.04 vs.1.06±0.12,t=-12.612,P<0.001;0.36±0.07 vs.1.06 ±0.14,t=-10.000,P<0.001;0.43 ±0.08 vs.0.97 ±0.12,t=-8.372,P<0.001;0.25 ± 0.03 vs.1.03±0.12,t=-14.100,P<0.001;0.19±0.03vs.0.99±0.13,t=-13.408,P<0.001).The mRNA expression of E-cadherin was significantly higher than that in NC-siRNA group (2.88 ± 0.42 vs.0.98 ± 0.12,t =9.726,P < 0.001).Conclusion Knockdown of Stat5 inhibits the invasion and EMT of thyroid carcinoma TT cells.
