1.Monitoring of Serum Concentrations of Western Medicine Ingredients in "Traditional Chinese Antiepileptic Medicines" and Analysis of Curative Effects

OBJECTIVE:To determine serum concentrations of western medicines in patients treated with "traditional Chinese antiepileptic medicine" alone and to evaluate the curative effects.METHODS:A total of 60 epileptic patients who visited our hospital between Feb.1997 and June 2006 were subjected to plasma drug level monitoring and during which the patients were treated with "traditional Chinese antiepileptic drugs" alone.Plasma concentrations of 4 kinds of western medicin-es were determined by FPIA.RESULTS:Of the 60 cases,valproic acid,carbamazepine,phenytoin and phenobarbitone were detected in 18,40,41,and 47 cases/times,respectively.On average,more than two kinds of western medicines were detected in every patient,and the blood concentrations were mostly beyond effective plasma drug concentration.The total curative effects were unsatisfactory.CONCLUSION:The fact that western medicine ingredients detected in these traditional Chinese antiepileptic medicines is inconformity with medication principle of epilepsy.Traditional Chinese antiepileptic medicines should be used with caution in the clinic in the treatment of epileptic patients.

Objective To explore the Oneomehnia distribution on electronic map based on gridding orientation pattern of marshlands in Changsha along Xiangjiang River,in order to manage snail information in a visual and digital way.Methods The data of geographic information and snail information were collected by global positioning system(GPS)and database was established using systematic sampling,and then an information management system of electronic map about the distribution of Oncomelania was designed.The degree of Oncomelania was expressed in different colors,the Oncomelania information in the database on the digital map wag reflected accurately using the geographic information system(GIS)technology.Results The electronic map of the distribution information of 20 marshlands in Changsha along Xiangjiang River were drawn.At the same time,the information could be grasped compreheusively.Digit map contains geographic information and database of snail information,such as the row and rnum,the density of alive and infectious snails,the area and geographic information et al.A method of managing and analyzing snail information with higher efficiency was offerred in this article.Conclusion Conventional methods of managing and analyzing Oncomelania information are reformed into digitalization and visualization.
5.Effect of nursing teaching competition to improve nurses' teaching ability

Objective To study the effect of nursing teaching competition to improve nurses' teaching ability.Methods Totals of 120 clinical teaching nurses from department of internal medicine and department of surgery were randomly divided into the control group and the observation group,each with 60 cases.Both groups received regular teaching training and the observation group received teaching competition once a month in addition.Teaching effect was evaluated by both teaching nurses and student nurses at the end of internship.Results Student nurses' satisfaction rates of teachers' knowledge level,professional quality,teaching methods,communication ability and teaching attitude were respectively 92.7%,95.3%,94.0%,93.3% and 94.7%,all higher than 80.0%,86.7%,80.0%,84.0% and 78.7% in the control group,and the differences were statistically significant (x2 =10.198 7,6.878 3,12.997 4,6.501 5,16.615 4,respectively; P < 0.01).Charge nurses' evaluation of student nurses' communication ability,quality of carefulness while alone,health education ability,professional dedication and ability to handle emergency were higher in the observation group than in the control group,and the differences were statistically significant (x2 =5.555 6,16.770 2,27.272 7,0.724 6,12.000 0,respectively; P < 0.01).Conclusions Nursing teaching competition can greatly improve nurses' ability of clinical teaching,which plays a decisive role to improve student nurses' comprehensive quality.
6.Effect of ribosome engineering on butenyl-spinosyns synthesis of Saccharopolyspora pogona.

Through introducing mutations into ribosomes by obtaining spontaneous drug resistance of microorganisms, ribosome engineering technology is an effective approach to develop mutant strains that overproduce secondary metabolites. In this study, ribosome engineering was used to improve the yield of butenyl-spinosyns produced by Saccharopolyspora pogona by screening streptomycin resistant mutants. The yields of butenyl-spinosyns were then analyzed and compared with the parent strain. Among the mutants, S13 displayed the greatest increase in the yield of butenyl-spinosyns, which was 1.79 fold higher than that in the parent strain. Further analysis of the metabolite profile of S13 by mass spectrometry lead to the discovery of Spinosyn α1, which was absent from the parent strain. DNA sequencing showed that there existed two point mutations in the conserved regions of rpsL gene which encodes ribosomal protein S12 in S13. The mutations occurred a C to A and a C to T transversion mutations occurred at nucleotide pair 314 and 320 respectively, which resulted in the mutations of Proline (105) to Gultamine and Alanine (107) to Valine. It also demonstrated that S13 exhibited genetic stability even after five passages.

7.Relationship between Expression of Vascular Endothelial Growth Factor and Cervical Lymph Node Metastasis in Papillary Thyroid Cancer: A Meta-analysis

The aim of the present study was to examine the relationship between the protein expression of vascular endothelial growth factor (VEGF) and lymph node metastasis (LNM) in papillary thyroid cancer (PTC).VEGF-related articles that had been published until August 2016 were searched from the PubMed,EMBASE,and MEDLINE to identify the risk factors of LNM in PTC.RevMan 5.3 software was used for the meta-analysis.Finally,9 articles met the inclusion criteria and were included in our meta-analysis.LNM was found to be present in 176 of 318 patients (57.8%) with high VEGF expression and in 71 of 159 patients (47.0%) with low VEGF expression.The overall OR was 2.81 (95% confidence interval,1.49-5.29).LNM occurred more frequently in patients with high VEGF expression than in those with low VEGF expression (P=0.001).Heterogeneity was markedly decreased in the subgroup analyses of LNM in terms of the patients' country of origin and the detection methods.Our meta-analysis concluded that the VEGF protein expression is associated with LNM in PTC.

10.Comparison of effects of acupuncture-assisted anesthesia with different acupoint combination in gynecologic laparoscopy operation.

OBJECTIVETo explore the best acupoint combination of acupuncture-assisted anesthesia in gynecologic laparoscopy operation.
METHODSNinety patients, with American Society of Anesthesiologists (ASA) physical status I - II, and scheduled for elective gynecologic laparoscopy operation, were randomly divided into 3 groups, 30 cases in each group. Group I received only general anesthesia, group II and group III received general anesthesia after Han's acupoint nerve stimulator (HANS) administered for 30 min, bilateral Zusanli (ST 36) and Sanyinjiao (SP 6) were selected for group II, and bilateral Hegu (LI 4) and Taichong (LR 3) were selected for group III. During operation, the concentration of Seveflurine was adjusted to maintain NTS at D1-D2. The change of ETsev value was recorded, the heart rate (HR) and blood pressure (BP) were observed, and the time from the end of operation to extubation, awake time were recorded adverse events such as restlessness, shivering, postoperative pain, nausea and vomiting, cases of respiratory depression, the analepsia quality, the time of first anus exhaust, the awaking of patient in operation and satisfactory of patients after surgery were recorded so as to evaluate the effect of acupuncture anesthesia.
RESULTSCompared with group I, the ETsev in group II was decreased 35% (P < 0.05), group ifi was decreased 25% (P < 0.05), with a significant difference between group II and group III (P < 0.05). BP and HR had more stability in group II and group III, the best in group II. As for the time from the end of operation to extubation and the time from the end of operation to open the eye on command, group I was the longest (all P < 0.05), group III was longer, group II was the shortest. The score of restlessness, postoperative pain, vomiting and in analepsia were the highest in group I (all P < 0.05), higher in group III and the lowest in group II. Consciousness score was lowest in group I (P < 0.05). As for the satisfactory of patient after operation, it was higher in group II and group IIII (both P < 0.05 ) than group I. The time of first anus exhaust in group II and group III were significantly shorter than that in group I (both P < 0.05).
CONCLUSIONHan's acupoint nerve stimulator has a certain adjuvant action for general anesthesia and analgesia and acupuncture point combination with both Zusanli (ST 36)and Sanyinjiao (SP 6) have the best effect in gynecologic laparoscopy operation. It can decrease the dosage of anaesthetic, has a stable effect on HR and BP during the surgery and has a better quality of analepsia.
