1.Development of an assessment tool for nursing professional values

Objective The purpose of the study was to develop an instrument to measure nursing professional values held by Chinese nurses.Methods 381 nurses were enrolled for the survey on their professional values by the draft of nursing professional values assessment tool(NPVAT).With the item analysis,such as dispersion degree,correlation efficient,discriminability,alpha coefficients index and exploratory factor analysis,the items were selected.Results As a result of the item analysis,the final version of NPVAT was composed of 20 items selected from a total of 32 items.Exploratory factor analysis resulted in a four-factor solution explaining 51.99% of the common variance.Conculsions The NPVAT developed in this research can be used for measuring the nursing professional values among nurses.

The holding of the national college students' clinical skills competition reflects the importance of medical education for clinical practice training . Although through intensive itemized skills drills, the competitors can complete each individual operation with satisfaction, due to the lack of clinical experience, in the integrated circuit training, they will easily stray into question stemtrap. The concrete analysis of the national college students' clinical skills contest competition reflects the medical students' insufficient recognition of clinical skills, lack of the overall concept of the intensive medical treatment, not flexible and insufficient mastery of the connotation of the clinical skills, which seriously restricts the improvement of clinical education. Therefore this article raises special sugges-tions, referring to training focusing on “Airway and Circulation”, developing the critical care thinking based on the relationship of multi-organ and improving the first-aid capability of the team work, so as to provide reference for the improvement of training effect.
3.CT feature of ovarian thecofibroma with clinical and pathologic correlation

Objective To observe CT features of ovarian thecofibroma and the clinical significance.Methods CT manifestations, clinical data and pathological diagnosis of 12 cases of ovarian thecofibroma proved by pathology were retrospectively analysed.Results The CT manifestation of ovarian thecofibroma was typical,which was showed as following:on one side of the attachment area tumor,single,ovoid or class circular shape,most boundary clear,equal or slightly low density solid masses,internal or boundary ar-ea accompanied by a small amount low density area,mild delay enhancement in solid ingredients or no obvious enhancement.Conclu-sion CT examination is important for qualitative diagnosis of ovarian thecofibroma and finding complications.

5.Cortex function and neuropsychological changing of non fluent aphasia type-frontal temple dementia

Objective To analyse the neuropsychological changing of nonfluent aphasia frontotemporal dementia (nf-FTD) with the FDG-PET, MRI and fMRI. Methods The language and other cognitive function were dynamically estimated by standard neuropsychological assessment which modified by Chinese. Compared with FDG-PET and fMRI, the relationship between the neuropsychologieal changing and the cortex function was studied. Results All the 5 patients met the criteria of nf-FTD by Association for Frontotemporal Dementias, with core syndrome and other supptted syndrome. And still they had the good word reading, calculation by pen, and the un-speechmemory. And they also had the related well living ability. They all had the related cortext metabolism support. Conclusions nf-FTD patients preserve the word reading, visuapacial ability and calculation by pen. These can help us to give the effective advise about the patients training and caring and to give them the chance to show their real social skill.
6.Immune dysregulation, polyendocrinopathy, enteropathy, X-linked syndrome in two cases.

OBJECTIVETo report on two cases affected with immune dysregulation, polyendocrinopathy, enteropathy, X-linked syndrome (IPEX).
METHODSTwo unrelated Chinese infants affected with IPEX were investigated. Case 1 was a 4-month-old boy with neonatal diabetes and severe enteropathy. Case 2 was a 6-day newborn boy with neonatal diabetes and ketoacidosis. DNA samples of the two infants and their parents were sequenced for FOXP3 gene mutations. Suspected mutations were verified among 100 unrelated healthy controls. The function of mutations was predicted with bioinformatics software.
RESULTSBoth infants had onset of the disease during neonatal period, and manifested insulin-dependent diabetes mellitus, persistent diarrhea, eczema and malnutrition. In case 1, a novel splice site mutation was identified in intron 9 (c.967+3A>T) of the FOXP3 gene, for which his mother was a carrier. For case 2, a missense mutation (c.1150G>A) was detected in exon 11 of the FOXP3 gene, for which his mother was also a carrier. The IVS9 c.967+3A mutation was not detected among the 100 healthy controls. As predicted with Human Splicing Finder software, the c.967+3A>T mutation may influence the splicing of mRNA and affect the function of protein.
CONCLUSIONBoth cases had typical clinical manifestation of the IPEX syndrome, among whom a novel splice site mutation (IVS9 c.967+3A>T) and a missense mutation (c.1150G>A) of the FOXP3 gene were identified. The clinical manifestation of the IPEX syndrome may be variable and the mortality is high. FOXP3 gene sequencing is recommended when insulin-dependent diabetes mellitus is diagnosed during the neonatal period.
7.Application of electronic signature technology in management and monitoring in vaccine cold-chain

In order to ensure the safety of the vaccine cold chain to the greatest extent, the newly regulations still require daily manual recording of temperature data and operation conditions based on the configuration of the automatic temperature measurement and alarm system.Therefore, on the basis of Shanghai Vaccine Cold-chain Monitoring System, Shanghai has introduced an electronic signature technology to implement a new model of remotely checking the temperature by mobile-phone and keeping records of the electronic signature as certificates.This technology reduces the on-site workload of cold chain management personnel.It also guarantees the authority, legitimacy, and non-tampering of recorded information through blockchain certificate storage technology.The application of the electronic signature technology is compatible with technical defense and civil defense methods, and is worthy of further promotion and use.
8.Association of the Asp312Asn and Lys751Gln polymorphisms in the XPD gene with the risk of non-Hodgkin’s lymphoma:evidence from a meta-analysis

Polymorphisms in DNA repair genes may alter DNA repair capacity and, consequently, lead to genetic instability and carcinogenesis. Several studies have investigated the association of the Asp312Asn and Lys751Gln polymorphisms in the xeroderma pigmentosum complementation group D (XPD) gene with the risk of non-Hodgkin’s lymphoma (NHL), but the conclusions have been inconsistent. Therefore, we performed this meta-analysis to more precisely estimate these relationships. A systematic literature search was performed using the PubMed, Embase, and Chinese Biomedical (CBM) databases. Ultimately, 6 studies of Asp312Asn, comprising 3,095 cases and 3,306 controls, and 7 studies of Lys751Gln, consisting of 3,249 cases and 3,676 controls, were included. Pooled odds ratios (ORs) and 95%confidence intervals (CIs) were calculated to assess the strength of each association. Overal , no association was observed between the Asp312Asn polymorphism and NHL risk (homozygous:OR=1.11, 95%CI=0.94-1.32;heterozygous:OR=1.00, 95%CI=0.89-1.11;recessive:OR=1.12, 95%CI=0.95-1.31;dominant:OR=1.02, 95%CI=0.92-1.13;and al ele comparison:OR=1.04, 95%CI=0.96-1.12) or between the Lys751Gln polymorphism and NHL risk (homozygous:OR=0.97, 95%CI=0.83-1.15;heterozygous:OR=0.96, 95%CI=0.86-1.06;recessive:OR=1.00, 95%CI=0.86-1.16;dominant:OR=0.96, 95%CI=0.87-1.06;and al ele comparison:OR=0.98, 95%CI=0.91-1.05). Furthermore, subgroup analyses did not reveal any association between these polymorphisms and ethnicity, the source of the controls, or the NHL subtype. These results indicated that neither the Asp312Asn nor Lys751Gln XPD polymorphism was related to NHL risk. Large and well-designed prospective studies are required to confirm this finding.
9.Observation on controlled hypotension during the peri-operation of endoscopic sinus surgery and nursing cooperation

Objective To discuss the mode of nursing cooperation of endoscopic sinus surgery for nasal sinus and nasal cavity under controlled hypotension, and to summary and accumulate the experiences. Methods The method of nursing cooperation and the operation program of 80 cases with endoscopic sinus surgery through the way of moving down the nasal endoscope under whole-anesthetization were retrospectively analyzed. These patients were divided into the controlled hypotension group and the non-controlled hypotension group. Results The effect of decreasing blood pressure was satisfied. The bleeding amount of the controlled hypotension group was smaller than that in the non-controlled hypotension group (P<0. 05). The amount of transfused liquid was smaller than that in the non-controlled hypotension group (P<0. 05). And the operation time was obviously shortened because of proper nursing cooperation. Conclusions Controlled hypotension can obviously reduce the bleeding amount and the transfused liquid during the operation, thereby shortening the operation time. The nursing measures should lay emphasis on the cycle and the bleeding amount during the operation, the breath and close observation of nervous system after the operation, and cooperation with the doctors to timely treat the symptoms.
10.Effect and nursing of the children under self-retaining laryngoscope papilloma surgery with intermittent ventilation and apneic anaesthesia

Objective To investigate the effect and nursing methods of children under the self-retaining laryngoscope papilloma surgery with intermittent ventilation and apneic anaesthesia. Methods Twenty children were selected as study subjects, who were undergoing selective self-retaining laryngoscope papilloma surgery assist with endoscope and with endotracheal intubation general anaesthesia. When the pulse oxygen saturation achieve 100% and the end expiration carbon dioxide concentration lower than 30mmHg, the intubation was pulled out and a surgery was carried out at once;When the SpO2 was 97% , endotracheal intubation was made again and hyperventilation. The treatment was repeated till the operation completed. The arterial blood gas,mean arterial pressure, heart rate, saturation of blood oxygen and the end expiration carbon dioxide concentration were monitored. The operation condition assessment by the surgeon, the time that patients come from consciousness;operation complications and the rate of papilloma recurrence three months after operation also were recorded. Results All operations were successfully completed and the time of consciousness was (7 ±3)min. PCO2 and PaO2 were a little fluctuation but in the permission extent. MAP、HR、SpO2 、pH and PaCO2 were maintained stable (P > 0. 05 );The operation condition assessment has reached ninety percent. No complications have happened. The rate of papilloma recurrence three months after operation was 15%.Conclusions Under the cooperation of the surgeon, anesthetists and nurses, the intermittent ventilation and apneic anaesthesia can be safely used in the children self-retaining laryngoscope papilloma surgery, and the breath and airway attendance should be emphasizes.
